Use of prostaglandin E2 in the management of missed abortion, missed labour, and hydatidiform mole.

Treatment of six cases of missed abortion and one case of hydatidiform mole with intravenous infusion of prostaglandin E(2) resulted in complete abortion in all cases. Of 15 patients with missed labour, 14 were delivered successfully with similar treatment. The technique appears to be a safe, reliable, and rapid method of managing missed abortion, missed labour, and hydatidiform mole.

Medical termination of missed abortion.

Missed abortions are a common occurrence and represent a heavy gynaecological emergency workload to both medical and nursing staff. The conventional method using vacuum aspiration of uterus is associated with morbidity and mortality. Medical termination of pregnancy is accepted as a safe and effective alternative method. However, medical evacuation of uterus in missed abortions had not been fully investigated. In this study we hope to examine the efficacy of medical methods for terminating missed abortions. Mifepristone and misoprostol prescribed to 100 women with an ultrasonically confirmed missed abortion was compared with 100 women who had surgical evacuation under general anaesthesia for their missed abortion. The disparity in complications between medical and surgical groups was not significantly different (Student's t-test, P=0.5). This study showed medical evacuation of missed abortion to be an effective, safe and cost-effective, alternative to surgical evacuation of the uterus and is particularly suited to women not wanting hospital admission or a surgical procedure under general anaesthesia. It represents an option of management that can be less intrusive and gives the woman some control but is both safe and effective.

Platelet function, coagulation and fibrinolysis during termination of missed abortion and missed labor by PGF2 alpha and oxytocin.

In cases of missed abortion and missed labor, labor was induced by PGF2 alpha i.a., i.v. and by oxytocin infusion. Platelet function (methods of Born and Breddin), the coagulation system and fibrinolysis have been studied within the three groups. Using PGF 2 alpha i.v., the initially increased platelet aggregation showed a tendency to become normal. There was no manifestation of activation of the coagulation system. Fibrinolytic activity showed a slight increase during PGF2 alpha i.v. No essential changes in platelet function, coagulation and fibrinolytic system were found after i.a. injection of PGF2 alpha. When inducing labor by oxytocin i.v., both the coagulation and the fibrinolytic system were slightly activated and platelet aggregation increased. The results and their clinical importance for hemostasis as well as therapeutic consequences are discussed.

Induction of abortion and labor by extraamniotic isotonic saline, with or without addition of oxytocin, in cases of missed abortion, missed labor and antepartum fetal death.

In a group of 16 women admitted for missed abortion, missed labor or antepartum fetal death, we induced abortion or labor by means of extraamniotic infusion of isotonic saline solution, using a Foley catheter inserted through the cervix. Eight patients also required parallel intravenous administration of 5-10 U of oxytocin drip. The average time from the onset of treatment until expulsion of the macerated fetus was 9.09 hours. No complications were recorded. This method is suggested in all such cases, especially when the classical methods are contraindicated.

Comparison of glycosylation patterns of placental proteins between normal pregnancy and missed abortion.

Contemporary understanding of missed abortion, as the case of spontaneous abortion where embryo is retained in uterus for four-weeks or more after its death, is very poor. Aiming to improve the level of knowledge about this process, we have compared glycosylation patterns of placental proteins in normal pregnancy and missed abortion. Oligosaccharide branches were detected by Western-blot using SNA, DBA and PHA-E lectins. The comparison of samples of the same gestational age enabled identification of changes in protein glycosylation between normal and pathological placentas. Lectin DBA detects in normal placenta the glycoprotein GP 105 during the eleventh week, which is absent in missed abortion. PHA-E identifies GP 71 during fourteenth week only in normal placenta. However GP 25 recognized by SNA in missed abortion was not found in normal pregnancy at tenth week. These results indicate that abnormal placental development is associated with changes in glycoprotein structures, and that glycoconjugates might have an important role in placental development.

Missed abortion and uterine contractility.

In missed abortion myometrial contractility is thought to be suppressed. However, such a simple procedure as sweeping of the fetal membranes by physiologic saline solution was found to provoke abortion within 24 hours in 10 of 12 cases of missed abortion, but not in any of 10 women admitted for therapeutic abortion. Myometrial contractility thus seems not to be diminished in missed abortion.

Hysteroscopic findings after missed abortion.

OBJECTIVE: To identify the relation of missed abortions and intrauterine pathology. DESIGN: A postabortal hysteroscopy was performed 8 to 12 weeks after a dilatation and curettage (D&C) for missed abortion. SETTING: Department of Obstetrics and Gynecology, Assaf Harofeh Medical Center, Zerifin, Israel. PATIENTS: Sixty patients after a D&C for a missed abortion. MAIN OUTCOME MEASURES: The hysteroscopic appearance of the uterine cavity. RESULTS: Intrauterine adhesions occurred in only 10 patients (16.7%) with most of them of the mild type. A previous missed abortion was recorded in 60% of these cases. Uterine anomaly was found in one quarter of the cases, mainly an incomplete uterine septum. CONCLUSION: We believe that missed abortion does not predispose for intrauterine adhesions to the extent that was previously believed. A partial uterine septum is a major factor predisposing for the occurrence of missed abortion. Hysteroscopy after a missed abortion is an easy and efficient means for both identifying such uterine anomaly and excluding intrauterine adhesions.
