Nonoptical definition of applanation surface.

Based on the Imbert-Fick law and the theoretical considerations of Goldmann, new applanation tonometers were designed, still using an applanation diameter of 3.06 mm to avoid a new biometric calibration. Three different electronic recording devices were used for the area assessment. An optical, a capacitance and a digital line sensor were tested in first calibration experiments.

Cortical surface tracking using a stereoscopic operating microscope.

OBJECTIVE: To measure and compensate for soft tissue deformation during image-guided neurosurgery, we have developed a novel approach to estimate the three-dimensional (3-D) topology of the cortical surface and track its motion over time. METHODS: We use stereopsis to estimate the 3-D cortical topology during neurosurgical procedures. To facilitate this process, two charge-coupled device cameras have been attached to the binocular optics of a stereoscopic operating microscope. Before surgery, this stereo imaging system is calibrated to obtain the extrinsic and intrinsic camera parameters. During surgery, the 3-D shape of the cortical surface is automatically estimated from a stereo pair of images and registered to the preoperative image volume to provide navigational guidance. This estimation requires robust matching of features between the images, which, when combined with the camera calibration, yields the desired 3-D coordinates. After the 3-D cortical surface has been estimated from stereo pairs, its motion is tracked by comparing the current surface with its previous locations. RESULTS: We are able to estimate the 3-D topology of the cortical surface with an average error of less than 1.2 mm. Executing on a 1.1-GHz Pentium machine, the 3-D estimation from a stereo pair of 1024 x 768 resolution images requires approximately 60 seconds of computation. By applying stereopsis over time, we are able to track the motion of the cortical surface, including the pulsatile movement of the cortical surface, gravitational sag, tissue bulge as a result of increased intracranial pressure, and the parenchymal shape changes associated with tissue resection. The results from 10 surgical patients are reported. CONCLUSION: We have demonstrated that a stereo vision system coupled to the operating microscope can be used to efficiently estimate the dynamic topology of the cortical surface during surgery. The 3-D surface can be coregistered to the preoperative image volume. This unique intraoperative imaging technique expands the capability of the current navigational system in the operating room and increases the accuracy of anatomic correspondence with preoperative images through compensation for brain deformation.

Linear focusing by a plane grating with curved grooves.

We study the field diffracted by a plane grating with curved (parabolic) grooves. We will demonstrate that when a monochromatic plane wave is incident on a grating with parabolic grooves the diffracted field has a focal line whose position depends on the curvature radius of the parabolas and the incidence angle of the light onto the grating. The effect described has potential applications in grating-based devices for focusing light without requiring any additional optics.

Method for embedding optical fibers in an aluminum matrix by ultrasonic consolidation.

The overall aim of the research, part of which is outlined in this paper, was to utilize the ultrasonic consolidation (UC) process for the fabrication of smart metal structures, capable of measuring an external stimulus and responding to this stimulus by adapting its structure accordingly through embedding both active and passive functional elements. This paper presents a fundamental study of embedding methods for the fabrication of optical fibers embedded within aluminum structures. The methods considered in this paper produced embedded optical fiber specimens in which large amounts of plastic flow were observed within the matrix. The matrix material deformed around the fibers, resulting in fully embedded optical fibers capable of transmitting a bright light source and without damaging the fibers. Based on light responses, a general process window was drawn to show the range at which optical fibers can be embedded within aluminum structures using the UC process. The outcomes lay down initial investigative principles for the further development of the technology for embedding or cladding of optical fiber sensors, such as fiber Bragg grating devices, within or on metal structures: for example, the cladding of large free-form metal structures or smart "skinned" metal foam or metal honeycomb structures.

Using shaded filter instruments for measurements of sky radiance: retrieval of the apparent sky-view factor from ultraviolet radiation measurements.

There is a growing need for sky radiometric measurements that encapsulate spatial as well as temporal variability. Since the advent of fast data acquisition systems in the 1980s, recent studies have utilized radiation filter instruments deployed in various sky-shading platforms. One cost effective method provides azimuthally averaged sky radiance distribution data at time scales down to fractions of a minute. Successful operation of this scheme requires knowledge of the apparent sky-view factor of the deployed sensor--an artifact of the instrument input optics, instrument filter design, and the instrument shading device employed. We provide a methodology for a determination of the sensor sky-view factor by using the employed shading device and the sun as a light source. The effect of an incorrect determination of instrument sky-view factor is also analyzed in context of the measurements.

Electrostatic micromachine scanning mirror for optical coherence tomography.

Compact electrostatic micromirror structures for use in the scanning arm of an optical coherence tomography (OCT) system are described. These devices consist of millimeter-scale mirrors resting upon micrometer-scale polyimide hinges that are tilted by a linear micromachine actuator, the integrated force array (IFA). The IFA is a network of deformable capacitor cells that electrostatically contract with an applied voltage. The support structures, hinges, and actuators are fabricated by photolithography from polyimide-upon-silicon wafers. These devices were inserted into the scanning arm of an experimental OCT imaging system to produce in vitro and in vivo images at frame rates of 4 to 8 Hz.

Ultraviolet wave plates based on monolithic integration of two fully filled and planarized nanograting layers.

We successfully fabricated a high-performance half-wave plate for the 405 nm wavelength based on monolithic integration of two nanograting layers. Each of the nanograting layers functions as a quarter-wave plate. Both of the nanograting layers were fully filled and planarized to achieve the monolithic integration. UV-nanoimprint lithography, along with thin-film deposition, high-aspect-ratio reactive ion etching, and trench-filling technologies, was used in fabrication and integration of the individual nanograting layers. High-aspect-ratio nanogratings with sub-50 nm linewidths and 100 nm spacing were fabricated to achieve good optical performance at the near-UV wavelength. The ability to integrate multiple nanostructure-based optical layers opens a path for integrated multifunction devices, as well as a new strategy for driving both miniaturization and cost.

A microchip CD4 counting method for HIV monitoring in resource-poor settings.

BACKGROUND: More than 35 million people in developing countries are living with HIV infection. An enormous global effort is now underway to bring antiretroviral treatment to at least 3 million of those infected. While drug prices have dropped considerably, the cost and technical complexity of laboratory tests essential for the management of HIV disease, such as CD4 cell counts, remain prohibitive. New, simple, and affordable methods for measuring CD4 cells that can be implemented in resource-scarce settings are urgently needed. METHODS AND FINDINGS: Here we describe the development of a prototype for a simple, rapid, and affordable method for counting CD4 lymphocytes. Microliter volumes of blood without further sample preparation are stained with fluorescent antibodies, captured on a membrane within a miniaturized flow cell and imaged through microscope optics with the type of charge-coupled device developed for digital camera technology. An associated computer algorithm converts the raw digital image into absolute CD4 counts and CD4 percentages in real time. The accuracy of this prototype system was validated through testing in the United States and Botswana, and showed close agreement with standard flow cytometry (r = 0.95) over a range of absolute CD4 counts, and the ability to discriminate clinically relevant CD4 count thresholds with high sensitivity and specificity. CONCLUSION: Advances in the adaptation of new technologies to biomedical detection systems, such as the one described here, promise to make complex diagnostics for HIV and other infectious diseases a practical global reality.

[Improvement of optic characteristics of the x-ray protective glass].

The optic characteristics of the roentgen protection glass (RPG) are shown to be capable of being improved through application of a translucent coating onto its working surface. This eliminates losses due to reflection from the RPG of a luminous flux proceeding to the examiner from the fluorescent screen of the X-ray apparatus. The proposed improvement of optic characteristics of the RPG will make it possible to cut down by 15 per cent the dose rate of ionizing radiation to which both the patient and the physician are exposed during roentgenoscopy.

[Development of wearable artificial endocrine pancreas--problems awaiting solutions for long-term clinical application].

By applying artificial endocrine pancreas, the perfect glycemic normalization could be achieved in diabetic patients. The major obstacle for long-term clinical application of artificial endocrine pancreas, however, is the shortage of the life-time of glucose sensor. Thus, it is crucial to develop continuous glucose sensing system with long-life. To augment the biocompatibility, most intensive investigations should be necessary on membrane design for indwelling glucose biosensor. Microdialysis sampling method could also be applied for extracorporeal measurement of subcutaneous tissue glucose concentrations. As an alternative way of long-term glycemic monitoring, the possibility of non-invasive method of glucose measurement by analyzing the infra-red absorption spectra through attenuated total reflection prism was indicated. In conclusion, as a future implantable artificial endocrine pancreas, the combined system of an implantable computer-insulin delivery device and an indwelling glucose biosensor or an optical glucose sensor with telemetric maneuver will be most feasible.

[Scanning laser ophthalmoscopy. Its value in macular diseases].

The scanning laser ophthalmoscope provided a high quality television image of the fundus with minimal illumination of the retina. This new device based on a totally new electro-optical principle allowed a detailed exploration of macular function directly under simultaneous fundus control. The focused beam of a yellow krypton laser (568,2 nm) was swept up and down, rights and links across the fundus to form a raster of parallel lines on the retina. The S.L.O. illuminated only a single retinal point at a time and illumination was reduced to less than 70 microw/cm2 versus 100,000 microns/cm2 for indirect ophthalmoscopy and 4,000,000 microw/cm2 for fluorescein angiography. The intensity of the laser beam could be modified with the microcomputer by means of an acousto-optic modulator. It was possible to produce static or dynamic graphic designs that were simultaneously viewed by the patient and observed by the examiner on the patient's fundus on the video monitor. Further computerized analysis of the videotaped scanning laser ophthalmoscopic images gave a functional retinal map with correction for shifts of stimulus position due to fixational saccadic eye movements. The map showed true retinal location of 1. fixation area; 2. scotoma. The clinical evaluation was completed with 3. measurement of visual acuity in any foveal or parafoveal location. The results of scanning laser ophthalmoscopy were illustrated with the report of datas in a patient with diffuse retinal pigment epithelial decompensation.

[The development of a new operating hysteroscopic fiberscope and its clinical application].

A new operating hysteroscopic fiberscope consisting of soft and rigid parts (4.8mm outer diameter) was developed with the support of Fuji Photo Optical Company. The working part of the scope can be divided into three sections: A flexible soft front section, a rotary rigid middle section and a flexible self retained semirigid rear section. With these functional parts the intrauterine target can be approached directly to perform the following operations. 1. Directed intrauterine biopsy. Thirty-five patients diagnosed as having endometrial polyp (13), submucous myoma (8), endometrial hyperplasia (4), endocervical polyp (3), endometrial carcinoma (2) and others (5) underwent direct biopsy with hysteroscopic control. No cervical dilatation or anesthesia was necessary. 2. Transcervical recanalization. In six cases of proximal tubal occlusion, a ureteral catheter or a percutaneous coronary balloon angiocatheter was introduced into the tubal ostium of the obstructed side to resolve the occlusion successfully with concomitant laparoscopy. 3. Hysteroscopic chorionic villus sampling. Chorionic villus sampling was performed with a ureteral catheter under direct hysteroscopic control and ultrasound guidance in eighteen pregnant women at from seven to fourteen gestational weeks. In fifteen cases, the samplings were performed satisfactory. 4. Removal of a lost IUD. Three cases of lost IUD underwent hysteroscopic removal without difficulty. Our results have proved that this scope is a very useful tool for intrauterine operations.
