[Treatment of acquired arteriovenous fistulas with interventional minimally invasive techniques].

OBJECTIVE: To evaluate the efficacy and safety of the interventional techniques for treatment of acquired arteriovenous fistulas (AVF). METHODS: Ten patients with acquired AVFs, including 4 with renal AVF, 3 with iliac AVF, and 3 with subclavian AVF, were treated with interventional procedures. The etiological factors of the AVFs were penetrating trauma in 5 cases, iatrogenic injury in 3, malignancy in 1, and intestine Crohn's disease in 1. The patients presented with peripheral venous hypertension (n = 6), local bruit (n = 10), cardiac overload (n = 10), the right cardiac failure (n = 2), and hematuria (n = 4). Three patients underwent transcatheter super-selective coils embolization and 7 underwent stent-graft placement in the involved arteries. RESULTS: The technical success was achieved in all cases. Completion angiography documented complete exclusion of the fistulas. Minor complications occurred in 3 patients, but without significant consequences. The patients experienced immediate relief of the limb swelling, peripheral venous hyperemia, and tachycardia. The local bruit was disappeared. The cardiac overload conditions were improved significantly, which was confirmed by ultrasound scan. Renal function tests in patients with renal AVF were stable. Radioactive isotopic scan revealed that the function was preserved in the treated kidney in two patients using stent-graft placement in the renal arteries. Follow-up time ranged from 6 months to 6 years. Three patients respectively died of unrelated AVF diseases in 6, 9, and 14 months after the treatment. Re-intervention with an another stent-graft placement was performed on 2 patients with recurrence of the AVF respectively at 3 weeks and two months after the procedures. The minor stenosis was found in stent-graft 2 of patients on the follow-up angiography respectively at 6 and 8 months after the treatment. Seven patients are still alive and in good condition without further intervention. CONCLUSIONS: Minimally invasive interventional procedures, including super-selective embolization and stent-graft exclusion, are safe and effective in the treatment of acquired arteriovenous fistulas.

[Assessment of the time course of stunned myocardium with cardiac magnetic resonance imaging: a case report].

A 77-year-old woman with chest pain was admitted to our hospital for evaluation and treatment. Electrocardiography showed T-wave inversion in the I, aVL and V2-V6 leads. Emergency coronary angiography showed 75% stenosis in the left anterior descending artery. Left ventriculography demonstrated akinesis of the left ventricular apical region. Iodine-123-beta-methyl-p-iodophenyl-pentadecanoic acid radioactive isotope imaging showed an uptake defect in the apical region during the acute phase, but the defect disappeared 1 month later. Cine cardiac magnetic resonance (CMR) in the acute phase showed apical akinesis and hyperkinesis of the mid region, as observed by left ventriculography. Contrast magnetic resonance imaging with gadolinium showed no delayed hyperenhancement. One month later, cine CMR showed disappearance of the abnormal wall motion and contrast magnetic resonance imaging demonstrated no delayed hyperenhancement. CMR is useful to monitor changes in wall motion and wall thickening in the stunned myocardium.

[Isolation and investigation of natural yeast strains resistant to heavy metal salts and radionuclides].

The collection of yeasts (more than 2000 strains) from extreme natural environment of Kamchatka peninsular and Kuril Islands was created. 448 strains were selected for their resistance to salts of such heavy metals as Cu, Cd, Co and to high temperature (37-52 degrees C). 72% of strains appeared to be resistant to one or more selective factors. We obtained several strains able to grow on medium with 0.1 M/L nonradioactive strontium and (or) cesium. Four of this strains accumulated radioactive isotope 90Sr with 45-80% efficiency. Thus, we demonstrated that yeast strains from nature could be used for bioremediation of industrial waste solutions, polluted by radionuclides and salts of heavy metals.

Effects of intestinal protozoa infection on gastrointestinal transit and contractility in experimental animals.

Three groups of animals each was orally infected with Cryptosporidia parvum oocysts, Enterocytozoon bieneusi spores or Cyclospora cayetanensis oocysts. At the time peak of colonization of infected animals, each group and its corresponding control were infused orally with radioactive isotope. Gastric emptying of isotope was significantly greater in infected compared to controls in both fasted and fed states, to determine the effect of each parasite on the contractility of longitudinal and circular muscle, isometric tension of jejunal segments was recorded. The development of active tension with stretch and the dose-response curve to muscarinic against were significantly increased in the longitudinal muscle of infected animals compared to controls. The circular smooth muscle did not show an increase in contractility. The results suggest that an altered gastrointestinal transit and smooth muscle contractility may be involved in the intestinal protozoa infections' pathophysiology.

[Sentinel-node biopsy for breast cancer: our experience].

The method of sentinel lymphnode is a new procedure, suitable for the cases of early diagnosed breast cancers: identifying the first lymphatic station of the cancer, it can avoid the axillary lymphadenectomy in those women with primitive invasive and unifocal neoplasis of not more than 1.5 cm in diameter and without lymphnodal invasion. So it is reduced the frequency of postoperative troubles (sensitive and motor diseases, lymphedema). In our study, we have researched and identified, using radioactive isotopes, the sentinel lymphnode in 20 women suffering from subclinic breast cancer. We have obtained a percentage of 5% of false negatives. The optimal level of identification of the sentinel lymphnode and the good level of predictivity about the axillary lymphatic status, that we have obtained, represent a further confirmation of the surety and validity of this method.

The accuracy of intraoperative frozen section analysis of the sentinel lymph nodes during breast cancer surgery.

BACKGROUND: The sentinel node biopsy (SNB) is a reliable and minimally invasive procedure, representing a new standard of care for patients with clinically node-negative breast cancer. Several studies have confirmed the reliability and the accuracy of this new procedure. Accurate intraoperative evaluation of sentinel lymph nodes (SLNs) can determine the need for immediate axillary node clearance thus reducing arm morbidity in SLN-negative patients and avoiding a second surgical procedure in patients with positive SLNs. The aim of this study is to determine the accuracy and the limitations of intraoperative frozen section (IFS) analysis of SLNs during breast cancer surgery. PATIENTS AND METHODS: 96 female patients with clinically node-negative T1-T2 breast cancer who underwent the SNB procedure and IFS analysis by single section standard staining in two hospitals were reviewed. The SNB procedure was performed using the patent blue dye method alone or the combination of the dye and radioactive isotope techniques. The results of IFS and the final histology were compared. RESULTS: The mean number of SLNs removed was 2. Overall, 22% (21/96) of patients were found to be SNB-positive on final histology. Based on patients, there was a 96.8% concordance between the results of the IFS and the final histology; the sensitivity was 86% (18/21), and the specificity was 100%. There were 3 cases of false negative all of which contained micrometastases only. When micrometastases were excluded, the sensitivity was 100%. Based on SLNs (n = 196), the sensitivity was 91%, and the specificity was 100%. CONCLUSION: Intraoperative frozen section of sentinel lymph nodes in patients with early breast cancer is highly accurate for macrometastases but not for micrometastases.

[Detection of cDNA end sequence of porcine fibrinogen like protein2 and it's structure analysis].

The purpose of this study is to detect the end sequence of porcine FGL2 gene cDNA and make a preliminary analysis of it's structure. Porcine DNA library was screened by a cDNA probe labeled with radioactive isotope alpha-32P dCTP. Rapid amplification of cDNA end (RACE): Retroverse transcription product of total RNA extracted from normal porcine tissue was used as the template, gene specific primers were designed and advantage 2 polymerase mix was used in PCR, of which using porcine genomic DNA as the template:forward primer was designed according to the acquired consensus region of human and pig FGL2 3' sequences while reverse primer was designed from human FGL2 3' end downstream sequence; TA cloning. Screening library failed to get any specific positive clone; the specific transcription initiation site and first poly A signal were successfully detected by RACE reaction although it fails again to detect the second poly A signal. The unknown sequence of porcine FGL2 3' end including the second poly A signal was successfully detected by PCR using genomic DNA as the template. RACE reaction can be applied as an effective method to detect the specific transcription initiation and termination sites. Using advantage 2 polymerase mix instead of regular DNA polymerase may significantly improve the sensitivity, accuracy and specificity of PCR reaction. If it met particular difficulties in the regular screening of DNA library, PCR reaction utilizing primers designed from the known consensus sequence and genomic DNA as template may be considered as an appropriate alternative.

[Chromosomal localization of the hormone-sensitive lipase gene (Hsl) in rice field eel].

Adipose tissue triacylglycerols are the quantitatively most important source of stored energy in animals. Hormone-sensitive lipase encoded by hormone-sensitive lipase gene (Hsl) is a multifunctional enzyme that catalyzes the hydrolysis of triacylglycerol stored in adipose tissue and cholesterol esters in the adrenals, ovaries, testes and macrophages. Using pig Hsl gene inserted into pBS labeled by the radioactive isotope and the digoxigenin as the probes respectively one band, 11.5kb, has been shown to hybridized with total DNA of rice field eel digested with Pst I by Southern blotting and Hsl gene has been assigned to metaphase chromosome 5, at the position of 78.35+/-1.26 from the centromere in rice field eel by fluorescent in situ hybridization (FISH). The mapping results are corresponding to that of "specific-chromosomal DNA pool" obtained by chromosome microisolation used to map gene and the mapping result is more accurate. The results of the study further illustrate the importance of the presence of Hsl gene in rice field eel genome and provide the first FISH mapping data for rice field eel chromosome 5. The current studies would advance the addition of known genetic markers and the construction of high resolution genetic map in rice field eel genome.

[Respiratory emergencies].

Respiratory emergencies in a patient with cancer can have their origin in pathologies of the airway, of the pulmonary parenchyma or the large vessels. The cause can be the tumour itself or concomitant complications. Obstruction of the airway should be initially evaluated with endoscopic procedures. Surgery is rarely possible in serious situations. The endobronchial placement of stents or radioactive isotopes (brachytherapy), tumoural ablation by laser or photodynamic therapy can quickly alleviate the symptoms and re-establish the air flow. Treatment of haemoptysis depends on the cause that is provoking it and on its quantity. Bronchoscopy continues to be the front line procedure in the majority of cases; it provides diagnostic information and can interrupt bleeding through washes with ice-cold serum, endobronchial plugging or topical injections of adrenaline or thrombin. External radiotherapy continues to be an extraordinarily useful procedure in treating haemoptysis caused by tumours and in carefully selected situations of endobronchial therapy with laser or brachytherapy, and bronchial arterial embolisation can provide a great palliative effect. Respiratory emergencies due to pulmonary parenchyma disease in the oncology patient can have a tumoural, iatrogenic or infectious cause. Early recognition of each of these will determine the administration of a specific treatment and the possibilities of success.

Detection of gastrointestinal and abdominal infections by 99mTc-ciprofloxacin.

BACKGROUND/AIMS: The aim of the study is detection and evaluation of the abdominal and gastrointestinal infective foci using 99mTc-ciprofloxacin (Laboratory for radioactive isotopes, Vinca). METHODOLOGY: In total 21 patients with clinical suspicion on abdominal or gastrointestinal infection were investigated. In all the patients, planar liver/spleen scintigraphy was performed. Ciprofloxacin chloride (3.5 mg) was mixed with 555 MBq of 99mTc in 3 mL of physiological solution and incubated for 20 min. After slow i.v. injection in a cubital vein, dynamic acquisition (1 f/min) was performed during the first 60 min in the position of interest, followed by static acquisition (500,000 imp) anterior and posterior view, abdomen and pelvis after 1 h and 4 h in all patients. When necessary, additional scintigrams were acquired after 24 h. In all the patients with negative or equivocal findings of planar scintigraphy, emission computerized tomography (SPECT) was performed (60 positions, 6 degrees). Interpretation was made by three independent observers. Additional data were provided using clinical findings, ultrasonography, computed tomography and magnetic resonance imaging, laboratory analyses, and surgical or microbiological confirmation of infection. RESULTS: There were eleven true-positive findings, seven true negative, two were false negative while one was false positive due to intestinal obstruction. Sensitivity was 79%, specificity 91%, positive predictive value 92%, negative predictive value 77%, accuracy 84%. CONCLUSIONS: According to our results, scintigraphy with radiolabeled ciprofloxacin is a useful method for detection and assessment of exact localization of abdominal and gastrointestinal infections.

Effects of protopine on intracellular calcium and the PKC activity of rat aorta smooth muscle.

We have previously shown that the vasodilator effect of protopine (Pro) on rabbit aorta is related to the elevations of cAMP and cGMP. In the present study, the vasodilator mechanisms of Pro were further explored by recording the isotonic contraction of the rat aortic strips, detecting directly the intracellular free Ca(2+) concentration ([Ca(2+)](i)) with Fura-2/AM loaded vascular smooth muscle cells (VSMCs) of rat aorta, and determining the activity of protein kinase C (PKC) in rat aortic tissue with radioactive isotope gamma-32P -ATP-catalyzing assay. By recording the aortic strips contraction induced by noradrenaline (NA) and high potassium (K(+)), Pro shifted nonparallelly the concentration-response curves of NA and high K(+) to right, in which the maximal response was depressed in the presence of Pro (30 and 100 micromol/L), and the values of pD'(2) were 3.70-/+0.25 and 3.97-/+0.15 for NA and high K(+), respectively. In the Fura-2/AM loaded VSMCs, Pro (50 and 100 micromol/L) could not produce any significant change on the resting [Ca(2+)](i), but significantly decreased the [Ca(2+)](i) elevated by NA and high K(+). Pro (30 and 100 micromol/L) had no significant effect on the activity of the cytosolic and membrane PKC in the aortic strips inpretreated by NA. However, in the aortic strips pretreated by NA, the activity of membrane PKC was significantly increased and the activity of cytosolic PKC tended to be decreased by Pro, while the activity of total PKC did not change. These results suggest that Pro seems to promote the translocation of PKC from the cytosol to the membrane in the presence of NA, its vasodilator effect may be the comprehensive result of its decreasing effect on the [Ca(2+)](i) and the increasing effect on cAMP and cGMP, as well as its influence on the PKC.

The relationship between protein kinase C signal pathway and the immunologic evaluation in patients with obstructive jaundice.

OBJECTS: To explore the roles of the protein kinase C (PKC) signal pathway in immunologic evaluation of patients with obstructive jaundice. METHODS: PKC from cytosolic and membrane fractions of peripheral blood lymphocytes (PBL) was isolated and purified, then its activities were determined by radioactive isotope gamma-32P-ATP-catalyzing assay. RESULTS: Total PKC activities in PBL in patients with obstructive jaundice were significantly (r = 0.64, P < 0.01) increased as compared to those in normal subjects. Moreover, the membrane PKC activities and their percentages of the total PKC activities were higher in the patients with obstructive jaundice than in normal subjects (r = 0.63, P < 0.05). The total PKC activities in PBL were significantly correlated with the levels of soluble IL-2 receptor (sIL-2R) (r = 0.67, P < 0.01) and the degree of jaundice (T-BIL) (r = 0.58, P < 0.01) in serum. CONCLUSIONS: The activities of the PKC signal pathway is related to the degree of T-BIL. PKC may be implicated in the activation of T-lymphocytes in patients with obstructive jaundice. It is suggested that the role of the PKC signal pathway may be important in the immune regulation and the assessment of pathosis in patients with obstructive jaundice.

[Monoclonal antibodies in the diagnosis and therapy of lung cancer].

The discovery of monoclonal antibodies (MoAbs) by Köhler and Milstein in 1975 has marked a fundamental step accounting for the present development of immuno-oncology. The use of MoAbs in the study of lung carcinoma has allowed to enlarge our knowledge on the biology and natural history of this tumor, on the diagnostic procedures both in vitro and in vivo, on the monitoring of patients as well as on the therapeutic potentialities. A number of MoAbs have been produced, directed either to the small cell lung carcinoma or to the other histotypes which are altogether defined as non-small cell lung carcinoma. Although these reagents are not specific sensu strictiori, their use has been shown to be of partial utility for tumor imaging by immunoscintigraphy and for immunotherapeutic procedures based on the administration of MoAbs conjugated with radioactive isotopes or immunotoxins. Several attempts are presently under way aiming at the production of MoAbs endowed with a more defined specificity and larger potentialities. In this context, a special mention deserve the so-called third-generation MoAbs, anti-idiotype MoAbs, the cocktails of MoAbs, and chimeric antibodies also termed humanized MoAbs.

[Misdiagnosis and mistherapy of ectopic thyroid gland].

OBJECTIVE: To explore the clinic characteristic, diagnosis and treatment of ectopic thyroid gland (ETG) and avoid the misdiagnosis and mistherapy of ETG. METHOD: The clinic materials of 4 cases with ETG were analysed. Four patients were misdiagnosed thyroglossal duct cyst. The clinical features, diagnosis and management of ETG were discussed. RESULT: All of patients had a normal thyroid gland in the neck and underwent a surgical resection of neck mass. Postoperative pathologic examination revealed the neck masses originated from accessory thyroid gland, of them, 1 gland enlargement, 1 adenoma and 2 adenocarcinoma. Fortunately, by the postoperative fellow up, no significant complication was found in the patients. CONCLUSION: The well understanding of ETG and the detailed examination before surgical treatment of neck mass, including ultrasonography, CT or MRI, radioactive isotope scanning and fine needle aspiration biopsy, are keys to adequate diagnosis and management of ETG.
