[Rapid preliminary diagnosis of anaerobic bacterial infections. I. Natural fluorescence of pus, purulent fluids and dressing under UV radiation].

For evaluation of usefulness of natural fluorescence of clinical materials in UV radiation as rapid diagnostic method of infections with anaerobes, 405 samples of pus, bloody-purulent fluids, blood, wound secretions, dressings and other materials were investigated. Occurrence of red-brick UV fluorescence of clinical materials was compared with results of culture aimed at isolation of non-sporeforming anaerobes from "B. melaninogenicus group (P. melaninogenica, P. intermedia and P. saccharolytics). Significant correlation red-brick fluorescence of clinical materials resulting from UV irradiation with presence in these materials of anaerobes such as P. melaninogenica, P. intermedia and P. asaccharolytics was detected. Investigation of clinical materials with application of fluorescence in UV radiation lasts only 1-2 minutes and together with preparation and microscopical inspection which is Gram-stained--only 15-20 min. Positive results of this test may constitute a basis for rapid, preliminary identification of the etiologic factor and for direction of chemotherapy in the early period of infection.

[Rapid preliminary diagnosis of anaerobic bacterial infections. II. Investigation of the presence of volatile and non-volatile fatty acids (C1-C6) in pus, purulent fluids and dressing by application of gas chromatography].

Studies were performed in the years 1986-1990 and dealing with 405 clinical materials originating from infections within the abdominal cavity, maxilla, reproductive organ, post-operative wounds as well as dressings, extremities, blood, bile and other types of material and usefulness of analysis by gas chromatography of presence of lower fatty acids in materials for rapid and preliminary diagnostics with strictly anaerobic bacteria was confirmed. Presence in chromatograms of isobutyric, butyric isovaleric, valeric, isocaproic and caproic acid, and partly of propionic acid, is a good indication of infection with strictly anaerobic bacteria. Moreover, presence of succinic acid may suggest presence of anaerobic infection, similarly as presence in chromatograms of three or more various lower fatty acids. Rapid, approximative diagnosis of anaerobic bacteria by application of gas chromatography is worth of broader application. Investigation of clinical material by this method lasts only 1-2 hours and obtained results my present a basis for preliminary diagnosis of the etiological factor and for direction of chemotherapy, which is very important in most early period of treatment of infections.

Beef and dressed chickens as sources of enterotoxigenic Staphylococcus aureus in Nairobi.

Staphylococcus aureus (S. aureus) isolates from beef carcasses, minced beef, and dressed chicken were assayed for production of enterotoxin A, B, C and D using reverse passive agglutination technique. The highest isolation rate was from chickens followed by minced beef. Chickens yielded the highest percentage of enterotoxigenic strains. Staphylococcal enterotoxin C (SEC) was the most frequently produced type from all the three sources while enterotoxin A ranked second and enterotoxin B third. These data show that chickens and minced beef are potential sources of food poisoning staphylococci in Kenya, and that increased handling of the products increases contamination suggesting that man is the major source.

The properties of hydrogel dressings and their impact on wound healing.

In recent decades, the management of difficult wounds has been enhanced by the introduction of amorphous hydrogels. This paper examines reports in the literature on the outcomes of studies into the efficacy of this group of dressings, discusses the types of wounds that are most suitable for hydrogel application and poses questions that merit further investigation.

Surgical wound care: current views on minimising dressing-related pain.

Managing surgical wounds healing by primary intention is likely to become less problematic with the development of more effective advanced wound dressings; increasingly effective methods of medicated pain management; education; and advances in minimal access surgical (MAS) techniques. Future research is likely to focus on management of patients with surgical wounds healing by secondary intention. If so, management of surgical wound pain must become an integral part of assessment. Theatre practitioners, clinical nurses and surgical practitioners, working as part of the multidisciplinary team, are ideally placed to undertake this work and to include multimodal, holistic approaches to caring for surgical wounds and controlling surgical wound pain (Gould, 1999). Patient support and education is likely to become an increasing priority, particularly in view of the significant move from inpatient care to short-stay and day-case surgery.

Use of mesh fibrous dressing covered with fibrin glue (TachoComb) in hemostasis after vascular anastomoses in the groin.

We present in this paper application of haemostatic device TachoComb onto bleeding after vascular anastomosis of dacron vascular prosthesis (branch of aortobifemoral or bypass aortofemoral) with common femoral artery in the groin. Hemorrhagic complications have influence onto clinical status of operated patients. Haemostatic TachoComb dressing was applied at 30 cases and results were compared to control group consist of 25 cases, in which gas compresses were applied. Mean loss blood in group I with the usage of TachoComb was statistical characteristic (p < 0.003) smallest than in group II. Also mean time of hemostasis was shortest than in group II (p < 0.01). We proved that use of TachoComb limits bleeding from suture line connecting artery with vascular prosthesis.

Silver dressings in clinical practice.

Silver is among several topical antiseptics that are once again gaining popularity due, in part, to the rise of antibiotic-resistant genotypes. Because of differences in wounds and the variety of products--including various silver vehicles--available to treat them, all of the dressing characteristics appropriate for the specific wound being managed must be considered. This article addresses why silver is (and is not) a good choice for particular wounds and offers suggestions about what silver products best serve individual situations.

Repellency of permethrin-treated battle-dress uniforms during Operation Tandem Thrust 2001.

Successful feeding by mosquitoes through permethrin-treated battle-dress uniforms (BDUs) was observed in the Shoalwater Bay Training Area (SWBTA), Queensland, Australia, during Operation Tandem Thrust 1997 (TT97). Therefore, during Operation Tandem Thrust 2001, the repellency of permethrin-treated BDUs was evaluated. Washing permethrin-treated BDUs 1 time reduced the permethrin concentration by 60%, but permethrin concentration was not reduced further after subsequent washings (up to 20 times). The probing time of Ochlerotatus vigilax landing on permethrin-treated BDUs was reduced to the greatest extent on the treated, unwashed uniform (7.5-fold reduction). Treated, washed uniforms also reduced probing time (2.5 times) compared to the untreated control uniform containing no permethrin. Reduced probing time should reduce feeding success and protect operational forces from bothersome mosquitoes and the associated risk of mosquito-borne disease in SWBTA. Observations of successful mosquito feeding made during TT97 could be attributed to the use of old and well-worn BDUs and seasonal differences in mosquito population density.

[DRESS syndrome to allopurinol: a case in Dakar].

We report a case of DRESS syndrome also called drug hypersensitivity reaction occurring a 47 years old Senegalese man who has been taking allopurinol for 3 months. That drug was prescribed for peripheric arthralgias associated to a hyperuricemia. He presented a generalised pruritus, cutaneous lesions, fever and facial oedema. On the biological examens, hyperleucocytosis with hypereosinophilia and hyperlymphocytosis associated to the presence of segmented basophiles. In addition, a hepatic cytolysis and cholestasis were documented. Liver ultrasound was normal. The hemocults were negative. These following serologies have been performed and were negative: hepatitis B and C, Epstein Barr-virus, cytomegalovirus, syphilis, toxoplasma and parvovirus B19. The anti-nuclear and anti-DNA antibodies were negative. A favourable clinical evolution was remarked after allopurinol treatment withdrawal. A desquamation occurred after 6 days and hemogram turned out to the normal as well as the hepatic tests after 2 weeks. The virologic examens performed 2 months later were unremarkable. This case point out the importance of the early diagnosis and quick withdrawal of the drug in order to prevent serious forms leading to the 10% of death.

Assessment of deposition of MEMC (seed dressing fungicide) in exposed rice grain through study of seedling growth parameters.

Rice seed grains exposed to 240 ppm of seed dressing fungicide, MEMC showed increase in deposition of mercury with increase in period of observation (0h to 48h) in 765+/-5 mg of intact endosperm. The gradual increase in mercury deposition in intact endosperm caused simultaneous decrease in mercury content of the seed coat. Exposure periods of 5 to 20 min with 240 ppm of MEMC showed 5.5 to 8.72 microg of mercury deposition in 765+/-5 mg of intact endosperm. Increase in period of exposure (3 to 20 min) to 240 ppm of MEMC and also increase in chronic treatment to lower concentrations (1.5 ppm to 24 ppm) to seed grains have shown similar results in seedling growth parameters. Lower period of exposure or exposure to lower concentrations, both showed a significant inhibition of root growth, however the shoot growth showed stimulation. But higher exposure period or exposure to higher concentrations brought inhibition of both root and shoot growth. The exposure period showing 50% of root growth inhibition of 72h old seedling was calculated to be 10 min So also the chronic treatment showing the 50% of root growth inhibition was calculated to be 9 ppm. Ten min exposure of rice grain to 240 ppm of MEMC showed deposition of 7.11 microg of mercury in 765+/-5 mg of intact endosperm at 48h of observation (optimal phase of germinating process). Regression analysis indicated that by the time of 72h the intact endosperm and developing seedling might have acquired with a minimum amount of 10.23 microg of mercury from the seed coat. Finally it can be said that 10.23 microg of MEMC might be bringing 50% root growth inhibition in 72h old seedlings and 16.71 microg of mercury causing 50% inhibition of both root and shoot growth of 120h old seedlings.

Allopurinol-induced DRESS syndrome.

Hyperuricemia is present in approximately 5% of the population, the vast majority of whom are asymptomatic and at no clinical risk. Complications, including renal calculi, uric acid nephropathy and gout, occur in a small proportion of patients. Allopurinol, an analog of hypoxanthine, has been widely used in clinical practice for over 30 years for the treatment of hyperuricemia and gout. Two percent of patients taking this medication develop a mild exanthema. A syndrome characterized by exfoliative dermatitis, hepatitis, interstitial nephritis and eosinophilia has been described previously. Termed allopurinol hypersensitivity syndrome, its etiology is related to the accumulation of one of the allopurinol metabolites, oxypurinol; clearance of oxypurinol is decreased in the setting of renal insufficiency and the use of thiazide diuretics. The term DRESS syndrome (Drug Rash with Eosinophilia and Systemic Symptoms) was recently introduced to describe a disorder associated with various drugs or viral infections and characterized by similar features. The pathophysiology of allopurinol-induced hypersensitivity, clinical presentation and treatment are reviewed.

[The use of fibrous wound dressing sheets made of carboxymethylcellulose natrium in the postoperative management of tracheostomy].

Postoperative management of tracheostomy is very important to reduce several complications such as abnormal granulation and subcutaneous abscess. One of the causes is the use of gauzes and a disinfectant because they induce the secondary wound injury and prevention of wound healing. We used a fibrous wound dressing, carboxymethylcellulose natrium sheet (Aquacel) on the postoperative management of tracheostomy for 5 patients. The sheets were very useful to absorb exudates including blood and to make suitable moist and clean surroundings in all patients.

Combining bioengineered human dermal replacement and multilayered compression dressings to manage ulcers in a person with diabetes mellitus: a case study.

Multiple modalities exist for the care of lower extremity ulcers associated with venous insufficiency and complications of diabetes mellitus, (eg, neuropathy). Although reports about the use of topical adjunctive treatment modalities in the treatment of foot ulcers in persons with diabetes mellitus exist, little is known about the safety of topical treatment when used in combination with compression therapy to manage venous insufficiency. A patient with diabetes mellitus, neuropathy, a 3.3 cm x 3.0 cm x 1 mm heel ulcer and a 8.1 cm x 4.9 cm x 3 mm lower leg ulcer secondary to venous insufficiency presented at the authors' clinic. After 8 weeks of therapy using a combination of bioengineered human dermal replacement and multilayered compression dressings, lower leg edema was reduced, the heel ulcer healed, and the leg ulcer continued to improve. No complications were observed. The results observed suggest that studies examining the effects of treatment regimens that address the multifactorial etiology of some lower extremity ulcers are warranted.

[Local wound care: evidence-based treatments and dressings].

Patients with open wounds require specific local-wound care. There is huge variety in methods of local-wound care. This is due not only to the many different types of wounds but also to the widely varying preferences of doctors and nurses, and to the lack of strong evidence and relevant guidelines regarding the most appropriate form of local-wound care. In 19 systematic literature reviews from the Cochrane Collaboration on surgical and traumatic wounds and ulcers (venous, arterial and diabetic) information can be found on local-wound treatment. Eleven of these reviews were limited to strong evidence for the use of: tissue adhesives for surgical and traumatic wounds; foam dressings in surgical wounds healing by secondary intention; (intermittent) compression, bilayer artificial skin and lidocaine-prilocaine cream for venous ulcers; hydrogel and in-shoe orthotics for diabetic-foot ulcers; negative pressure for chronic wounds, and cleansing of acute wounds with clean tap water.

[DRESS syndrome to sulfasalzine].

We report a case of drug hypersensibility syndrome (DRESS syndrome) developed one month after initiation of a treatment by sulfasalazine. Due to the severity of the cytolytic hepatic damages as observed in this case, we emphasize the importance of suggesting this diagnosis in any patient developing a cutaneous rash and an alteration of the general health status after initiation of a new treatment. The hematologic alterations which represent a key feature for the diagnosis may rash. develop only a few days after the cutaneous rash.
