[Telethermography in the examination and treatment of patients with acute respiratory viral infections].

Results are reported of thermographic examination of 51 patients with acute respiratory viral infections. It was established that the patients showed an increase of the radiation temperature of the skin of the nose, floor of the auricle, in the projection of the sternocleidomastoid muscles and increased submaxillary lymphatic nodes independent of the etiological factor and disease severity grade. Hyperthermia of the nose on thermograms was more frequently observed in patients with rhinitis. Informative were differences in the temperatures between the external auditory meatus and the lobule of the ear which decrease in the acute period of the disease in accordance with its severity. In most convalescents normalization of the thermal picture of the face coincides by terms with clinical convalescence while hyperthermia of the neck is maintained for a longer time.

[Cases of hemorrhagic fever with renal syndrome in Amur Province associated with the rat serotype of the virus].

To carry out serodiagnosis and to determine the serotype of the virus causing hemorrhagic fever with the renal syndrome (HFRS), paired sera obtained from 28 HFRS patients, 42 persons with a history of HFRS (1-17 years after convalescence), and 268 serum samples from healthy persons residing at the areas with the natural foci of this infection have been studied by indirect immunofluorescence techniques at the territory of the Amur Province. This study has demonstrated for the first time that, alongside the diseases caused by HFRS virus of serotype Apodemus, HFRS viruses of serotype Rattus and an unidentified serotype serve as the source of infection for the population of the Amur region. The leading role of serotype Rattus in HFRS has been confirmed by the detection of antibodies mainly to this serotype in serum samples taken from convalescents after HFRS and from the healthy population of the areas with the foci of HFRS virus infection.

[The sequelae of tuberculosis].

During the 20th century, tuberculosis has been the most prevalent and most harmful disease in Japan. Enormous medical researches have ever been performed to conquer the disease. Nevertheless tuberculosis has left various somatic and psychological residues on vast convalescents. On the other hand, researches to conquer tuberculosis have made considerable contribution to other fields of medicine. 1. Somatic and psychological residues on convalescents from tuberculosis. Chest x-ray findings, cardio-pulmonary disturbance, secondary infection, serum-hepatitis due to mass transfusion during the chest surgery, streptomycin-deafness and psychological disorder. 2. Sequelae of phthisiology. a. In the field of basic medicine. Respiratory physiology, immunology and genetic pharmacology. b. In the field of epidemiology. Methodology to control the disease. c. In the field of clinical medicine. Chest x-ray diagnostics, bronchoscopy, thoracoscopy, randomized controlled trial, regimens of chemotherapy, open chest surgery, anesthesiology, treatment of respiratory failure, informed consent, terminal care and cooperative study system. d. In the field of rehabilitation. Medical, vocational and social rehabilitation of the handicapped. e. In the field of public health. Comprehensive control system of the chronic disease. Smallpox has been eradicated, but the elimination of tuberculosis is still far away. Studies as excellent as past ones should intensively be carried out.

Plasma immunoreactive leukotriene C4 levels in patients with Kawasaki disease.

The incidence of wheezing in Kawasaki disease (KD) has been investigated retrospectively. We measured plasma immunoreactive-leukotriene C4 (i-LTC4) levels of patients with KD. Wheezing was observed in 32 (12.5%) of the 256 patients with KD. Patients who had a prior history of bronchial asthma wheezed more frequently than other patients. During the acute stage of KD, plasma i-LTC4 levels both of wheezing and nonwheezing were significantly higher than those of healthy children. During the convalescent stage, plasma i-LTC4 levels of wheezing cases were higher than those of nonwheezing cases. We speculate that LTC4 contributes to the appearance of inflammatory symptoms during the acute stage of KD. During the convalescent stage when patients were taking aspirin, the presence of wheezing was associated with increased plasma levels of i-LTC4. More attention should be paid to the appearance of wheezing during the course of KD, especially in those cases in which the patient has a prior history of bronchial asthma.

[Blood and serum iron values in children at child day care centers. 1].

The parameters of iron in blood and in serum were studied with 90 children aged from 13 to 36 months from eight crèches of a large city. The investigations were performed in state of health, in state of acute respiratory disease and until the fourth week of convalescence. In the acute state iron of serum, transferrine and saturation index of transferrine are situated outside the limits of the normal state. The initial values of the state of health were not reached yet in the fourth week of the convalescence.

[The dynamics of the peptide hydra morphogen in meningitis].

The content of peptide hydra morphogen (PHM) was measured for the first time in blood plasma and CSF by RIA in patients with purulent meningitides. 36 patients were examined. Blood and CSF were withdrawn in the acute period and during early convalescence. In the reference group (patients with acute respiratory diseases of virus etiology, quinsy, pneumonias), the PHM concentration in the blood was 128 +/- 62 fm/ml, that in CSF 66 +/- 14 fm/ml. In purulent meningitis of medium gravity, the content of PHM in blood plasma and CSF did not significantly differ from the control. Grave purulent meningitis was marked by a high content of PHM in blood plasma and CSF, in particular, in the acute period and during early convalescence. It was 1250.30 +/- 191 and 544.03 +/- 159 fm/ml in blood plasma and 1504.38 +/- 475 and 1811.39 +/- 375 fm/ml in CSF, respectively. In grave complicated purulent meningitis, which was mainly observed in persons over 50 years, the content of PHM in blood plasma exceeded but insignificantly the content in the control during the acute period (327.49 +/- 76 fm/ml), followed by further increase (1131.41 +/- 209 fm/ml) by days 8-12 of the disease; within the same time, the content of PHM in CSF did not significantly differ from the control (319.52 +/- 152 and 625.69 +/- 275 fm/ml, respectively). In view of the fact that hypothalamic PHM is of crucial importance in the processes of nervous tissue regeneration, the content of the latter one in CSF in patients with grave purulent meningitis may serve as a criterion for disease course tendencies.

[Echocardiography in assessing myocardial function in influenza].

A study was made of the echocardiographic signs of heart lesion in 23 influenza patients in the acute disease period and in 19 of them, during early convalescence. Echocardiography permits detecting alterations in cases where clinically or with the aid of the ECG, the diagnosis of myocardial lesions could not be established. Moderate enlargement of the right ventricular cavity in the main, appearance of local hyperkinesia of the myocardium, changes in the central hemodynamics with a tendency towards the hyperkinetic type turned out to be the most frequently occurring alterations. During early convalescence, these alterations do not undergo complete normalization.

[Serological proof of the existence of a variety of non-A, non-B hepatitis with a fecal-oral transmission mechanism].

An outbreak of hepatitis in one of Siberian regions mainly among adults of 19-35 years of age was studied. The epidemiological studies showed the infection to spread by the fecal-oral mechanism of transmission. In 90% of the patients the disease was mild, in 10% of moderate severity; no severe forms or fatal outcomes were observed. This refers also to pregnant women irrespective of the term of pregnancy. Among 78 subjects examined, antibody to hepatitis A virus of the IgM class (anti-HAV-IgM) were detected in 14%, HBsAg in 6%. No rise in titer of total anti-HAV in the convalescent period (within 3-6 months) was observed. Immune electron microscopy studies of fecal extracts from patients revealed immune complexes of virus particles 32-35 nm in size which were formed only with sera from patients in the acute stage and convalescents from this outbreak (to a lower degree). Negative results were obtained with sera containing anti-HAV and antibody to hepatitis non-A-non-B virus (HnAnB) with fecal-oral transmission mode from India, Central Asia, and Afghanistan. These data attest to the distinct nature of this causative agent among HnAnB viruses with the fecal-oral transmission mechanism.

[Clinical study on hemodynamics, blood gases and hemorheologic monitoring of patients with cor pulmonale].

14 patients of cor pulmonale in convalescent stage were studied with Swan-ganze catheter technique. The pulmonary arterial pressure, cardiac output, arterial blood gases, transcutaneous gases tension and hemorrheologic measurement were monitored simultaneously at definite intervals within 24 hours. The results showed that the mean value of the pulmonary arterial pressure (MPAP), arterial carbon dioxide tension (PaCO2), transcutaneous carbon dioxide tension (tcPCO2), hematocrit(H), whole blood viscosity (eta b) and plasma viscosity (eta p) were higher at night than those at daytime. While the value of cardiac output (CO), arterial oxygen tension (PaO2) and transcutaneous oxygen tension (tcPO2) showed a converse result, most of them showed more predominant at daytime and tended to be lowered at night. The MPAP in some of convalescent patients might decrease to normal, but in the pulmonary hypertension group [MPAP greater than or equal to 2.66 Kpa (20 mmHg)] the day and night measurements of MPAP, CO, PaO2, PaCO2, tcPO2, tcPCO2 were fluctuate more obviously than in normal group (P less than 0.01). The day and night hemorrheologic measurements such as H, eta b, eta p etc. showed that the blood viscosity increased more significantly at night than at daytime. The cardiac output (CO, CI) seemed to be a little bit lower in some patients especially in those with high pulmonary arterial pressure and at night. These results suggest that the variation of the day and night measurements of hemodynamics, blood gas, hemorrheologic changes have some rule.(ABSTRACT TRUNCATED AT 250 WORDS)

[The characteristics of the clinical picture and outcome of viral hepatitis B].

Results are reported of a study of the clinical course and outcomes of viral hepatitis B. The etiology was confirmed in all patients by detection of the superficial antigen of hepatitis B virus in the blood serum. A clinico-physiological and biochemical follow-up revealed that complete clinical recovery is slow, though the condition of most convalescents does not suffer practically. Viral hepatitis B convalescents revealed in the long-term follow-up residual clinico-biochemical changes.

[Antibody formation dynamics in a patient with Central Asian tick-borne borreliosis].

Antibody titres were examined in patients suffering from Central Asian tick-borne borreliosis by means of indirect immunofluorescence. In acute period of the disease and in early convalescence period a rapid increase of antibody titres was observed; they were still increasing even after clinical recovery. The maximum titres (1:320) were marked in 1.5 months after the onset of the disease. Decrease of antibody titres began in 2-3 months after the peak of the disease. Antibodies in relatively low titres are detectable at least up to the 11th month of convalescence.

[Bile protein fractions in patients with viral hepatitis].

A study of bile protein fractions by means of polyacrylamide gel electrophoresis in patients with viral hepatitis revealed changes in the protein fraction range in those with a prolonged course of nonicteric convalescence manifested in an increase of the amylolytic and reduction of proteolytic fractions. It is suggested that enzymatic disorders of the pancreatic function may lead to development "enzymatic" cholecystitis and protracted convalescence of patients with viral hepatitis.

Characteristic T cell dysfunction in patients with chronic active Epstein-Barr virus infection (chronic infectious mononucleosis).

We evaluated immune functions in 16 patients with chronic active Epstein-Barr virus (EBV) infection (chronic infectious mononucleosis). Chronic infectious mononucleosis is an illness characterized primarily by chronic and occasionally disabling fatigue and other constitutional complaints, only sometimes beginning with an episode of acute infectious mononucleosis, and associated with an abnormal pattern of serum antibodies to EBV. In these patients, the frequency of circulating EBV-infected B cells that manifested spontaneous outgrowth in vitro was comparable to that found in EBV-seropositive normals, and the levels of EBV-specific suppressor activity were also normal. Upon stimulation with polyclonal activators, unseparated cells from these patients produced a relatively normal number of immunoglobulin-secreting cells. However, when purified T cells from these patients were mixed with normal mononuclear cells in co-culture, immunoglobulin production was strikingly suppressed. The degree of this T cell suppression correlated directly with the abnormally elevated titer of antibody to the early antigens of EBV. Interestingly, during normal convalescence from acute EBV-induced infectious mononucleosis a period is also seen during which T cells suppress the response of allogeneic but not autologous cells. Thus, from an immunologic viewpoint, patients with chronic active EBV infection appear "frozen" in a state typically found only briefly during the convalescence from acute EBV infection.

[The importance of secondary immune deficiencies in the pathogenesis of dysentery].

Some immunity indices were studied in patients with acute dysentery. The number of total and active T-lymphocytes, T mu- and T gamma-cells, T-rabbit, T-staphylococcal, B-total and B-lymphocytes forming rosettes with mouse erythrocytes were determined for assessment of the immune status. Changes in the above indices with relation to the severity of the disease were noted. Suppression of the T- and B-systems of immunity were noticeable in patients with a severe and lingering course of disease. Delayed time of complete convalescence was observed in patients with a marked imbalance of indices of the T- and B-systems of immunity during convalescence.

[Lymphocyte blast transformation to HBsAg and HBcAg in patients with various clinical forms of hepatitis B].

Sensitization of the peripheral blood lymphocytes to HBsAg and HBcAg in 51 patients with acute hepatitis B (HB) in the time course of infection, in 13 with chronic active hepatitis B (CAH) and 8 HBsAg carriers was studied by lymphocyte blastogenesis assay. In patients with mild or moderately severe forms of acute HB at the peak of the disease lymphocyte blastogenic response to HBcAg was observed; sensitization to HBsAg was lacking and could be detected only in the stage of convalescence when the specific response to HBcAg was already undetectable in most patients. No lymphocyte sensitization to HBsAg either at the peak of the disease or in convalescence was observed in patients with a lingering form of acute HB as well as in those with CAH and HBsAg carriers. At the peak of severe HB blastogenic response was demonstrated to both antigens under study but was more marked to HBcAg. Most patients with CAH showed lymphocyte blastogenic response to HBcAg. It is concluded that specific lymphocyte response to HBV antigens varies in relation to the severity and course of the infection.

[The relation of the acetylation phenotype to the predisposition to typhoid fever and its clinical course].

Altogether 33 typhoid fever convalescents and 36 contacts of a control group were examined during an epidemic outbreak. In one year 17 former patients and 65 controls (persons who had been at the center of the outbreak but contracted no disease) were examined. Norsulfazole was used as a model agent to study an acetylation phenotype. Unimodal distribution of a degree of norsulfazole distribution was noted in the convalescents, bimodal distribution in the controls and patients examined in one year. Persons with a rapid type of acetylation were more predisposed to typhoid fever, and intoxication in them was more noticeable.

[The level of fecal excretion of free amino acids and lipids in children with acute intestinal infections].

The level of excretion of free amino acids and lipids in the stool was determined in children of young age with a severe course of acute intestinal infections (dysentery, salmonellosis, acute gastroenteritis and gastroenterocolitis of unknown etiology). The study has shown that excretion of free amino acids in the stool at the height of the disease considerably exceeds that in healthy children. Normalization of these disorders during convalescence is much behind clinical convalescence.

[Capacity of mononuclear cells to produce a factor that promotes E-rosette formation in different clinical forms of erysipelas].

The author has examined the capacity of mononuclear cells in peripheral blood samples obtained from erysipelas patients for the in vitro secretion of E-rosette formation promoting factor (E-RPF) in response to polyclonal stimulation with phytohemagglutinin (PHA) P or antigenic stimulation with hemolytic streptococcal allergen. At the acute stage of the disease, mononuclear cells spontaneously secreted E-RPF in a half of the examined patients; at the same time, a statistically significant decrease in the PHA-induced secretion of E-RPF was observed, especially in patients with the primary bullous form and relapses of erysipelas. The optimum lymphokine production in response to PHA in patients with primary erysipelas was observed simultaneously with a low level of spontaneous lymphokine production, while in relapses it was significantly suppressed at all levels of the spontaneous secretion of E-RPF. The antigen-stimulated secretion of E-RPF was observed more frequently in patients with primary bullous erysipelas, while in bullous relapses it was completely absent. At the early stage of convalescence the intensity of the spontaneous secretion of E-RPF decreased, while the PHA-induced secretion of E-RPF enhanced. In relapses the secretion of E-RPF in response to stimulation with the specific allergen remained at a low level, while in convalescents having had primary erysipelas the level of this secretion was high. These data indicate that mononuclear cells in the peripheral blood essentially differ in their capacity to secrete E-RPF in response to polyclonal and antigenic stimulation in various clinical forms of erysipelas.
