Integrated analysis of amide proton transfer weighted MRI and proteomics uncovers altered protein dynamics in glioblastoma.

PURPOSE: Elevated amide proton transfer-weighted (APTw) MRI signals in glioblastoma (GBM) are often linked to increased intracellular mobile proteins, but the associated molecular patterns in human tissue remain unclear. We examined the relationship between regional APTw features and cellular protein composition and profiled proteomic differences between tumor and peritumoral tissue. METHODS: In this single-center prospective study, preoperative MRI data were integrated with intraoperative neuronavigation for 12 image-guided tissue samples (8 tumor and 4 peritumoral). Total, cytoplasmic, and nuclear proteins were quantified using bicinchoninic acid (BCA) assay. Data-independent acquisition (DIA) proteomics identified exploratory differentially expressed proteins (DEPs), followed by functional enrichment and protein-protein interaction (PPI) network analyses. Transcript-level expression patterns and survival associations were queried in The Cancer Genome Atlas (TCGA) and Genotype-Tissue Expression (GTEx) datasets to provide indirect external clinical context. RESULTS: Tumor regions showed higher APTw signals than peritumoral regions (p < 0.001) and increased cytoplasmic protein concentration (p < 0.05), without a corresponding increase in total or nuclear protein levels. DIA identified 654 DEPs. Further analysis highlighted 36 higher-significance DEPs, and prioritized 12 hub proteins in the PPI network. In public transcriptomic datasets, ERBB2, RUNX1, and SHC1 showed higher expression in GBM and were associated with poorer overall survival. CONCLUSION: These findings suggest that elevated APTw signal in GBM may be associated with increased cytoplasmic protein content and distinct proteomic alterations. This imaging-proteomic framework provides exploratory regional context for future mechanistic and follow-up studies, but larger, spatially matched and independently validated cohorts are required to confirm the molecular contributors to APTw contrast.

A global livestock production and health atlas (GLiPHA) for interactive presentation, integration and analysis of livestock data.

An interactive electronic atlas has been developed with the purpose of providing a scaleable overview of spatial and temporal variation in animal production and health-related information for decision and policy makers in national and international institutions. The information contained in the atlas is currently managed and presented using the Key Indicators Mapping System (KIMS), and will also be integrated using the Key Indicators Database System (KIDS). Both systems were developed by the World Agricultural Information Centre of the FAO (FAO-WAICENT), the former as a stand-alone application and the latter for access via the Internet. Components of the atlas include vector maps, livestock disease and production databases, rules for country-level disease risk classification and 'disease cards' containing basic background information on diseases included in the atlas. The disease data is currently based primarily on Office International des Epizooties (OIE) disease reports, and the livestock production data on the FAO-WAICENT database. The atlas is highly interactive and allows visual presentation of information using maps, tables and charts. It also contains links to relevant resource information on the Internet. Diseases covered in the animal health layer include most OIE List A diseases and a subset of OIE List B diseases. Extensive analyses have been conducted to develop a set of qualitative and semi-quantitative criteria that allow improved disease status classifications based on 5-years cumulative OIE disease reports, and official disease control declarations. Classification rules were determined depending on the epidemiological features of each disease and considering spatial heterogeneity of disease presence in local regions.

A study of the attitudes of a rural Indian community toward people with physical disabilities.

OBJECTIVE: To examine the attitudes of a rural community in southern India toward its disabled members. DESIGN: Prospective cross-sectional study. METHOD: One hundred and twenty randomly selected villagers around the town of Vellore in southern India were invited to take part in the study. The attitudes towards individuals with physical disabilities were examined using a modified version of the Scale of Attitudes Toward Disabled Persons (SADP). The questionnaire was administered in its original English format with simultaneous translation into Tamil. RESULTS: Data were complete in 111 cases, 82% of whom showed a positive attitude toward people with disability. Gender and employment status did not appear to have an influence on whether the respondent regarded disabled people in a positive or negative way but most older individuals expressed prejudice and/or challenged the rights of disabled people to equal opportunities in education, employment and social integration. Further data analysis demonstrated that more subjects with than without a disabled family member supported the rights of disabled individuals to education, employment and social integration. By contrast, the items of SADP which described the personal attributes of disabled people did not discriminate between the two groups. CONCLUSION: The attitudes of the rural community studied toward people with disabilities were unrelated to the sex or educational status of the respondents but was influenced by their age. This suggests that older subjects in rural communities in developing countries should be the main target for educational programmes which promote positive images of disability.

The social assimilation of immigrants.

"This paper provides an empirical analysis of the determinants of migrants' [social] integration, using data for Germany. Ordered probit models are estimated, where the dependent variable is an ordered response on the feeling of national identity. The results show that personal characteristics, the nationality and the family context affect the migrant's integration, while labor market variables appear to be quite unimportant."

Methodologic issues in collecting data from debilitated patients with cancer near the end of life.

PURPOSE/OBJECTIVES: To report the experience of a group of researchers who have had a year of experience in a clinical trial with homecare hospice patients. SAMPLE: 150 hospice patients with cancer and their primary caregivers who were accrued to a National Cancer Institute-funded clinical trial focusing on quality of life. METHODS: The investigative team kept careful records of the numbers of patient/caregiver dyads accrued to the study and the reasons for nonaccrual as well as reasons for attrition. Data were analyzed using descriptive statistics. FINDINGS: During a nine-month period, the hospice admitted 2,517 patients; 75% had cancer and 95% had caregivers, making them eligible for the study. However, after further screening, only 19% were eligible for contact and only 5% finally were accrued to the study. For the 125 patient/caregiver dyads actually accrued to the study, baseline data were obtained on only 50% and evaluable follow-up data on only 50%. CONCLUSIONS: Accruing patients to clinical trials and retaining them when they are critically ill and near death are extraordinarily difficult tasks. The inability to recruit and retain subjects for clinical trials has implications for integrity of data, data analysis, success of the project, and the cost of conducting such projects in the future.

Teaching information management.

Managing in the present chaotic hospital financing environment requires integrated clinical/financial data systems and people who know how to use them. Health care management students of the present and future will need to understand how these information systems are structured and used. This article describes a graduate course in integrated clinical/financial information management as it has evolved at Yale University. The course provides students a vicarious experience in information management through class sessions and exercises using a database of real patient and cost center level information. Health care management of the future will depend on interdisciplinary collaboration and partnerships in education between provider organizations and academic programs. Both are modeled in the course.

Integrating AlphaFold2 models and clinical data to improve the assessment of Short Linear Motifs (SLiMs) and their variants' pathogenicity.

Short Linear Motifs (SLiMs) are protein functionally relevant regions that mediate reversible protein-protein interactions. Variants that disrupt SLiMs can lead to numerous Mendelian diseases. Although various bioinformatic tools have been developed to identify SLiMs, most suffer from low specificity. In our previous work, we demonstrated that integrating sequence variant information with structural analysis can enhance the prediction of true functional SLiMs while simultaneously generating tolerance matrices that indicate whether each of the 19 possible single amino acid substitutions (SASs) is tolerated. However, the scarcity of representative crystallographic structures of SLiM-receptor complexes posed a significant limitation. In this study, we demonstrate that these interactions can be modeled using AlphaFold2 (AF2) to generate high-quality structures that serve as input for our MotSASi method. These AF2-derived structures show robust performance, both in reproducing known structures deposited in the Protein Data Bank (PDB) and in reflecting the deleterious effects of known sequence variants. This updated version of MotSASi expands the repertoire of high-confidence predicted SLiMs and provides a comprehensive catalog of variants located within SLiMs, along with their respective deleteriousness assessments. When compared to AlphaMissense, MotSASi demonstrates superior performance in predicting variant deleteriousness. By contributing to the accurate identification and interpretation of variants, this work aligns with ACMG/AMP standards and aims to improve diagnostic rates in clinical genomics.

Quadriceps force and myoelectric activity during flexed knee stance.

The purpose of this study was to determine the quadriceps demand during single limb stance with the knee in five positions of flexion (0 degree -60 degrees). Two variables were used to estimate the quadriceps demands: the integrated electromyogram (EMG) of three vasti and the torque about the knee joint. Ten normal subjects 23-29 years of age were tested. Myoelectric activity was recorded with intramuscular electrodes and knee joint angle with an electrogoniometer. The EMG data were integrated and normalized. A visible vector system was used to determine knee flexion torque. A significant linear correlation was found between values of patellar ligament force and knee angle (R2 = 0.86). The patellar ligament force increased 4.16% of body weight per degree of knee flexion between 0 degree and 60 degrees. Both vector-estimated quadriceps force and normalized EMG showed significant correlations with knee angle (R2 = 0.91 and R2 = 0.88, respectively). Both exhibited a slower rate of rise below 30 degrees flexion and a higher rate of rise above 30 degrees. The authors' findings suggest that the stabilizing effects of plantar flexors (except gastrocnemius) on the knee joint, changing muscle moment arm length, and force ratio between the patellar ligament and quadriceps muscle with respect to knee angle are primary reasons for the quadratic normalized integrated EMG (NIEMG)-force nonlineality during flexed stance. This might indicate the existence of a critical angle of knee flexion contracture beyond which patients cope poorly with standing and functional ambulation.

Kinetic ratio as a parameter for product stability calculations.

A new parameter, the kinetic ratio, is suggested for estimating product potency during storage under ambient warehouse temperatures. Actual warehouse temperature data were integrated using Arrhenius kinetics, and the resulting potency errors, due both to the integration method and to temperature averaging, were evaluated. Kinetic ratios and virtual temperatures then were compared to show seasonal and Arrhenius constant effects. Equations interrelating the kinetic ratio and virtual temperature are given. Finally, sample calculations using typical vitamin A data were analyzed to demonstrate the calculational and interpretation characteristics of the kinetic ratio compared to the virtual temperature.

Combined method to determine the therapeutic position for occlusal rehabilitation.

Differences of opinion concerning the correct reference position for occlusal rehabilitation and the correct methods for diagnosis are partly due to the tendency to overemphasize either the clinical or radiologic approach. However, careful comparison of data from radiography, clinical examination, and occlusal analysis offers the best opportunity for correct diagnosis and treatment. A combined method has been described for the determination of mandibular displacement in the IP and the correct reference position for occlusal rehabilitation. Integration of data from radiology, clinical examination, and occlusal analysis allows location of the condyles and the lower midline, thus providing the best opportunity for a correct diagnosis. The selected therapeutic position is then achieved at the beginning of treatment through a splint constructed on casts mounted on an articulator.

A model for the assessment of medical workstations for health care support.

The development of medical workstations for the support of patient care, the assessment of care, management support, and education is just at its beginning. During the Working Conference on the Health care Professional Workstation held in Washington DC, June 1993, several aspects of such workstations were discussed, but it was also recognized that prototyping or learning by experience could be a rich source to further promote the progress in this field. Eight such prototypes or already operational medical workstations were demonstrated and a preliminary user assessment was done to obtain a first insight in the advantages and the type of criteria of such evaluations. It was concluded that such assessments were of great value to (i) give feedback to the designers of medical workstations, (ii) indicate areas of strength and for further research, and (iii) to offer criteria to potential users of such workstations for making decisions on using such systems. The assessment criteria deal with functionality, architecture, user interfaces, communications and integration, and data and knowledge management.

A DBMS-based medical teleconferencing system.

This article presents the design of a medical teleconferencing system that is integrated with a multimedia patient database and incorporates easy-to-use tools and functions to effectively support collaborative work between physicians in remote locations. The design provides a virtual workspace that allows physicians to collectively view various kinds of patient data. By integrating the teleconferencing function into this workspace, physicians are able to conduct conferences using the same interface and have real-time access to the database during conference sessions. The authors have implemented a prototype based on this design. The prototype uses a high-speed network test bed and a manually created substitute for the integrated patient database.

Cerebellar mediation of the complexity of bimanual compared to unimanual movements.

OBJECTIVE: To study rapidly alternating movements under fMRI in order to identify the brain regions that mediate increased complexity in bimanual vs unimanual movements and to verify the localization of a clinical test of limb ataxia (diadochokinesis). METHODS: Unimanual and bimanual movements, that is, palm(s) pronated then supinated, served as stimulation in a block design fMRI investigation at 1.5 T. Analyses compared bimanual movements and rest for each hand separately and the unimanual conditions combined. A pronation/supination task was chosen as it provides the same objective motor output during unimanual and bimanual formats. The increased coordination demand of the bimanual format (phase/antiphase movements) was expected to result in distinct activation in supplementary motor, primary motor, prefrontal, and cerebellar regions. RESULTS: The bimanual task uniquely elicited responses in specific anterior medial and posterior (vermal) cerebellar regions. CONCLUSIONS: The study corroborated clinical use of diadochokinesis tasks to test for aspects of cerebellar integrity. The data do not support the literature emphasizing basal ganglia mediation of this type of coordinated movement. Cerebellar medial and vermal regions (in connection with central nuclei) are proposed as the locus within the cerebellum for mediating complexity, that is, the effective integration of separate limb movements that proceed in an asynchronous but systematic fashion.

Human exposure to endocrine disrupters: carcinogenic risk assessment.

Human exposure to endocrine disrupters (EDs) is widespread and is considered to pose a growing threat to human health. Recent advances in molecular and genetic research and better understanding of mechanisms of blastic cell transformation have led to efforts to improve cancer risk assessment for populations exposed to this family of xenobiotics. In risk assessment, low dose extrapolation of cancer incidence data from both experimental animals and epidemiology studies has been largely based on models assuming linear correlation at low doses, despite existence of evidence showing otherwise. Another weakness of ED risk assessment is poor exposure data in ecological studies. Those are frequently rough estimates derived from contaminated items of local food basket surveys. Polyhalogenated hydrocarbons are treated as examples. There is growing sense of urgency to develop a biologically based dose response model of cancer risk, integrating emerging data from molecular biology and epidemiology to provide more realistic data for risk assessors, public, public health managers and environmental issues administrators.

Excitation functions of 85Rb(p,xn)(85m,g,83,82,81)Sr reactions up to 100 MeV: integral tests of cross section data, comparison of production routes of 83Sr and thick target yield of 82Sr.

The beta+ emitter 83Sr (T(1/2) = 32.4 h, Ebeta+ = 1.23 MeV, Ibeta+ = 24%) is a potentially useful radionuclide for therapy planning prior to the use of the beta+ emitter 89Sr (T(1/2) = 50.5 d). In order to investigate its production possibility, cross section measurements on the 85Rb(p,xn)-reactions, leading to the formation of the isotopes (85m,g)Sr, 83Sr, 82Sr and 81Sr, were carried out using the stacked-foil technique. In a few cases, the products were separated via high-performance liquid chromatography. For 82Sr, both gamma-ray and X-ray spectrometry were applied; in other cases only gamma-ray spectrometry was used. From the measured excitation functions, the expected yields were calculated. For the energy range Ep = 37 --> 30 MeV the 83Sr yield amounts to 160 MBq/microA h and the level of the 85gSr (T(1,2) = 64.9 d) and 82Sr (T(1/2) = 25.5 d) impurities to <0.25%. In integral tests involving yield measurements radiostrontium was chemically separated and its radioactivity determined. The experimental production data agreed within 10% with those deduced from the excitation functions. The results of the 85Rb(p,3n)83Sr reaction were compared with the data on the production of 83Sr via the 82Kr(3He,2n)-process. In the energy range E3Hc = 18 --> 10 MeV the theoretical yield of 83Sr amounts to 5 MBq/microA h and the 82Sr impurity to about 0.2%. The method of choice for the production of 83Sr is thus the 85Rb(p,3n)-process, provided a 40 MeV cyclotron is available. During this study some supplementary information on the yield and purity of 82Sr was also obtained.

Bayesian integrated functional analysis of microarray data.

MOTIVATION: The statistical analysis of microarray data usually proceeds in a sequential manner, with the output of the previous step always serving as the input of the next one. However, the methods currently used in such analyses do not properly account for the fact that the intermediate results may not always be correct, then leading to cumulating error in the inferences drawn based on such steps. RESULTS: Here we show that, by an application of hierarchical Bayesian methodology, this sequential procedure can be replaced by a single joint analysis, while systematically accounting for the uncertainties in this process. Moreover, we can also integrate relevant functional information available from databases into such an analysis, thereby increasing the reliability of the biological conclusions that are drawn. We illustrate these points by analysing real data and by showing that the genes can be divided into categories of interest, with the defining characteristic depending on the biological question that is considered. We contend that the proposed method has advantages at two levels. First, there are gains in the statistical and biological results from the analysis of this particular dataset. Second, it opens up new possibilities in analysing microarray data in general.

Temporal integration, frequency resolution, and off-frequency listening in normal-hearing and cochlear-impaired listeners.

Temporal integration for a 1000-Hz signal was determined for normal-hearing and cochlear hearing-impaired listeners in quiet and in masking noise of variable bandwidth. Critical ratio and 3-dB critical band measures of frequency resolution were derived from the masking data. Temporal integration for the normal-hearing listeners was markedly reduced in narrow-band noise, when contrasted with temporal integration in quiet or in wideband noise. The effect of noise bandwidth on temporal integration was smaller for the hearing-impaired group. Hearing-impaired subjects showed both reduced temporal integration and reduced frequency resolution for the 200-ms signal. However, a direct relation between temporal integration and frequency resolution was not indicated. Frequency resolution for the normal-hearing listeners did not differ from that of the hearing-impaired listeners for the 20-ms signal. It was suggested that some of the frequency resolution and temporal integration differences between normal-hearing and hearing-impaired listeners could be accounted for by off-frequency listening.
