Testosterone therapy for anaemia in maintenance dialysis.

Intramuscular testosterone 250-500 mg weekly given to 25 patients on long-term haemodialysis produced a significant rise in haemoglobin level in all patients. The response was reversible when the drug was stopped. Treatment was effective in bilaterally nephrectomized patients and those with intact kidney tissue. The use of intramuscular testosterone is recommended for all adult patients on long-term haemodialysis in order to eliminate the need for routine blood transfusions.

Acute complications of sickle cell disease in children.

Sickle cell disease is a recessively inherited condition in which synthesis of haemoglobin is abnormal. The disease, which occurs mainly in people of African, African-Caribbean, Indian, Mediterranean and Middle Eastern descent, is characterised by chronic anaemia, susceptibility to infection, bouts of severe pain and organ dysfunction. While the life expectancy for patients has improved, from a median survival age of 14 years in the 1970s among those homozygous for the sickle haemoglobin gene to survival into the mid-40s, childhood remains a period of peak mortality and morbidity. Here, we discuss the acute complications of sickle cell disease in children, concentrating on the management of the acutely unwell child.

Coital emergencies.

The act of heterosexual coitus is associated with morbidity due to a variety of conditions as well as with a small risk of sudden death. Awareness of the presentation of coital emergencies is essential to allow appropriate medical management and sexual counselling.

Prolonged penile erections induced by hydroxyzine: possible mechanism of action.

We describe a subject who developed prolonged penile erections on hydroxyzine. Molecular modelling showed that the hydroxyzine metabolite, norchlorcyclizine, has structural and conformational similarities to the trazodone metabolite, m-chlorophenylpiperazine. This suggests that a common pharmacological mechanism may underlie the ability of trazodone and hydroxyzine to induce erections.

How safe is the treatment of impotence with intracavernous autoinjection?

Intracavernous administration of papaverine and phentolamine has become an essential and indispensible element in the diagnosis and treatment of erectile dysfunction. This paper assesses the frequency and meaning of the main side effects of intracavernous injections (prolonged erections, fibrosis, etc.). It is concluded that the benefit from this new method definitely exceeds the possible risks. However, for the safe implementation of this method, close and intensive cooperation between the physician and his patient is a condition that has to be guaranteed. Each patient carrying out autoinjection therapy has to be examined at reasonable intervals to evaluate the to date unknown effects of long-term therapy.
