[Health threats for children in the modern building trade].

Changes in building industry during last 30 years in search of economic technologies and building materials ended in creating airtight and energy-saving constructions with reduced ventilation. As a result, diminish exchange between internal and external air, leads to cumulation of various chemicals and organic substances in our houses. Concentrated oxidation products (CO2, CO, NO2, nicotine smoke), vaporizing substances (formaldehyde, NH3, aromatic hydrocarbons) might irritate mucouses, promote or intense respiratory tract infections, allergic diseases or bronchial asthma. Higher risk of atopic diseases and bronchial asthma also arises from proliferation of fungus, bacteria and mites in modern interiors, which are quite convenient for it. Buildings with microclimate, which is dangerous for human health, are termed "sick buildings". Diseases that develop under their influence are called BRI (building related illnesses). Children are most exposed on them, especially under municipal conditions, because they spend majority of their time at home.

The provision of modern medical services to a nomadic population: a review of medical services to the Bedouins of southern Sinai during Israeli rule 1967-1982.

Southern Sinai, a mountainous desolated arid area, is inhabited by Bedouin nomad tribes composed of Arabic-speaking Moslems. Until the Six Day War between Egypt and Israel in 1967, healthcare services in the region were based on traditional medicine performed by the Darvish, a local healer. Over the course of Israeli rule (1967-1982) an elaborate healthcare service was established and maintained, providing modern, up to date, comprehensive medical services that were available to all free of charge.

The evolution of human occlusion--ancient clinical tips for modern dentists.

Man evolved in an environment in which the occlusion was worn down quickly, resulting in flattened occlusal and interproximal surfaces. This rapid wear reduced occlusal decay, traumatic occlusion, malaligned teeth, impactions, and temporomandibular disease (TMD). In the last 250 years, however, new food production techniques created an environment that was less dentally abrasive than earlier diets. Teeth were not worn down as programmed in our "evolutionary blue-print." This lack of wear resulted in increased caries, cusp fractures, bruxing, malocclusion, periodontal disease, and TMD. A practical re-creation of ancient dental wear patterns can help to reduce these modern dental diseases.

Microsurgical repair at early stage for soft tissue defect of limbs wounded by modern firearm.

OBJECTIVE: To explore an early stage repair method for soft tissue defect of limbs of modern firearm wound, and to improve treating result. METHODS: Defects of the hind limbs of dogs were repaired with skin, muscle and myocutaneous flaps. RESULTS: Wounds healed within 2 weeks in the experimental group except one that healed in 3 weeks because of infection. Limb function was close to normal. The treatment result was better in the experimental group than the control. CONCLUSIONS: Skin, muscle and myocutaneous flaps can cover soft tissue defect at an early stage, prevent and reduce infection, promote the healing and recovery of combined injury, reduce the time of treatment and disability rate.

[Modern variant of hanging: use of a time-programmed winch].

Hanging is one of the most frequent types of suicide. Descriptions of unusual ligatures can be read about again and again. The paper reports the suicide of a 41-year-old electrician, who, using an electric winch, hung himself in nearly free suspension. The winch that was secured to the timberwork of the attic had been programmed by an electric time clock, the toggle switch of the remote control was fixed in the on-position using adhesive tape. The man had handcuffed his hands behind his back against any attempts to save himself. The noose was partly cushioned with a bike tube with the obvious aim of reducing the pain. Due to the state, in which he was found and the results of the post-mortem examination, a third party responsibility was excluded. The case shows that not only certain types of knots and nooses can be associated with various occupational groups, but the choice of a special strangulation device can possibly also come from the occupational sphere. As a modern variation of hanging, the presented casuistry can partly complete the familiar documentation of historical cases with the use of block and tackle and similar constructions.

[Modern therapy of intestinal helminthiasis (author's transl)].

The modern chemotherapy of intestinal worm diseases is essentially based on niclosamide, thiabendazole and mebendazole. Thus an effective therapy with few side effects has become possible for these helminthiases. In infections with Taenia, Ascaris, Trichuris, Ankylostoma, Strongyloides, Trichostrongylus and Oxyuris, as the most frequently imported helminths, an elimination rate of over 90% of the parasites can be attained. Infections with nematode larvae as with Larva migrans and trichinoses are more problematic therapeutically because the anthelmintics are directed principally against the adult worms.

[Contradictions in modern data on the structure and function of dog hepatoid circumanal glands].

Since the 1950s, an erroneous viewpoint has gained a foothold in the special and reference literature that dog hepatoid circumanal glands have an abortive nature, lack efferent ducts, and show no signs of secretory activity. Nevertheless, most modern researchers indicate that these glands are actively functioning structures differentiated to exercise some function according to their histological pattern; however, no one has managed to define this function. The latest international publication (Atoji et al., 1998) considers dog circumanal hepatoid glands as a "circumanal body" similar to epidermis rather than glands. We discuss strong and weak points of the data on dogs hepatoid circumanal glands published in 1950s-1990s in comparison with those obtained in our purposeful long-term investigation of various hepatoid glands. We strongly disagree with the viewpoint of Atoji et al. (1998); hepatoid circumanal glands of dog and other canids should be considered as odor glands, their abundant protein secretion covers the skin surface in the circumanal region, it is preserved on fecal marks, and its smell is perceived during mutual nosing of the anal region.

Salvaging legacy data: mapping an obsolete medical nomenclature to a modern one.

The Veterinary Medical Database (VMDB) is a repository containing abstracts of over six million case records from 24 veterinary colleges throughout the U.S. and Canada. These case record abstracts, spanning almost 40 years, represent a valuable resource for outcomes analysis and hypothesis generation. Database records are currently encoded using the Standard Nomenclature of Veterinary Diseases and Operations (SNVDO), a precoordinated, hierarchical coding system. SNVDO has not been updated since 1977 and is outdated and inadequate to express the current state of medical knowledge. We undertook to manually map a subset of the SNVDO codes to a modern medical nomenclature, SNOMED-RT (Version 1.0), and to evaluate the quality of the resultant mappings and the acceptability of the mapping method used. We found that the distribution of frequency of use of the SNVDO codes in the VMDB records is highly skewed, with a small number of codes accounting for a large percentage of the records. We targeted our mapping efforts on that subset of codes. We found that our targeted manual mapping of the SNVDO codes to SNOMED-RT codes was feasible and produced good quality results, based on separate evaluations performed by two domain experts. However, a significant proportion of the SNVDO codes could not be mapped to a single SNOMED-RT concept, necessitating construction of multiple-code post-coordinated terms. Additionally, this manual mapping was very labor-intensive.

Modern endodontic therapy for an incompletely developed tooth.

An important goal of modern dentistry is to preserve a functional, esthetic, and pain-free dentition. If irreversible pulp pathology occurs in an immature permanent tooth from traumatic or carious exposure, normal apical maturation may be interrupted. Conventional endodontic obturation techniques, if necessary, will be difficult and may not be adequate in such a tooth. The endodontic techniques of apexogenesis and apexification have been successful in achieving adequate root development and apical construction. These conventional techniques and materials are reviewed along with a promising material called mineral trioxide aggregate.

Comparative study of the introduction of modern botany in Japan and China.

Prior to the eighteenth-century, a similar approach towards the vegetable kingdom, mainly influenced by the tradition of the Chinese pharmacopoeias, could be observed in China and Japan. During the eighteenth-century, the interest for "Dutch learning" led some Japanese physicians and interpreters to be more and more interested in Western knowledge about medicinal plants. At the beginning of the nineteenth-century, a few scholars, through direct contact with foreigners or with foreign books, realised that there was a specific scientific field called "botany" and began to introduce the Japanese scholarly community to this new science which became one of the subjects taught at the "University of Tokyo" in 1877. In China, up to the middle of the nineteenth-century, no trace of modern botany can be found in any published document. In the second half of the century, a few botanical treatises were published, all being adaptations or translations of Western books, done by foreign-Chinese teams of translators. This situation began to change when Chinese students had the opportunity to go and study abroad, mainly to Japan, at the beginning of the twentieth-century, and, actually, it is between 20 and 30 years later that botany became a real scientific practice in China. We will analyse these two processes, their specificities and their interactions.

[Communication and teamwork in a modern hospital].

The problems facing a modern hospital, which mirror the crisis confronting medicine as a whole, are reviewed. In the search for solutions to these difficulties it is necessary to envisage the hospital in a larger context and examine the role it should play in society. And a comprehensive approach that takes into account all the different elements involved in the work of a hospital-administration, medical care, teaching, research-is fundamental. All teamwork presupposes certain dimensional factors such as organization, dynamics, and productivity. And each member of the health team must identify with the work of the group as a whole, providing for a degree of "realization of individual potential" which ultimately comes together with the primary goal-namely to work toward the health of the patient. The various forms of communication in the hospital, both between individual departments and between the members of each team that has a part to play, are also reviewed.

Bernardino Ramazzini's De morbis artificum diatriba or three hundred years from the beginning of modern occupational medicine.

This review opens with the most important examples from the history of medicine until the end of the 17th century, which anticipated the affirmation of occupational medicine as separate medical discipline. The article remembers the famous Italian physician Bernardino Ramazzini (1633-1714) and his book De morbis artificum diatriba (Diseases of Workers) published in Modena in 1700 in which he described the effects of work on health for some fifty professions. By doing that, Ramazzini had laid scientific foundation to modern industrial hygiene, that is, today's occupational medicine. The authors conclude that even 300 years after the first publication of Diatriba, Ramazzini is still relevant, and that it deserves much greater attention than it is given on occasions such as this year's anniversary.

Modern aspects of continuous medical education.

Continuous medical education represents the final and often most poorly understood stage of physician education. It is defined as any and all ways by which doctors learn after the formal completion of their training. While evidence-based medicine is defined as "the conscientious, explicit and judicious use of current best evidence in making decisions for the care of individual patients. The practice of evidence-based medicine means integrating individual clinical expertise with the best available external clinical evidence from systematic research." The primary purpose of this article is to discuss the modern and technological aspects of continuing medical education and how it helps to maintain and improve clinical performance of practicing doctors and patient care. The article also emphasizes evidence-based medicine as a newly evolved, rapidly growing discipline for learners and researchers, which has extended the application of its principles to all professions associated with health care, including purchasing and management.

The role of colposcopy in modern gynecology.

UNLABELLED: The purpose of this review is to demonstrate that colposcopy, introduced in 1925--which is, notably before the development of great technological advances in modern gynecology--continues to be a valid technique without essential innovations to the original method described at the beginning of the last century. Colposcopy was developed in Germany during the rise of Nazism with the Second World War being an important barrier for the spread and diffusion of the technique. Colposcopy, however, continued to progress in a few countries such as Spain, Italy, Brazil, France and Switzerland. When colposcopy was introduced in the United States during the 70s, its use was mostly restricted to specialists who were almost exclusively dedicated to cervical pathology and knowledgeable about cytopathology, anatomic pathology, and colposcopy and who were competent both in the diagnosis and treatment of cervical lesions. These circumstances were completely different from what happened in the majority of European countries where colposcopists were trained as gynecologists and their histocytological knowledge, which was focused on the lower genital tract, was somewhat more extensive than that acquired by specialists in gynecology. There are two clearly different trends in relation to the use of colposcopy with characteristic geographic distribution: countries with an Anglo-Saxon influence in which colposcopy is performed selectively, and countries with a German medical inheritance in which colposcopy is carried out routinely during a standard general gynecological consultation. However, this difference is not restrictive and by no means can it be stated that colposcopy is systematically being used by all European or Latin American gynecologists for reasons related to training in the colposcopic technique. In 1977, we introduced the concept of dynamic colposcopy with the aim of differentiating it from the descriptive immobility of the original classification of Hinselmann (1954) that had remained almost unchanged by his immediate followers. Briefly, the objective was to turn colposcopy into a diagnostic tool able to identify the pathological substrate corresponding to traditional colposcopic images. We established ten differential signs that allow us to classify an ATZ area as subsidiary or not to be biopsied. The classification system proposed in Rome (International Federation of Cervical Pathology and Colposcopy [IFCPC], 1990) supports our original concept because by identifying major or minor changes in the original images, a diagnosis of the severity of the lesion can be established. With regard to specificity, the figures range between 48% and 10% with 96% for sensitivity. Obviously, a wide range of colposcopic specificity must be related to the expected efficacy of the method. When after biopsy of an atypical colposcopic image, only a low-grade lesion is detected, should this be considered a false positive colposcopic result? Although histopathologic findings are accepted as the "gold standard"...it is well known that a certain degree of subjectivity can be present. Inter- and intra-observer differences (when the same pathologist is reviewing the diagnosis after a certain time lapse) may be present. It has been argued that microbiopsy under colposcopic control gives rise to a wide error range and that it cannot be considered representative of the lesion. It is likely that this situation may occur when colposcopy-guided biopsy is performed by inexperienced hands or when biopsy is limited to small and insufficient sampling. A very important colposcopic sign, such as complete visual inspection of the squamocolumnar junction is frequently missed. Any lesion with boundaries in the endocervix, cannot be simply assessed by means of microbiopsies from the ectocervix unless there is no doubt regarding the severity of the lesions. Microcolpohysteroscopy (MCH) may be of great value in these cases by showing the limits of endocervical involvement. CONCLUSIONS: According to the evidence presented here, it can be concluded that "colposcopy is in good health" and that probably the popularity of this technique in the field of gynecology would increase if cytopathologists and gynecologists' tasks were limited to their own fields rather than turning them into improvised specialists for their counterpart disciplines. The coordinating role of the gynecologist as a specialist for integral women's health should continue to be defended and in this respect, colposcopy should be considered a routine technique in daily practice.

[Modern poro-elastic biomechanical model of bone tissue. I. Biomechanical function of fluids in bone].

The modern biomechanical two-phase poroelastic model of bone tissue is presented. Bone tissue is treated in this model as a porous elastically deformed solid filled with a viscous newtonian fluid. Traditional one-phase biomechanical model of bone tissue, which is characterized by the Young modulus and the Poisson's coefficient, is still valid and it can be treated as an approximate model in comparison with the more realistic two-phase model of bone tissue. The biomechanical function of fluids in bone is considered. Bone biodynamics is presented in form of the scheme which illustrates the mechano-adaptive, the mechano-electric and the electrophysiologic properties of bone tissue. Essentials of the poroelastic model of bone tissue is the mechanical load induced flow of intraosseous fluid and the associated strain generated electric potentials SGPs.

International migration in modern economic development: with special reference to Yugoslavia.

"This article analyzes international migration within the framework of modern socioeconomic development, taking as an example the Yugoslav external migration in the period after World War II. How these migration flows fit with generalizations derived from long-run experiences with external migration of other countries is examined. General theoretical reasoning is applied in order to elucidate the basic determinants of migration, thus making a contribution to the integration of the theory and policy of economic development of emigration countries."

Spatial patterns of age-sex structures in Costa Rica: a study in demographic modernization.

The relationship between economic development and demographic factors in Costa Rica is examined. "Specifically, the paper illustrates the evolution of spatial patterns in age-sex structures over three points in time for a single case study area." The authors suggest that there is order in the evolving patterns and that this order may be explained by the economic modernization process. Data are from the 1950, 1963, and 1973 censuses. Although some spatial order is indicated, the patterns are confused primarily by an increase in fertility that apparently occurred between 1950 and 1963.
