Vestibular evoked myogenic potentials in response to laterally directed skull taps.

In recent years it has been demonstrated that loud clicks generate short latency vestibular evoked myogenic potentials (VEMP). It has also been demonstrated that midline forehead skull tap stimulation evokes similar VEMP. In the present study, the influence of skull tap direction on VEMP was studied in 13 normal subjects and in five patients with unilateral vestibular loss. Gentle skull taps were delivered manually above each ear on the side of the skull. The muscular responses were recorded over both sternocleidomastoid muscles using skin electrodes. Among the normals, laterally directed skull taps evoked "coordinated contraction-relaxation responses", i.e. skull taps on one side evoked a negative-positive "inverted" VEMP on that side and a positive-negative "normal" VEMP on the other side. Among patients with unilateral vestibular function loss, skull taps above the lesioned ear evoked similar coordinated contraction-relaxation responses. However, skull taps above the healthy ear did not evoke that type of response. These findings suggest that laterally directed skull taps activate mainly the contralateral labyrinth.

Pressure pain threshold of the posterior aspect of the temporomandibular joint measured with a semi-spherical probe.

AIMS: To develop and test a probe for measurement of the pressure pain threshold (PPT) over the posterior aspect of the temporomandibular joint (TMJ) in healthy individuals, including determination of PPT levels, reliability, and the smallest detectable difference (SDD) between measurements. METHODS: A semi-spherical probe was designed to measure PPT levels over the posterior aspect of the TMJ through the external auditory meatus. The probe was connected to an electronic algometer. Three consecutive measurements were performed with this probe over the posterior and lateral aspects of the left and right TMJs as well as over a reference point on the forehead (glabella) in 31 healthy subjects: 10 male and 21 female. Measurements were also performed for comparison with a conventional flat probe with a 1 cm2 area over the lateral aspect of the TMJ and the reference point. RESULTS: The PPT measured with the semi-spherical probe and the conventional probe showed similar degrees of interindividual variation and reproducibility. The relative SDD, expressed as the percentage of the mean PPT for 2 measurements, showed similar levels for the flat and semi-spherical probes, i.e., 28% to 32% of the mean PPT at the TMJ. CONCLUSION: The semi-spherical probe shows similar reliability and relative SDD for measurement of PPT levels over the posterior aspect of the TMJ in healthy individuals as measurement over the lateral aspect with a flat probe. Measurement of the posterior PPT with a semi-spherical probe may be a useful adjunct to conventional lateral PPT measurements.

[Circadian variation in the latency of auditory brainstem response].

The auditory brainstem response (ABR) has been found to reflect many pathological conditions within the auditory system and brainstem. And now, many neurosurgeons are using it to monitor the integrity of the auditory pathway during neurosurgical procedures. It is said that ABR shows little variation from person to person or laboratory to laboratory, nor is it easily affected by anesthesia, level of consciousness, fluctuation of blood pressure or hypoxemia. On the other hand, previous studies have shown that component waves of the ABR increase in latency and decrease in amplitude with lowered temperature. We reported here that naturally occurring circadian variations in body temperature were correlated with similar changes in the latency of the ABR. Tympanic temperature (Tty), deep forehead temperature (Thd) and ABR were recorded every 3 hours during a 24-hour period for a total of 8 recording sessions from each of 6 healthy persons (2 males and 4 females, mean age 24.3 years). The subjects were free to come and go during the day but slept overnight in the laboratory. All subjects had circadian variations in each temperature on the order of one degree. Thd had a tendency to fluctuate and its range of difference from Tty was -0.5-0.4 degree C. There was a more significant negative correlation between the latency of the ABR and Tty than that of Thd. It has become apparent that ABR latency is affected by small temperature changes such as circadian variation. The rate of a latency change in the ABR was 0.15msec per degree (C).(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of dirofilariasis in the town of Magnitogorsk of the Cheliabinsk region].

A local case of dirofilariasis was first reported in a 70-year-old female in Magnitogorsk, Chelyabinsk Region, in November 2002. The primary diagnosis is atheroma of the skin of the forehead. A round mass having a smooth surface and slightly solid gray-whitish walls was surgically excised, which yielded a mobile rolled nematode measuring 7 cm in length and 0.8 mm in width. The parasitological diagnosis is an immature Dirofilaria repense female. It has been found that the female has neither dogs nor cats in her flat and she had not been left Mangitogorsk for approximately 18 years. A histological examination of the fibrotic capsule around the parasite revealed inflammation in the inner layer of the capsule, which was regarded as proliferative and having no signs of a granulomatous process. Fragments of cuticles of a round helminth having a well-defined alimentary tube was found. To encode the case, the authors have used Category B74.8 "Other types of filarisis (dirofilariasis)" of the first class, ICD-10.

Brooke-Spiegler syndrome with parotid gland involvement.

Salivary gland involvement in Brooke-Spiegler syndrome (BSS), an autosomal dominantly inherited disease, is known though not frequent. A case of familial cylindromatosis with parotid gland adenoma is herein reported. A 67-year-old lady presented with multiple scalp nodules and papular coalescent lesions over the nasolabial folds and the forehead. The clinical examination also revealed a left preauricular lump. Multiple biopsies of the scalp lesions and the nasolabial papules revealed cylindromas and trichoepitheliomas respectively. CT scan and FNA of the preauricular lump were suggestive of parotid gland adenoma. The patient underwent excision of the scalp cylindromas and total left parotidectomy. There is no evidence of recurrence after 4 years. The association of BSS with salivary gland tumours, emphasizes the necessity of thorough salivary gland examination in all patients with skin lesions. Knowledge of the genetic background of BSS allows for genetic counseling of patients.

The zygomaticotemporal branch of the trigeminal nerve: an anatomical study.

This study was conducted to determine the site of emergence of the zygomaticotemporal branch of the trigeminal nerve from the temporalis muscle and to identify the number of its accessory branches and their locations. A pilot study, conducted on the same number of patients, concluded that the main zygomaticotemporal branch emerges from the deep temporal fascia at a point on average 17 mm lateral and 6 mm cephalad to the lateral palpebral commissure, commonly referred to as the lateral canthus. These measurements, however, were obtained after dissection of the temporal area, rendering the findings less reliable. The current study included 20 consecutive patients, 19 women and one man, between the ages of 26 and 85 years, with an average age of 47.6 years. Those who had a history of previous trauma or surgery in the temple area were excluded. Before the start of the endoscopic forehead procedure, the likely topographic site of the zygomaticotemporal branch was marked 17 mm lateral and 6 mm cephalad to the lateral orbital commissure on the basis of the information extrapolated from the pilot study. The surface mark was then transferred to the deeper layers using a 25-gauge needle stained with brilliant green. After endoscopic exposure of the marked site, the distance between the main branch of the trigeminal nerve or its accessory branches and the tattoo mark was measured in posterolateral and cephalocaudal directions. In addition, the number and locations of the accessory branches of the trigeminal nerve were recorded. On the left side, the average distance of the emergence site of the main zygomaticotemporal branch of the trigeminal nerve from the palpebral fissure was 16.8 mm (range, 12 to 31 mm) in the posterolateral direction and an average of 6.4 mm (range, 4 to 11 mm) in the cephalad direction. On the right side, the average measurements for the main branch were 17.1 mm (range, 15 to 21 mm) in the lateral direction and 6.65 mm (range, 5 to 11 mm) in the cephalic direction. Three types of accessory branches were found in relation to the main branch: (1) accessory branch cephalad, (2) accessory branch lateral, and (3) accessory branches in the immediate vicinity of the main branch. This anatomical information has proven colossally helpful in injection of botulinum toxin A in the temporalis muscle to eliminate the trigger sites in the parietotemporal region and surgical management of migraine headaches triggered from this zone.

[Cranial fascitis--case report and literature review].

Cranial fascitis is a rare disease that is considered as a subset of nodular fascitis. Lauer and Enzinger described the disease for the first time in 1980 as a cranial fascitis of childhood (CFC), based on pathologic findings in nine cases of children from three weeks to six year old. The disease is benign proliferation of immature fibroblasts infiltrating the cranial bones with tendency to form tumor like shapes. Surgery excision gives total cure and recurrence is very rare. We present the case of a twenty years old male patient with head trauma in medical history and two years history of recurrent forehead headache. Performed cerebral computerized tomography and skeleton scintigraphy showed erosion of the lateral parts of both frontal skull bones and upper-lateral parts of the right and left orbital cavity. In general anesthesia, biopsy specimen was taken. After histopathological examination the diagnosis of cranial fascitis was established. Eight months after the diagnosis we observed progression of the skull bones destruction. Therefore our recommended treatment was surgical excision but we did not obtain patient agreement.

[Rejuvenation of the forehead/temple by limited incision].

OBJECTIVE: To utilize multiplane, subcutaneous and subperiosteal, dissection through small incisions in scalp to rejuvenate aging signs of forehead/temple. METHODS: Forehead: We make four small incisions in scalp, widely separate tissues between subperiosteum skin and frontal muscle to form galea frontal muscle-periosteum flap, the flap is tightened and sutured with the galea at the posterior border of the incision. This method avoid to excise scalp. Temple: there are two small incisions in the scalp of temple at each side. Superficial temple fascia and orbicularis oculi muscle as well is dissected from deep temporal fascia and skin respectively, then tightened and sutured with the superficial part of deep temporal fascia, excising scalp is not necessary. RESULTS: Twenty patients have received this treatment, the effects are satisfying. CONCLUSIONS: Multiplane dissection through small incisions can remove wrinkles of skin, correct the prolapse of eyebrow and avoid the complications of coronary incision. This approach brings more rapid recovery of patients, it is safe and affective.

Opportunistic coryneform organisms--residents of human skin.

Opportunistic infections are usually caused by endogenic flora originated from physiological flora. In this context we studied coryneform bacteria recovered from deeper layers of epidermis of the forehead (278 isolates) and the back (196 isolates) of healthy men. It was observed that coryneform bacteria are in dynamic equilibrium with coagulase-negative staphylococci and they amount 4,7% of resident aerobic flora. On the base of biological and metabolic features 49 different biotypes were indicated. Biotypes of lipophilic rods were the basic part of coryneform flora existing for long period. The most frequently isolated taxa were C. jeikeium (31%), CDC group G2 (23,4%), next in order CDC group G1 (11%) and C. afermentans ssp. lipophiluni (7%). These isolates were phenotypically differentiated. Nonlipophilic species did not play significant role in creating resident flora of the skin. The significance of coryneform bacteria in opportunistic infections is growing, especially in immunocompromissed patients. Isolated lipophilic taxa belong to these taxa coryneforms which are described in literature as the main etiologic factors of opportunistic infections.

Differences in the skin surface pH and bacterial microflora due to the long-term application of synthetic detergent preparations of pH 5.5 and pH 7.0. Results of a crossover trial in healthy volunteers.

Skin cleansing preparations consisting of identical synthetic detergents but differing in pH-value (pH 5.5 and 7.0) were applied twice daily on the forehead and forearm of healthy volunteers in a randomized crossover trial. The skin surface pH was found to be significantly higher when the neutral preparation had been used, as was the propionibacterial count (p less than 0.05). The number of propionibacteria was significantly linked to the skin pH. Hence even minor differences in the pH of skin cleansing preparations seem to be of importance for the integrity of the skin surface. This should be taken into account when planning the formulation of optimal skin care products.

Perineural invasion of the orbit by squamous cell carcinoma.

Two cases of orbital extension by squamous cell carcinoma along the supraorbital nerve are reported. Important clinical aspects of these cases include previous incomplete removal of squamous cell carcinoma of the forehead and altered sensation in the dermatome of the supraorbital nerve. Axial spread of carcinoma along periorbital nerves is an important avenue of local metastasis and should be suspected in all cases of periorbital squamous cell carcinoma.
