Effect of ionic and nonionic contrast media on regional distribution of thallium-201 in rabbits.

Cardiac output is effectively redistributed to each part of the body by regional changes in vascular resistance. Thallium-201 distribution reflects the fractional distribution of cardiac output and parallels regional blood flow. The side effects of iodinated contrast media in different organs have been well documented in animal experiments and clinical studies. However, no simultaneous assessment of regional blood flow change in each part of the body after the administration of contrast media has been reported. Using a double-dose thallium-201 method, we investigated the effect of contrast media on the regional distribution of thallium-201 in each organ as an estimate of regional blood flow change in rabbits. The distribution of 201T1 and 99mTc-MAA was similar in the heart, spleen, kidneys, and leg muscles in the control animals (r = 0.99). The regional distribution of thallium-201 significantly increased in the heart and leg muscle, but decreased in the liver in the nonionic (Iohexol) and ionic (Iothalamate) contrast medium groups as compared with the control group. There were no significant changes in the kidneys or intestine in these three groups in our study. The intravenous infusion of contrast media in rabbits causes uneven redistribution of regional blood flow in the heart, leg muscle and liver. These changes are more prominent in the ionic contrast medium group.

[Increase of kidney enzymes in urine after administration of contrast media: comparison of the nephrotoxicity of ionic and nonionic substances].

In a double blind clinical study, 50 patients with healthy kidneys were tested for the discharge of renal marker proteins before and after receiving a high or low osmoloar contrast medium (Meglium-Ioglicinat, Imeprol) using i.v. urography. The renal excretion of the tubular indicator enzymes alanine aminopeptidase, beta-Glutamyltranspeptidase and alkaline phosphatase increased in all patients after administration of the contrast media. The enzymuria was significantly lower with the non-ionic contrast medium with the ionic one, thus demonstrating that the non-ionic contrast medium has a lower nephrotoxic potential than the conventional ionic contrast medium. Three independent investigators evaluated the radiograms with regard to the contrast quality using prearranged criteria in a 5 point system. The diversity of the evaluations clearly favoured the non-ionic contrast medium.

Constant infusion clearance is an inappropriate method for accurate assessment of an impaired glomerular filtration rate.

Recently renewed interest has been focused on constant infusion clearance to assess GFR accurately. In this study we compared GFR and ERPF calculated from the constant infusion method (CIM = I x V/P) with that calculated from the standard method (StM = U x V/P), in 100 patients with renal disease who were subdivided in four groups according to their GFR-StM (< 30; 30-60; 60-90; > 90 ml/min). After a priming dose, a constant infusion of 125I-iothalamate (= GFR) and 131I-hippurate (= ERPF) was started at 9 a.m. The infusion rates were individually adjusted to the GFR which was approximated from the serum creatinine concentration. After a 90-min equilibration period, GFR-StM and ERPF-StM were determined for two 2-h periods. These values were compared with GFR-CIM and ERPF-CIM calculated from the plasma concentration of the respective tracers at the end of each 2-h period (= 210 and 330 min). In the patient group with GFR-StM < 30 ml/min, the 125I-iothalamate plasma concentration increased progressively over time. Consequently, average GFR-CIM at 210 min (34.2, SE +/- 2.1 ml/min) was higher than the GFR-CIM at 330 min (31.9, SE +/- 2.0 ml/min; P < 0.001). In addition both values were significantly higher than the corresponding GFR-StM values (18.1 +/- 2.4 and 15.3 +/- 1.6 ml/min respectively). In the two patient groups with GFR-StM > 60 ml/min, the 125I-iothalamate plasma concentration decreased progressively over time.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of renal function before and after intravenous injection of non-cholangiographic water soluble contrast media in rats.

In the present study, normal laboratory rats (n = 22), were injected intravenously with water soluble contrast media (CM) or saline. Renal function was monitored before and followed after challenge. Seven animals were injected with saline, the others with 3 different types of contrast media. The absolute urinary creatinine output decreased significantly in the saline group, from 0.0247 mumol/min 100 g BW (IR: 0.0052) to 0.0167 mumol/min 100 g BW (IR: 0.0019) (p < 0.01), while in the CM groups only a significant decrease was seen after ioxaglate injection, from 0.0250 mumol/min 100 g BW (IR: 0.0014) to 0.0174 mumol/min 100 g BW (IR: 0.0027) (p < 0.01). ANOVA between the groups showed no difference. The median values for serum creatinine after injection of the test products did not differ from the control values. It seems therefore that the challenge of a normal laboratory rat with CM is not a suitable model for the detection of subtile nephrotoxic properties of CM.

Diagnosis of traumatic rupture of the thoracic aorta using dynamic rapid sequence axial computerized tomography.

The use of intravenous contrast enhanced rapid sequence (dynamic) computerized tomography (CT) of the chest led to the successful diagnosis and treatment of traumatic rupture of the thoracic aorta in three patients. Early diagnosis of this often fatal injury may result in a favorable outcome; and in the hemodynamically stable patient with multisystem injury a well-performed CT may preclude the use of more invasive techniques. When aortic disruption is suspected, dynamic rapid sequence axial CT of the chest may not only exclude but also confirm the diagnosis, in addition to accurately detecting other major intrathoracic injuries.

[Pulmonary edema secondary to the intravenous administration of iodized contrast. A clinical case with high-resolution tomodensitometry studies].

Contrast media related reactions are numerous and of variable gravity. We report a case of pulmonary edema related to the intravenous administration of contrast media. Cardiogenic and non cardiogenic pulmonary edema are pathophysiologic possibilities. High resolution CT findings associate patchy ground glass areas and smooth thickening of subpleural septa, both rapidly disappearing.

[Adverse reactions to high osmolarity contrast media. A comparative-prospective study].

The purpose of this study was to verify and compare the different types of reactions to iodinated contrast media available at the Social Security Metropolitan Hospital Complex: Conray 60%, Vascoray and Uromiron. These are ionic, high osmolality compounds. The authors found that, in 400 patients not only the osmolality had an important role in the reactions observed, but that the intrinsic nature of the molecule of the contrast medium used was the determining factor.

[Arterial time of intravenous pyelography: initial results (author's transl)].

The authors consider that attention should be paid to the arterial time in intravenous pyelography in all patients who undergo the examination. Manual injection or an injector containing 80 to 100 ml of 38% iodine contrast medium set at a speed of 20 ml/sec, on the one hand, and films taken during the arterial phase on the other hand, are the two conditions necessary to obtain a satisfactory arteriogram. Slight abdominal compression and subtraction improve the definition of the films. 600 cases have been collected so far, in the exploration of 105 patients with hypertension, 17 cysts, 7 malignant tumours, extra-renal pathology and various nephropathies in which no indication for arteriography existed. Thus pyelographic arteriography made it possible to avoid renal arteriograms in almost all our patients. Only one selective and one complete arteriogram were necessary. Unusual arterial lesions in nonsurgical renal conditions may be seen (glomerulonephritis, nephroangiosclerosis...) and unexpected images in abdominal conditions with renal manifestations or in association with nephropathies (aneurysms of digestive arteries and the aorta, collagen diseases etc.).

[Experience with the contrast medium Telebrix 300 in neuroradiology (author's transl)].

Clinical experience with a new tri-iodized contrast medium (Telebrix 300) in cerebral angiography is reported. In one of its forms, Telebrix is available as the pure methylglucamine salt of the newly developed ioxitalaminoacid in 66% aqueous solution, with an iodine content of 300 mg/ml. The results of the use of Telebrix 300 in 302 patients examined with over 600 single vascular visualisations (direct puncture and catheter angiography) are communicated. Unforessen incidents with permanent neurological disturbances were not observed. Local and general tolerance of Telebrix 300 is excellent, even after large quantities of the contrast medium (up to 300 ml).

Angiographic demonstration of the cervical vertebral venous plexus in man.

Opacification of the cervical vertebral venous plexus was achieved by injecting contrast material into the upper part of the internal jugular vein while the lower portion of this vein was compressed externally with a tourniquet. The injection was easily performed via a percutaneous catheter which was advanced upwards into the jugular vein. Demonstration of the cervical plexus is made easier with the patients sitting up. Both sides of the plexus may be shown with a single injection if both jugular veins are injected simultaneously through bilateral catheters joined with a Y connector.

Absence of pathological changes following intravenous methamphetamine and intra-arterial iothalamate meglumine.

Eleven Rhesus monkeys received injections of intravenous methamphetamine hydrochloride (Desoxyn) and/or intra-arterial 60% iothalamate meglumine (Conray) according to a schedule previously reported to produce marked radiological and pathological changes in the cerebral vasculature of the Rhesus monkey. While radiological changes consistent with impaired cerebral circulation were observed, they could not be correlated directly with the administration of intravenous methamphetamine because of the trauma and variability of the technique utilized. Moreover, significant pathological changes could not be found in animals given intravenous methamphetamine alone, intra-arterial iothalamate meglumine alone, or both drugs together. This was true for both drug naive and drug experienced monkeys. These results suggest: (1) radiological changes consistent with decreased cerebral blood flow are not necessarily pathognomonic for reported morphological lesions, and (2) further investigation is required to determine the specific factors necessary to cause the previously reported changes.
