[Yield of bronchofibroscopy in patients with hemoptysis and normal thorax radiography].

The clinical histories of all the patients with hemoptysis who underwent bronchofibroscopy (FB) during a period of 5 years (1981-1986) are revised. 25.6% (108 out of 702) of the endoscopic explorations were performed in patients without radiographic pleuropulmonary abnormalities with a definite pathology. In these patients, an early FB results in one of the diagnostic methods with greatest sensibility to diagnose subglottic bleeding. Its efficacy decreases when it is performed more than 24 hours after the bleeding apparently ceased. As a whole, the side of the bleeding was determined in 50% of cases with the localization of the segment involved in 53 occasions (29.4%). In 8 (4.4%) patients, hemoptysis was secondary to bronchial neoplasia and in 6 of them FB contributed with the diagnostic material. In 5 cases hemoptysis lasted for more than seven days. Seven patients suffering bronchial carcinoma were males over 40 years old and smokers of more than 40 years/packet. There was a 77 year old female, with a carcinoid tumor who was a non smoker. In other 3 patients (2 suffering endobronchial angiomas and one case of tracheal bronchus ) the cause of bleeding could be determined with FB. We conclude that in patients presenting hemoptysis and normal chest x ray, FB is a useful procedure to localize the origin of bleeding and in a significant percentage of cases permits the etiological diagnosis.

[Current status of the emergency surgical management of closed trauma of the thorax].

The authors discuss problems related to emergency surgical management of non penetrating thoracic injuries. With reference to their own experience and current literature, they underline the etiologic, pathophysiologic, diagnostic and therapeutic features of the most frequent indications to the surgical treatment.

[Once again, the routine preoperative thorax photo].

A prospective study of the influence of radiological findings upon operation policy was carried out in 810 patients over 40 years of age who underwent elective non cardiopulmonary surgery. Only in 5 patients did it appear that a preoperative X-ray yielded any relevant extra information, and only in 3 cases was the operation planning influenced. If no preoperative chest X-rays has been taken, all except one of the significant abnormalities would have been discovered anyway. By differentiating between patients graded ASA 1 and those classed ASA 2 or higher, it would be possible to reduce the number of preoperative chest X-rays by 41% without any loss of precision. Preoperative chest X-rays in patients over 40 years of age scheduled for non-cardiopulmonary surgery can be restricted to those whose history requires further radiological analysis.

[Changes in the lungs in the early period of closed trauma of the thorax in rats].

In experiments on rats it has been demonstrated that in the acute period of uninjured parts of the lungs periodic changes of protein, lipid and glucose concentrations developed. In uninjured parts of the lungs one can observe more concentration of lipids and in injured parts-more glucose. The changes of substrates, concentrations in the lungs are due to not only the changes of venous blood excess in the lungs, but are connected with metabolic changes in the cells of the injured and uninjured parts of the lungs. The metabolic changes in the lungs in the acute period of chest trauma influence the creation of such indices of gas exchange as PaCO2 and PACO2.

[Closed thorax esophagectomy in caustic stenosis of the esophagus].

The technique of oesophagectomy without thoracotomy using a stripper for oesophageal eversion has been applied to 6 cases of caustic stenosis of oesophagus in infants. This technic has been realized 6 to 18 months after the initial lesion in 5 cases, 13 years in a case. There is neither morbidity nor mortality. This procedure seems well adapted to pediatric indications of oesophagectomy.

[Surgical treatment of the unstable thorax in respiratory insufficiency].

Sixteen patients with traumatic flail chest underwent surgical stabilization using procedures introduced by Rehbein, Judet and Vecsei. Compared with sixteen patients treated by internal stabilization with intermittent positive pressure respiration, there are some advantages: time of artificial respiration is shortened, early mobilization is possible, nursing is easier, the costs are lower. The procedure is indicated in those patients, in whom long-term artificial respiration is not necessary for other reasons.

[Recording of the his bundle potential at the surface of the thorax in healthy volunteers. Application to the study of propranolol].

New analogic-digital technology contributions to standard electrocardiography may allow to record the his bundle potential from the body surface. The aim of this work has been to study the validity of this method by its application to estimate the dromotropic effect of propranolol in healthy volunteers. His bundle potential from body surface was recorded in 19 out of 26 enrolled subjects. The negative dromotropic effect of propranolol (40 mg orally) was observed in only 10 volunteers. The direction of the PR and PH interval changes was used to estimate sensibility (0.9) and specificity (0.89) of this method. There was also a significant correlation (0.89 less than p 0.05) between PR and PH interval changes. This new non-invasive method could be used in clinical pharmacology but the subjects must be preselected.
