[Importance of oral and dental care in home medical care].

Recently, the importance of home medical care has been stressed. But patients, those assisting them and home care nurses do not attach much importance to oral and dental care in home medical care. When we visit patients who have asked for dental treatment through home care nurses, they appeared to have very poor oral hygiene; cavities were uncared for, dental plaque and dental calculus built up, and they used ill-fitting dentures. Through home dental care visits, it was not just the dental treatment but advice on dental hygiene management and diet that helped patients regain their appetite and bring back a smile to their face, while giving a rhythm to their life. This created a certain human feeling and brought about better patient quality of life (QOL), while at the same time helping to improve the QOL of the family as well.

Microcephaly, macrotia, unusual mimics and mental retardation syndrome: new syndrome or variant of De Lange type 2 syndrome.

We report a boy who shows a severe microcephaly, with mild mental retardation and hypotonia, and a dysmorphic facies: (flat profile, arched eyebrows, mild ptosis, short nose with raised basis, large tip and anteverted nares, long, smooth philtrum, narrow mouth with down turned corners, very large, backward tilted ears, with a prominent lobule, retrognathism and very small and widely spaced, although normally shaped teeth. Vesicoureteral reflux was present. The mother showed similar aspect, large ears, and a grinning smile. This appear to represent an undescribed phenotype which share some resemblance to mild Cornelia de Lange and Kabuki syndromes.

Advanced instant photography.

With the popularization of cosmetic and esthetic dental procedures, the importance of various photographic techniques has increased. The dentist's choice of a particular system depends on whether the intended use of the dental image is to assist in treatment acceptance, or to use the patient's "initial phase of excitement" to contribute to photomarketing. This paper uses before-and-after photos to demonstrate a dramatic alteration of the patient's smile. Photomarketing may be enhanced through the introduction of instant cameras, including the Polaroid Macro 5 SLR.

Vital bleaching for aesthetic improvement.

Vital bleaching is one of the least invasive, most conservative, and most cost-effective procedures to dramatically improve the aesthetic appearance of a patient's smile and self-confidence. Bleaching began as an in-office procedure and required multiple visits. With the arrival of dentist-monitored at-home procedures, bleaching has become widely known and used. This presentation evaluates the advantages of the higher viscosity adhesive formulation bleaching materials, which prevent dehydration and reduce the subsequent color change upon rehydration.

The typical pattern of superficial punctate keratopathy in wearers of extended wear disposable contact lenses.

PURPOSE: We investigated both the incidence and types of corneal epithelial complications associated with extended wear of the Acuvue disposable soft contact lens, in particular with respect to risk factors for the typical pattern of superficial punctate keratopathy (SPK). METHODS: We conducted a retrospective study of 5,478 eyes. Clinical characteristics of the corneal epithelial complications, tear volumes, and patient compliance were analyzed. RESULTS: A total of 3.6% (197 eyes/5,478 eyes) were observed to have a corneal epithelial complication on at least one examination during the entire follow-up period. Among the 197 eyes, 112 eyes were observed to have a complication during the initial 2 weeks of trial lens use. This included nine eyes that developed SPK with only daily wear use of the lenses. Sixty-eight eyes were beginners with no previous contact lens experience. SPK was observed in 193 eyes and corneal infiltrates in four eyes, although there were no cases of corneal epithelial erosion or corneal ulcer. With respect to type of SPK, a "smile mark" pattern confined to the area below the pupil was observed in 179 eyes (3.3%). CONCLUSIONS: The typical pattern of SPK was observed in 3.3% of users of extended wear disposable soft contact lenses, although the condition usually improved with the use of artificial tears.

The maxillary anterior single tooth implant aesthetic-health compromise.

With implant function established as a predictable treatment modality, the emphasis during the last decade has shifted to aesthetic of the restoration, particularly in the maxillary anterior area, sometimes to the extent of compromise of hygiene in the cervical region. The author reviews the aesthetics-hygiene compromise, in the context of long-term maintenance and survival, and suggests a compromise in cervical aesthetics in "unaesthetic" areas and even in the maxillary anterior if the cervical region is not exposed with broad smile. The presentation is a reminder that aesthetics is not the only criteria for an implant crown. The aesthetic-health compromise must be considered and understood prior to the treatment so that the most appropriate treatment plan can be developed.

Treatment of severe physiologic gingival pigmentation with free gingival autograft.

Melanin is normally found in the skin of all people, and normal pigmentation of the intraoral tissues is a relatively frequent finding. The highest rate of gingival pigmentation has been observed at the incisors. Until recently, however, very little attention has been paid to the esthetic treatment of severe gingival pigmentation. To answer the cosmetic demand for a pleasing smile, pigmented gingiva was replaced with an unpigmented free gingival autograft in 10 patients. At least two areas in each patient were grafted. In all 10 areas in which the recipient site received full-thickness bed preparation, no evidence of repigmentation was found after 4.5 years. Of the 10 areas that received partial-thickness bed preparation, only one exhibited repigmentation (after 1 year).

The value of IDPN as a supplemental therapy when elderly patients fail to thrive--two case studies.

On the last day of his life, H.J. came into the dialysis unit smiling and joking. He spent the time watching a movie and visiting with visitors and staff. He reported feeling better than he had in a long time. His predicted 3-day death watch had become nearly 5 months of improved quality of life and precious time spent with his large extended family. He frequently expressed gratitude for the chance to prolong those few days into such an unexpected extension. He and his family attributed this gift to the nursing care and encouragement he received in the dialysis unit. Everyone was aware that the IDPN therapy, along with improvement in his own daily nutritional intake, was a critical element. Later that night, H.J. had a cardiac arrest, and he died peacefully at home with his family. There is no doubt that without the financial resources available to him to pay for his IDPN, the outcome would not have been the same. As it was, all the intended patient outcomes were achieved.

Color communication, laboratory fabrication, and cementation.

Once considered to be revolutionary, porcelain veneers are now the foundation of most aesthetic dental practices. Requiring only conservative preparations, porcelain veneers can dramatically change a smile. This article discusses the technique necessary to prepare and place porcelain veneers, using a new pressed-ceramic system. Close and thorough cooperation and communication between the clinician and the laboratory are essential throughout the procedure, and this article is coauthored by a dentist and a ceramist.

Humour as a nursing intervention.

Extensive study of the brain has resulted in the development of interventions to augment and preserve function. However, there exists a phenomenon that affects the vital brain centres including speech and memory the control of vital functions, and the immune system. What has this power? Humour. Recognized as hazardous to illness, humour leads to laughing, smiling and good feelings. Humour has been described as a basic need, a coping mechanism, and a form of communication, all integral in the care of patients and families. The use of humour gives one the permission to laugh, and to relax. While the value of humour is unique to each person and culture, its deliberate use is an effective nursing intervention to decrease tension and anxiety. This paper will examine the use of humour as a nursing intervention in the care of neuroscience patients. It will look at using humour as an adjunct to pain control and an augment to the efficacy of treatment. Nurses will learn what constitutes therapeutic humour, and the pathways to humour. Participants will come away with a repertoire of strategies to incorporate humour in neurosciences nursing care infusing it with joy.

Evoking communication in Rett syndrome: comparisons with conversations and games in mother-infant interaction.

Girls with Rett syndrome retain a responsiveness with care-givers that corresponds in many respects with the preverbal communication observed with normal infants. This has characteristic rhythmic patterns and phrases, mutual imitation, reciprocal emotional phases and rudimentary oral, vocal and gestural expressions. After individuals with Rett syndrome have passed the critical stage in dissolution of their attention, co-ordination and voluntary control, they retain positive orientation to human faces and eyes with smiling. Video analyses show that they can engage with rhythms and phrases of conversation, sometimes showing a sense of humour and sensitivity to playful teasing. They respond to repeated patterns of expression in rhythmic/prosodic play and to certain forms in music. It is suggested that sensitive and appropriately attuned support for the rudimentary motives for human contact that survive in Rett syndrome can help stabilisation of self-regulatory states, alleviate panic and confusion and facilitate learning. The effects of the disorder may be a consequence of a genetic fault in the elaboration of an Intrinsic Motive Formation in the reticular core of the embryo brain, leading to dysregulation of differentiation in higher cognitive systems and learning, but leaving partially intact motive principles for human intersubjective response.

Anterior crowns in re-establishing vertical dimension of occlusion: overcoming fear of heights.

The rejuvenation of this dentition is initiated with the placement of six anterior crowns. Re-establishing the vertical dimension has allowed the restoration of pleasing proportions to the centrals. This has led to the re-assertion of the dominance of the centrals and thus corrected the reverse smile line. The restoration of these teeth demands harmonious accommodation of the requirements of esthetics, phonetics and function. These considerations are inseparable and are in fact, determinants of each other. Esthetics and phonetics help determine the degree of vertical opening and incisal edge placement. This location influences anterior guidance, lingual and labial contours, the pitch of the anterior teeth and proper lip support. The principles of proportion, so important in esthetics, allow us, to calculate incisor length. Thus, function, phonetics and esthetics are intimately intertwined and enhance one another in the restoration of vertical dimension of occlusion.

Oral and maxillofacial surgery: a new wrinkle.

A number of facial altering surgical procedures have been presented. Several areas are important to note. Most of these procedures were devised by practitioners lost to antiquity and individuals in a plethora of different fields, including orthopedics, dermatology, oral surgery, plastic surgery, and otolaryngology. Just as in the other specialties that perform these procedures, not all surgeons of the specialties perform the entire scope of their specialty. However, dentistry now has the capability to change the jaws, soft tissue drape of the face, neck, nose, and eyes, as well as smile lines and the shape and color of the dentition. No other profession in the history of human-kind has ever had this potential to make such facial changes.

Five- and eight-month-old infants recognize their faces and voices as familiar and social stimuli.

Five- and 8-month-old infants were presented with silent moving and static video images of self, peer, and doll and with sounds of self, peer, and nonsocial objects. In the visual conditions, infants at both ages showed a significant looking preference to peer over self when the faces were moving. When the faces were static, older infants showed the same significant discriminations, but the younger infants showed a significant looking preference for their own faces over peer and doll. These data suggest that recognition of one's own image develops through experience with dynamic facial stimulation during the first 8 months of life. In the auditory conditions, infants at both ages showed significant looking preferences for sounds of peer over self or nonsocial objects. In general, infants of both ages smiled and produced more vocalizations to social faces and social sounds than to nonsocial faces and nonsocial sounds. Thus, at 5 months infants treat their faces and voices as familiar and social stimuli. The findings forge important links among studies of self-perception, self-recognition, and social knowledge.

Case report: adult Class I, constricted arches, crowding and impacted cuspid.

The patient presented with constriction of both arches, moderate crowding and an unerupted and impacted maxillary cuspid. The Crozat appliance therapy efficiently developed the arches, reduced skeletal asymmetry on the sagittal plane, and distalized the upper left posteriors although this was aided by the removal of the second molar. Alignment and the development of a good functional occlusion was accomplished efficiently with the straight wire appliance. Facial balance and good lip support was maintained. No stripping was present and the periodontal health remains excellent. A significant improvement was made in the patient's smile. The teeth have been stabler during the two-year period following the initial placement of retainers. And the patient is very pleased with the treatment results.

The cosmetic practice: building from the ground up.

Is this practice management, patient relations, public relations or simply marketing? The name is insignificant as long as one realizes what it is and how it can work for each and every one of us. Call it what you will: practice management, patient relations, etc. Simply stated, marketing is a systematic approach to practice growth which consists of analysis, development of a philosophy or focus, strategies, techniques, and skills. The elements involved include image, identity, environment, and communication, both verbal and nonverbal. Using a structured program that takes into consideration each of these elements will educate patients regarding quality care. It will provide patients with expanded services. It will give patients alternative esthetic options, and give them something to smile about. It will change any practice into a growing and thriving cosmetic practice. One picture is worth a thousand words. The practices that say "cosmetic dentistry" will succeed in the 1990s.
