Novel intrapulmonary model for orthotopic propagation of human lung cancers in athymic nude mice.

A major impediment to the study of human lung cancer pathophysiology, as well as to the discovery and development of new specific antitumor agents for the treatment of lung cancer, has been the lack of appropriate experimental animal models. This paper describes a new model for the propagation of human lung tumor cells in the bronchioalveolar regions of the right lungs of athymic NCr-nu/nu mice via an intrabronchial (i.b.) implantation procedure. Over 1000 i.b. implantations have been performed to date, each requiring 3 to 5 min for completion and having a surgery-related mortality of approximately 5%. The model was used successfully for the orthotopic propagation of four established human lung cancer cell lines including: an adenosquamous cell carcinoma (NCI-H125); an adenocarcinoma (A549); a large cell undifferentiated carcinoma (NCI-H460), and a bronchioloalveolar cell carcinoma (NCI-H358). When each of the four cell lines was implanted i.b. using a 1.0 X 10(6) tumor cell inoculum, 100 +/- 0% (SD) tumor-related mortality was observed within 9 to 61 days. In contrast, when the conventional s.c. method for implantation was used at the same tumor cell inoculum, only minimal (2.5 +/- 5%) tumor-related mortality was observed within 140 days (P less than 0.001). Similarly, when a 1.0 X 10(5) or 1.0 X 10(4) cell inoculum was used, a dose-dependent, tumor-related mortality was observed when cells were implanted i.b. (56 +/- 24% or 25 +/- 17%) as compared with the s.c. method (5 +/- 5.7% or 0.0 +/- 0%) (P less than 0.02 and P less than 0.05, respectively). Most (greater than 90%) of the lung tumors propagated by i.b. implantation were localized to the right lung fields as documented by necropsy and/or high-resolution chest roentgenography techniques which were developed for these studies. The intrapulmonary model was also used for establishment and propagation of xenografts derived directly from enzymatically digested, fresh human lung tumor specimens obtained at the time of diagnostic thoracotomy and representing all four major lung cancer cell types as well as a bronchioloalveolar cell carcinoma. Approximately 35% (10 of 29) of the fresh primary human lung tumor specimens and 66% (2 of 3) of tumors metastatic to the lung were successfully propagated i.b. at a 1.0 X 10(6) tumor cell inoculum, whereas only 20% (1 of 5) of the specimens were successfully grown in vivo via the s.c. route from a 1.0 X 10(7) tumor cell inoculum.(ABSTRACT TRUNCATED AT 400 WORDS)

[Evaluation of the effectiveness of medical prevention of pneumoconiosis based on studies of its incidence in selected coal mines].

In a group of 3,938 persons--pit miners of 3 coal mines--employed over 8 years, full-size X-ray pictures of the chest have been taken and through a special questionnaire information has been collected on the length of employment, workstation, risk of dustiness, exposure to fibrogenic dusts outside the mine. Basing on the studies, the mean annual incidence of pneumoconiosis in particular mines has been calculated. The data has been compared with the mean annual incidence rate of pneumoconiosis calculated for 10 years prior to epidemiological studies and based on the primary selection by means of small-size pictures. This comparison indicated that the full-size pictures revealed 3-7 times more pneumoconioses as compared to the hitherto applied system. The comparison of full-size and small-size X-ray pictures demonstrated that the lesions visible in full-size pictures have not been registered in small-size pictures in as many as half of the cases. The obtained results indicate that the effectiveness of the hitherto applied system of medical prevention of pneumoconiosis is insufficient. A change in this system is indispensable; miniature roentgenography should be replaced by full-size X-ray pictures.

[Present status and place of sialography].

Roentgenography without contrast media and sialography, as well as the basic facts concerning the radiodiagnostic of the salivary gland, are all discussed with regard to the most current equipment, examination techniques, the positioning of patients, and the problems involved in contrast media. The technique used in more than 4,000 sialograms is recommended. The diagnostic value of this method is illustrated by means of representative patient series: sialolithiasis, inflammatory disorders, and tumors. The limitations of the method are demonstrated and complementary examinations shown.

[A case of recurrent gastric cancer successfully treated with a combination of cisplatin and carmofur].

A 51-year-old man with recurrent gastric cancer was treated by combined administration of Cisplatin and Carmofur. The target sites were the abdominal lymph nodes and the area of invasion to the stomach. Cisplatin (50 mg/body/day) was given for 3 days, while Carmofur (400-800 mg/body/day) was administered daily in 1 course. After 1 course of administration, the target tumor was reduced in size and the therapy was continued. A complete response was confirmed by upper gastrointestinal roentgenography, endoscopy and echography after 8 courses of Cisplatin administration. The patient has survived for 2 years 6 months in a state of CR. This case suggests that a combination therapy of Cisplatin and Fluoropyrimidine derivatives might be effective for gastric cancer.

Occult sinus abnormalities in the asthmatic patient.

Unrecognized sinusitis has been cited by others as a significant unrecognized factor in patients with asthma, chronic rhinitis, and urticaria. The current study examines the utility of roentgenography of the paranasal sinuses in such patients. Forty-seven percent of patients with flare-ups of asthma had abnormal sinus roentgenograms, a highly significant difference from the 29% prevalence in patients presenting with complaints of rhinitis. The patient's clinical history was most often not helpful in predicting whether or not roentgenographic abnormalities of the sinuses would be detected. Since maxillary sinusitis is the most frequent finding, the erect Waters' view is the most useful single roentgenogram.

Endobronchial carbon-dioxide laser therapy.

Twenty-four patients underwent single and multiple carbon-dioxide laser treatments for endobronchial lesions at the Mount Sinai Hospital in Toronto. Fifteen had malignant disease, 2 had benign tumours and 7 had benign strictures. Rigid bronchoscopy with Venturi ventilation was used in all, with the CO2 laser therapy directed down the open end of the bronchoscope. In the majority of cases laser therapy was combined with mechanical débridement, consisting of removal with biopsy forceps and use of the end of the rigid bronchoscope to "core out" the tumour mass. Assessments were made subjectively by the patient and objectively by bronchoscopic examination and post-treatment roentgenography. Worthwhile improvement was noted in patients who had proximal malignant lesions and those with benign lesions at all levels. Overall there was an effective improvement in 18 of the 24 patients.

Colonoscopic assessment of radiologic strictures of the colon.

The authors studied 47 patients with radiologic evidence of stricture to determine whether colonoscopy in this situation increased the accuracy of diagnosis and how this procedure might influence patient management. Colonoscopy increased to 81% the diagnostic accuracy, which was established at 38% with the single-contrast enema. Failure of both techniques to establish a precise diagnosis was 19%. From the clinical and radiologic data, surgical exploration for the 47 strictures would have been necessary in 28, not indicated in 9, and indicated solely for diagnostic purposes in 10. The addition of colonoscopy prevented unnecessary laparotomy in 5 (18%) of the first group and in 8 (80%) of the last. The authors conclude that colonoscopy should be complementary to the standard radiologic examination for colonic strictures, especially when carcinoma is suspected or if roentgenography does not provide a definitive diagnosis.

[Hemomicrocirculatory bed of the muscles of the shoulder and forearm].

By means of classical anatomical techniques: injection of contrast masses into the vascular network, macro-microscopic preparation, translucency, roentgenography, and some histological techniques, peculiarities of the hemomicrocirculatory bed in muscles of the human arm and forearm have been revealed. Small arteries of the 3d-4th order run along the muscle fiber fasciculi. In the center of the 2d order muscle fasciculus, in its internal perimysium, arteriole and venule (or 2 venules) run; from them into the 1st order fasciculus, precapillary arterioles and postcapillary venules, connected by means of capillaries, run. The arteriole and the venule, accompanying it, together with the precapillary arterioles and postcapillary venules, branching off them, form a unit of the microcirculatory bed of the arm and forearm muscles (module). Well developed intramuscular arterial anastomoses, presence of isolated structural-functional units of the hemomicrocirculatory bed ensure functional prosperity of the human muscles.

Long-term inversion stability of the ankle after rupture of the lateral ligaments.

The late results of surgical and nonsurgical treatment of recent ruptures of the lateral ligaments of the ankle are compared in two groups of 52 patients. By means of a questionnaire, physical examination and inversion stress roentgenography (at an average of three years after the injury), the functional and mechanical stability of the ankle were determined. After both surgical repair and treatment with plaster cast alone, more than 90% of patients achieved inversion stability of the ankle.

The virgin, modified, human, umbilical vein graft: morphologic characteristics and mechanical properties.

The authors used macrophotography, endoscopy, roentgenography, light microscopy, and transmission and scanning electron microscopy to assess the morphologic characteristics of 37 virgin, human, umbilical vein grafts. The specimens showed deep longitudinal folds (22%) and multiple transverse folds of the intimal surface (27%), irregularity of wall thickness (41%) and intimal breakdown exposing thrombogenic layers of the vein wall to blood (19%). The mechanical properties also studied were dynamic compliance, Young's modulus, breaking strength and breaking strain. The mean dynamic compliance was 6.44 X 10(-4) mm Hg-1 (SD = 2.02 X 10(-4) mm Hg-1, i.e., 31%), which is in agreement with results of others. The specimens showed great variability in Young's modulus, breaking strength and breaking strain. The structural deficiencies and variable mechanical properties of human umbilical vein grafts require investigation to determine their effect on patency rates due to increased thrombogenicity and thus to establish whether there are potential hazards associated with their use.

[Roentgen morphology following interventions of the lumbar intervertebral disk].

We compared x-ray films of 204 patients, evaluating plain radiographs, computer tomograms and myelographies. This study showed that plain radiographs serve as a guide, enabling to localise the level at which bone defects have occurred as a result of surgery. Conventional tomography is important for detecting changes associated with postoperative discitis. In about 50% of the patients, computed tomography enables satisfactory indication for a second intervention if used together with plain roentgenography and clinical examination. Myelography remains an indicated procedure if the disease pattern is not clear and if findings are ambivalent.

[Pulmonary malignant lymphoma--a case report].

A case of pulmonary malignant lymphoma is reported. A 65-year-old woman had no complaint. But chest roentgenography and CT revealed an infiltrating shadow with marked airbronchogram in the left S3. TBLB revealed marked subepithelial lymphocytic infiltration of the bronchial wall. Because the shadow had gradually enlarged, malignancy was suspected and lobectomy was performed. The resected specimen showed a 60 X 56 X 30 mm greyish-yellow, medullary tumor with intrapulmonary metastasis. Microscopically, well-differentiated small lymphocytic tumor cells severely infiltrated the pulmonary parenchyma, pleura and bronchial cartilage. There was no lymph node metastasis or extrapulmonary involvement. Immunohistochemical study proved the monoclonality of the K chain. She was treated with chemotherapy postoperatively.

Selective operative cholangiography.

This is a study of 505 patients who underwent surgical treatment of the gallbladder. Of this group, 343 were operated upon without operative cholangiography with an incidence of retained stones of 0.03 per cent. One hundred and ten underwent operative cholangiography according to five indications. These are: stones in the common duct on ultrasound or roentgenography; obstructive jaundice; dilation of the common duct greater than 1.2 centimeters; previous cholangitis, and preoperative pancreatitis. The advantages of selective operative cholangiography versus routine cholangiography are discussed in detail. We concluded that the selective operative cholangiography should be the preferred procedure.

[Axial projections of the patella. Apropos of a new accessory].

The author explains how to use an accessory allowing axial patella roentgenography without patient helping with different angulations: 30 degrees, 60 degrees and 90 degrees. Radiographs are realised with low rate contraction of quadriceps and immobilisation, with two femoro-patella joints on the same view with simple table and moving X-ray tube.

Operative ultrasonography in the biliary tract during pregnancy.

Operative roentgenographic cholangiography entails problems during pregnancy because of the hazard of radiation to the fetus and mother. In 450 biliary tract operations in which real-time B-mode operative ultrasonography was performed, five were for diseases of the biliary tract during pregnancy. Operative ultrasound correctly revealed non-palpable stones in the gallbladder in one patient and in the common bile duct in three patients. Furthermore, operative ultrasound provided anatomic information, such as dilation or stenosis of the common bile duct. Of seven possible occasions in which roentgenography would customarily be expected to be performed, operative cholangiography was used only once. Thus, ultrasound significantly reduced the number of operative roentgenographic studies. Operative ultrasonography was not associated with any complications and significant time delays. It is concluded that operative ultrasonography can replace operative roentgenographic cholangiography as a screening procedure for gallbladder and common duct calculi at biliary tract operations during pregnancy.

[Method of determining the quality of irrigation of the vessels of parenchymatous organs before injection].

The method suggested for qualitative estimation of the renal and hepatic vessels perfusion before an injection is based on measuring oscillations of the light beam diffused by the organ's surface (the source--a gas laser of continuous action of red spectrum). An absolute photometer protected by a red filter from external effects of light is used as a registrator. Successive injection of the renal and hepatic vessels with different masses and their investigation by means of roentgenography, preparation, carrosion demonstrate a satisfactory effectiveness of the method presented.
