[Clinical course and surgical treatment of complex forms of recurrent acute paraproctitis].

Clinical course and results of surgical treatment of 94 patients with complex forms of recurrent acute paraproctitis were analysed. Radical surgical intervention was conducted in 68 patients, of whom in 35--using the pararectal abscess opening with cryptectomy, in 17--with translocation of a distal rectum mucosa, in 16--with a ligature conduction through internal aperture of fistula. One-nine year follow-up of 89 patients is available. After radical intervention the recurrence and postoperative complications had occurred in 19%, after nonradical one--in all the patients.

[Clinico-radiobiological evaluation of radiation dosage and complications of radiotherapy for endometrial neoplasm metastasis to the vagina].

The data on 132 patients with metastases of endometrial carcinoma into the vagina (MECV) were analysed. They had received complex (remote + contact) and contact radiotherapy alone with low-, medium- and high dosage. The following radiobiological models were employed to assess tissue response: time-dose-fractionation factor (TDFF), cumulative radiation effect (CRE) and linear-quadratic dose effect equation (LQDE) ("extrapolated dose of response"). There was no correlation between survival and dose in excess of connective tissue tolerance calculated for each radiobiological model or a dose below tolerance limit. When dosage exceeded tolerance limit the incidence of early- and late-onset radiation injuries increased significantly. In MECV patients who had received primarily radiotherapy and surgery, early-onset radiation injuries occurred in cases of overdosage from the most recent exposure. When high-dose brachiatherapy was employed as a component of complex treatment, late-onset moist epithelite of the vagina could develop, irrespective of whether radiotherapy had been given or not and whether tolerance limit had been exceeded. Late-onset injuries were recorded in cases of over-dosage who had received both radiation and surgery for primary endometrial carcinoma.

Metastatic ulcerative penile Crohn's disease.

Metastatic Crohn's disease (MCD) is a cutaneous granulomatous reaction which affects patients with bowel disease in areas of the skin distant from the affected bowel. Penile involvement is a very uncommon observation. We report a case of penile ulcerative MCD in a 37-year-old man, which requested surgical management. The lesion developed during reactivation of the bowel disease.

Colonoscopy. A review.

The technique of colonoscopy has introduced an exciting new modality for the diagnosis and treatment of diseases of the colon. It is not a panacrea, for, as with any technique, limitations are always present; but it offers the single most far reaching advance for the diagnosis and treatment of diseases of the colon that has been offered in the last decade. The next 10 years should indeed prove exciting.

[Treatment of acute paraproctitis by latex ligature method].

A new procedure of operation in acute paraproctitis with transsphincteral purulent canal is proposed. The method is based on deferral transection of sphincter's portion by latex ligature, which ensure minimal diastasis of muscular ends and prevents the development of anal sphincter insufficiency. 35 patients with different forms of transsphincteral purulent canal were operated by this technique. There were other recurrences or anal sphincter insufficiency.

[Ambulatory surgical interventions in patients with proctological diseases].

The results of 629 operations for the most frequent proctological diseases (hemorrhoid, anal scissure, rectal fistula, acute paraproctitis) were analyzed. 134 operations were performed in outpatient clinic, 225--in one-day hospital and 270 (control group)--in coloproctology department. The place of operation did not influence postoperative period and temporary disability time. The mean stay of patient in outpatient department and hospital surgical unit was 3.8 and 324 hours, respectively. Since the second day after operation in specialized department it is possible to perform therapeutic manipulations outpatient in coloproctology room without negative sequences for patient's health. Cost of the ambulatory operation constituted 28.4% of hospital stay for patients with similar interventions. Surgical treatment of the above diseases could be performed in outpatients setfing in more than 50% patients. The bed resources must be used for patients who need long-term and intensive treatment in specialized department.

[Lesions around the rectum. Diagnosing and treatment abscesses and fistulas].

Anorectal abscesses and fistulas are among the most common anorectal lesions. They represent different phases of one and the same condition, the abscess being the acute manifestation, the fistula the chronic form. In the event of a strong suspicion, rapid referral to hospital, preferably one specializing in proctology, is indicated. The treatment of choice of both abscesses and fistulas is surgery. Abscesses are widely incised and decompressed. Fistulas showing signs of fresh secretion may be treated initially by the insertion of a thread, and subsequently undergo definitive surgical treatment. The surgical procedure is determined by the site of the fistula. Attendant risks include recurrence and incontinence. When fistulas occur in Crohn's disease, the indication for surgical treatment must be stringently applied. In the absence of complications, postoperative healing of the wound should be checked regularly.
