[Genotypic specificity of schizophrenic psychoses].

A total of 610 probands with a disease manifestation in childhood, middle and old age and their families were examined by the clinico-genealogical method. The results allowed conclusions that (1) there is an undoubted genetic relationship between schizophrenia of childhood, middle and old age; and that (2) among the closest relatives in families of the probands there is no significant (in comparison to the general population) accumulation of non-schizophrenic pathology. The latter indicates a high genotypic specificity of schizophrenia.

[Constitutional-somatotopic factors in the clinical picture of alcoholic psychoses and obvious signs of paranoid schizophrenia].

The clinical and anthropometrical methods were used to study the constitutional traits of 240 patients with psychotic forms of alcoholism and 300 patients with paranoid schizophrenia. The somatotypes were assessed by the Rees-Eysenck method. The prepsychotic parameters of alcoholism and the structure of alcoholic delirium were studied in 78 patients, as well as the manifest symptoms of paranoid schizophrenia. The study demonstrated a syndromal modified expressivity in exo- and endogenous processes, depending upon the somatotypical constitution. Simple syndromes were more frequently found in patients with a pyknic somatotype, and complicated in normosthenical and asthenic types of body built. It is suggested that the constitutional and somatotypical factors be included into the multifactorial analysis of the syndrome structure and prognosis in the development of the psychosis.

[The effect of hallucinogenic drugs on the development of psychoses].

Psychotic behavior and suicide which occurred after it induced us to study a connections among LSD use, psychotic behavior and suicidal risk ones again. Hereby we have a case of a young man who showed a psychotic behavior a week after taking LSD which ended tragically with suicide. What was the cause of this action, was it a late consequence of drug intake, flush-back or the first episode of a dissociative process which possibly started by change of behavior and sphere of interest? It is hard to answer this question by psychological autopsy. The dilemma remains constant, but we still think that this case report is of importance because we must not neglect such a clinical picture after taking LSD. It is particularly important now as drug addition is abruptly increasing in our society.

[Forensic psychiatric expert evaluation of so-called drug-induced psychoses].

A considerable number of schizophrenic persons also abuse illegal drugs. This comorbidity raises problems of diagnosis which become particularly important when concerning forensic questions. The hypothesis of a distinct psychopathological syndrome of drug induced psychosis is widely criticized in the literature. A case report is presented to demonstrate the problems of differential diagnoses and the difficulties for psychiatric expertizing arising from these problems.

[Patterns of the syndrome transformations in recurrences of alcoholic psychoses (clinico-statistical research)].

Psychopathological aspects of recurrent attacks of alcoholic psychosis has been investigated. The article presents results of examination of 689 patients with alcoholic psychosis (407 with delirium, 189 with hallucinations, 93-paranoids), who have suffered 2093 psychotic attacks within 10 recurrences. It was found, that change of syndromes from the 1st to the 10th attack followed in a declining order and the character of syndromologic transformation was steadily-advanced. Two variants of transformation of alcoholic psychosis were revealed: the tendency to preservation of an initial clinical form during the following recurrent attacks and the tendency to changing psychopathologic syndrome.

[Mefloquine-induced psychoses--problems in etiologic classification based on 2 case reports].

Psychiatric disorders following treatment with Mefloquine were described several times since the introduction of this antimalarial drug in Germany in 1985. On the basis of two case reports problems of the diagnosis and of the correlation of psychiatric disorders with the medication are discussed. Particularly are an analysis of the development of the ADR and a comparison of the psychiatric symptoms with known ADR-cases and with other psychiatric diseases important. This seems to be significant for an optimal treatment as well as for the assessment of the prognosis.

[Glutamatergic hypothesis of schizophrenia: psychoses induced by phencyclidine and cortical-subcortical imbalance].

Glutamatergic hypothesis stemmed from the observation of phencyclidine-induced psychosis. Phencyclidine is able to induce in healthy subjects negative and positive schizophrenic-like symptoms, as well as thought disorganization. Phencyclidine acts as an antagonist of NMDA receptor, one of the glutamatergic receptors. Experimental studies in animals have demonstrated that compartmental effect of phencyclidine is due to its action at striatal level, allowing the disinhibition of down-stream structures. The organization of the two striato-thalamocortical loops, which exert, respectively, a positive and negative retro-control on cortical activity, may explain how a glutamatergic deficiency induces both positive and negative symptoms. Positive symptoms could also be due to a secondary hyperdopaminergia, since a part of striatum, the striosomes, connected with limbic system, control the activity of dopaminergic neurons. This model validates the hypothesis that a single anomaly can lead to the different symptomatic dimensions of schizophrenia and supports the implication of basal ganglia in the expression of mental disease.

[Neuropsychological parameters of droperidol-induced psychoses].

To each of twelve normal subjects 140 to 200 mg of droperidol were administered. The psychopathological (temporary hallucinatory syndrome), electroencephalographic (distinct posterior theta groups), electromyographic and electroneurographic (H amplitude depression), as well as electrocardiographic (tachycardia, extrasystoles) results are discussed with reference to possible mechanisms of action.

[Marihuana psychoses].

Five chronic consumers of marihuana (through periods of 1 to 3 years) were studied and given a complete set of psychological tests (Rorschach, Wechsler, Phillipson, Psicodiagnóstico Miokinético). Brief case histories and tests results are presented for each subject. In all cases, marked depression, and anxiety, schizoid features, and poor control were detected. In four patients self-aggresion, inversion of sleep cycle, weakness of will and mental confusion were registered. In four subjects a definite psychotic profile was detected through Rorschach tests. Three patients suffered from visual hallucinations, and trends to sexual promiscuity. Memory failure and gaps were apparent in two cases. One patient developed a short-lasting paranoid delusion. Two patients showed no sign of abnormality in their E.E.G. The syndrome shows that marihuana abuse may lead to toxic psychosis similar to schizophrenia. The importance of a systematic use of complete sets of psychological tests is discussed, although their value for a differential diagnosis between toxic marihuana-psychosis and schizophrenia remains to be established. The need for careful search on marihuana abuse is emphasized. Regarding the physiopathology of marihuana-psychosis, some recent findings of neurochemistry are discussed, in particular those of biological theories of schizophrenia. This approach allows a parallel between both disorders. The possibility of both being accounted for by a common metabolic defect is suggested. Finally, the current contention that marihuana is harmless or innocuous is called to revision.
