[Ultrasonography in the diagnosis of testicular tumors].

Ultrasonographic examination of scrotum has become a stable part of urological practice. A palpation finding of scrotum or a condition when it cannot be examined by palpation represent the most frequent indications, followed by search for primary tumour, etc. The technique of water sac is useful and rapid. The examination may differentiate intratesticular processes, mostly of tumorous origin, e.g. seminoma, embryonal carcinoma and teratocarcinoma from extratesticular processes, e.g. a large hydrocoele, hematocoele, spermatocoele, epididymal cyst, orchitis, etc. Even though the ultrasonographic picture of the testis tumours are not specific, there is a possibility to hypothesize their histological structure in some cases. Ultrasonographic examination represents an unquestionable diagnostic progress.

[Pilomatrixoma. A series of 18 surgically treated cases. A sign for its diagnosis].

We present a personal casuistic of 18 cases of pilomatrixoma. We make a reference of the clinical and histological situation of our patients. We remind that even if it isn't a frequent pathology, it possesses however, in almost every cases, certain characteristics very typical that permit to arrive to a correct diagnosis. We bring out a sign that made possible this diagnosis: the palpation of a tumor with a rocky consistency where united planes forming salient angles like a stone with many facets are evident: "Faceted stone sign". All the cases were treated surgically with complete extirpation of the tumor and up to now we haven't observed any relapses. The palpation of the sign of the "faceted stone" is pathognomonic of pilomatrixoma.

[Limitations in diagnosing laryngeal lesions by physical inspection as against achieving diagnoses histologically].

In diagnosing a lesion in the ENT area, an inspection that includes a physical examination and palpation is important. For a laryngeal lesion, however, the physical examination inspection becomes especially important, since palpation cannot be done. In most instance, the macroscopic diagnosis corresponds well with the findings uncovered by the histological diagnosis. However, we have found that a histological examination is crucial in diagnosing cases of a "White-pathy" lesion in the larynx, to confirm if the lesion is a so-called leukoplakia, a pre-cancerous manifestation, or an early cancer.

Congenital inguinal hernias associated with a rent in the common vaginal tunic in five foals.

Five foals were admitted because of colic that developed within 24 hours after birth. Physical examination revealed swelling from the inguinal region to the cranial aspect of the prepuce. Through the skin overlying the swelling, distended loops of bowel could be palpated. The hernias could be reduced with the foals restrained in dorsal recumbency. Surgical exploration showed the swelling to be an inguinal hernia with small bowel herniation through a rent in the common vaginal tunic. Concurrent rupture of the vaginal tunic should be suspected when a congenital inguinal hernia is associated with colic, swelling that extends to the prepuce, or intestine that is easily palpated within the hernial swelling. Repeated manual reduction in these cases would be of little benefit and surgical intervention would be recommended.

Surgical treatment of sand colic in equids: 48 cases (1978-1985).

Medical records of 48 equids (47 horses, 1 pony) with surgical sand colic were reviewed. The diagnosis of sand colic was made if a sand impaction(s) was palpated during exploratory abdominal surgery or if a large quantity of sand was found during colotomy. Most equids did not experience a previous episode of sand diarrhea or sand colic. Clinical findings and results of clinicopathologic determinations were not diagnostic. Rectal palpation findings in 40 of 46 horses were compatible with large-colon and/or cecal distention. Impactions were palpable per rectum in only 7 horses, but emergency abdominal surgery revealed one or more impactions in 46 of 48 equids. Single impaction most commonly was observed at the pelvic flexure. Twenty-six horses had concurrent large-colon torsion or displacement. Pelvic flexure colotomy was performed in 44 horses. Of 48 equids, 44 survived and were discharged.

[The value of clinical aspects, laboratory and circulatory parameters in blunt abdominal trauma].

Within seven years 506 patients with blunt abdominal trauma were included into a prospective trial. The aim of the study was checking of the validity of clinical parameters, routinely performed laboratory examinations and of the initial circulatory situation in relation to an abdominal organ lesion. Three groups were separated out of the total collective: Group 1: Patients without abdominal lesion (N = 274). Group 2: Patients with abdominal lesion, verified by operation, sonography or CAT scan (N = 232). Group 3: Patients with rupture of the spleen (N = 107) (subgroup of 2). Among the clinical parameters: spontaneous abdominal pain, contusions marks, abdominal tenderness, shoulder pain, and abdominal palpation, the latter does have a high validity (92%). However, in group 1, more than half of the cases also had palpation pain. Shoulder pain has a high sensitivity. Of the laboratory parameters: hemoglobin, hematocrit and leucocytes, only the leucocyte count provided a certain importance: 83% of group 2 had values above 10,000. The circulatory parameters blood pressure and pulse as initial spot picture are of minor validity. Continuous registration of these values at clinical observation has much higher relevance indicating trends towards improvement or deterioration.

[A case of retrovesical abscess due to Crohn's disease].

A 29-year-old man was admitted with pain on urination and high grade fever-up. Mass was palpated above the right lobe of prostate. Cystoscopy revealed an edematous region in the upward to the right ureteral orifice. CT revealed the retrovesical mass in the same region. The surgical specimen obtained by transurethral resection showed severe inflammatory changes, but no malignancy was found. Antimicrobial chemotherapy had been continued, but the mass did not disappear on palpation and computed tomography and cystoscopy revealed pus-discharge from the center of the edematous region was found. An operation was performed under the diagnosis of retrovesical abscess. The terminal ileum had formed adhesion to the posterior bladder wall. Segmental resection of the ileum and partial resection of involved segment of the bladder were performed. The pathological diagnosis was Crohn's disease. Inflammation of ileum seemed to infiltrate the bladder wall and formed an abscess. After the operation symptoms disappeared.

[Regional neck dissection in carcinoma of the cervical esophagus].

The metastatic spread in the neck of carcinoma of the cervical esophagus was studied in 10 patients from 1970 to 1985. The recurrent chain was involved in 6 patients: 2, unilateral and 4, bilateral. The metastatic nodes in each patients were counted 1, 2, 2, 2, 4 and 6, 17 in total. Their sizes ranged from 5 mm to 15 mm. However, they were not palpable preoperatively. Therefore, this area always should be dissected. The internal jugular chain including supraclavicular nodes had seven metastatic nodes in 6 patients; only one node, 3 cm in diameter, palpated preoperatively; 3 nodes, 2.5 cm, 6 mm and 5 mm, detected microscopically; and 3 nodes, 4 cm, 3 cm, a tip of small finger, palpated by clinical emergence in course of follow-up. Seven nodes distributed 0 in upper jugular, 3 in mid jugular (one node in the level of carotid bulbus was the highest one), 1 in lower jugular and 3 in supraclavicular. From this data the author feel that upper jugular nodes need not be dissected in the elective neck dissection for carcinoma of the cervical esophagus. The author recommend elective procedure for negative neck since 2 out of 4 patients developed positive nodes later in undissected necks, and 2 out of 4 patients revealed microscopically positive nodes in electively dissected necks. It is interesting that the rate of metastasis in jugular chain is high (5/6) in patients who have positive nodes in recurrent chain. There were two unusual sites of metastasis; one Delphian node in patient whose primary lesion invaded to thyroid gland, and pretracheal as well as upper mediastinal nodes in patient who had 6 positive nodes in recurrent chain and one positive node in jugular chain.

[Clinical studies against chronic prostatitis and prostatitis-like syndrome (1). Clinico-statistical observation].

We clinically evaluated 605 cases of chronic prostatis and other resembling disorders. The incidence was 4.34% of all out-patients (7.54% of male patients). The average age was 38.1 years, and many were adolescents. A greater number of out-patients were seen in October, November, and December (p less than 0.01) and June (p less than 0.05). The type of disease was chronic prostatitis in 48.1% (bacterial in 5.3%, non-bacterial in 42.8%) and prostatitis-like syndrome in 51.9%. The incidence among male patients was 3.63% for chronic prostatitis (bacterial in 0.40%, non-bacterial in 3.23%), and 3.91% for prostatitis-like syndrome. In the case of chronic prostatis, the abnormal finding rate (p less than 0.01), tenderness (p less than 0.01), induration (p less than 0.05) on prostatic palpation, non-pretreated rate (p less than 0.01), urological past history (p less than 0.05) except prostatitis and urological complications (p less than 0.01) were significantly higher. On the other hand, the negative rate on urine culture (p less than 0.01), no abnormal findings on palpation (p less than 0.01), discomfort (p less than 0.01), discomfort with tenderness (p less than 0.01) and past prostatitis episodes (p less than 0.01) were seen significantly more in the case of prostatitis-like syndrome. In conclusion, chronic prostatitis with inflammatory change on prostate and prostatitis-like syndrome without inflammation have similar clinical features, but should be diagnosed as totally different types of disease.

Correlation of the presence of seminal white blood cells and the prevalence of separated spermatozoal heads with subclinical Brucella ovis infection in rams.

A breeding soundness evaluation was conducted on 824 Colorado range rams. These rams were determined to be free from epididymitis via testicular palpation. Semen evaluation included microscopic observation for the presence of WBC. Of the 824 rams, 15.5% failed the breeding soundness evaluation on the basis of the semen evaluation: 10.6% had WBC in the semen and 4.9% had poor sperm morphology. The prevalence of Brucella ovis isolation varied from 0% to 16.2% within flocks. The prevalence of subclinical B ovis infection was 10% in the control flocks. Brucella ovis was isolated from 71.9% of the rams that had WBC in their semen. From this study, it appeared that palpation and vaccination may be inadequate for control of ram epididymitis.

[Transrectal prostatosonography. Follow-up in prostate cancer therapy].

Thirty-five cases of prostate carcinoma were followed after orchiectomy and during drug therapy using both ultrasound and digital palpation to measure the prostate. Following orchiectomy and drug therapy the prostate shrunk an average of one third within six months independent of the grading and staging of the carcinoma. The biggest shrinkage was observed in the first two months after therapy. There was agreement between transrectal ultrasonography (TPS) and digital palpation in judging the prostate capsule in two thirds of the cases. Ultrasonographically, a downstaging of the capsule in twenty per cent and an upstaging in fourteen per cent of the cases was found. Three quarters of the G II prostate carcinomas improved ultrasonographically in structure and capsule in the twelve month period of observation while two thirds of the G III prostate carcinomas did not improve. According to our results the routine use of TPS seems not to be indicated when only gross measurement is desired. However TPS is an objective and reliable method für testing the effects of medication on prostate carcinomas.

Congenital hypertrophic pyloric stenosis.

In a large metropolitan general hospital, a high incidence of congenital hypertrophic pyloric stenosis was noted in non-Caucasian groups. Bile-free emesis was consistently reported, and admission was frequently delayed. A prompt diagnosis following admission was not always possible. Unequivocal palpation of a right upper quadrant mass was successful in less than half of the patients in this series, and radiographic studies were helpful in establishing the proper diagnosis in the remainder. Liver fracture can occur with improper abdominal palpation techniques. Despite a surprisingly high complication rate, the ultimate result of operative therapy is uniformly excellent. Three patients not operated upon who were followed for more than two years still have evidence of gastric dysfunction. Postoperative emesis following adequate operation is not unusual, occurring approximately one-third of the time. When postoperative emesis is protracted, incomplete pyloromyotomy should be considered.

[Clinical course characteristics of polymyalgia arteritica. Study of 16 cases].

Clinical and biological characteristics are studied in 16 patients with polymyalgia arteritica. 12 of them were diagnosed by biopsy of the temporal artery and the other 4 because they presented clinical, biological data and a high response to corticosteroids. There were no differences according to sex. Most patients (75%) had symptoms since 1-6 months, headache being the most common (75%). Fever (56%), polymyalgia (50%), weight loss (37%), intermittent claudication, loss of vision and arthritis (12%) were the symptoms seen in these patients. ERS was high in all cases, hemoglobin was less than 8 g/100 ml in 8 cases and an increase of alfa-2-globuline was found in 8 patients. Temporal artery palpation was abnormal in 11 patients. Two of 5 patients who presented a normal arterial palpation had a positive biopsy. All patients received 6-metil-prednisolone. 3 are well after 3, 4 and 6 months therapy. 8 are also well but receiving small doses of steroids as treatment.

Effect of estrogen therapy on metastatic carcinoma of the prostate.

Forty-one patients with metastatic carcinoma of the prostate (stage IV) were treated with diethylstilbestrol or bilateral orchidectomy or both and followed for a period of two years. The effect of treatment was determined every six months and was based on the size and consistency of the primary lesions on rectal palpation, the effects on pain, obstructive symptoms, osseous metastases, level of serum prostatic acid phosphatase and on the overall clinical evaluation of the patient. Bilateral orchidectomy was as effective as a combination of bilateral orchidectomy and diethylstilbestrol therapy. Diethylstilbestrol given alone was less effective. The poorer results obtained were attributed to the failure of many patients to adhere strictly to their estrogen regimen. Rectal digital palpation of the prostate as well as an estimation of the level of serum prostatic acid phosphatase is recommended in developing countries for all male patients over 50 years of age seen at the hospital.

[Delayed skin sensitivity in cattle against adenovirus antigen].

An investigation was carried out concerning the allergic sensitivity of a delayed type with cows infected with adenoviruses. The experiments were made on 30 heads of cattle between the age of 3 and 6, 3 out of which by intradermal injecting of concentrated adenoviral antigen with serotype 1 in a quantity 0.2 cm3 into the neck, whereas on the opposite side an applied control antigen was used. The results were accounted for visually through palpation and measuring of the thickness of the skin fold before the experiment and 24, 48, 72 and 96 hours after the injection. As a positive skin allergic reaction was adopted that one during which a swelling was noticed in the spot of the injected antigen from a distance and through palpation it proved tempered and sore, whereas the difference in thickness of the skin fold before the experiment and 48 hours after the injection of the antigen was over 2.5 mm. With 17 cows out of the 30 treated cows changes were found proving a positive allergic reaction with a most pronounced character in the 48th hour. On the spot of application of the control antigen there were no data of a positive reaction. A certain parallelism was proved. A conclusion was drawn out that the skin allergy reaction could be used for a herd diagnosis of adenoviral infections with cows.

[The range of removing fatty tissue and lymph nodes by a modified radical mastectomy with preservation of the major and minor pectoral muscles].

Thirty five patients with mammary carcinoma were selected and completeness of lymph node dissection in modified radical mastectomy (MRM) was evaluated by two step procedures, i.e., MRM was performed at the first step and then standard radical mastectomy was added immediately following the first step. Complete axillary dissection was possible in all the cases with MRM, however, in some cases, excision of the other areas was insufficient, especially in an obese patient. There were number of cases in which the number of subclavicular lymph nodes and Halsted lymph nodes felt by palpation during the MRM operation did not correspond to the actual number of excised lymph nodes. MRM may be indicated in those cases where no metastasis is observed in the histological examination of the frozen section of axillary lymph nodes during operation and/or where there is no recognizable swelling of axillary lymph nodes under palpation when the size of breast lump is less than 2 cm in diameter.

Ceruloplasmin as a marker of neoplastic activity in rabbits bearing the VX-2 carcinoma.

The establishment of an experimental model with rabbits in which VX-2 carcinoma was implanted in the gastrocnemius muscle and subsequently successfully cured by a second tumor cell inoculation has been reported previously. Tumor growth and regression could be followed by manula palpation. The changes in serum ceruloplasmin (CP, EC 1.10.3.2) levels of individual rabbits during tumor development and regression were followed. CP levels increased 4- to 8-fold of normal during the progression of the malignant process, often before tumors could be detected by palpation. With tumor regression CP levels returned to normal. When metastasis developed, the CP levels remained high. This phenomenon seems to be related to the VX-2 carcinoma, since CP levels in rabbits challenged with various antigens and suffering from induced multiple s.c. abscesses did not change significantly, while in pregnant rabbits CP levels increased up to at most 3-fold. It is concluded that serum CP level can serve as a reliable biochemical marker of the activity of this malignant process. The practical application of this finding lies in the follow-up of malignant processes in humans and is now under investigation.

Effects of gonadotrophin releasing hormone on reproductive performance of dairy cows with retained placenta.

The effects of gonadotrophin releasing hormone (GnRH) on the reproductive performance of dairy cows with retained placenta were studied. Three hundred and seventy-eight cows diagnosed as having retained placenta received intramuscular injections of either 2 mL sterile water or 200 micrograms of GnRH in 2 mL sterile water between day 8 and day 14 postpartum. Rectal palpation was performed at the time of treatment and ten to 20 days after treatment in order to determine the rate of uterine involution. Thereafter, monthly rectal examinations were carried out until insemination. Pregnancy diagnosis was made by rectal palpation at 40 days or more after breeding. Using the entire experimental population, there were no significant differences between GnRH-treated and control cows for the rate of uterine involution, the occurrence of reproductive problems, the interval from parturition to first observed estrus, the interval from parturition to first insemination, the interval from parturition to conception, the number of services per conception, the total number of services per cow regardless of conception and the incidence of culling for infertility. When the data for herds in which breeding began earlier in the postpartum period (herds having a mean less than or equal to 80 days from parturition to first service for retained placenta cows) were considered, the GnRH treatment resulted in a significantly shorter (p less than or equal to 0.01) calving to conception interval as compared to control cows. Also, there was a significant reduction (p less than or equal to 0.05) in the total number of services per cow regardless of conception and a significant reduction in the interval from parturition to first service.(ABSTRACT TRUNCATED AT 250 WORDS)
