[Documentation of digital dacryocystography using video technique].

Digital dacryocystography is considered the optimal radiodiagnostic method in obstructions of the lacrimal pathways. The examination is carried out by a computer-controlled X-ray unit with a C-arc attached to an image-intensifying TV system. In this way the advantages of this technique and of modern video recording are linked. The bilateral filling process of the contrast medium is recorded and the findings simultaneously recorded on videotape. Consequently, pictures of the dynamic flow are retrievable at any time. This provides good diagnostic documentation and an outstanding basis for any scientific analysis.

[Computer documentation of allergological data on patients suffering from allergies of the ENT-tract (author's transl)].

After a brief discussion of the purpose of EDP in medicine an EDP system is described that serves the collection of data on patients of an allergy department. It was taken into account that in addition to data concerning case histories and the general and special status data on the different courses of therapies and test results can be collected. The system is specifically adapted to the problem of allergies of the respiratory tract, but may also be used--with minor additions or alterations--for internal or dermatological cases. The aim of this documentation is the systemization and at the same time the rationalization of the examination course, without, however, ruling out individual extensions. Such a system facilitates and accelerates not only the collection of data, it also allows a faster systematic disclosure of errors and makes a meaningful scientific evaluation possible.

[Development and initial results of AmBADO. Ambulatory basic documentation in outpatient psychiatric clinics of Bavarian district hospitals].

As of 1.1.2002 a new documentation system (AmBADO) has been introduced to the out-patient departments of all Bavarian psychiatric clinics. With the use of this new system, practitioners are for the first time allowed to collect data concerning structure, process, and outcome parameters in this field of care. A pilot study was conducted in 2000 before statewide introduction; this occurred at the "Atriumhaus", a psychiatric crisis- and treatment centre in Munich. Experiences gathered in this study will be presented and examples for possible benefits of using the new system will be discussed.

[Basic documentation in child and adolescent psychiatry: first evaluations and experiences].

A lightly modified version of the basic documentation of the Child and Adolescent Psychiatric Associations was introduced in three different clinics. Relevant items were analysed for about 5300 patients. The data of the 3 clinics were compared. There was a fair amount of agreement in the age structure and the distribution of the diagnoses between the clinics. A regular and detailed comparative analysis of the data will be developed as a measure of quality. The results of these analyses will be discussed in the 3 clinics and implemented in the daily routines.

[Researching documents on the history of hansen's disease in Brazil].

This article corresponds to part of the results of a research on leprosy-related sources developed in several institutions in the city of Rio de Janeiro. At Real Gabinete Português de Leitura, Arquivo Nacional and Biblioteca Nacional, banks, indexes, official documents and photos on the administration of leprosaria and articles on the treatment of the disease have been investigated. At Centro de Pesquisa e Documentação de História Contemporânea do Brazil (CPDOC-FGV), several files have been researched, mainly those with information on the health policies of the first Vargas administration (1930-1945). This research is part of the International Leprosy Association Global Project on the History of Leprosy. Its results can be accessed at the site http://www.leprosyhistory.org

A process for consolidation of redundant nursing documentation forms.

Form redundancy can lead to duplicate documentation which can be the source of patient and staff frustration, as well as data errors. A data-driven systematic process for consolidation of the multiple redundant forms used in various patient care areas across a health care delivery system is proposed and tested using the 21 intake forms currently used by a regional HMO.

A comparison of self-documentation in diabetics: electronic versus paper diaries.

Attention needs to be given to how patients can capitalize on the benefits of Personal Digital Assistant technology. The specific aims of this pilot study are to compare the efficacy of diabetic patients documenting their health maintenance data (i.e. blood glucose levels, meal intake, and exercise) using an electronic patient diary (ED) versus a traditional pen and paper (PD) diary; and determine participants' satisfaction with each diary approach.

[Photographic documentation of peritoneoscopic findings with the polaroid camera (author's transl)].

Photographic documentation of peritoneoscopic findings is technically well developped, relatively easy to perform, and should be used, as a routine procedure. There are different methods available. The polaroid method is the method of choice, when a paper copy with adequate technical quality is needed at once; this is especially useful for the pathologist, who may evaluate polaroid copies along with bioptic material. The surgeon as well may preoperatively get some visual information if emergency surgery is to be done. The endoscopist is responsible for the quality of the picture himself; a photo laboratory is not needed. Own results and technical procedures are demonstrated.

Professional DOC (PDOC): a software environment for authoring and consulting of electronic documentation. INTERACTIVE DIAGNOSIS and THERAPY: a medical application.

Professional DOC (PDOC) is a software environment for electronic documentation authoring and consulting based on the Hypermedia technology. It allows to manage different kinds of information (pictures, sounds, texts, graphics, video) organized according to hyperstructures. The user-friendliness and effectiveness of its man-machine interface, and its availability on the most common hardware platforms makes it a suitable product for a very broad category of users. INTERACTIVE DIAGNOSIS AND THERAPY is an electronic medical book designed to help general practitioners to make diagnosis and treat patients in their daily clinic practice.

Photographing pressure ulcers to enhance documentation.

Complete and careful observation, description and documentation of pressure ulcers is an essential component of practice when caring for people with pressure ulcers. Photographing pressure ulcers and including the photo as part of the patient's chart provides a useful adjunct to the written record. While a "picture is worth a thousand words," photographs can be misleading unless they are consistently taken according to specific guidelines. Techniques to enhance photographic results and avoid common errors are discussed.

Document classification and searching--a neural network approach.

This article introduces the basic concepts of Artificial Neural Networks and the related work that is currently being carried out at ESRIN in the field of document classification and searching. Some of the authors' ideas on the future directions that these techniques might take are also presented.

International Conference on Harmonisation; guidance on the M4 Common Technical Document--Quality: Questions and Answers/Location Issues; availability. Notice.

The Food and Drug Administration (FDA) is announcing the availability of a guidance entitled "M4: The CTD--Quality: Questions and Answers/Location Issues." The guidance was prepared under the auspices of the International Conference on Harmonisation of Technical Requirements for Registration of Pharmaceuticals for Human Use (ICH). This guidance provides further clarification for preparing the quality components of an application file in the common technical document (CTD) format. The guidance addresses the relationship between linked sections for certain parameters (such as polymorphism and particle size), and it addresses location issues (by indicating the section in which to place requested information). The guidance is intended to ease the preparation of paper and electronic submissions, facilitate regulatory reviews, and simplify the exchange of regulatory information among regulatory authorities.

Stop shuffling paper. Case study: document management offers efficiency, access.

Document image management (DIM) technology for patient charts can organize tasks, automate redundant chores, improve access to data and decrease error. DIM allows multiple users to share patient information and has audit and security features. The technology offers a strategic benefit for practices preparing for health care's ongoing transition from paper-based systems to electronic ones.

A one-step approach to data retrieval, analysis and documentation.

In this paper we propose a system architecture to support data retrieval from patient records, together with data analysis, and report generation. Moving from retrieval all the way to documentation should be done as in a pipeline, and computer support must minimize manual steps in this knowledge refinement scenario. A case study is drawn from urology for surgery on patients with lower urinary tract symptoms.

Step-by-step mark-up of medical guideline documents.

The quality of document-centric formalisation of medical guidelines can be improved using a decomposition of the whole process into several explicit steps. We present a methodology and a software tool supporting the step-by-step formalisation process. The knowledge elements can be marked up in the text with increasing level of detail, rearranged into an XML knowledge base and exported into the operational representation. Semi-automated transitions can be specified by means of rules. The approach has been tested in a hypertension application.

[Possibilities in clinical quality assurance founded on a basis documentation system and extended evaluation studies].

Although the topic of quality control (QC) has been under discussion at least since the "Health Reform Legislation" (GRG) in almost all areas of the public health system, there is quite often a lack of models for practicability. Even for inpatient medical rehabilitation--at least concerning empirically founded QC-measures suitable methods and instruments have still to be developed. These must be relatively simple and economical to be integrated in the daily routine within a special clinic, if quality control should function. Starting from a broad concept of evaluation, the authors present a practical empirical QC-model for clinics, specifically for psychosomatic rehabilitation, which has been developed and proven for more than five years. To investigate the "programme" of the clinic, or particular program components (e.g. patients, treatments, outcome criteria) with systematically obtained data and analysis, is the main goal of this model. Where statements are replaced by empirically proven data. Aspects of process and outcome quality are considered to be a main task, and transparency is a necessary supposition. This model is particularly based on questionnaire procedures, is two-tracked, e.g. it is composed by continuous data collection ("treatment documentation") on the one hand and by time-restricted and defined studies for programme evaluation on the other. The treatment results represent the true target variable.
