Outcomes research is the fifth discipline of the fifth generation of managed care: utility of a single troponin-T.

OBJECTIVE: The fifth generation of managed care is disease management. Diseases have measurable risk in providing laboratory and medical services. The link between managing services and managing risk can be aided by leveraging the laboratory. We wish to remodel laboratory services to fit the needs of the use, thereby using the laboratory for competitive advantage by redesigning a desired output using a formal structured process. Outcomes research is the systems framework for the remodeling process through the link of laboratory output to clinical and financial outcomes. A process redesign model connects the use of laboratory tests to improved medical services by leveraging resources to achieve measurable improvement over current results. This view of outcomes research seeks both competitive advantage and measurable improvements in quality. METHODOLOGY: This approach is illustrated by the patient presenting with chest pain (CP). A majority of the patients rule out for acute myocardial infarction (AMI), including patients with indigestion, shortness of breath, and other clinical findings. This is the basis for an emergency department (ED) CP observation unit to reduce coronary care unit admission rates. When the Goldman algorithm for discharging low-risk patients with CP from the ED using only clinical features and electrocardiographic findings proved difficult to implement, we turned to measuring the diagnostic efficiency of a new cardiac marker to replace the evolutionary changes in creatine kinase (CK) isoenzyme MB. The physicians making the decision were blinded to the results of the study. We fitted the expected characteristics of the test to the expected results for our program. The test was done on the presenting specimen of 293 evaluable patients with a median of 6.5 hours from the time of onset of CP to the time the specimen was drawn. The result was compared with the evolutionary pattern of CK-MB. RESULTS: The sensitivity of the test at presentation to the ED was 85% compared with < 50% for the presenting CK-MB, the false negative results taken earlier than 3 hours or 10 days after the onset of symptoms. Troponin-T effectively identifies non Q-wave AMI much earlier than the CK-MB. This study led to a prospective randomized clinical trial to demonstrate an improved medical and financial benefit from an early rule in or rule out of severe coronary artery ischemia. CONCLUSION: The study supports our hypothesis that the laboratory can systematically redesign its technology strategy and participate in the construction of a clinical pathway for the discharge from ED or admitting decisions with a test 98% sensitive for identifying patients with serious coronary ischemia by 3.5 hours after the onset of symptoms.

Nurses gain more time with patients.

Clinical pathways that incorporate charting by exception eliminate repetitive documentation and give nurses more time to educate and care for patients. In this case, nurses report a gain of 15 minutes per patient each day.

The effect of clinical pathway implementation on total hospital costs for thyroidectomy and parathyroidectomy patients.

Clinical pathways have long been used to guide the delivery of patient care in varied practice settings. There is little information in the literature to document the effectiveness of pathway implementation in general surgical populations. This study reports the effect of clinical pathway implementation in two general surgical patient groups, thyroidectomy and parathyroidectomy. Clinical pathways were implemented to serve patients undergoing thyroidectomy and parathyroidectomy surgery. The effects of both clinical pathways on total hospital costs, length of hospitalization, variances, and outcomes were collected and evaluated from July 1998 through July 1999. These data were compared to data from the previous year. The average length of stay for parathyroidectomy patients decreased from 2.4 to 1.5 days (P = 0.26) for pathway patients as compared to prepathway patients. The average cost per case decreased from $5071 to $4291 (P = 0.50) for parathyroidectomy pathway versus prepathway patients. The average length of stay decrease for thyroidectomy patients was 1.4 to 1.2 (P = 0.16) for the pathway to prepathway comparison. The average cost per case decrease was minor at $4117 to $4111. Pharmacy costs and laboratory utilization were effectively reduced. Perioperative costs rose dramatically during this period, operating room/central sterile supply cost per case rose 12 per cent, anesthesia supply cost per case rose 15 per cent, and surgical pathology costs increased 110 per cent overall for both patient groups. Clinical pathway implementation has allowed us to reduce or maintain total hospital costs in the face of rising perioperative costs. We conclude that implementation of these clinical pathways has allowed us to improve consistency with which we deliver care while maintaining the quality of patient outcomes and reducing the costs of care and length of hospital stay.

[Quality of care in chronic obstructive pulmonary disease].

INTRODUCTION: Chronic obstructive pulmonary disease (COPD) has been a public health priority for many years and many recommendations have been published on its detection and management. STATE OF THE ART: Many studies have shown that practice remains sub-optimal, at both diagnostic and therapeutic levels, in the stable state and during exacerbations. There are multiple factors responsible including the recommendations themselves (method of development, format, distribution and implementation) and persisting grey areas in the literature upon which they are based. PERSPECTIVE: To improve this situation it will be necessary to clarify the place of various treatment options during the evolution of the disease, to define the best organisational care pathways and to produce precise and didactic guidelines that will be widely distributed and implemented. CONCLUSIONS: Real progress can be anticipated in view of the increasing quality of the guidelines and the clinical trials upon which they are based.

Clinical pathways in subacute care settings.

There is a dramatic growth of subacute care settings because of mandated shorter hospitalization and the growing older adult population. The cost parameters of a fixed reimbursement model of payment led to development of clinical pathways to enhance quality care, maximize resources and decrease costs, facilitate the patient's recovery and reduce length of stay. Seven essential elements are described that comprise the comprehensive framework, along with suggested criteria.

The effects of water-odor preference conditioning in the preadolescent nucleus accumbens septi.

The mesolimbic dopamine (DA) pathway is critical in reward-mediated behavior. Water, sucrose, and drugs of abuse all increase DA in the nucleus accumbens septi (NAcc) in adult animals. Recently our laboratory has shown that cocaine and alcohol increase DA efflux in preadolescent animals. The present study used a natural reinforcer (i.e., water) at postnatal day 25 (PND 25) to determine the sensitivity and responsiveness of this pathway. Repeated pairing of a peppermint odor with water resulted in a behavioral odor preference and an odor-elicited increase in accumbal DA. Results show that this developing pathway is functional and responsive to conditioning using a natural reinforcer and that these behavioral and neurochemical responses can be conditioned to a previously novel environmental stimulus.

The caspase-8 inhibitor FLIP promotes activation of NF-kappaB and Erk signaling pathways.

BACKGROUND: Activation of Fas (CD95) by its ligand (FasL) rapidly induces cell death through recruitment and activation of caspase-8 via the adaptor protein Fas-associated death domain protein (FADD). However, Fas signals do not always result in apoptosis but can also trigger a pathway that leads to proliferation. We investigated the level at which the two conflicting Fas signals diverge and the protein(s) that are implicated in switching the response. RESULTS: Under conditions in which proliferation of CD3-activated human T lymphocytes is increased by recombinant FasL, there was activation of the transcription factors NF-kappaB and AP-1 and recruitment of the caspase-8 inhibitor and FADD-interacting protein FLIP (FLICE-like inhibitory protein). Fas-recruited FLIP interacts with TNF-receptor associated factors 1 and 2, as well as with the kinases RIP and Raf-1, resulting in the activation of the NF-kappaB and extracellular signal regulated kinase (Erk) signaling pathways. In T cells these two signal pathways are critical for interleukin-2 production. Increased expression of FLIP in T cells resulted in increased production of interleukin-2. CONCLUSIONS: We provide evidence that FLIP is not simply an inhibitor of death-receptor-induced apoptosis but that it also mediates the activation of NF-kappaB and Erk by virtue of its capacity to recruit adaptor proteins involved in these signaling pathways.

Tumor promoter arsenite stimulates histone H3 phosphoacetylation of proto-oncogenes c-fos and c-jun chromatin in human diploid fibroblasts.

Although epidemiological studies have long established that inorganic arsenic is a potent human carcinogen, the underlying mechanisms are still poorly understood. Recent studies suggest that inorganic arsenic may act as a tumor promoter by perturbing key signaling transduction pathways. We have shown previously that arsenite can potently activate the mitogen-activated protein kinase cascades and induce the expression of proliferation-associated genes, including proto-oncogenes c-jun and c-fos. In order to elucidate further the molecular mechanisms underlying its tumor-promoting properties, we investigated the signaling events involved in arsenite-mediated induction of c-fos and c-jun. We found that induction of both c-fos and c-jun by arsenite can be substantially inhibited by the MEK- selective inhibitor U0126, suggesting that the ERK pathway is critically involved in their up-regulation. Interestingly, arsenite dramatically induced the phosphorylation and acetylation of histone H3 preceding the induction of mRNAs encoding c-fos and c-jun. Finally, chromatin immunoprecipitation assays revealed that arsenite treatment markedly induced the phosphorylation/acetylation of histone H3 associated with the c-fos and c-jun genes through an ERK-dependent pathway. Our results strongly suggest that arsenic-triggered alterations in chromatin structure perturb specific gene transcription, including that of proto-oncogenes c-jun and c-fos, and may thereby contribute to the carcinogenic process.

What is a clinical pathway?

Clinical pathways are patient care algorithms based on best evidence. They are intended to minimize variance in treatment and thus reduce cost, increase efficiency, and ultimately improve patient care outcomes. Clinical paths are more commonly used in the United Kingdom, and there is evidence that they are effective in the medical context. Some limitations that dentists may want to keep in mind regarding critical paths are that they may not be appropriate for all patient especially those with complex conditions); they present the potential for being misunderstood or misapplied; and practitioners often find them time-consuming, restrictive, and intrusive.

Outpatient tonsillectomy and adenoidectomy clinical pathways: an evaluative study.

PURPOSE: The purpose of this study is to examine two different dimensions of an outpatient pediatric tonsillectomy and adenoidectomy (T&A) clinical pathway at a tertiary care children's hospital. First, the analysis investigates whether the T&A clinical pathway effectively aids in the decision to discharge a pediatric patient as a day surgery (DS) (less than 12 hours) rather than as an outpatient observation surgery (OPO) (12 to 23 hours). Second, the pathway's impact on quality and financial outcomes is explored. PATIENTS AND METHODS: Forty prepathway pediatric T&A patients were randomly selected and matched to 40 pathway pediatric T&A patients by age, gender, medical history, and surgeon to form a retrospective cohort. Using chi-square and analysis of variance, the two groups were compared by type of discharge (DS or OPO), length of stay, readmission rates, and costs. RESULTS: The results show that patients on a pathway were more likely to be discharged as a DS. The shift toward DS discharges effected significant reduction in average length of stay and overall direct costs. Furthermore, there was no difference in readmission rates. CONCLUSION: These results indicate that the development and implementation of a pathway is an effective method in reducing length of stay and overall direct costs while maintaining quality outcomes.

Measuring quality and cost of care: evaluation of an amputation clinical pathway.

This study was based on nursing literature supporting the use of clinical pathways in case management systems to improve quality, reduce cost, and increase efficiency. Its purpose was to provide a descriptive analysis of outcomes in lower-extremity amputation caused by arterial occlusive disease. Patients were admitted between Jan. 1, 1989, and Sept. 1, 1995, with arterial occlusive disease for an above- or below-knee amputation and were cared for in three consecutive phases of nursing case management: (1) without a clinical pathway (group I), (2) with a consultation to rehabilitation services in the postoperative stay (group II), and (3) with a rehabilitation-focused clinical pathway (group III). The results of the study indicated more patients were able to return home from acute care and rehabilitation (52%) in group III than in group I and group II (40% and 44%, respectively). In addition, the hospital length of stay was reduced from 11 days to 8 days (geometric mean) once a rehabilitation consultation was included in the plan of care (group II). Although hospital charges rose initially with rehabilitation consultation ($18,627), they were reduced significantly when the pathway was used ($12,629; adjusted for rate increases). When patients were separated by level of amputation, those with below-knee amputations in group III had a significantly shorter length of stay and lower hospital charges than those in the other two groups, but comorbidity remained high. Nurse case managers can use this clinical pathway as a model for care as the findings support its cost effectiveness. They also demonstrate its effectiveness in enhancing the of quality care through a rehabilitation-focused, goal-oriented plan to return patients home.

Perceptions on the development of a care pathway for people diagnosed with schizophrenia on acute psychiatric units.

Policy development and practice for hospital mental health care has shifted towards a user-focused and evidence-based direction. Important within this policy development has been a guideline for inpatient care, particularly the establishment of an inpatient Acute Care Forum. A vehicle to both commission and develop this agenda is the Implementation of a care pathway. A research study was designed to explore how a care pathway could be developed for inpatients diagnosed with schizophrenia. Interviews with a range of health care professionals and observation of the process of care pathway development were the data-collection tools. Analysis was driven by emergent themes across the data set. Themes were then presented as one possible interpretation of the factors to be considered for the development of a care pathway for people diagnosed with schizophrenia. Clinicians experienced many difficulties in finding and including evidence-based practice (EBP) within a care pathway. Professions on the whole felt that there was a certain futility to psychiatric care given the paucity of evidence to support practice. This may contribute towards the poor use of hospital care as a therapeutic intervention as part of the wider spectrum of care. Difficulties arise when trying to develop a care pathway with EBP, given the paucity of knowledge on why certain interventions are only partially effective. The development of a care pathway may inform the priorities of the inpatient Acute Care Forum for people diagnosed with schizophrenia. A care pathway should not be constrained, however, to EBP and should incorporate therapeutic activities to improve the overall experience of service users. Limitations on the study and the collection of evidence supporting these conclusions conclude the paper.

Clinical care pathways: decreasing resource utilization in head and neck surgical patients.

In this era of decreasing reimbursement, health systems have been forced to become more efficient and decrease resource utilization to remain financially viable. One of the methods of internal cost control has been the use of clinical pathways. Given the complexity of treatment of head and neck cancer patients, clinical pathways can help to standardize decision making and introduce uniformity in resource utilization. The objective of this study is to compare resource utilization and outcomes before and after implementation of a clinical pathway for head and neck surgical patients. We observed significant decreases in hospital costs as well as shorter lengths of stay after pathway implementation. It is our belief that a uniform management tool is beneficial in this complex disease.

Implementation of a clinical pathway system in maternal newborn care: a comprehensive documentation system for outcomes management.

This article describes the design, implementation, and evaluation of an interdisciplinary clinical pathway system for maternal newborn care in a perinatal regional referral institution. Core issues in the design of this system are addressed to promote outcomes management and ongoing performance improvement. A discussion of the implementation follows, illustrating the lessons learned, changes made, and associated evaluation. This clinical pathway system has improved communication and collaboration among all disciplines, enhanced the discharge coordination process, and established protocols available to all members of the health care team.

The evolving health-care environment: new arguments for closer collaboration between cardiac surgical intensive-care nurses and clinical engineers.

"Not vain the weakest, if their force unite." Homer, Iliad (9th century, B.C.). In today's evolving health-care environment, the cardiac surgical intensive-care nurse is required to provide care to patients whose acuity levels are increasing. Simultaneously, these nurses are asked to use more technologically complex devices to deliver that care. In addition, practice protocols are being integrated into hospitals' clinical information systems. To meet these challenges, cardiac surgical intensive-care nurses must collaborate with clinical engineers on the evaluation and installation of new products and on the automation and refinement of clinical pathways and other outcomes measurement tools. Each discipline also must ensure that the other keeps pace with and maintains its levels of proficiency in the technology used to care for patients undergoing cardiac surgery.

Heart failure: strategies to enhance patient self-management.

Successful management of heart failure requires an active partnership between the patient and health care providers. This can be facilitated through a focused patient education plan that begins in acute care and has continuity into the community. Elements of the education plan involve both teaching content areas and self-management behaviors. Clinical pathways for heart failure incorporate teaching and educational strategies to guide the work of the multidisciplinary team, and the advanced practice nurse has tremendous potential in facilitating improved patient outcomes.

Clinical pathway for hip fractures in the elderly: the Hospital for Joint Diseases experience.

Hip fractures are common injuries in the elderly and are associated with considerable morbidity and mortality. Although technical advances in the treatment of the elderly have resulted in improved fracture fixation and surgical outcomes, clinical pathways have been developed to further improve patient outcome while shortening hospital length of stay after hip fracture. We describe the clinical pathway used since 1990 at the Hospital for Joint Diseases. The outcomes of 747 patients treated before 1990 were compared with outcomes of 318 patients treated at our hospital after initiation of the clinical pathway. Use of the clinical pathway was associated with significant decreases in the acute care hospital length of stay, in-hospital mortality, and 1-year mortality.

Acute urinary retention: developing an A&E management pathway.

Acute urinary retention (AUR) is a painful and distressing condition which is associated with significant morbidity requiring hospitalisation, catheterisation and often surgery (Kirby, 1998). AUR often develops from benign prostatic hyperplasia (BPH) and its incidence increases with age. The management of AUR is highly dependent on the cause, mainly BPH and clot/haematuria. Medical management will often necessitate the insertion of a urinary catheter, a procedure which is associated with an increased risk of urinary tract infections (Mulhall et al, 1998). The use of alpha-adrenegic antagonists (alpha-blockers), 5-alpha-reductase inhibitors, a trial without catheter and the catheter valve are recommended to delay or prevent the need for surgery and future complications. Proper assessment and documentation, particularly in relation to the catheterisation process is of vital importance. The authors demonstrate how audit was used in the development of a management pathway to provide guidance to accident and emergency (A&E) practitioners in their delivery of care to patients who presented to the A&E with AUR.
