The authors report a retrospective series of 45 cases of blunt renal trauma from a total of 685 cases of blunt abdominal trauma observed between 1989-1993. This multicentre trial was designed to illustrate the severity of this type of trauma and to place it in the context of road accidents in Tunisia.
The authors report a retrospective study of 34 cases of blunt renal trauma affecting a pathological kidney out of a total of 156 cases of renal trauma. The patients were between the ages of 3 and 60 years, with a male predominance (sex-ratio: 2.4). The predominant cause of trauma was a household accident, in 15 cases (44%). The clinical features were dominated by haematuria and pain. The underlying renal disease was dominated by renal stones (15 cases, 44%) and ureteropelvic junction (UPJ) abnormality (10 cases, 29%). Twelve patients with benign trauma and minimal underlying renal disease were treated conservatively. Surgery was indicated in the remaining 22 patients, but only 20 were actually operated. Nephrectomy was performed in 9 patients and partial nephrectomy was performed in 3 patients. Treatment consisted of UPJ plasty in 2 cases, uretero-caliceal anastomosis in one case, stone surgery in 4 cases and suture of ruptured renal pelvis in one case. The postoperative course was marked by the development of a urinary fistula in 1 patient, cured by drainage and deterioration of hydronephrosis in one patient. Lastly, one patient died from Wilms' tumour. This disease therefore tends to have a fairly benign course, which nevertheless depends on the underlying renal disease.

A report is given of the pathological changes in two eyes which were enucleated because of anterior luxation with rupture of the orbital optic nerve two hours after a traffic accident with frontal head injury. In the first case there was rupture of the trabecular meshwork and Schlemm's canal, the ciliary body and epithelium of the pars plana, a giant retinal tear at the ora serrata and a retinal avulsion at the optic disc, a partial rupture of the peripheral and a complete one of the central lamina cribrosa with a longitudinal tear in the optic nerve which lead to a complete rupture of the orbital optic nerve together with its sheats 22 mm behind the globe. The lesions produced arised probably from a slightly oblique course of the frontal trauma with a suggested additional rotating force followed by a sudden anterior movement of the eye with subsequent luxation. In the second case there was a giant tear at the ora serrata and at the pars plana with dislocation of unpigmented epithelium of the pars plana over the ciliary body, a rupture of the optic fascicle behind the lamina cribrosa, and a rupture of the optic dura mater 16.5 mm behind the globe.

Histochemical and biochemical evaluation of cerebral tissue fibrinolysis following cranioencephalic trauma was studied in 20 cases seen at necropsy, with varying periods of survival after injury. Fibrinolysis was mainly located at the vasal intima and was very intense in areas rich in vascularised connective tissue, such as the dura mater. Its activity diminished soon after injury to reach minimum values during the first day. Thereafter it remained at very low levels in sites with gross lesions, whereas it returned to nearly normal values elsewhere on the 2nd or 3rd day. Attention is drawn to the territorial character of this pattern. Extension of these results to the fields of diagnosis and treatment, however, is considered premature.

After spinal cord injury (SCI), the absence of an adequate blood supply to injured tissues has been hypothesized to contribute to the lack of regeneration. In this study, blood vessel changes were examined in 28 adult female Fischer 344 rats at 1, 3, 7, 14, 28, and 60 days after a 12.5 g x cm NYU impactor injury at the T9 vertebral level. Laminin, collagen IV, endothelial barrier antigen (SMI71), and rat endothelial cell antigen (RECA-1) immunoreactivities were used to quantify blood vessel per area densities and diameters in ventral gray matter (VGM), ventral white matter (VWM), and dorsal columns (DC) at levels ranging 15 mm rostral and caudal to the epicenter. This study demonstrates an angiogenic response, defined as SMI71/RECA-1-immunopositive endothelial cells that colocalize with a robust deposition of basal lamina and basal lamina streamers, 7 days after injury within epicenter VGM. This angiogenesis diminishes concurrent with cystic cavity formation. GAP43- and neurofilament- (68 kDa and 210 kDa) immunopositive fiber outgrowth was associated with these new blood vessels by day 14. Between 28 and 60 days after injury, increases in SMI71-immunopositive blood vessel densities were observed in the remaining VWM and DC with a corresponding increase in vessel diameters up to 15 mm rostral and caudal to the epicenter. This second angiogenesis within VWM and DC, unlike the acute response observed in VGM, did not correspond to any previously described changes in locomotor behaviors in this model. We propose that therapies targeting angiogenic processes be directed at the interval between 3 and 7 days after SCI.
