A patient with the classic signs and symptoms of temporal tendonitis was treated with excisional surgery of the temporal tendon and its respective mandibular coronoid process. The excised tissue was submitted to pathology for microscopic analysis, and the results were confirmed by two pathologists. Degenerative signs of focal atrophy and tissue necrosis were evident and served to describe the focal nature of the painful condition of temporal tendonitis. The description for the pain pattern is given, along with the recommended method for diagnostic testing. The current method of surgical management using radiofrequency thermal ablation is described, which replaces the more tissue-destructive, excisional approach.
An 83-day-old male infant had convulsions, hypertrophic cardiomyopathy, and lactic acidosis. Cranial computed tomography revealed low-density areas in both parieto-occipital lobes and in the left temporal lobe. Muscle biopsy did not reveal ragged-red fibers, but abnormal mitochondria were found in the capillary endothelial cells as well as in the muscle fibers. At 5 months of age, the patient developed purpura on the soles and palms. Skin biopsy showed degeneration of the endothelial cells with abnormal mitochondria in the arterioles and capillaries. Myelinated nerves in the skin had vacuolated axons with swollen mitochondria, and their myelin sheaths showed vacuolation. At 9 months of age, he died of heart failure, and autopsy revealed abnormal mitochondria in the myocardium but not in the coronary vessels. Our findings indicate that the symptoms of the mitochondrial encephalopathy, myopathy, lactic acidosis, and strokelike episodes (MELAS) syndrome cannot be fully explained by the mitochondrial angiopathy alone.

The effects of temporal fascia graft repair of temporomandibular joint disk perforation on intra-articular structure were examined on four sheep. Four sheep had bilateral disk perforations. At 20 weeks the perforation in the right joint was repaired with a temporal fascia graft. At 40 weeks the sheep were killed and the joints were examined radiologically and histologically. It was found that the temporal fascia graft did not survive and the disks remained perforated. However, histologically and radiologically there were fewer changes than in the joints where the perforated disks were not repaired.
The first case of a rare and unusual solitary neurofibroma of the mandible that caused bony erosion and extension into the adjacent soft tissues of the infratemporal fossa is presented. The clinical, radiographic, and histopathologic features of the neurofibroma are reviewed. The diagnostic procedures and the surgical treatment of this tumor in a young child are discussed.
The mandible coronoid process is another site subject to various pathoses that may result in limited jaw movement. Six different pathoses that prevented the free rotation of the coronoid are presented. Differential diagnosis and treatment approach are emphasized.

A study was undertaken to investigate the current practices in the UK in the management of zygomatic complex fractures. The results are presented and discussed with reference to recent literature on the subject.
The morphology of the developing articular disc of the temporomandibular joint in eight human fetuses, ranging in age from 13 weeks to 17.5 weeks, was studied. The shape of the articular disc at the time of initial delineation resembled the shape of the adult articular disc, suggesting a predominantly genetic determination. Elastin fibers were not demonstrated. The vascularity of the fetal articular disc was confined to the anterior and posterior thickened peripheral zones, with no evidence of capillaries in the thinner intermediate zone. Lateral pterygoid muscle fibers inserted into the medial aspects of the developing articular disc.
The use of prefabricated custom silicone rubber implants for frontal and nasofrontal deformities produces predictable esthetic results with minimal operative and postoperative morbidity and/or complications in selected patients. Over the past 8 years, 15 custom silicone rubber implants have been placed with good to excellent results. Only one implant was removed due to postoperative infection. This implant was successfully replaced upon resolution of the infection.

To date, the temporalis myofascial flap has been used only to a limited extent for reconstruction in the maxillofacial region. Experience gained with 26 temporalis flaps in reconstruction of tissue defects in the periorbital region, skull base, maxilla, and oral cavity is presented. Only one patient developed total necrosis of the flap; significant necrosis did not occur in any other patient. The anatomy of the flap and surgical technique are briefly presented.

PURPOSE: This study was undertaken to investigate the demographic and clinical features of mandibular coronoid hyperplasia and to assess the response to current treatments. PATIENTS AND METHOD: Case notes were studied and records made of age, sex, duration of symptoms, operative procedures, and response to treatment in all patients presenting at the Queen Victoria Hospital, East Grinstead, over a 20-year period from 1970 to 1990 with a confirmed diagnosis of mandibular coronoid hyperplasia. RESULTS: Thirty-one cases were recorded, 23 bilateral and 8 unilateral. The average age on presentation was 27.8 years for bilateral and 23.6 years for unilateral cases, with symptoms predating presentation by an average of 9 years and 6.75 years, respectively. Surgery was disappointing in terms of improving mouth opening. CONCLUSION: The results of this large series are supported by meta-analysis of the previous literature. They suggest that surgical management may be improved by a greater understanding of the pathogenesis of this condition.
