Collection and characterization of a bulk PM2.5 air particulate matter material for use in reference materials.

A contemporary PM2.5 (particulate matter smaller than 2.5 microns aerodynamic diameter) aerosol material from an urban site has been collected for the production of a new standard reference material that will be made available for the development of new PM2.5 air quality standards. Air particulate matter corresponding to the PM2.5 fraction was collected at an established Environmental Protection Agency monitoring site in Baltimore, Maryland. The air-sampling system that has been constructed for this collection separates fine particles with a cyclone separator and deposits them onto an array of Teflon membrane filters. The fine air particulate material is removed by ultrasonication or by mechanical means and collected for further preparation of standards. The composition of the collected PM2.5 aerosol, as well as the composition of the deposited PM2.5 aerosol, are determined by instrumental nuclear activation analysis and other techniques.

Anticardiolipin antibodies and thrombosis: buffer's influence on the detection and quantitation of anticardiolipin antibody measured by ELISA.

The influence of adult bovine serum (ABS) and fetal calf serum (FCS) on IgG anticardiolipin antibodies (ACA) levels measured by ELISA has been investigated. In control subjects the frequency of the optical density (O.D.) distribution was found to be non-gaussian. The O.D. of the 97th percentile was similar with both buffers, although the distribution of O.D. was statistically different (P less than 0.004). The slope of the dilution curves of two house standards as well as the curves obtained with reference standards were found to be very different depending on the buffer. In 55 consecutive patients with thrombotic events, recurrent fetal loss or auto-immune disorders, ACA positivity was also dependent on the way to express the data and the choice of the buffer: when O.D. values were read with their respective standard curves, IgG ACA positivity was 98% with FCS and 20% with ABS (P less than 0.001), but when the 97th percentile O.D. was considered, 40% were positive with FCS and 51% with ABS (n.s.). Furthermore 10 patients were found to be negative with FCS and positive with ABS and 4 others showed the inverse picture. Levels of positivity were also dependent on the buffer. These differences arise from the solution used to dilute the sera and not of that to block the plates. These methodological problems may explain some conflicting data found in the literature about the prevalence of ACA positivity.

Comparison of isotope dilution mass spectrometry methods for the determination of total homocysteine in plasma and serum.

Two independent methods have been critically evaluated and applied to the measurement of total homocysteine in serum and plasma: solid-phase anion extraction (SPAE) gas chromatography/mass spectrometry (GC/MS) and protein precipitation liquid chromatography/tandem mass spectrometry (LC/MS/MS). In addition, analysis of samples prepared by SPAE was accomplished by liquid chromatography/mass spectrometry (LC/MS) and LC/MS/MS. These methods have been used to determine total homocysteine levels in several existing serum-based Standard Reference Materials (SRMs) from the National Institute of Standards and Technology and in patient plasma samples provided by the Centers for Disease Control and Prevention. The precision of the homocysteine measurements in serum and plasma was critically evaluated, and method comparisons were carried out using Bland-Altman plots and bias analysis. On the basis of the excellent precision and close agreement of the mass spectrometric (MS) methods, the MS-based methods will be used for certification of a serum-based SRM for homocysteine and folates.

Determination of aluminium in different tissues of the rat by atomic absorption spectrometry with electrothermal atomization.

Atomic absorption spectrometry with electrothermal atomization was used for the determination of aluminium in brain, liver, spleen, kidney cortex, skeletal muscle and bone of the rat following digestion by nitric acid and in serum following simple dilution and in situ oxygen ashing. The method of standard additions in the presence of a chemical modifier, ammonium dihydrogen-phosphate, was essential for bone tissues. The detection limits ranged from 3 to 58 ng per gram of wet mass of tissue and were 4-19 times lower than the observed physiological levels of aluminium. The between-day precision for serum was 8.9% at a mean concentration of 6.8 micrograms I-1 and 2.4% at a mean concentration of 125.3 micrograms I-1. Additionally, repeated analyses of National Institute of Standards and Technology Standard Reference Material 1577b Bovine Liver gave a relative standard deviation of 12.2% (mean concentration = 0.8 microgram g-1). Of the tissues studied, bone had at least ten times higher levels of aluminium than others (0.959 +/- 0.322 micrograms g-1). The aluminium concentration in cerebellum (0.073 +/- 0.043 micrograms g-1) was approximately twice that in the cerebral hemisphere (0.034 +/- 0.009 micrograms g-1).

[Standardized shoulder examination with reference to computerized isokinetic strength testing (Cybex II)].

The complexity of shoulder movement makes it difficult to test normal or pathological shoulder function, as well as therapy results after injury. There are also problems to classify therapy results and to compare those with the results of other persons or other treatment methods. The aim of this paper is to introduce a standardised examination method of the shoulder and a standardised score regarding "subjective feelings", "clinical examination", "technical examination" such as x-ray, CT, sonogram, and "computerised isokinetic strength test", which enables us to conduct a follow-up case study of different patient groups after shoulder injury. Standardised examination programmes of the most common shoulder injuries like AC separations, rotator-cuff tears and luxations of the shoulder are given in this paper. A retrospective study of surgically treated AC separations Type III at the Staatliche Orthopädische Klinik München/Harlaching showed that therapy results can be established with the help of this score. The efficacy of different treatment methods (e.g. Bosworth and tension-band wire) can be both elucidated and compared.

Relationship between lead levels on painted surfaces and percent lead in the particles aerosolized during lead abatement.

Quantifying airborne lead on lead abatement work sites is critical in assessing worker lead exposures. Airborne lead levels depend on both the concentration of aerosolized particles and the percent lead in those particles. The lead level on the painted surface being abated may affect the percent lead in aerosolized particles. Experiments were performed in the University of Cincinnati Environmental Test Chamber (volume approximately 24.3 m3) using wood doors painted with lead-based paint. Three methods were used for paint removal: dry scraping, wet scraping, and dry machine sanding. Particles aerosolized during lead abatement activities were collected on filters using the Button Personal Inhalable Aerosol Samplers (SKC Inc., Eighty Four, PA) mounted in the workers' breathing zone. The filters were subsequently analyzed for percent lead in the particles. A portable X-ray fluorescence (XRF) instrument (NITON-700, NITON Inc., Bedford, MA) was used to measure surface lead levels of the doors. The accuracy of the XRF instrument was verified by testing standard reference materials prepared by the National Institute of Standards and Technology (NIST) and by Princeton Gamma Tech Inc. It was also verified by relating XRF results from painted door surfaces to laboratory lead analysis data obtained from paint chip samples taken from the same painted surfaces (r2 = 0.81, p < 0.001). A highly significant relationship (r2 = 0.83, p < 0.001) was found between the XRF readings and the percent lead in the particles aerosolized during dry scraping. No significant relationship was found for wet scraping (r2 = 0.09, p = 0.56) or dry machine sanding (r2 = 0.002, p = 0.92). The relationship between surface lead levels and percent lead in particles was found to be dependent on the paint removal method. This variation was attributed to the difference in water absorption property of the paint layers and the different particle aerosolization mechanisms inherent in each paint removal method.

Assessment of cardiac function using multidetector row computed tomography.

In patients with suspected or documented heart disease, a precise quantitative and qualitative assessment of cardiac function is critical for clinical diagnosis, risk stratification, management and prognosis. Cardiac CT is increasingly being used in diagnosis of coronary artery disease. Initially multi-detector row computed tomography (MDCT) was used chiefly for detecting coronary artery stenosis and assessment of cardiac morphology. Electron beam computed tomography has been shown to provide a highly accurate ejection fraction (+/-1%), with 50 ms image acquisition per image. Retrospective electrocardiographic gating allows for image reconstruction in any phase of the cardiac cycle. Thus, end systolic and end diastolic images can be produced to assess ventricular volumes and function. Despite lower temporal resolution than electron beam computed tomography, the ability of MDCT to assess ejection fraction is preserved. In the assessment of cardiac function, MDCT has been shown to be in good agreement with echocardiography, cineventriculography, single photon emission computed tomography and magnetic resonance imaging. The fast technical development of scanner hardware along with multisegmental image reconstruction has led to rapid improvement of spatial and temporal resolution and significantly faster cardiac scans. The same data that is acquired for MDCT angiography can also be used for evaluation of cardiac function. Considering contrast media application, radiation exposure, and limited temporal resolution, MDCT solely for analysis of cardiac function parameters seems not reasonable at the present time. However, because the data is already obtained during coronary evaluation, the combination of noninvasive coronary artery imaging and assessment of cardiac function with MDCT is a suitable approach to a conclusive cardiac workup in patients with suspected coronary artery disease. MDCT seems suitable for assessment of cardiac function by MDCT when results are held in comparison to magnetic resonance imaging as the reference standard. Given the radiation dose and contrast requirement, referring a patient to MDCT only for evaluation of function is not warranted, but rather adds important clinical information to the already acquired data during retrospective triggering for MDCT angiography.

Growth and energy and protein intake of preterm newborns in the first year of gestation-corrected age.

CONTEXT: There are few longitudinal studies that analyze the growth and nutritional status parameters of children born prematurely. OBJECTIVE: To evaluate the growth and dietary intake of preterm newborns in the first year of gestation-corrected age. DESIGN: Prospective clinical study. SETTING: Tertiary care hospital. PATIENTS: 19 children (7 male) who were born prematurely, with birth weight between 1000 g and 2000 g, which was adequate for the gestational age. PROCEDURES: At 3, 6, 9 and 12 months of gestation-corrected age, children were evaluated in relation to weight, height and cephalic perimeter, using the National Center for Health Statistics as the standard reference, and the Rozalez-Lopez and Frisancho standards for brachial perimeter and triceps and subscapular skinfolds. The calculated dietary intake was compared to the Recommended Dietary Allowances. MAIN MEASUREMENTS: The Z score was calculated for the weight/age, height/age and weight/height relationships, and the percentiles of the perimeters and skinfolds were considered. Dietary intake records were made using the 24-hour Dietary Recall and the Food Frequency Intake Questionnaire methods. The Virtual Nutri software was used to calculate energy and protein intake. RESULTS: The weight/age, height/age and weight/height relationships and the brachial perimeter and triceps skinfold were statistically greater in the first semester in relation to the second. The cephalic perimeter remained above the 50th percentile for the ages studied and there was no difference in the subscapular skinfold between the first and second semesters, remaining below the 50th percentile. The calorie and protein intake, although statistically lower in the first than in the second semester, always remained above the recommended. CONCLUSIONS: The pace of growth is greater in the first semester than in the second, not reaching the standard expected for full-term newborns, with the exception of the cephalic perimeter, which remains adequate. Calorie/protein intake shows an inverse relationship with growth speed, remaining above the recommended for full-term newborns, although with difficulty in depositing subcutaneous fat, in spite of the high caloric intake.

Application of linear programming to dose optimization in intracavitary implant therapy.

Linear programming can be used to optimize intracavitary brachytherapy for carcinoma of the cervix. A method has been developed which gives meaningful output and is described. A set of reference points were necessary in addition to the standard reference points. Point A as well as an array of points for adjacent radiation sensitive normal structures were used in order to obtain isodose curves conforming to those commonly used for therapy. In addition, arbitrary upper and lower limits of dose at selected points were needed and were set to conform to systems commonly used clinically for intracavitary therapy. It was immediately evident that a wide variety of loadings can be used that deliver appropriate or improved doses to reference points while minimizing normal tissue dose. The loadings represent arrangements which are not commonly used in many clinics but offer potential for clinical use.

Colorimetric determination of potassium in plasma and serum by reflectance photometry with a dry-chemistry reagent.

We evaluated a colorimetric assay of potassium in plasma and serum with the Boehringer Mannheim Reflotron reflectance photometric analyzer, which is designed for near-patient testing in hospitals and physicians' offices. This potassium method does not require calibration or instrument maintenance by the operator. Analysis of 30 microL of plasma or serum takes approximately 140 s. Within-day imprecision (CV) was 1.0-1.2%. Total CVs over a 1-month period were 1.0-1.4%. Patients' results from the Reflotron correlated well with those from the IL 643 flame photometer and the Beckman Synchron CX3 ion-selective electrode methods. The accuracy of Reflotron values was also verified with Standard Reference Material 956 from the National Institute of Standards and Technology.

[The effect of Fenbion, an anthelmintic, on blood in heifers].

The complex effects of the anthelminthic Fenbion (prepared on the basis of fenbendazole, Bioveta Nitra) were investigated; its effects on the blood picture were studied in heifers with the liveweight of 400 kg, until the sixth day after its single application at doses of 15 and 30 mg per kg liveweight. The haemoglobin (Hb) content decreased in keeping with the physiological standard. The haematocrit (Hk) value decreased below the reference standard from 0.31 and 0.32 l per l on days five and six to 0.28-0.29 l per l. The total leucocyte counts increased from 8.60-9.64 G per l to 10.54-11.0 G per 1. This means that on the first days after administration Fenbion has negative effects on erythropoiesis, and as a stressor it evokes defense reactions of the organism. Differential blood counting: leucocyte counts increased to 0.71-0.73 arb. u. on days three to four. The increase in lymphocyte counts in the blood after Fenbion administration is likely to be related to the cellular immunobiological defense reaction of the organism. The counts of segmented leucocytes decreased, the counts of eosinophils increased irregularly, those of basophils and monocytes were highly variable, and the counts of monocytes increased irregularly in a large span, or they decreased.
