Neck injuries: I. Occipitoatlantal dislocation--a pathologic study of twelve traffic fatalities.

Twelve of 155 persons killed in traffic crashes had occipitoatlantal dislocations. Nine were vehicular occupants, 2 were cyclists, and one was a pedestrian. The dislocations involved various combinations of lacerations of the alar ligaments, the occipitoatlantal joint capsules, the dura mater, the tectorial membrane, the rectus capitis muscles, and the suboccipital muscles. In 2 instances, an occipital condyle failed instead of the corresponding alar ligament, producing condyle fractures. Atlas ring fractures occurred in 3 instances. Axial and subaxial cervical trauma were uncommon. Facial or mandibular fractures occurred in a majority of cases, vault skull fractures were uncommon, and basilar fractures were absent. Pontomedullary brainstem lacerations occurred in 9 of the 12, and 4 had midbrain lacerations. The majority of the victims succumbed to acute neurogenic shock as the sole or the major mechanism of death. The biomechanical basis for occipitoatlantal dislocation is discussed, and the author suggests that distraction, in concert with variable combinations of extension, rotation, and posterior translation is responsible for occipitoatlantal dislocations.

[Benign-thyroid nodule: what's therapy? Personal experience].

INTRODUCTION: Solitary thyroid nodule is an important pathology with an incidence of 2-5% in Italian population. The diagnosis was based on clinical exam, laboratory tests and ultrasonographic evaluation. MATERIALS AND METHODS: The study undertook has the purpose to assess the usefulness between lobe-isthmusectomy (with corresponding risk of possible relapse and complications for reintervention) and total thyroidectomy, (with corresponding post-surgical treatment complications), for benign thyroid disease with solitary nodule, on selected cases for disease and corresponding risk factors, in the retrospective clinical study, using laboratory tests and ultrasonographic evaluation. The study was conducted on 80 patients admitted from 1994 to 2000 with diagnosis of benign thyroid nodule and operated with lobeisthmusectomy. In seven patients the operation had to be converted in total thyroidectomy. RESULTS: Operative mortality was nil and long-term results at a mean follow-up of three years are encouraging. DISCUSSION: The main advantages of lobe-isthmusectomy for benign solitary thyroid nodule consist in less postoperative complications and less hospital stay. CONCLUSION: Therefore our experience has carried us to consider the lobectomy which treatment choice for all those benign thyroid diseases, with, solitary nodule.

[Pathology of bone diseases in elderly subjects].

In elderly persons, structural changes occur in bone tissue. The bone histology and histomorphometry demonstrate that the trabecular volume and thickness are reduced and trabecular connection is disrupted in cancellous bone, and that the cortical layer becomes thinner and the bone lacuna is enlarged in compact bone. These features become more serious in osteoporotic patients. Even weak forces can cause fragility fractures in osteoporotic patients. However, a prolonged period is not always required for fracture healing in elderly patients. Elderly persons can suffer from many disorders, such as osteomalacia, bone diseases associated with joint diseases and bone tumors.

[History of the pathologico-anatomical service of the Red Army in the Great Patriotic War 1941-45].

The history of the rise of the pathology service in the military-medical service of the Red Army is described along with an explanation of its indispensible contribution to the combat effort. The goals of the service include evaluation of the work of the military-medical establishments using summarized clinico-anatomical analyses of pathology data. This allowed the heads of the military-medical service to rapidly recognize and alter organizational and therapeutic-diagnostic weaknesses in various stages of medical evacuation and to improve the activity of the therapeutic evacuation centres. The pathology service also established the causes of death among persons wounded in battle. It evaluated the significance of the type, timing, intensity and shortcomings of treatment prior to death. The pathology of war trauma, regeneration, the pathogenesis of infections complications and influence of treatment on their development and course were also studied. The organizational structure of the service, its branches (front and army laboratories of pathology, department of pathology of major hospitals) working under the guidance of the Central Laboratory of Pathology (CLP), is described. The documents which were the basis of the work of these laboratories are reviewed. The great services of M. F. Glazunov and N. A. Krajevskij - chiefs of the CLP - in organization and guidance (both practical and scientific) are honored.

[Changes in the configuration of the synaptic membranes in the human brain in aging and vascular pathology].

The structure of synapses in the brain of persons who had died of aging and vascular disease was studied by electron microscopy which indicated synaptic contacts with the convex and concave surface of pre- and postsynaptic membranes. There were also synapses with more complicated alterations in the configuration of synaptic membranes. The functional significance of the phenomenon is discussed in the paper. It is assumed that the alterations in the curvature of synaptic membranes are manifestation of plasticity of a synapse and they are of compensatory value.

[The evolutionary principles of inguinal hernioplasty through our personal experience].

Inguinal hernia is a multi-factorial pathologic condition. A constitutional weakness of transversalis fascia, probably genetically transmitted, represents a fundamental cause. Hernioplasty has evolved year by year from the "Bassini" to the "tension-free" techniques with Marlex or Prolene prosthesis, on the rational base of no tension on constitutionally weak structures. The data reported confirmed the absolute validity of "tension-free" hernioplasty.

Locus of control in women with eating pathology.

Locus of control scales have been used to measure 'effectiveness' or personal control in patients with eating disorders. A locus of control of behaviour scale was used in a community sample of women with a spectrum of eating pathology from bulimia nervosa to chronic dieting as well as in a comparison group of non-dieters. This scale appeared to add nothing over and above general measures of psychopathology as assessed by the Clinical Interview Schedule and the Symptom Rating Test, or more specific measures such as the Eating Attitudes Test.

[Personality disorders and psychopathology following trauma. Reflection on diagnostic classification].

Pervasive personality disorders have been shown to be long-term sequelae of cumulative childhood physical and sexual traumatization. This finding is not reflected in DSM-IV and ICD-10 classifications where post-traumatic stress disorder is confined to intrusions, avoidance, numbing, and hyperarousal. However, there is growing evidence that trauma etiology should be taken into account in planning treatment for personality disorders. It is not yet clear whether childhood traumatization is more strongly associated with borderline personality disorder than with other personality disorders. The finding of a substantial overlap between borderline personality disorder and dissociative identity disorder gives rise to discussions concerning the relationship of these two pathologies.

[The role of immune mechanisms in the pathogenesis of chronic bronchopulmonary pathology].

A clinicoimmunological investigation of 1230 patients with chronic nonspecific pulmonary diseases (CNPD) and 380 persons of the control group revealed significant variations in the immune status of patients with bronchopulmonary pathology. Changes in cellular immunity were characterized by a decrease in the number of T-lymphocytes, altered quantitative structure of lymphocyte subpopulations, a decrease in the activity of natural killer cells, leukocyte interferon producing capacity and lymphocyte function revealed by lymphocyte blast-transformation and leukocyte inhibition migration reactions. Changes in humoral immunity were characterized by imbalance of immunoglobulins. Variations in normal indices of local and nonspecific immunity were revealed. Certain relationship between clinical and immunological indices in CNDP patients indicated the presence of immunodeficiency in them confirming the importance of immune mechanisms in the development of chronic bronchopulmonary pathology.

[The sociomedical problems of disability in elderly persons].

Indices were analyzed for invalidity in pensionable persons residing in Chernivtsi Province, presenting with disorders of respiratory organs and circulatory system. By comparing intensive indices for primary disablement in pensioners and those able to work and data on distribution of primary disablement according to forms of diseases it is shown that in persons of greater age complications of chronic unspecific lung diseases are "hidden" in the cerebrovascular pathology. Ways for realization of the program intended to rehabilitate disabled persons of greater age are outlined.

Predispositions, personality traits, and posttraumatic stress disorder.

This review suggests an explanation for the finding that trauma is a necessary but insufficient condition for the development of posttraumatic stress disorder. Predispositions affecting vulnerability to stress can help to account for discrepancies between traumatic exposure and pathological outcome. Such predispositions may be rooted in personality traits that shape the cognitive processing of stressful events, but they are also influenced by both prior life experiences and social supports. All of these observations are consistent with a biopsychosocial model of posttraumatic stress disorder.

Some comments on the treatment of the borderline personality.

A discussion of "Some Clinical Manifestations of Structural Defects in a Borderline Personality," by Alan Krohn, M. D. Because the borderline patient presents the gamut of symptomatology and therapeutic responses, diagnosis and treatment have been problems since the beginning of psychoanalysis. With her key conceptualization of the "as if" personality, Helene Deutsch, in the 1930s (1942), stimulated the clarification of both the diagnostic and therapeutic problems. The primitive ego defenses were delineated by Klein (1946), and Mahler's studies (1968) of symbiosis, separation, and individuation provided the developmental framework for the growing understanding of these conditions. Since then, many have contributed, none more than Kernberg (1975). He has proposed limiting the term borderline to a relatively specific, more or less stable personality with typical symptom constellations, defensive operations, pathology of internal object relations, and instinctual vicissitudes. These conditions dictate a modification of therapeutic technique practiced by many but formulated by Kernberg, Masterson (1972, 1976), Boyer (1977), and others. Essential to the implementation of this modified technique is the understanding of the defensive operations and the pathology of the internal object relations. Particular emphasis is given to two defensive operations, splitting and projective identification, and to aspects of the other concepts especially pertinent to the therapy of the borderline. The work of Kernberg has been drawn on extensively.

Non-invasive dynamic assessment of the elasticity of intracranial structures.

The dynamics of the speed of transcranial propagation of ultrasonic impulses has been established in humans. The possibility of detecting the changes in mechanical state of intracranial structures by dynamic measurement of the time of transcranial propagation of ultrasonic impulses (TUPT) has been ascertained for 10 healthy persons and 11 patients with different neurological pathology. The cerebrospinal fluid pressure (CSFP) was simultaneously monitored for 4 of the patients. Spontaneous fluctuations in TUPT and the changes caused by jugular veins, inferior vena cava compression, hyperventilation and temporary retention of breathing were detected for all investigated subjects. Spontaneous TUPT changes in the range of 0.25-0.85 x 10(-4) of initial mean value, which are related with cardiac and respiratory cycles, have been found in healthy persons while in patients they ranged from 0.07 x 10(-4) to 4.5 x 10(-4) of mean time. In healthy persons bilateral jugular vein compression leads to a decrease in TUPT by 1.8-3.0 x 10(-4), while the compression of inferior vena cava gives an increase in TUPT by 1.6-2.6 x 10(-4). Changes in TUPT during these maneuvers in neurological patients were found to be of different amplitude and direction. The rise of CSFP by 8.4-20.2 mmHg (mean 12.4 mmHg) was observed in 4 investigated patients during the compression of jugular veins or during the inferior vena cava compression, while the changes in TUPT obtained during each of such maneuvers were of opposite character. The results suggest that dynamic evaluation of acoustic impedance of intracranial system reflects the changes in the mechanical state of brain parenchyma.

Insular Alzheimer's disease pathology as a cause of "age-related" autonomic dysfunction and mortality in the non-demented elderly.

Only a few brain structures have been implicated in the autonomic control of blood pressure and heart rate. Among them are heteromodal association areas in the cortex, especially the insular cortex. Ischemic insular lesions have been associated with both cardiac arrhythmias and mortality. However, stroke may not be the only insular pathology with the potential to disrupt autonomic function. Alzheimer's disease (AD) is associated with both insular pathology and autonomic dysfunction. Alzheimer's dementia is merely the final stage of a pathological process that spans decades. Recent studies have demonstrated a hierarchichal sequence of AD pathology that includes the insular cortex. This may explain why AD has effects on BP and central autonomic cardio-regulatory functions. However, AD reaches the insular cortex at a "preclinical" stage in its development (i.e., before "dementia" can be diagnosed). Thus, AD pathology should also be considered as a possible explanation for autonomic morbidity and mortality in non-demented elderly persons. We hypothesize that autonomic dyscontrol, commonly seen in non-demented well elderly persons without significant cardiovascular disease (CVD), reflects subclinical stages of AD pathology affecting the insular cortex. If true, then preclinical AD pathology should be considered as a possible explanation for arrhythmia/fall related morbidity and mortality in non-demented elderly persons.

Pathological changes in frontal cortex from biopsy to autopsy in Alzheimer's disease.

We evaluated the change in density of total senile plaques, plaque subtypes, and neurofibrillary tangles, from biopsy to autopsy in left frontal cortical sections from four patients with clinically typical Alzheimer's disease (AD). Comparisons were made on sections stained with modified Bielschowsky and Thioflavin S. In two cases, comparisons were also made on tissue stained with a monoclonal Alz-50 antibody and an antiserum to A beta (beta-amyloid protein). Despite a marked decline in mental status over several years of follow-up clinical evaluations, there was no consistent significant change in numerical density of plaques or tangles among the four cases. However, we did find fewer primitive plaques in the autopsy specimens. These results from longitudinally evaluated persons with typical AD suggest that although plaques and tangles may serve as adequate markers of the presence of AD, their numerical density within a single neocortical region may not reflect dementia severity. This conclusion supports the results of recent cross-sectional studies on the progression of pathology among persons with AD.

Person drawings by transsexual clients, psychiatric clients, and nonclients compared: indicators of sex-typing and pathology.

The drawings by 31 transsexual clients, 61 psychiatric inpatients, and 62 nonclient college students were compared to derive information about whether they differed with regard to sex-typing and psychopathology. Transsexuals did not differ from nonclients or psychiatric patients as far as their conceptualization of masculinity is concerned, nor are they more or less sex-role stereotyped. However, their conceptualization of femininity of women differs significantly from that of the other groups in that transsexuals view women as more feminine than either of the other groups. Finally, transsexuals differed from both nonclients and psychiatric patients in inconsistent patterns across other variables, indicating that they are neither similar to nonpathological controls, nor to psychiatric inpatients. This uniqueness of the transsexual group is discussed in the context of multiple possible interpretations.
