[External fistulas of the small intestines in the newborn infant and child. Medical aspects and digestive resuscitation (19 cases)].

The prognosis in fistulae of the small intestine in the infant and child is totally different according to whether they develop following a simple operation involving the healthy small intestine or multiple or complicated operations on pathological bowel. Under these latter conditions, constant medico-surgical cooperation, modern techniques of renutrition (total parenteral renutrition or constant flow enteral nutrition), together with restoration of water/electrolyte balance and the treatment of infections give increasingly satisfactory results. In a series of 19 children, with 26 enterocutaneous fistulas, deaths were reduced to 4. Independently of recourse to surgery determined by local abdominal conditions (peritonitis, obstruction) or persistence of the fistula, the choice between total parenteral renutrition and constant flow enteral nutrition (C.F.E.N.) depend essentially upon the site of the fistula. When the latter is proximal the digestive tract may not be used, while C.F.E.N. may be employed when it is distal, sometimes resulting in cure without further surgery.

A new method: double Thiry-Vella fistulas in rats to compare the effects of small and large intestinal stimulation on behaviour.

A new method for comparing the effects of small and large intestinal mechanical stimulation was developed. Isolated intestinal loops were created by the Thiry-Vella method from the lower duodenal--upper jejunal, and from the upper colonic portion of the gut, respectively. This preparation allows parallel evaluation of stimulation effects and affects from both the small and the large intestine of the same animal. A balloon could be inserted into the isolated loops before each experimental session and removed afterward. Behavioral thresholds to changes in the volume of the gut were recorded and compared in the two loops. No essential differences between the two portions of the intestinal tract were detected by this method.

Laparoscopically assisted ileocecal resection for Crohn's disease associated with intestinal stenosis and ileovesical fistula.

We describe a 22-year-old man with Crohn's ileocolitis accompanied by intestinal stenosis and ileovesical fistula in whom laparoscopically-assisted surgery was successfully performed after thorough nutritional therapy. Laparoscopic procedures are characterized by minimal access and minimal invasion, features which can contribute to the early recovery of patients who undergo surgery. It is suggested that laparoscopic (or laparoscopically-assisted) surgery after strict nutritional therapy can be effective in the treatment of patients with Crohn's disease who have intestinal complications.
