Outcomes of continuous process improvement of a nutritional care program incorporating TTR measurement.

Early assessment of protein calorie malnutrition (PCM) can improve the outcome for hospitalized patients by allowing the initiation of nutrition support if required. In addition, monitoring nutritional status during the hospital stay can identify a decline in or improvement of PCM so that alterations to treatment regimens can be made if needed. The visceral protein albumin is the traditional laboratory indicator of PCM. In the past decade another protein has been lauded as a superior marker that can be used in conjunction. We undertook several studies to test the effectiveness of TTR as an aid in nutritional assessment. We found TTR to be a sensitive measure of nutritional status, allowing for earlier assessment and intervention, thus reducing length of stay and other hospital associated costs. Based on these findings, our hospital generated and implemented a multidisciplinary nutrition care program. Transthyretin is an integral portion of this program; levels are determined on admission and repeated twice weekly until discharge.

Psychoanalytic and psychodynamic education in the 21st century.

Psychoanalytic and psychodynamic education over the past 50 years is reviewed in the context of the author's personal experience, as well as the changes that have taken place in psychiatry and psychoanalysis over the same time span. Several key issues are identified, including (1) the increasing biological reductionism of psychiatry; (2) the gradual de-emphasis of subjective experience along with the biopsychosocial explanation of human behavior and psychopathology; and (3) the increasing attention paid to neuroscience, psychopharmacology, and brief therapies during residency training. In order to address these issues, it is suggested that education in psychodynamic and psychoanalytic psychiatry be integrated into residency training. This would take the form of subspecialty training in either psychodynamic psychiatry or psycho analysis. Such training would promote an understanding of the connections between neurobiology and psychodynamics, versatility with a variety of dynamic psychotherapeutic techniques, and familiarity with research methodology. Psychiatrists educated in this type of integrated training program will be optimally equipped to advance psychodynamic and psychoanalytic treatment, theory, and research in the 21st century.

The maize macrohairless1 locus specifically promotes leaf blade macrohair initiation and responds to factors regulating leaf identity.

The leaf surfaces of almost all plant species possess specialized epidermal cell types that form hairs or trichomes. Maize leaves produce three distinct types of hairs, the most prominent being the macrohairs that serve as a marker for adult leaf identity and may contribute to insect resistance. This report describes the maize macrohairless1 (mhl1) locus, which promotes macrohair initiation specifically in the leaf blade. Each of seven recessive mhl1 mutant alleles significantly reduces or eliminates macrohairs in the leaf blade. The mhl1 mutations block macrohair initiation rather than interfering with macrohair morphogenesis. Genetic mapping placed mhl1 within bin 4 on chromosome 9. A second independently segregating locus was found to partially suppress the mhl1 mutant phenotype in certain genetic backgrounds. Macrohair density was observed to increase during early adult vegetative development and then progressively decline, suggesting macrohair initiation frequency is affected by factors that act throughout shoot development. Genetic analyses demonstrated that mhl1 acts in the same pathway but downstream of factors that either promote or repress adult leaf identity. Thus, mhl1 plays a key role in integrating developmental programs that regulate leaf identity during shoot development with those that specify macrohair initiation within the leaf blade.

Combining surgery and chemotherapy for invasive bladder cancer: current and future directions.

More than 13,000 patients died from invasive bladder cancer in 2005 alone. Radical cystectomy is the most commonly prescribed treatment for patients with muscle-invasive bladder cancer, or for those with a nonmuscle-invasive disease that is refractory to intravesical therapy. Despite advances in surgical technique and improved understanding of the role of pelvic lymphadenectomy, 5-year survival probabilities suggest that improvements in treatment are necessary. The maturation of several randomized clinical trials on perioperative chemotherapy, and particularly neoadjuvant chemotherapy, clearly suggest that an integrated treatment program of systemic chemotherapy and definitive locoregional therapy may improve the outcome for bladder cancer patients. The next frontier is the molecular characterization of this spectrum of diseases that make up invasive bladder cancer and targeted therapeutics. Prospective validation of molecular markers and evaluation of novel therapeutic agents, alone or in combination with established cytotoxic agents, provide hope of better outcomes for bladder cancer patients.

[Knowledge of the motives that lead a woman from Vargem Grande Paulista to undergo a Papanicolaou smear--I].

In this first approach the authors discusses the state of the Program for Integral Assistance por Woman's Health in São Paulo and emphasize the deficit of information on social epidemiology of women's behaviors related to health practices. They focus on the preventive Pap smear and associate the motives that yeld women of Vargem Grande Paulista to have it made. They analyse this knowledge from criterian proposed by KULBOK for posteriory apply it in practice.

[Domestic and institutional violence in health services: women's experiences].

Developed in state capitals Recife, Salvador and Aracaju, in three state maternity hospitals, this quantitative study aimed at identifying the manifestations of domestic and institutional violence suffered by women assisted in subprograms of the Program of Integral Assistance to the Woman's Health. Data collection was performed by using a semi-structured form. The sample featured young women, housewives, and those that are economically dependant on their significant others. Most women experience matrimonial violence, and financial dependency is the most often mentioned reason to bear it. The lack of vacancies characterized institutional violence due to omission. Research showed that experiencing such violence brings health problems to women, or aggravates them.

Ecology of malaria vectors in the Americas and future direction.

The resurgence of malaria in the Americas has renewed interest in Anopheles biology. Anopheles darlingi, An. albimanus, An. nuneztovai and An. aquasalis are reconfirmed as major malaria vectors and other species are playing important roles in regional malaria transmission. Adult biting activity and larval ecology are discussed in detail. Seasonal abundance and daily biting activity of Anophelines vary considerably among species and geographically for the same species. Anopheles albimanus has the least amount of variation in biting activity over its range and An. darlingi has the greatest. All species studied are more exophilic and exophagic than endophilic and endophagic. Anopheles darlingi is more anthropophilic, endophilic and endophagic than other Anophelines. Larval studies remain more descriptive than comprehensive. Research on Anophelines is becoming more integrated and biologists are using new biochemical techniques and ecological principles to answer critical questions. This "pluralization" will help us understand species complexes, population dynamics and malaria transmission. Integrated control programs will require more regional, in-depth ecological studies.

Adult abundance, biting behavior and parity of Anopheles aquasalis, Curry 1932 in two malarious areas of Sucre State, Venezuela.

The principal vector of malaria in eastern Venezuela, Anopheles aquasalis, is exophagic and exophilic. Control using indoor insecticide house sprays has failed to lower the number of malaria cases. Therefore, studies were initiated in two villages of the eastern coastal state of Sucre to better understand this vector's biology and develop a more integrated control program. An. aquasalis was found to have a crepuscular biting behavior with a major peak at dusk and a minor peak at dawn. Mosquitos were collected more outdoors than indoors. Forty-seven percent of the biting took place before people went to bed (22:30 hr) and 69% of the mosquitos biting during this time period bite outdoors. Outdoor biting could be the reason why indoor spraying alone did not lower malaria cases. Seasonal abundance was greater in the rainy season compared to the dry season. Seasonal parous rates were high (78.3%-100%) and similar indoors and outdoors and between dry and wet season in Santa Fé. In Guayana, the seasonal parity was lower (34.6%-42.2%) than Santa Fé with indoor parity slightly higher than outdoors. Malaria cases were higher in Santa Fé, but adult mosquito density was much lower than in Guayana. This difference could have been due to higher parity in Santa Fé compared to Guayana. The greater distance to the nearest breeding site and presence of alternative hosts in Guayana can not be discounted as factors which contributed to the difference in malaria transmission between locations. We concluded that knowledge on seasonal occurrence, biting activity, resting behavior and breeding site location can be used to design a new control strategy for this vector.

[Rereading contraceptive policy: health professionals' views of daily routine in public health services].

This essay is part a larger study on the relationship between the Brazilian government and contraceptive policy and precedes a survey performed in health services in Rio de Janeiro. It is also intended to analyze what health care professionals and users think about contraception. It presents considerations by a social worker with experience in family planning activities in outlying public health care services and also provides data facilitating activities in this area. It thus touches on some elements that interfere in practical work in this field, such as academic life and social representation, in addition to submitting a written critique to PAISM (the Brazilian Ministry of Health's Program for Integrated Women's Health Care) as an official contraceptive policy.

The effectiveness of community-based interventions to improve maternal and infant health in the Northeast of Brazil.

OBJECTIVE: To evaluate the effectiveness of a community-based intervention project aimed at reducing maternal and infant mortality in a poor urban district in the city of Natal, in the Northeast of Brazil. METHODS: The intervention, called the ProNatal project, introduced a program of integrated community health care to a geographically defined population. The interventions included the establishment of antenatal clinics at the district's health centers, the opening of the maternity facilities at the polyclinic for low-risk deliveries, the introduction of a family planning clinic and a breast-feeding clinic, support from pediatricians for under-5 (well-baby) clinics, children's outpatient services and children's emergency care, and the introduction of health agents recruited from the local community. Representative surveys of the population were taken at the project's inception (July 1995) and then 30 months later (December 1997), using a general health questionnaire adapted to the local conditions. Mortality data were collected from local registration systems as well as from an autopsy survey of perinatal and infant deaths. RESULTS: During 1995 there were 4 maternal deaths from 1 195 pregnancies (maternal mortality of 335/100 000); three of the deaths were related to hypertension and one to uterine perforation after an illegal abortion. During 1998 (post-intervention), there were no maternal deaths in pregnancy or childbirth. In 1993 no deliveries took place at the polyclinic, but in 1998 there were 946 deliveries at the clinic without any serious complications. The method of delivery, the incidence of prematurity, and the incidence of low birthweight did not change significantly over the study period. In the post-intervention survey, 75% of women reported receiving contraceptive advice from a doctor in the preceding year, compared to 50% in the first sample. A mortality survey carried out in 1993-1995 estimated the infant mortality rate to be 60/1 000 live births. By 1998, using data collected locally by active surveillance, the infant mortality rate was 37/1 000 live births. The causes of infant death in both those periods were dominated by respiratory infections and diarrheal disease. Over 95% of both samples initiated breast-feeding, but a higher proportion of the post-intervention sample reported breast-feeding for longer than 6 months (41% vs. 32%, P = 0.0005). No differences were apparent in the use of under-5 clinics, but immunization rates improved. Post-intervention, significant improvements were documented in the mothers' understanding of basic hygiene, their knowledge of causes of common diseases, and their management of acute respiratory infections and diarrhea in children. This was particularly true for the households visited by a community health agent. CONCLUSIONS: Inequalities in health care in poor urban populations can be reduced by integrated community-based interventions, including the use of health agents recruited from the local community.

Live Varroa jacobsoni (Mesostigmata: Varroidae) fallen from honey bee (Hymenoptera: Apidae) colonies.

The proportion of Varroa jacobsoni Oudemans that were alive and mobile when they fell from honey bees, Apis mellifera L., in hives was measured during a 20-wk period to determine the potential use of systems that prevent these mites from returning to the bees. Traps designed to discriminate between the live, fallen mites and those that are dead or immobile were used on hive bottom boards. A large fraction of the fallen mites was alive when acaricide was not in use and also when fluvalinate or coumaphos treatments were in the hives. The live proportion of mitefall increased during very hot weather. The proportion of mitefall that was alive was higher at the rear and sides of the hive compared with that falling from center frames near the hive entrance. More sclerotized than callow mites were alive when they fell. A screen-covered trap that covers the entire hive bottom board requires a sticky barrier to retain all live mites. This trap or another method that prevents fallen, viable mites from returning to the hive is recommended as a part of an integrated control program. It also may slow the development of acaricide resistance in V. jacobsoni and allow the substitution of less hazardous chemicals for the acaricides currently in use.

Field evaluation of a lethal ovitrap for the control of Aedes aegypti (Diptera: Culicidae) in Thailand.

In 1999 and 2000 we evaluated a lethal ovitrap (LO) for the control of Aedes aegypti (L.) in three villages in Ratchaburi Province, Thailand. Two blocks of 50 houses (a minimum of 250 m apart) served as treatment and control sites in each village, with each house in the treatment area receiving 10 LOs. Thirty houses in the center of each treatment and control block were selected as sampling sites, with larval and adult mosquito sampling initiated when LOs were placed. Sampling was conducted weekly in 10 of the 30 houses at each site, with each block of 10 houses sampled every third week. Sampling continued for 30 wk. Efficacy of the LO was evaluated by determining number of containers with larvae and/or pupae per house and number of adult mosquitoes collected inside each house. In 1999, the LO had a negligible impact on all measures of Ae. aegypti abundance that were assessed; however, fungal contamination of insecticide-impregnated strips may have been responsible for the low efficacy. In 2000, significant suppression was achieved based on changes in multiple entomologic criteria (containers with larvae, containers with pupae, and number of adult Ae. aegypti); however, control was not absolute and neither immature nor adult Ae. aegypti were ever eliminated completely. We conclude that the LO can reduce adult Ae. aegypti populations in Thailand; however, efficacy of the LO is lower than desired due primarily to the high number of alternative oviposition sites. LO efficacy may be improved when used as part of an integrated control program that places emphasis on reduction of adjacent larval habitats. Further studies are required to assess this issue.

Targeted irradiation of Mammalian cells using a heavy-ion microprobe.

The existing focusing heavy-ion microprobe at the Gesellschaft für Schwerionenforschung in Darmstadt (Germany) has been modified to enable the targeted irradiation of single, selected cells with a defined number of ions. With this setup, ions in the range from helium to uranium with linear energy transfers (LETs) up to approximately 15,000 keV/microm can be positioned with a precision of a few micrometers in the nuclei of single cells that are growing in culture on a thin polypropylene film. To achieve this accuracy, the microbeam traverses a thin vacuum window with minimal scattering. Electron emission from that window is used for particle detection. The cells are kept in a specially designed dish that is mounted directly behind the vacuum window in a setup allowing the precise movement and the imaging of the sample with microscopic methods. The cells are located by an integrated software program that also controls the rapid deflection and switching of the beam. In this paper, the setup is described in detail together with the first experiments showing its performance. We describe the ability of the microprobe to reliably hit randomly positioned etched nuclear tracks in CR-39 with single ions as well as the ability to visualize the ion hits using immunofluorescence staining for 53BP1 as a marker of DNA damage in the targeted cell nuclei.

Chemogenomics: bridging a drug discovery gap.

With the successful completion of the human genome sequencing and the resulting plethora of genetic information now available novel technologies and applications have to be established to translate the huge amount of data generated into successful biological and biomedical research programs. The integration of various drug discovery disciplines within the parallel quest for novel targets and new molecular entities has meanwhile given rise to a quite popular term in pharmaceutical research named "chemogenomics". This review article gives an overview of the disciplines involved in this field and discusses the possible implications of this novel paradigm in drug discovery for the near future.

Overpopulation as crisis: redirecting health care services in rural Bangladesh.

This article examines the consequences of a "population-as-crisis" theme on the institutional configuration and resource endowments of health care services in an integrated Ministry of Health and Population Control in Bangladesh. The Ministry's focus on women as child bearers and its emphasis on sterilization supported by incentives has contradictory consequences as women become vulnerable to a limited health service and incentives encourage a focus on meeting sterilization targets. Both undermine people's access to and use of primary health care services. Findings from three studies, undertaken between 1978 and 1983, support the argument that despite international concern with preventive and promotive primary health care, simultaneous support for and emphasis on population control inhibits meeting the goals of a broad-based rural primary health care service.

Disclosure of basic strengths and basic weaknesses in demented patients during morning care, before and after staff training: analysis of video-recordings by means of the Erikson theory of "eight stages of man".

In the field of care for aging persons, it is commonly understood that personality changes occur in dementia patients. It is reported to be a consistent part of the clinical syndrome and to occur early in the disease. The aim of this study was to investigate if strengths and weaknesses, described by E.H. and J.M. Erikson as basic quality in the person, could be interpreted in severely demented patients during a caring activity, and if a difference in these qualities could be seen after staff completed a training program in "integrity promoting care." The morning care of five patients was video-recorded and a phenomenological-hermeneutic analysis of the patient's and staff's interaction was conducted. The findings indicate that the complex qualities of someone's personality are more preserved than could be expected considering the cognitive handicap. It seems, however, that demented patients need a special, supportive environment for their full mental potential to be realized. It is reasonable to assume that, if the staff are given knowledge of how to create a positive climate for the demented patients and the opportunity to implement it, the patients will show a rich pattern of mental reactions in spite of their dementia.

Right hemispheric contribution to motor programming of simultaneous bilateral response.

The purpose of this study was to verify the hypothesis that the motor program which integrates the left- and right-hand responses in the bilateral reaction time (RT) task is located in the right hemisphere. 8 female and 50 male students performed bilateral simultaneous RT tasks to lateralized light stimuli. Uncrossed RT based on the relationship between the unifying center in the right hemisphere and hemifield stimulus was shorter than crossed RT for the bilateral response. Therefore, the hypothesis was supported. Some areas of the right hemisphere contributed to unification of the movements of the right and left hands for bilateral movements in reaction time.
