A statistical study on the comparison of the comprehension skill levels between the group with and the group without each maladaptive behavior of nine items was done for autistic and mentally retarded children. Results indicated that the group of the autistic children with hyperactivity or withdrawal had slightly, not significantly, lower comprehension skill levels than the group of those without each of these maladaptive behaviors. On the other hand, the correlation between the prevalence of hyperactivity or withdrawal and lower comprehension skill levels was seen more clearly, with statistically significant differences, rather among the mentally retarded children.

Researchers have examined adaptive behavior in autism, but few studies have looked for different patterns of adaptive skills according to age and intelligence. Domain scores from the Vineland Adaptive Behavior Scale (VABS) were compared in relation to age and Performance IQ for 72 children and adolescents with autism and 37 nonautistic children and adolescents with mental retardation. Age and IQ were positively related to each of the Vineland domains. Children with autism had lower scores in the socialization domain. An interaction was present between Performance IQ and group: With increasing IQ, children with autism showed smaller increases in social functioning than children with mental retardation. A similar trend was present for daily living skills. Results suggest that (a) the relationship between the two groups' adaptive behavior profiles is stable from preschool age through adolescence, and (b) increasing IQ is associated with less of an increase in certain adaptive skills for children with autism.
Since 1986, we have seen a rise in the occurrence of tuberculosis in the United States. Long considered defeated in this country, the disease is returning with distressing vigor. Outbreaks of MDR-TB, tuberculosis resistant to more than one medication, have been reported around the country. This article analyzes the Centers for Disease Control and Prevention's National Action Plan to Combat Multidrug-Resistant Tuberculosis, with particular focus on the moral dimensions of mandatory directly observed treatment (DOT) and involuntary quarantine. It is proposed that a moral response to the control of tuberculosis must be one which is sustainable and which can effectively curtail the spread of the disease at a minimal cost to individual rights.
Brain-computer interfaces are devices for enabling patients with severe motor disorders to communicate with the world. One method for operating such devices is to use movement-related potentials that are generated in the brain when the patient moves, or imagines a movement of, one of his limbs. An algorithm for detecting movement-related potentials using a small number of EEG channels was developed. This algorithm is a combination of the matched filter, a non-linear transformation previously developed as part of a similar detector, and a classifier. The algorithm was compared with previous designs of similar detectors by both theoretic analysis and off-line evaluation of performance on data recorded from five subjects. It is shown that the performance of the algorithm was superior to that of previous methods, improving the area under the receiver operating characteristic curve to 87.8%, an improvement of 25% compared with the best previously suggested detection method. Finally, the probable sources for false detections were identified, and possible ways to minimise them are proposed.
The paper seeks to describe the way in which changes were brought about in the delivery of speech therapy to children in special schools. Changes were necessary in order to make most use of existing staff resources and to investigate the effects of treatment. Both aims were achieved, but improvements to practice are continuing. A change in service organisation across the two state services of Health and Education takes time to bring about.
The training of communication skills of professional caregivers in six homes for elderly people has been developed and evaluated in a model project. The purpose of the project was to strengthen the staff's orientation towards the residents, their needs, handicaps and abilities. Therefore, a series of 8 in-house training courses as well as procedures to establish the contents of the program into daily care-giving (transfer) have been developed and implemented with six teams during one year. The evaluation included interviews, questionnaires and observations and was realized with participants and non-participants of the program once before the implementation of the training program and once afterwards. We found evidence for positive effects of the training: although the staff's positive self perception of the climate of communication remained stable and mainly not affected by the training, this was contradictory to the observations. The way of giving information to the residents was improved by the training program as well as the quality of relations between staff and residents. Again, sending messages about oneself which are not care-oriented had not been affected by the training-as to the observations of care giving situations. Although the staff's self perception about the change of sending these messages was highly positive. Thus, the further development of the training program has to consider these effects. To establish the transfer of the training program into daily care giving, it proved to be helpful to specify exercises after each session which had to be carried out and discussed by the participants until the next training session.
To determine the reason for differing shunt rates based on electroencephalographic (EEG) and neurologic changes during general and regional anesthetic, respectively, we compared simultaneous EEG tracings and neurologic status in 135 patients undergoing carotid endarterectomy (CEA) under cervical block over a 30-month period. The decision to shunt in these patients was made on the basis of neurologic changes only irrespective of EEG findings. This group was then compared to the 288 patients undergoing CEA under general anesthetic with EEG monitoring over the same period. EEG changes occurred in 7.4% of awake patients and 15.3% of asleep patients (p < 0.03). The rates of ipsilateral hemispheric changes were similar, but no awake patient manifested global EEG changes with clamping while 3.5% of patients under general anesthesia did (p < 0.04). Global, but not hemispheric, changes were correlated with systolic blood pressure variability during clamping. This implies that global EEG changes in anesthetized patients may be the result of the anesthetic technique itself, and that cervical block may in fact be cerebroprotective.

This study investigated perceptions of the communication performance of 31 young adults with severe learning disabilities enrolled in a vocational-education program. Communication assessment questionnaires, designed to rate communication skills of these students, were completed by the students themselves and vocational educators. These questionnaires included items in the areas of comprehension, verbal expression, social communication, and problem solving. Overall, the students' perceptions of their own communication performance were remarkably similar to those of the vocational educators. However, students perceived their communicative performance as less problematic than did others. Of the four areas pertinent to communication, students and vocational educators perceived problem solving as most difficult. Implications of these findings are discussed with reference to the development of assessment procedures for adults with learning disabilities.
