Clearance of pulmonary edema fluid is accomplished by active ion transport, predominantly by the alveolar epithelium. Various ion pumps and channels on the surface of the alveolar epithelial cell generate an osmotic gradient across the epithelium, which in turn drives the movement of water out of the airspaces. Here, the mechanisms of alveolar ion and fluid clearance are reviewed. In addition, many factors that regulate the rate of edema clearance, such as catecholamines, steroids, cytokines, and growth factors, are discussed. Finally, we address the changes to the alveolar epithelium and its transport processes during acute lung injury (ALI). Since relevant clinical outcomes correlate with rates of edema clearance in ALI, therapies based on our understanding of the mechanisms and regulation of fluid transport may be developed.

The human cardiovascular system and regulation of fluid volume are heavily influenced by gravity. When decreasing the effects of gravity in humans such as by anti-orthostatic posture changes or immersion into water, venous return is increased by some 25%. This leads to central blood volume expansion, which is accompanied by an increase in renal excretion rates of water and sodium. The mechanisms for the changes in renal excretory rates include a complex interaction of cardiovascular reflexes, neuroendocrine variables, and physical factors. Weightlessness is unique to obtain more information on this complex interaction, because it is the only way to completely abolish the effects of gravity over longer periods. Results from space have been unexpected, because astronauts exhibit a fluid and sodium retaining state with activation of the sympathetic nervous system, which subjects during simulations by head-down bed rest do not. Therefore, the concept as to how weightlessness affects the cardiovascular system and modulates regulation of body fluids should be revised and new simulation models developed. Knowledge as to how gravity and weightlessness modulate integrated fluid volume control is of importance for understanding pathophysiology of heart failure, where gravity plays a strong role in fluid and sodium retention.
Mechanisms exist in the ruminant to detect changes in osmolality and volume of plasma during feeding to maintain fluid and electrolyte homeostasis. Feed intake during a meal can be limited by the rise in osmolality of ruminal fluid, which is sensed in the wall of the rumino-reticulum. Ruminal microbes appear to be resilient to the short-term changes in ruminal fluid tonicity, but in vitro growth rates are inhibited when the tonicity of the culture medium is increased beyond physiological levels. Although mixing contractions of the rumen are not inhibited by the normal increases in tonicity of ruminal fluid, time to first rumination is increased. This aspect of motility requires further research. The tonicity of plasma increases toward the end of a large meal as a consequence primarily of absorption of VFA and Na+ from the rumen and fluid shifts into the gut. This hypertonicity is sensed centrally to inhibit parotid secretion by a reduction in the parasympathetic stimulation to the gland. Increases in animal production may result from future research directed toward developing ways of counteracting these negative effects of hypertonicity in body fluids on feed intake and ruminal function.
