Soft tissue profile changes from 5 to 45 years of age.

The purpose of this study was to describe the changes in five soft tissue parameters that are commonly used by orthodontic practitioners in their diagnosis and treatment planning as well as in their evaluation of profile changes that occur with growth and orthodontic treatment. The subjects in this study were 20 males and 15 females for whom lateral cephalograms were available between 5 and 45 years of age. The parameters evaluated were two angles of facial convexity, the Holdaway soft tissue angle, and the relationship of the upper and lower lips to Rickett's esthetic line. Descriptive statistics for the absolute and incremental changes were collected on a yearly basis between 5 and 17 years of age as well as at early (25 years) and middle (45 years) adulthood. Growth profile curves were constructed for each parameter to describe the age-related changes in the five parameters for both males and females. The analysis of variance was used to compare the absolute and incremental changes both longitudinally and cross-sectionally. Significance was predetermined at P </=.05. The present findings indicated that (1) in general, the changes in males and females were similar in both magnitude and direction. On the other hand, the timing of the greatest changes in the soft tissue profile occurred earlier in females (10 to 15 years) than in males (15 to 25 years); (2) the angle of soft tissue convexity that excludes the nose expressed little change between 5 and 45 years; (3) the upper and lower lips became significantly more retruded in relation to the esthetic line between 15 and 25 years of age in both males and females; the same trends continued between 25 and 45 years of age; (4) the Holdaway soft tissue angle progressively decreased between 5 and 45 years of age. It is important for clinicians to be aware of these changes when planning the orthodontic treatment of the still growing adolescent patients because the changes might influence the extraction/nonextraction decision.

Comparison of conventional standing knee radiographs and magnetic resonance imaging in assessing progression of tibiofemoral joint osteoarthritis.

OBJECTIVE: Although the current recommendation is to measure radiographic joint space width (JSW) to assess structural change in osteoarthritis (OA), there is increasing interest in direct measurement of cartilage volume from magnetic resonance imaging (MRI). We performed a longitudinal study to compare change in both JSW and articular cartilage volume in subjects with symptomatic knee OA. METHODS: JSW was measured in 28 subjects with knee OA (57% females, mean age 62.8+/-9.8 years) who had standing radiographs in full extension, where both radiographs had satisfactory alignment. Each subject had femoral, tibial and combined femoral and tibial cartilage volumes determined from T1-weighted fat saturated sagittal knee MRI. All subjects had a repeat of the knee radiograph and MRI 1.96+/-0.4 years later. RESULTS: At baseline there was a moderate, but statistically significant, correlation between JSW and femoral and tibial cartilage volumes in the medial tibiofemoral joint, which was strengthened by adjusting for medial tibial bone size (R=0.58-0.66, P=0.001). Although we observed a reduction in JSW and femoral and tibial cartilage volumes over the study period, there was no significant association between reduction in JSW and cartilage volume (R<0.13). There was a trend towards a significant association between change in medial tibiofemoral cartilage volume and joint replacement at 4 years (OR=9.0, P=0.07) but not change in medial tibiofemoral JSW (OR=1.1, P=0.92). CONCLUSIONS: Although there was a modest correlation between cartilage volume and JSW in the medial tibiofemoral compartment, there was no correlation between longitudinal change in these measures. Change in cartilage volume appears to be a better predictor of joint replacement. Further work in larger samples over a longer period of time will be needed to confirm these findings.

Deformation-based morphometry of brain changes in alcohol dependence and abstinence.

Brain atrophy associated with chronic alcohol consumption is partially reversible after cessation of drinking. Recovering alcoholics (RA, 45+/-8 years) were studied with MRI within 1 week of entering treatment, with follow-up at 8 months. Light drinkers (LD) were studied with MRI twice 1 year apart. For each participant, deformation maps of baseline structure and longitudinal size changes between baseline and follow-up scans were created using nonlinear registration techniques. ANCOVA assessed group differences and regression methods examined relationships between deformation maps and measures of drinking severity or baseline atrophy. At baseline, RA showed significant atrophy in the frontal and temporal lobes. Longitudinally, abstainers recovered tissue volumes significantly faster than LD in parietal and frontal lobes. When comparing abstainers to relapsers, additional regions with significantly greater recovery in abstainers were temporal lobes, thalamus, brainstem, cerebellum, corpus callosum, anterior cingulate, insula, and subcortical white matter. Gray matter volume at baseline predicted volume recovery during abstinence better than white matter. Drinking severity was not significantly related to brain structural changes assessed with this method. Longitudinally, deformation-based morphometry confirmed tissue recovery in RAs who maintain long-term sobriety. Abstinence-associated tissue volume gains are significant in focal parts of the fronto-ponto-cerebellar circuit that is adversely affected by heavy drinking.

Longitudinal follow-up study of smoking-induced lung density changes by high-resolution computed tomography.

To evaluate the ability of high-resolution computed tomography (HRCT) to detect longitudinal changes in structural abnormalities caused by smoking, HRCT and pulmonary function tests were used to examine nonsmokers, current smokers, and past smokers annually for 5 yr. Inspiratory HRCT was taken for the upper, middle, and lower lung fields, while expiratory images were obtained for the upper and lower lung fields only. We estimated the three quantitative CT parameters including MLD (mean CT value), HIST (CT value with the most frequent appearance), and %LAA (relative area of low attenuation with CT values less than -912 HU). Most of the pulmonary function tests, excepting FEV(1), did not change annually, whereas many of the inspiratory HRCT parameters did. In nonsmokers, only %LAA in the middle or lower lung fields exhibited an annual increase. In current smokers, %LAA in the upper lung field was augmented, while inspiratory MLD or HIST in the middle or lower lung field became more positive. In past smokers, %LAA in any lung field examined increased. The annual change in %LAA in the upper lung field was larger for past smokers than nonsmokers, with little difference between past and current smokers. Expiratory CT parameters showed few annual changes in all groups. In conclusion, (1) aging increases airspace abnormalities, mainly in the lower lung field; (2) although continuous smoking worsens airspace abnormalities mainly in the upper portion of the lung, this trend does not seem to slow down even after smoking cessation; and (3) inspiratory HRCT images are superior to expiratory images for longitudinal estimation of structural abnormalities caused by aging and smoking.

Progression of radiographic changes in the temporomandibular joints of patients with rheumatoid arthritis in relation to inflammatory markers and mediators in the blood.

The aim of this study was to investigate longitudinal radiographic changes in the temporomandibular joint (TMJ) with clinical involvement of rheumatoid arthritis (RA) and its relation to the blood level of inflammatory mediators and markers. Sixteen patients were investigated by computed tomography on two occasions 25-46 months apart. The radiographs were assessed independently for changes in presence of erosions, sclerosis, flattening, osteophytes, and subchondral pseudocysts. The serum (S) or plasma (P) concentrations of C-reactive protein (CRP), thrombocyte particle concentration, scrotonin (S-5-HT and P-5-HT), tumor necrosis factor alpha, interleukin-1 receptor antagonist, tumor necrosis factor soluble receptor type II, interleukin-1 soluble receptor type II (P-IL-1sRII) and interleukin 6 as well as the erythrocyte sedimentation rate (ESR) were measured. The radiographic status showed no consistent or significant change during the observation period, but the individual variation was considerable. The radiographic signs of erosion and sclerosis varied most. Regression of erosions was associated with high S-5-HT and P-IL-1sRII, while progression of erosions was associated with high P-5-HT. Regression of sclerosis was associated with an increase in P-5-HT and high ESR. Progression of flattening was associated with high CRP. In conclusion, this study indicates that the progression of radiographic changes that occurs in the TMJ of patients with well-controlled RA during a period of 25-46 months seems to be related to the blood levels of CRP, 5-HT, and IL-1sRII. However, only minor progression can be expected to occur, and with considerable individual variation.

Changes in insulin sensitivity and glucose and bone metabolism over time in patients with asymptomatic primary hyperparathyroidism.

OBJECTIVE: The clinical profile of asymptomatic primary hyperparathyroidism (PHPT) has shifted from a symptomatic disorder toward a more subtle, asymptomatic state. The lack of knowledge about the natural course of asymptomatic PHPT contributes to the controversy regarding the optimal management of these patients. The aim of this study is to evaluate the natural course of calcium and glucose metabolism abnormalities, insulin sensitivity, and bone mineral density (BMD) in subjects with asymptomatic PHPT over 18 months. DESIGN: The study was designed as a prospective observational examination of asymptomatic PHPT patients at baseline and at 6-month intervals for 18 months. METHODS: Our study examined 61 patients with asymptomatic PHPT and 80 healthy control subjects matched for age, sex, and body mass index. We evaluated calcium metabolism, glucose metabolism by oral glucose tolerance test, insulin sensitivity by homeostasis model assessment index, and BMD by dual-energy x-ray absorptiometry at distal radius, lumbar spine, and femur. RESULTS: Although in asymptomatic PHPT patients, homeostasis model assessment index at baseline was slightly higher than in controls (3.0 +/- 2.2 vs. 2.2 +/- 1.3; P = 0.035), the prevalence of preexisting diabetes mellitus (13.1 vs. 11.3%), undiagnosed impaired fasting glucose (34.4 vs. 30%), impaired glucose tolerance (13.1 vs. 18.8%), and newly diagnosed diabetes mellitus (4.9% vs. 2.5%) was similar. Baseline BMDs (g/cm2) at all three sites were not different between two groups. Six-month interval measurements showed no change in calcium metabolism parameters including serum calcium, phosphorus, 25-hydroxyvitamin D, and 24-h calcium excretion. No significant longitudinal adverse changes were noted in glucose metabolism, insulin sensitivity, or BMD at any site over the 18-month observation period. CONCLUSIONS: Our follow-up of patients with asymptomatic PHPT revealed no progression of calcium and glucose metabolism abnormalities, insulin sensitivity, and loss of BMD during the 18-month study period.

A longitudinal study of brain volume changes in normal aging using serial registered magnetic resonance imaging.

OBJECTIVE: To investigate the effect of age on global and regional brain volumes and rates of atrophy, and to compare directly results based on cross-sectional and longitudinal data. METHODS: Thirty-nine healthy control subjects (age range, 31-84 years) underwent serial magnetic resonance imaging assessments. Measurements included the whole-brain, temporal lobe, hippocampal, and ventricular volumes at baseline and for repeat scans. RESULTS: We found significant decreases in cross-sectional whole-brain (P<.001), temporal lobe (P<.001), and hippocampal (P =.003) volumes and a significant increase in ventricular volume (P<.001) with increasing age. Cross-sectional and longitudinal estimates of atrophy rates were similar. We also found directional evidence of acceleration in atrophy rates with increasing age in all analyses, with the most marked changes occurring after 70 years of age. This increase in rates after 70 years of age was particularly marked in the ventricles (P<.001) and the hippocampi (P =.01). CONCLUSIONS: We found a significant age-associated decrease in global and regional brain volumes. Some evidence indicates that this decline in brain volumes may be due to a nonlinear acceleration in rates of atrophy with increasing age. A better understanding of this process may help to discriminate normal age-related changes from neurodegenerative diseases.

Postural changes in dental hygienists. Four-year longitudinal study.

Numerous surveys identify the occurrence of musculoskeletal complaints as a concern in dentistry. However, no longitudinal data exist to indicate whether postural changes occur as a result of practicing dental hygiene. The purpose of this preliminary, four-year longitudinal study was to investigate whether any postural changes developed during the hygienists' clinical education and/or during subsequent dental hygiene practice after one and/or two years. It was anticipated that the awkward positions and intense physical demands placed on hygienists might initiate musculoskeletal problems, but that no postural changes would occur over this short period of time. Nine of 10 dental hygienists in the graduating class of 1987 were surveyed for existing musculoskeletal complaints, and the subjects were photographed for a measurement of postural change. Responses from participants indicated an increase in musculoskeletal-related complaints in each of the six areas investigated. The photographic findings indicated that one of the nine hygienists showed an increase in forward head posture, a postural change.

Predictors of parenting behavior trajectories among families of young adolescents with and without spina bifida.

OBJECTIVE: To evaluate the utility of familial and parental variables in predicting trajectories of parenting behaviors among families of young adolescents with and without spina bifida (SB). METHOD: Sixty-eight families with a child with SB and a demographically matched comparison group (CG) of 68 families of an able-bodied child participated. Observational and questionnaire assessments of parenting behavior were collected via home visits at three time points, as were reports of parent and family functioning. RESULTS: Family conflict was negatively associated with adaptive parenting behavior at Time 1 (T1), but positively associated with adaptive parenting change. Although the direction of this effect was the same across both groups, findings were more robust for the SB sample. Among fathers of children with SB, parenting stress was positively associated with adaptive parenting at T1 but negatively associated with adaptive parenting change. In contrast, within the CG, paternal parenting stress was negatively associated with adaptive parenting at T1 but showed no enduring negative effects in longitudinal analyses. CONCLUSIONS: Family conflict and parenting stress were significant predictors of parenting behaviors and longitudinal parenting change. Findings are interpreted within a developmental context such that variables associated with maladaptive (or adaptive) parenting in the short run, may facilitate adaptive (or maladaptive) parenting over time based on young adolescents' changing developmental needs.

Assessment of individual changes in body fatness in boys during early pubertal period.

BACKGROUND: In order to know whether the changes in indexes of overweight, body mass index (BMI; kg/m2) and percentage of overweight (POW) (%), really represent the tendency toward obesity, we examined longitudinal individual changes in these indexes, and the change in body composition in boys during early pubertal period. METHODS: The subjects were 50 school boys who lived in Shizuoka, Japan. Standing height and weight were measured, and BMI and POW were obtained. Body fat percentage (BF%), fat mass (FM) and lean body mass (LBM) were estimated by bioelectrical impedance method. The influence of predictive variables (LBM, FM) on the changes in BMI and POW was investigated by multiple regression analysis. We examined the 3-year changes in BMI, POW and predictive variables in each individual, from 9 to 12 years of age. RESULTS: The mean change of BMI was 1.7 +/- 0.3 (mean +/- SEM) kg/m2 and that of POW was 2.2 +/- 1.9%. The influence of predictive variables on the changes in BMI and POW was determined. The variance of the change in POW could be explained by that in FM (r(2) = 0.737, P < 0.0001), while the change in BMI was influenced by both LBM and FM (r2 = 0.891, P < 0.0001). CONCLUSIONS: Based on the definition of obesity, POW is the better index of body fatness to assess its individual change for boys during early pubertal period, because the index independent from the change in LBM is supposed to be the appropriate index for obesity in practical use.

The knife-edge tendency in mandibular residual ridges in women.

To investigate the bone resorption pattern of the residual alveolar bone, the morphologic change that occurred in mandibles was analyzed with standardized lateral cephalographs of 30 edentulous patients (15 women and 15 men). The longitudinal morphologic changes were measured at the sagittal sections of the mandibular bony contour at the symphysis area on superimposed cephalographic tracings. To quantify the morphologic change, a knife-edge index (KEI) was developed as the area change divided by the height change. Geometrically, the higher value of KEI represents the greater tendency to become a narrow residual ridge. The KEI values were statistically higher in the women than in the men (p less than 0.002). In addition, the value of KEI seems to correlate with osteopenic change at the center point of the body of the second vertebra (p less than 0.01). The continuous bone resorption activity in the edentulous mandible of women seems to be emphasized at the labial and lingual surfaces of the residual alveolar bone, resulting in a knife-edge type of residual ridge.

Effects of thyrotropin releasing hormone on cardiac and extracardiac flows of appropriately grown and growth-retarded fetuses.

The objective of this study was to establish the effect of thyrotropin releasing hormone on the fetal circulation of appropriately grown and growth-retarded fetuses. Doppler recordings were performed immediately before and 15 min after maternal administration of 400 micrograms of thyrotropin releasing hormone in 14 women with appropriately grown and 19 with growth-retarded fetuses. Furthermore, in six women with growth-retarded fetuses, serial recordings were performed at 2-hourly intervals for 8 h until a second thyrotropin releasing hormone dose was administered. Velocity waveforms were recorded from the outflow tracts, inferior vena cava, umbilical artery, middle cerebral artery and descending aorta, and different Doppler indices calculated. In appropriately grown fetuses, thyrotropin releasing hormone induced a significant increase of peak systolic velocity values in the aorta and pulmonary artery and an increase of Doppler-estimated cardiac output. In growth-retarded fetuses, these changes were more evident and associated with a significant decrease in reverse flow in the inferior vena cava during atrial contraction. No changes were observed in fetal heart rate or in the other fetal vessels investigated. In the growth-retarded fetuses followed longitudinally, these changes were evident for the following 8 h and were potentiated by the second thyrotropin releasing hormone administration. In conclusion, thyrotropin releasing hormone induces significant hemodynamic effects on the fetal heart that may temporarily improve the impaired cardiac function of growth-retarded fetuses.

The relationship between corneal eccentricity and improvement in visual acuity with orthokeratology.

BACKGROUND: Due to the renewed attention given to reduction of myopia, interest in orthokeratology has dramatically increased. This study was performed to determine whether or not a predictor for orthokeratologic changes can be identified. METHODS: Fifteen subjects enrolled in the study. For each subject, corneal eccentricity (e) was measured, subjective refraction and autorefraction were performed, and intraocular pressures were taken. Each subject was then fitted with a rigid contact lens for orthokeratology and followed for a minimum of 16 weeks. Eleven subjects completed the study. RESULTS: No correlation between corneal eccentricity and improvement in visual acuity was found. Correlations were found between e and changes in subjective refraction, and between e and changes in autorefraction. There was also correlation between lower intraocular pressure and changes in subjective refraction, and between lower intraocular pressure and changes in autorefraction. CONCLUSIONS: These findings suggest that corneal eccentricity and lower intraocular pressures may be predictors of orthokeratologic changes. A longitudinal study with a larger subject size will be needed to conclusively determine if these two measurements are predictors of orthokeratologic changes.

Job experiences over time, multiple roles, and women's mental health: a longitudinal study.

Are changes over time in the quality of a woman's job associated with changes in her psychological distress? Do family roles moderate these relationships? We addressed these questions using longitudinal data from a 2-year 3-wave study of a stratified random sample of 403 employed women who varied in occupation, race, partnership, and parental status. After estimating individual rates of change for each woman on each of the predictors and the outcome, we modeled the relationships between family role occupancy and change in job-role quality on the one hand, and change in psychological distress on the other. Among single women and women without children, as job-role quality declined, levels of psychological distress increased. Among partnered women and women with children, change in job-role quality was unrelated to change in psychological distress.

Predictors of change in plasma total cysteine: longitudinal findings from the Hordaland homocysteine study.

BACKGROUND: Total cysteine (tCys) in plasma has recently been linked to cardiovascular risk and is also associated with cardiovascular risk factors, including body mass index (BMI) and cholesterol. Changes and predictors of change in tCys concentrations over a mean follow-up time of 6.0 (5.2-7.2) years were assessed in this study. METHODS: Baseline data from the Hordaland Homocysteine Study recorded in 1992-1993 included tCys, total homocysteine (tHcy), and various lifestyle and cardiovascular risk factors. In 1998-1999, the same measurements were repeated in 3,732 individuals born in 1950-1951 and 3,339 individuals born in 1925-1927. Most of the statistical analyses were done separately in the four age and sex groups. RESULTS: The overall mean values of tCys were higher at follow-up [mean (SD), 296 (41) micro mol/L] than at baseline [278 (36.5) micro mol/L]; P <0.0001. The mean percentage of increase in tCys in the different age and sex groups ranged from 4.9% to 8.5%. There was a significant correlation between the tCys values measured on the two occasions (Spearman correlation coefficient, 0.55-0.59 in the different age and sex groups; P <0.0001). The change in tCys correlated with changes in BMI, cholesterol, and diastolic blood pressure in the younger age group, whereas only changes in BMI predicted changes in tCys in the older age group. CONCLUSIONS: tCys increased in the 6 years between the two measurements. Factors related to the baseline tCys values, including BMI and the change in BMI, predicted the tCys changes over time.

Changes in the values of dental students and dentists over a ten-year period.

This paper investigates value changes in dental students and dentists over a ten-year period through a cross-sectional study of all dental students at the University of Minnesota in 1966 and 1976, and a ten-year longitudinal study of a class of dental students who entered in 1967. Findings in both studies showed significant differences in values over the ten-year period, but they could not be attributed to the dental school experience. Findings also showed that the values of the dentists in the longitudinal study changed significantly over the ten years from 1967 to 1977, to resemble those of dental school freshmen in 1976. Comparisons of the two studies indicated that changes in social values and the social pressures of professional role development could have accounted for the findings.

Near infra-red interactance for longitudinal assessment of nutrition in dialysis patients.

OBJECTIVE: Serial nutritional assessment of dialysis patients is important because of the high incidence and prevalence of malnutrition in these patients. Near-infrared interactance (NIR) technology may provide a practical and reliable method to evaluate body fat and its changes over time in dialysis patients. DESIGN: Longitudinal study consisting of 2 cross-sectional measurements, 2 months apart. SETTING: Outpatient dialysis unit affiliated to a tertiary care community medical center. PATIENTS: Seventy-one dialysis patients (35 men, 36 women), 57 +/- 15 years old, who have been on dialysis between 5 months and 11 years (43 +/- 30 months). Twelve additional patients with similar features were studied during the second round. INTERVENTION: None. MAIN OUTCOME MEASURES: NIR was used to estimate the body fat percentage. Other simultaneous measurements included subjective global assessment, anthropometric indices including midarm circumference, triceps and biceps skinfold thickness, and body mass index, and some laboratory values including albumin, transferrin, and cholesterol. NIR measurement was performed by placing a Futrex sensor on the nonaccess upper arm for several seconds, after logging the required individual data (sex, weight, height, and body frame), along with uniform physical activity levels for all patients, into a mini-computer. RESULTS: Seventy-one dialysis patients underwent nutritional and laboratory measurements. A second measurement round was performed 8 to 9 weeks after the first one and included 12 additional patients. Within each cross-sectional round, Pearson correlation coefficients (r) between the NIR score and nutritionally relevant variables were significant for anthropometric values (0.56 to 0.82) as well as low cholesterol and creatinine (0.22 to 0.30). The two serial NIR measurements on the same patients were highly consistent over the 2-month study interval (r = 0.96), whereas anthropometric values showed greater variability. The within-person coefficient of variation for NIR was low, indicating high consistency between 2 measurements. Moreover, the timing of the NIR measurement (predialysis v postdialysis) did not have any impact on consistency of the NIR results. The longitudinal changes of NIR had significant correlations with anthropometric and laboratory changes over time. CONCLUSION: The NIR, which can be performed within seconds, may serve as a reliable and practical tool for objective measurements of nutritional status in hemodialysis patients. The NIR not only seems to have a high degree of reproducibility but may also be an optimal tool to detect longitudinal changes in body fat over time. The NIR measurement is independent of the fluid status in dialysis patients. More comparative and longitudinal studies are needed to confirm the validity of NIR measurements in longitudinal evaluation of dialysis patients.
