[Positron emission tomography of the thorax. The current clinical status].

The non-invasive diagnostic assessment of thoracic lesions has been greatly enhanced by the use of positron emission tomography with metabolic active radio-compounds such as fluor-18 labeled deoxyglucose. Four clinical fields of use were established: differential diagnostic evaluation of lesions (T-staging); classification of adenopathy (N-staging); therapy response monitoring; diagnostic evaluation of recurrence. The upcoming challenges for clinical use are limited availability and necessary technical and qualitative standardizations.

[The connection between hospital volume and outcome in thorax surgery].

A review of current literature with respect to a possible correlation among hospital volume, caseload and outcome of thoracic and cardiovascular surgery supports quite convincingly the theory that, from a probability point of view, the combination of high-volume hospitals with high-volume surgeons produces the best results. During the last decade, thoracic and cardiovascular surgery has to a certain degree been centralized in Denmark, reducing the number of public centres to the five university hospitals. None of these centres is low-volume, and two are high-volume hospitals.

[Descending necrotizing mediastinitis treated by mediastinal drainage through bilateral thorax; report of a case].

Descending necrotizing mediastinitis is a severe infection spreading from the cervical region to the mediastinal connective tissue. The mortality rate was high and appropriate treatment is necessary to obtain favorable results. The present study describes a case of a 69-year-old man with severe descending necrotizing mediastinitis due to parapharyngeal abscess. The patient was successfully treated with cervical drainage, surgical debridement of the mediastinum via small thoracotomy. Postoperatively, continuous mediastinal and pleural irrigation with saline was performed. Swallowing disturbance due to dissection of cervical muscles prolonged duration of hospital stay. Eventually, the patient recovered and was able to eat by muscle rehabilitation.

[War injuries of the thorax].

Chest injuries are very common in war circumstances. Chest injury is commonly associated with other injuries of intrathoracic organs and development of shock, which has to be treated immediately. Our patients with war chest injuries are presented, along with our approach to the treatment of these injuries. "The conservative treatment" is stressed, with particularly importance of proper chest drainage, as a corner stone of proper and successful therapy. Shock treatment and prevention of atelectasis does present an unavoidable procedure of this type of therapy.
