[The x-ray picture of pulmonary circulation in the course of acute myocardial infarct].

A study was made of the time course of lesser circulation (LC) in 80 patients with acute transmural myocardial infarction and in 20 patients with chronic coronary heart disease with the help of chest x-ray. A possibility of preclinical detection of left ventricular insufficiency was confirmed. The capacity of roentgenography to reflect objectively the time course of LC disorder and its correlation with primary determination of a degree of changes, age, cardiac sizes, the expression of aortic atherosclerosis, pleurocardiac reactions and site of myocardial infarction were defined.

A pathological study of experimental chemonucleolysis with collagenase.

This study was undertaken to examine and quantitate the chemical injury and repair of the dog nucleus pulposus in collagenase-induced chemonucleolysis. Thirty three adult mongrel dogs were used into which the purified collagenase was injected at a rate of 200 units. The dogs were sacrificed after roentgenography. Disc specimens were stained primarily with safranin-O for microscopic study, and fresh tissues were used for electron microscopic study. For immunohistochemical analysis, paraffin sections were reacted with anti-bovine types I, II and III collagen antibodies. Two weeks after the injection, the height of the injected disc was reduced by about 50% and there was no sign of recovery for 52 weeks. There was a profound loss of safranin-O stainability in the nucleus pulposus, annulus fibrosus and cartilagenous endplate. By 8 weeks, regeneration of the nucleus pulposus began and chondrocytes appeared from the junctional area. The regenerated nucleus pulposus was stained well with safranin-O and consisting largely of type II collagen immunohistochemically. In the regenerated nucleus pulposus, collagen fibers and proteoglycans were reconstructed in a net-work with hyaluronic acids.

[Mesenchymal chondrosarcoma. Anatomico-clinical, immunohistochemical and ultrastructural study. Apropos of a case].

One case of mesenchymal chondrosarcoma of the mandible is reported in a 18 years-old man. The patient noted swelling and pain and roentgenography showed an osteolytic lesion of the right mandible. Histologically, the feature was obvious with chondroid sheets of round ovoid or elongated cells. Immunohistochemically, numerous cells had positive S-100 protein and vimentin. The electron microscopy showed numerous undifferentiated cells and also fibroblastic and cartilaginous cells. The prognosis of this tumor, usually bad, cannot be yet appreciated in this case (follow-up of only 3 months). The histogenesis of this peculiar variety of chondrosarcoma is discussed.

CT scanography for limb length determination.

The authors present an alternative to classic techniques used to measure limb length discrepancy radiographically. CT scanography seems to have advantages over currently-used Bell-Thompson roentgenography in that it uses less radiation and is of no increase in cost.

[A case of Krukenberg tumor associated with elevation in blood testosterone].

One case of Krukenberg tumor associated with elevation in blood testosterone levels was reported. The patient was a 31-year-old woman. She visited the outpatient service of our hospital with the chief complaint of uterine bleeding and infertility. She was diagnosed to have an anovulatory cycle associated with elevation in blood testosterone levels. However, after an examination at our hospital, a tumor of the left ovary was pointed out and she underwent bilateral ovariectomy. Postoperatively, tarry stool was passed, for which she received an examination at the department of internal medicine. Roentgenography of the stomach and examination by a gastric camera revealed undifferentiated carcinoma of Borrmann's type III in the greater curvature of the angular area. This lesion was diagnosed to be Krukenberg's tumor and a gastrectomy was performed. On the other hand, the blood testosterone level which had shown elevation before resection of the ovaries was restored to the normal range after the operation. Various reports have been published concerning the endocrinological activity of this tumor. It remains to be elucidated whether it is due to metastasis of the carcinoma or if it originates from other ovarian parenchyma. However, it has been reported that elevation in endocrinological activity is apt to occur when the tumor originates from the digestive tract.

Radiologic and ultrasonographic diagnosis of gallstones.

Several new nonsurgical modes of therapy for gallstone disease are being introduced almost simultaneously in the United States. These include orally administered bile acids, contact dissolution with methyl t-butyl ether (MTBE), and extracorporeal shock wave lithotripsy. Paralleling these developments is a similar period of transition in the field of gallstone imaging. Technical considerations in performing ultrasonographic, roentgenographic, and cholecystographic studies become more important, since the criteria for use of the nonsurgical techniques place a greater emphasis on gallstone morphology. In ultrasonography, sizing artifacts from reflective surfaces, optimizing measurements, magnifying hard-copy images, positioning the patient, and optimizing transducer frequencies will need to be addressed. In roentgenography, coned views of the right upper quadrant, with the patient prone, will give the best results. Oral cholecystography (OCG) has largely been replaced by ultrasonography for the detection of gallstones, but it is still an essential part of the imaging workup because it permits adequate visualization of the gallbladder to assure patency of the cystic duct. Correlation of the findings from the several techniques has disclosed new insights and encouraged the use of tailored techniques. Based on the current criteria for the use of orally administered ursodeoxycholic acid, an estimated 60-70% of all patients who have gallstones may be suitable candidates for such treatment.

[Drug-induced bronchial asthma].

In any case of acute bronchoconstriction the possibility of an adverse reaction to a drug should be considered. In many of such side reactions no allergic mechanism can be detected. Therefore, they are included into the category of pseudoallergic reactions (PAR). The clinically most important form of drug-induced bronchial asthma, analgesics asthma, belongs to this PAR group. A further risk for asthmatics are intravenous applications of contrast-media for roentgenography which in about 15% induce a severe, sometimes life-threatening pseudo-allergic adverse reaction. In asthmatics, the application of any beta-receptor blocking agents and also the use of parasympathicotonic eye drops for treatment of glaucoma are contraindicated. Paradoxical bronchial constriction following application of antiasthmatics are preponderantly caused by locally irritative actions, less frequently by genuine allergic phenomena or additive intolerance. The most reliable prophylaxis against drug-induced bronchial asthma consists in strong avoidance of all derivatives possibly capable to trigger any intolerance. A respective warning should entered into the emergency passport.

[Diagnostic strategies of the small and large intestine].

The selection and sequence of different diagnostic procedures, such as abdominal X-ray, contrast roentgenography, ultrasonography, endoscopy and angiography, depend on the presenting symptoms and the presumptive diagnosis. In addition, the acuteness of the disease and the requirements for planning the operation determine the diagnostic time course. Novel image processing devices, such as endosonography and computerized tomography, may improve diagnosis, especially in rectal diseases. Successful outcome of diagnostic examinations can be achieved only by differentiated use of radiologic and endoscopic methods.

An epidemiological survey on ossification of ligaments in the cervical and thoracic spine in individuals over 50 years of age.

An epidemiological survey on ossification of the spinal ligaments was performed on a total of 1,058 subjects over the age of 50 years by means of roentgenography of the cervical and thoracic spine. Ossification of the posterior longitudinal ligament (OPLL) of the cervical spine was detected in 34 subjects (3.2%) with a predilection for men, whereas OPLL in the thoracic spine was found in 8 (0.8%). There were 325 cases (30.7%) of ossification of the anterior longitudinal ligament (OALL) of stage II or above by Forestier's classification in the region from the cervical to thoracic vertebrae, and these cases included a significantly greater number of men. Ossification of the ligamenta flava (OLF) was observed in 48 cases (4.5%). As for the coexistence of ossification of these ligaments, 364 individuals (34.4%) had at least one instance of OPLL and OALL (stage II or above) in the region from the cervical to thoracic spine, and OLF in the thoracic spine.

[Successful treatment of advanced gastric cancer by combined cisplatinum, mitomycin C and 5-FU administration. A case report].

A 55-year-old woman with gastric cancer underwent laparotomy and was found to have an unresectable tumor at the MC region, characterized by S3 (invasion into the pancreas), N3, P0, H0, Borrmann-II and moderately differentiated tubular adenocarcinoma. She was then treated by combined administration of cis-dichlorodiammine platinum (CDDP), mitomycin C and 5-fluorouracil (5-FU). A complete response was confirmed by upper gastrointestinal roentgenography and endoscopy. Upon reoperation 4 months after the initial laparotomy, a successful total radical gastrectomy with R2 (partially R3) lymph node dissection was performed. Histological examination of the specimens, including the stomach and lymph nodes, revealed no cancer cells in any region.

[X-ray examination for the detection of complications after abdominal surgery (author's transl)].

The incidence of complications in a consecutive series of 515 abdominal surgical operations was studied. Abdominal complications of varying degree of severity were observed in 62 cases (16%). They were of four types: haemorrhage, ileus, peritonitis and complications on the part of the biliary tract and pancreas. The use of radiological methods for detecting these complications is reviewed. Direct roentgenography is the method of choice in cases of suspected ileus, perforation or foreign body. Administration of contrast media should be reserved almost entirely to cases of stress ulcer and to distinguish between incomplete and complete ileus. If perforation of penetration is suspected only water-soluble contrast media should be used. Detection of a leaking anastomosis or an abscess is by injection of the fistula. The value of angiography for revealing a haemorrhage or embolism and for assessing the response to vasoactive agents is discussed.

[A case of renal angiomyolipoma with bone formation].

A case of renal angiomyolipoma with bone formation is reported. A 61-year-old woman was admitted to our hospital with one month history of fever. About 15 cm in diameter mass was palpated in her right lower abdomen. Plain abdominal roentgenography, ultrasonography and computed tomography showed marked calcification in the cephalic portion of the right kidney, tumors surrounding right kidney and another tumor in the right lower abdomen. All these tumors and the kidney constituted a big mass continuously, and no fatty component was demonstrated. Total excision of the mass including the kidney was performed. The mass was 30 X 24 X 10 cm in size and 3,240 g in weight. Histologically, the tumor was compatible to renal angiomyolipoma. But, radiopaque shadows which looked like calcification were disclosed to be bones, and fatty tissues were not seen so much. Therefore, the name "benign mesenchymoma" was thought to be more acceptable than angiomyolipoma in this case. Preoperative diagnosis of renal angiomyolipoma is the demonstration of fatty densities in the tumor, and the characteristic extrarenal development should also be taken into consideration. Furthermore, the bone formation of angiomyolipoma, which is very rare might be an important finding to angiomyolipoma.

[Contribution to x-ray diagnostics of tumors of the cerebellopontine angle (author's transl)].

A scrutiny of the palette of possibilities in the radiological examination for the discovery and identification of neurinomas of the auditory nerve, shows that, apart from plain roentgenography, which continues to occupy the first rank after clinical examination, computer tomography represents the most important contribution towards identifying tumors of the cerebellopontine angle. For the identification of neurinomas (of the Brunner type) of the medial type this method of examination is the method of choice. We are not so confident with regard to the small lateral neurinomas which are situated within the auditory meatus. We consider that the discovery of these extremely small lateral neurinomas of the auditory nerve remains extremely difficult and is mainly left to positive cisternography. Probably, the discovery of these neurinomas remains one of the happy chances of early radiological diagnosis. Nevertheless, the knowledge acquired during the past few years must be considered an essential step forward in radiodiagnostics, and hence represents a great aid in the curative treatment of the patient.

[Topographic anatomical interrelation of the lumbar triangles with the organs and adipose tissue formations of the retroperitoneal space].

The investigation has been carried out in 96 corpses of fetuses, newborns and mature persons by means of preparation methods, injection of fatty formations of the retroperitoneal space, superior urinary pathways and vessels with x-ray contrast masses and subsequent roentgenography. The area of the lumbar triangles is subjected to certain fluctuations and only in some cases they are absent. The kidneys, peritoneal part of the ureters, duodenum, pancreas, aorta and inferior vena cava in the projection of the lumbar triangles are not situated. The lumbar triangle can be used for performing drainage, when the organs and formations of the retroperitoneal space are operated on.

Developmental and radiological anatomy of the superficial cerebral convexity vessels in the human fetus.

The superficial cerebral vessels on the convex side of the cerebral hemisphere were investigated using 40 brains of aborted human fetuses (ranging from 2 to 7 months of gestational age), injected with radiopaque material either through the arterial or venous system. These brains, particularly their vessels, were studied in detail through stepwise roentgenography in stereo-pairs, photography and careful dissections. Morphological changes of the superficial vessels of the cerebral convexity reflect the developmental alterations of the cerebral structures, particularly those of the opercula and cerebral sulci. Between the gestational ages of 3 and 4 months, the middle cerebral artery and its tributaries run radially on the sylvian fossa and over the convexity. Anastomoses among the peripheral branches of the anterior, middle and posterior cerebral arteries take place on the convex side of the hemisphere. At the ages of 3 and 4 months, the middle cerebral vein and its tributaries are well developed compared with the superior and inferior cerebral (or ascending and descending cortical) veins. The middle cerebral vein and its tributaries rapidly regress from the 5th to 7th month. The superior and inferior cerebral veins, on the other hand, develop rapidly during this period. Owing to the development of the opercula and resultant depression of the insula, after seven months of gestation the middle cerebral artery and its branches, begin to differentiate into the vallecular (related to the sylvian vallecula or fronto-temporal notch), insular (related to the insular cortex), opercular (related to the frontal, parietal and temporal opercula) and convexity segments (related to the convex side of the hemisphere). The anastomotic veins of Trolard and Labbé are also recognizable at this stage. The superficial sylvian vein in the process of formation may also be detected. The configuration of the cerebral vessels on the convex side of the hemisphere is also affected greatly by the development of the cerebral sulci. These cerebral vessels may run along, or dip into the sulci. Since the development of the superficial cerebral vessels is closely related to that of the cerebral mantle, these studies should be correlated to detail with those of developmental alterations of the intraparenchymal vessels (medullary arteries and veins). The latter investigation will be presented in a future communication.

Iodine-123 SPECT of the thyroid in multinodular goiter.

Iodine-123 single photon emission computed tomography (SPECT) imaging of the thyroid was performed in two patients with multinodular goiter and swallowing difficulty to provide the functional and anatomic orientation of the goiter in relation to the airway. Transaxial slices showed the retrolaryngeal extension of the enlarged thyroid and the tracheal compression by the goiter in both patients. Sagittal and coronal sections confirmed the posterior extension of the goiter. Tracheal displacement was confirmed by roentgenography of the neck in both patients. Vocal cord paralysis demonstrated by fiberoptic laryngoscope and esophageal compression shown by esophagography were found in a patient with toxic multinodular goiter with coexisting papillary carcinoma of the thyroid. In this patient, both the tracheal compression noted in SPECT imaging and the tracheomalacia suggested by the flow volume loop pattern in pulmonary function test were confirmed at the time of thyroidectomy. Our observation suggests that SPECT imaging of large multinodular goiter may be useful in preoperative delineation of the functional anatomy and the extension of goiter in relation to the airway.

Radiohistology as a new diagnostic method for barium granuloma.

Barium granulomas are rare complications of the barium enema. They pose diagnostic problems to the gastroenterologist, who may suspect a carcinoma, and to the pathologist, who may have difficulty in determining the precise nature of the foreign body. From four cases we suggest a simple and quick diagnostic method: paraffin-block roentgenography.
