Subcutaneous fat necrosis of the newborn: Light and electron microscopic studies.

Specimens from two cases of subcutaneous fat necrosis of the newborn were studied by light and electron microscopy. Crystallization of the fat cell started with clearing of the central fat droplet, followed by the appearance of cylindrical crystals. The fat crystals were phagocytosed by cytoplasmic projections of macrophages as well as by foreign body giant cells. Calcium deposits were demonstrated within the central fat droplet of the degenerated fat cell of Case 2.

Comparison of computed radiography and conventional film-screen radiography of the equine stifle.

Major advantages of computed radiography are the potential reduction of dose and the possibility of postprocessing. In our study, we compared conventional radiographs to digital radiographs of the equine stifle by subjective evaluation of diagnostic quality when using a decreasing photon flux (mAs). Twelve equine stifle joints from horses of different weight and size were examined. Conventional and digital radiographs were performed identically in a caudocranial projection with the tube angled 15 degrees. A series of four radiographs was performed in each technique with an increasing photon flux starting with 2.5 mAs and going up to 5, 10, and 20 mAs. All radiographs were evaluated subjectively in a blinded fashion by seven readers in terms of contrast, bone structure, and diagnostic value and were graded using a 1-5 scale. Results from conventional and digitized radiographs were compared, and differences between the individual observers were analyzed statistically. Contrast, bone structure, and diagnostic value from digital images were rated significantly better than from conventional images (p < .001). For both techniques, a decrease in ranking was found with a decrease of photon flux. There was only slight interobserver variability. A dose reduction up to a factor of 4 compared to a 100 speed film-screen system seems to be possible without loss of information. Weight and size of the horse are not major influences.

United States: perspectives in school health.

An overview is presented of school health programs being conducted in the U.S. as well as the U.S. education system. Major health problems of youth are identified and initiatives to improve the health of school-aged youth are reviewed. With the start of the new decade, it will be important to use and disseminate the findings from ongoing school health programs and research projects, to address the growing social issues for children and adolescents that directly affect their physical health, and to broaden the scope of school health to include issues of psychological or mental health, issues related to social competencies, dysfunctional family lives, or major life events.

A university rural teaching practice: a model for collaboration in rural health care.

After nearly two decades of Federal, State and local incentives, primary medical services in rural areas remain for short of target. Numerous demonstration projects supported by public and private funds have had some success but models with both broad replicability and independence of external start-up and/or maintenance support are rare. The Department of Family Medicine at the State University of New York at Buffalo has established a novel collaborative network of public and private organizations to emplace a four-physician rural group practice concurrently accomplishing three major goals: high quality primary and specialty medical services to two medically underserved populations; enriched training opportunities for students, residents, and fellows; and financial viability. Beyond meeting regional needs, however, the components and nature of the collaborative effort indicate reproducibility in many communities throughout the country. To emphasize exportable features we will focus on: opportunities, operational strategies, financial feasibility, training advantages, and impact.

Research in epidemiology and community health in the medical curriculum: students' opinions of the Nottingham experience.

Medical students in Nottingham all complete an honours Bachelor of Medical Sciences course in one department including a research project. The honours programme in community health is described, with results from a survey of the 122 graduates since the medical school started. The 98 (80%) responses showed high levels of satisfaction and gain from the programme, the great majority regarding the course as valuable, enjoyable, and giving both research skills and skills in interpersonal relationships.

[Development and cell dynamics of PLCbeta2 positive cells in mouse taste buds].

Taste buds, the sensory end organs for the sense of taste, consist of taste sensing cells, supportive cells and basel cells. Taste bud cells are heterogeneous in terms of morphology as well as functional profiles. Although the lineage of mammalian taste bud cells is largely unknown, it is generally accepted that undifferentiated epithelial basal cells surrounding taste buds enter the taste buds to form and maintain this specialized corpuscle. To analyze taste bud formation during development, we conducted morphological observations and examined differentiation marker expression. Thickening of epithelia starts at 13 dpc foetus and immunohistochemistry against a neural marker, PGP 9.5, revealed that the change of epithelial morphology precedes neural projection observed at 14 dpc foetus. Taste sensing cells appear 6 days after birth indicated by expression of the single transduction component phospholipase Cbeta2 (PLCbeta2) as differentiation marker. To further investigate the maintenance and cell lineage in the taste buds in adults, we injected 5'bromo-2'deoxyuridine (BrdU) solution to young growing mice for a week. BrdU label retaining cells (LRCs) could be observed even 8 weeks after injection. LRCs were examined the differentiation by PLCbeta2 and proliferative activity by Ki-67. The results suggested two possibilities. (1) Part of PLCbeta2 positive cells retain proliferative activity and multipotentiality, or (2) precursor cells (stem cells) stay in the taste buds and produce at least part of the taste sensing cells through proliferation and differentiation processes.

Panel discussion. Data needs in cancer.

A prospective, comprehensive outcomes database was recently initiated by the National Comprehensive Cancer Network (NCCN) after a 2-year study to test data collection methods and systems. It started with data on 400 patients with newly diagnosed breast cancer at five NCCN sites, and over the next 3 years is projected to grow to include more than 12,000 patients with common cancers treated at all eligible NCCN sites. Among the goals of the database are: 1) to establish the capability to select, analyze, and report patterns of care and outcomes; 2) to allow NCCN members to assess their compliance with NCCN clinical practice guidelines and benchmark their performance against the rest of the NCCN; 3) to establish a true databased continuous quality improvement program; 4) to support clinical disease-oriented research and methodologic studies; and 5) to provide the NCCN with a vehicle for forging partnerships with others in the health-care field, such as the pharmaceutical industry, regulatory agencies, and accrediting bodies. Many of those potential partners were represented on this panel. Panelists discussed the data needs of their organizations, what they are doing to meet those needs, and how a comprehensive database will ultimately help improve patient care.

[From planning to realization of an electronic patient record].

The high complex requirements on information and information flow in todays hospitals can only be accomplished by the use of modern Information Systems (IS). In order to achieve this, the Stiftung Orthopädische Universitätsklinik has carried out first the Project "Strategic Informations System Planning" in 1993. Then realizing the necessary infrastructure (network; client-server) from 1993 to 1997, and finally started the introduction of modern IS (SAP R/3 and IXOS-Archive) in the clinical area. One of the approved goal was the replacement of the paper medical record by an up-to-date electronical medical record. In this article the following three topics will be discussed: the difference between the up-to-date electronical medical record and the electronically archived finished cases, steps performed by our clinic to realize the up-to-date electronical medical record and the problems occurred during this process.

Implementation of evidence-based treatment for schizophrenic disorders: two-year outcome of an international field trial of optimal treatment.

According to clinical trials literature, every person with a schizophrenic disorder should be provided with the combination of optimal dose antipsychotics, strategies to educate himself and his carers to cope more efficiently with environmental stresses, cognitive-behavioural strategies to enhance work and social goals and reducing residual symptoms, and assertive home-based management to help prevent and resolve major social needs and crises, including recurrent episodes of symptoms. Despite strong scientific support for the routine implementation of these 'evidence-based' strategies, few services provide more than the pharmacotherapy component, and even this is seldom applied in the manner associated with the best results in the clinical trials. An international collaborative group, the Optimal Treatment Project (OTP), has been developed to promote the routine use of evidence-based strategies for schizophrenic disorders. A field trial was started to evaluate the benefits and costs of applying evidence-based strategies over a 5-year period. Centres have been set up in 18 countries. This paper summarises the outcome after 24 months of 'optimal' treatment in 603 cases who had reached this stage in their treatment by the end of 2002. On all measures the evidence-based OTP approach achieved more than double the benefits associated with current best practices. One half of recent cases had achieved full recovery from clinical and social morbidity. These advantages were even more striking in centres where a random-control design was used.

Intraperitoneal cisplatin-based chemotherapy for ovarian carcinoma.

Ovarian carcinoma demonstrates a steep dose-response curve for cisplatin, but even very small levels of acquired resistance at the cellular level are sufficient to block the efficacy of intravenous (IV) cisplatin. The intraperitoneal (IP) route of administration produces a 12-fold to 15-fold greater exposure for the peritoneal cavity, and concurrent use of IV thiosulfate permits the safe IP injection of 200 mg/m2 cisplatin. In this study, two phase II trials of an IP regimen containing cisplatin 200 mg/m2 and etoposide 350 mg/m2 with IV thiosulfate were conducted; the first trial enrolled patients with residual disease less than 2 cm who had failed primary cisplatin-based IV chemotherapy and the second trial newly diagnosed ovarian carcinoma patients irrespective of the size of residual disease after primary surgery. As salvage therapy, the IP cisplatin/etoposide regimen produced a median survival of 26 months from the start of IP therapy and 51 months from diagnosis. As first-line therapy, the median survival has not yet been reached; projected survival is 68% at 27 months. In both studies the major toxicity was myelosuppression; the use of concurrent thiosulfate almost completely eliminated serious nephrotoxicity and neurotoxicity. The size of the largest tumor mass was an important determinant of efficacy in both settings. The results of these trials are consistent with the hypothesis that increased drug delivery will result in higher response rates and improved survival. Data are sufficiently encouraging to mandate phase III randomized trials of this program.

The evaluation of glucagon infusion algorithms for a counterregulatory system in artificial endocrine pancreas.

As counterregulatory system of artificial endocrine pancreas, glucagon infusion algorithm has been developed and its usefulness has been examined in pancreatectomized dogs and a pancreatectomized diabetic patient. Glucagon infusion rate (G1nIR(t)) was determined depending on proportional plus derivative action to blood glucose concentration (BG(t] with the time delay (tau) to start infusion as follows. G1nIR(t) = Gp(BGp- BG(t-tau)) + Gd(-dBG(t-tau)) + Gc where BGp is the projected value of blood glucose concentration, Gp, and Gd are the coefficients and Gc is the constant for basal glucagon supplement. Glucagon infusions based only on proportional action (Gp/Gd/Gc/tau) = (0.2/0/0.4/10 or 0.4/0/0.4/10) failed in simulating the pattern of blood glucose or glucagon response seen in normal dogs. Glucagon infusion based on proportional plus derivative action (Gp/Gd/Gc/tau = 0.2/0.4/0.4/10) successfully mimicked the pattern of blood glucose concentration and plasma glucagon profile seen in normal dogs. This glucagon infusion algorithm has been applied to control insulin-induced hypoglycemia in a pancreatectomized patient with the same infusion parameters. Hypoglycemia was recovered to normoglycemia in 80 min and the plasma glucagon patterns showed no significant difference from those in healthy volunteers. These data indicate that glucagon infusion algorithm thus developed is effective to render hypoglycemia to normoglycemia and mimic plasma glucagon response seen in normal subjects.

[The horror story--a contribution of horror literature to psychoanalysis].

Using the example of the vampire motif origin and psychic function of the ghost-story in context of the pertaining historical situation are presented. A comparison to the development and function of night-mares is drawn.--The vampire motif in Europe originally developed at the end of the Middle Ages. As with the collective madness of witch persecution it was a superstition which was in fact supported by the church of this time. The belief in vampires was used for splitting of, projection and acting-out of taboo aggressive, oral and sexual drives. Although the era of Englightment quenched this superstition, the motif started to crop up in sublimated form: the vampire became a favourite motif in serious as in trivial literature of the 19th century. At first it can be seen as symbolic expression of rejected anxiety and guilt feelings of the bourgoisie after having thrown the absolutistic institution from power. In the course of the century the motif developed increasingly to an enciphered representation of sexuality perverted due to repression and rejection.--Our current social and literary life has now changed--also under the influence of Freuds work. The hypothesis is presented that meanwhile science fiction literature has become the successor of the ghost story, since it allows the preconscious presentation of contemporary anxiety and conflict and their rejection. Since fictions therefore can be viewed as a serious collective nightmare of the second half of the 20th century.

Strategy as a portfolio of real options.

In financial terms, a business strategy is much more like a series of options than like a single projected cash flow. Executing a strategy almost always involves making a sequence of major decisions. Some actions are taken immediately while others are deliberately deferred so that managers can optimize their choices as circumstances evolve. While executives readily grasp the analogy between strategy and real options, until recently the mechanics of option pricing was so complex that few companies found it practical to use when formulating strategy. But advances in both computing power and our understanding of option pricing over the last 20 years now make it feasible to apply real-options thinking to strategic decision making. To analyze a strategy as a portfolio of related real options, this article exploits a framework presented by the author in "Investment Opportunities as Real Options: Getting Started on the Numbers" (HBR July-August 1998). That article explained how to get from discounted-cash-flow value to option value for a typical project; in other words, it was about reaching a number. This article extends that framework, exploring how, once you've worked out the numbers, you can use option pricing to improve decision making about the sequence and timing of a portfolio of strategic investments. Timothy Luehrman shows executives how to plot their strategies in two-dimensional "option space," giving them a way to "draw" a strategy in terms that are neither wholly strategic nor wholly financial, but some of both. Such pictures inject financial discipline and new insight into how a company's future opportunities can be actively cultivated and harvested.

[Malaria vector control in Cameroon: past, present, future. Reflections].

During the fifties, large scale malaria vector control projects based upon house spraying were implemented in Southern and Northern parts of Cameroon in line of malaria eradication concept. In the South, the pilot zone of Yaounde gathered about 150,000 inhabitants, in the forest area. First operations started in 1953 but the programme became actually operational in 1956. It was divided in two parts: the western part was treated with DDT, while the eastern one was treated with dieldrin. At the same time, the whole forested area was also treated with dieldrin until 1960. Yaounde itself was not treated because it was free of anopheles and malaria. House spraying in the pilot area of Yaounde was a complete success and plasmodic index dropped below 1%. The same success was observed in most of the southern treated areas. Unfortunatly dieldrin resistance of An. gambiae hampered this programme which stopped in 1960. The northem pilot project dealt with some 250,000 inhabitants around Maroua, in a savanna area. To avoid dieldrin resistance observed in 1956, DDT was selected and house spraying started in 1959. From a strictly operational point of view, the campaign was considered as a success. But after two years, it was noticed that plasmodic index remained still around the same value of 35% and the programme stopped. It was thus stated that according to available techniques it was not possible to reach the ultimate goal of eradication even when chemoprophylaxis (chloroquin + pyrimethamin) was added. The comparison between south (= success) and north (= failure) was very interesting as it underlined the big differences between epidemiological faces, an unaccepted concept at that time. Now ecological and epidemiological diversity is the well acknowledged. It also underlined the need of diversity of strategies according to the epidemiology of the disease and the ecology of its vector Vector control was then stopped for a while. In the eighties, Primary Health Care was promoted and malaria control shifted from vector to parasite control, vector control remaining as a prevention method. But chemioresistance of Plasmodium falciparum appeared and. quickly spread, at different levels, across the country. A new emphasis was therefore given to vector control thank to the newly developed technique of insecticide impregnated mosquito nets. Two kinds of studies were undertaken: - what people were actually doing in term of mosquito control at family level, the main reason and the costs as well as current use of mosquito nets - the efficacy of pyrethroid treated mosquito nets (IMN) in different areas of southern forested area against different malaria vectors: An. gambiae, An. nili, An. moucheti. It thus clearly appeared that IMN were very successful in sharply reducing malaria transmission aAd morbidity. But its promotion is limited by the current poor use of mosquito nets in Yaoundé (1 mosquito net for 27 "beds") while in Douala, where IMN are largely used against the bite of the huge population of Culex. quinquefasciatus, the implementation of the first riational centre for impregnation of mosquito net was a great success, as long as it was headed by a motivated and skill manager Impregnated mosquito nets appear thus as a tool of great efficacy but their sustainability is still matter of concern and promotional campaigns must be developed involving private and public, political and scientific spheres as well as the general population who should be encouraged to become partners and even actors of vector and malaria control at their household level.

North Hammersmith stroke prevention project.

The North Hammersmith stroke prevention project was designed to reduce the number of deaths from stroke in this health district by improving the detection of hypertensive patients and thereafter reducing the default rate from treatment. Starting in May 1979 general practitioners in the district were asked to register hypertensive patients over the age of 40 so that the proportion of such patients in each practice was known and could be compared with the average for all practices. This confidential information was fed back every year to the individual general practitioners so that they could assess their performance. Forty of 44 eligible general practitioners agreed to participate and 29 proceeded to register patients. Over a three year period 1006 patients were registered, representing 4.3% of the project population over the age of 40. Individual practice registration rates ranged from 1.1% to 9.1%. Sixty five per cent of the registered patients were women, 34% of all registrations were in the age group 60-69, 31% in the group 50-59, 12% in the group 40-49, and 23% in the group over 70. The average blood pressure before treatment was 190/111 mm Hg. After one and two years the patients were contacted or their notes examined to ensure that they still receiving treatment. Persistant default from treatment occurred in under 12% over the three year period.

Community-based antenatal education in Istanbul, Turkey: effects on health behaviours.

In this article we describe the implementation and evaluation of a community-based antenatal education programme for first-time expectant mothers carried out in Istanbul, Turkey, as part of the Healthy Beginnings Project. We hypothesized that women participating in an antenatal education programme would be more likely to have a vaginal birth, practice recommended maternal and infant health-related behaviours, and adopt a contraceptive method after the birth than a control group, even after statistically controlling for differences in background characteristics. Methods used to evaluate the antenatal education programme included pre- and post-tests with programme participants and a household survey with participants and a control group. All antenatal education course participants, as well as a control group composed of women giving birth to their first child at the same hospitals, were interviewed in their homes by a trained interviewer 2.5-3 months after the baby's birth, starting in October 1998. Results of statistical analyses indicate that a community-based antenatal education programme, such as the one offered as part of The Healthy Beginnings Project in Istanbul, may increase the chance that women will adopt behaviours beneficial to health in the period following a birth. In particular, behaviours related to infant health (breastfeeding and infant check-up) and contraception appeared to be influenced by participation in the programme. In a developing country such as Turkey, with persistent maternal and child health problems, there is an urgent need to increase both demand for and quality of reproductive health services. Antenatal education is not the only answer, but it can be part of the answer. Reaching couples early in their reproductive lives may encourage them to adopt healthy behaviours and make use of available services during their reproductive years.

Social health maintenance organizations' service use and costs, 1985-89.

Presented in this article are aggregate utilization and financial data from the four social health maintenance organization (S/HMO) demonstrations that were collected and analyzed as a part of the national evaluation of the S/HMO demonstration project conducted for the Health Care Financing Administration. The S/HMOs, in offering a $6,500 to $12,000 chronic care benefit in addition to the basic HMO benefit package, had higher start up costs and financial losses over the first 5 years than expected, and controlling costs continues to be a challenge to the sites and their sponsors.

Fully differentiated Xenopus eye fragments regenerate to form pattern-duplicated visuo-tectal projections.

In order to determine if differentiated Xenopus retina is capable of undergoing regeneration and duplicative pattern formation, we devised a new surgical technique for removal of the temporal two-thirds of the retina. In a series of progressively older larval eyes starting with late tailbud stage embryos (stage 38) and extending to limb-bud stage tadpoles (stage 48), nasal one-third-sized eye fragments successfully regenerated to form nearly normal sized eyes over 75% of the time. Histological preparations showed that early wound healing involved the formation of a neuroepithelium at the ventro-temporal region of the fragment. The pigmented retinal epithelium and associated retinal tissue appeared to be involved in this process. Animals from each stage were reared through metamorphosis and electrophysiologic techniques were employed to determine visuo-tectal projections. Seventy percent of stage 38 animals showed evidence of pattern-duplicated projections. Ninety percent of their responding tectal points showed duplicate innervation from two retinal regions. Older animals (stages 44 to 48) showed less duplication. Only 52% of their responding tectal points duplicated (P less than 0.001). Thus, fully differentiated Xenopus retina can undergo regeneration and duplicative pattern formation similar to that shown by embryonic retinal tissue.
