Acute intestinal obstruction caused by endometriosis mimicking sigmoid carcinoma.

Endometriosis is a relatively frequent disease in fertile women. The intestine is involved in 12-37% of cases. Intestinal endometriosis is usually asymptomatic and complete obstruction of the bowel lumen occurs in less than 1% of cases. We report a case of endometriosis of the sigmoid, which caused complete intestinal obstruction and mimicked carcinoma of the sigmoid colon. This case demonstrates the difficulty of establishing an accurate pre- and peroperative diagnosis and the propensity of intestinal endometriosis to mimic colon cancer.

Intestinal obstruction from a forgotten artery forceps: a case report.

68-year-old male patient who had had two previous gall bladder operations presented with an acute abdomen. Upon investigation an Artery Forceps was seen on plain abdominal X-ray, and subsequent laparotomy showed that it entrapped a segment of the small intestine, which was gangrenous. The gangrenous loop was delivered from the forceps followed by resection and end-to-end anastomosis. The patient had an uneventful post-operative period and remained well on follow up twenty four months later.

[X-ray contrast examination in surgery of adhesive intestinal obstruction].

The nature, frequency, and informative value of contrast X-ray examination of the gastrointestinal tract after oral administration of liquid barium in the adhesive disease and adhesive ileus were studied in two groups of patients (123 and 162 persons). Signs such as prolonged retention of the contrast medium in the small intestine and "sagging" of the intestine with the appearance of an X-ray contrast level in the absence of gases above it are of most diagnostic significance. Goal-oriented X-ray contrast examination made it possible to reduce the time needed for preoperative diagnosis, reduce the number of operative interventions in which the intestinal lumen is opened, and improve the immediate outcomes of treatment; mortality reduced from 19.8% to 10.5%.

Segmental defect of intestinal musculature: a rare cause of intestinal obstruction in children.

A 3 years old female child was brought to the emergency department with acute abdomen. Laparotomy disclosed multiple segmental dilatations in the middle third of the ileum which was resected and anastomosed. Post-operative recovery was uneventful. Histopathological findings showed lack of smooth and longitudinal muscles in dilated segments of the intestine with intact mucosa without any evidence of necrosis and inflammation.

[The clinical picture and surgical treatment of adhesive intestinal obstruction].

Results of examination and treatment of 128 operated patients were analyzed. In addition to clinical and roentgenological data the ultrasonic investigation and laparoscopy were also made for diagnosis of commissural ileus. Prevailing was terminal ileus (109 patients). Dissection of the commissures was performed in 115 patients, dissection of the commissures with resection of the intestine was made in 29 patients. Combined operations were fulfilled in 17% of the patients. Sonic stimulation of the intestine was used parallel with medication at the postoperative period which led to earlier restoration of the intestine function. Serious complications were noted in 19 patients, 14 patients died. Main causes of the death were intoxication and peritonitis, hepato-renal and cardio-vascular insufficiency.

Small-bowel intestinal obstruction caused by an unusual internal hernia.

We report a rare case of transomental small-bowel herniation in a 91-year-old lady who presented with central abdominal pain and mild distension. Urgent abdominal computed tomography (CT) showed a segment of dilated ileum with features suggestive of strangulation. Emergency exploration revealed a segment of congested small-bowel loop herniated through a defect over the greater omentum. Reduction of the bowel loops and division of the omental defect was performed without the need for bowel resection. The patient made an uneventful recovery. We discuss the value of CT scan and highlight the importance of recognizing this rare cause of small-bowel obstruction.
