Electromyography: its potential as an adjunct to other monitored parameters during conscious sedation in children receiving dental treatment.

The purpose of this study was to evaluate the effect of a combination of a low dose of chloral hydrate and hydroxyzine on the frontalis muscle electromyogram in addition to other physiologic parameters in pediatric dental patients. A double blind, two-appointment cross-over design was used. Either a placebo or a combination of chloral hydrate and hydroxyzine was given to children during the first visit with the sequence of placebo/drug conditions being randomly determined. During the second visit, the children received that agent not given during the first visit. Baseline physiologic data was obtained at the beginning of each visit and the physiologic measures were again recorded during topical and local anesthesia administration, high-speed tooth preparation, and at the end of the dental appointment. The results indicated that the frontalis muscle activity and cardiovascular parameters were significantly affected by the drug and dental procedures. Oxygen saturation was least affected. The frontalis muscle appears to be a sensitive physiologic parameter to monitor during conscious sedation as an index of the amount of patient relaxation.

[Evaluation of pyralgin usefulness in premedication of patients before stomatological treatment].

Pyralgin (metamizole sodium) usefulness was tested in premedication of 90 patients subjected to processing of hard tooth tissues by grinding or drilling. The results of the study demonstrated that about 2/3 of the patients failed to observe any difference in the painfulness of the procedure with and without premedication with Pyralgin. In 1/3 of the patients Pyralgin reduced pain, improved wellbeing and raised the emotional status during the procedure. This effect might be regarded as a placebo result.

[Amalgam filling in clinical practice].

The quality of treatment of dental defects with amalgam is closely associated with the preparation of the cavity and its filling as well as with completion of the filling and its finish. Strict adherence of a correct working procedure improves markedly the standard of treatment and makes it possible to make use of the positive properties of amalgam from which the filling is prepared.

[Koferdam and its application].

For perfect dental treatment it is essential in the majority of cases to ensure a readily surveyed and perfectly dry working field. This is met best by the use of the rubber membrane--koferdam. In the submitted work the authors inform the stomatological public on basic terms and principles of work with koferdam which is considered irreplaceable under certain conditions.

[History of the beginnings of Czech stomatology].

The authors present a brief account of the history of dental surgery--stomatology--in Bohemia and Moravia from the very beginnings to the period after the Second World War with indicators of the brisk development of the discipline in the new democratic state which occurred in particular after the foundation of the stomatological branch of medical faculties in 1950/51. The authors mention in more detail only the main personalities and representatives of the discipline who contributed most to the development, being aware that they collaborated with several tens of teachers and research workers of more than four generations who also deserve admiration and gratitude.
