Among these, mean Kupperman index was decreased from 28.38 to 11.6 after 3 months and up to 5.47 after 6 months therapy with Klimonorm. These patients estimated this agent as very good and good in 93.21% tolerated as very good and good in 91.43% and 91.59% of these patients planned to continue treatment with Klimonorm.

[Medical evaluation of the climacteric patient].

A pilot program with primary care physicians was established in Clinica Cuauhtémoc y Famosa, focused to evaluate women older than 35 years with climateric symptoms. This program included a survey, a complete gynecological examination with Pap smears, ultrasound pelvic examination and mamography. Also, blood samples were collected for cholesterol, tryglycerides, calcium, phosphorus and alkaline phosphatase. An ECG, bone densitometry of the radius and X-rays of the vertebral column were obtained. This group was formed by 69 women with an age media of 50 years (SD 7.6 years, median 49 years). The survey disclosed that 34/65 women had been hysterectomized, and only 34/64 had received antitetanic immunization in the last 10 years. A preexistent chronic disease occurred in 36/59 women, these were diabetes mellitus, arterial hypertension or degenerative osteoarthritis. Alcohol consumption was recorded in 5/66 women and tabac consumption in 9/66 women, and 32/68 women accepted to be sedentary. Laboratory examinations disclosed hypercholesterolemia in 19/66 women and hypertryglyceridemia in 8/64 women. Osteopenia was detected in 33/60 women. This findings support a systematic plan to provide medical assistance for women in this age, specially if previous epidemiologycal studies have disclosed that coronary disease is the main cause of death for women older than 50 years in this region, that also belongs to the area of highest incidence of diabetes.

[Metabolic changes in climacteric women in terms of body mass index].

The objective was to identify climateric woman with cardiovasculary risk by the relation between size and weight, known as corporal mass index. Seven hundred and ninety seven postmenopausal patients, were studied; they attended the CECLIMEN, with one or two years of spontaneous menopause, and they were not receiving, nor had received replacement hormonal therapy (RHT). In all of them clinical history was taken, and blood pressure, weight, size and corporal mass index (CMI) (weight/(size)2), were evaluated; considering normal weight when CMI was < or = 27, and obesity when this index was greater than 27. Fastening and postprandial glucose at two hours, was determined, as well as total cholesterol, HDL-C, cholesterol of low density lipoproteins (LDL-C), triglicéridos (Tg), and aterogenic index was calculated by CtHDL-C. Besides, estradiol (E2) and follicle stimulating hormone (FSH) were measured in serum by radioimmunoanalysis RIA. Results were considered in the two groups, according to CMI: Group I with CMI < or = 27 (n = 566); and group II, with CMI > 27 (n = 231). Blood pressure, systolic and diastolic, triglicerides and aterogenic index were significantly greater in group II. There were higher levels of glucose in group II (tendency). CMI > 27 is useful to differentiate the postmenopausal women with a higher cardiovasculary risk.

[Climacteric. The first level physician's approach to this phase at the Mexican Institute of Social Security].

The objective was to know about physician's knowledge, attitude and practice toward climateric syndrome. An observational, descriptive and transversal study was realized in 520 general practitioners and family practice specialists in ten out patients clinics at the Institute of Social Security in Mexico City. A questionnaire of 22 items, which was validated by the Delphi's Technique, was applied to the selected population by one of the researchers (SM). The results were: 5.4% good, 78.9% regular and 15.7% non-acceptable about knowledge; 33.7% good, 44.2% regular and 22.1% non-acceptable for attitude; 5.4% good, 63% regular and 31.6% non-acceptable about medical practice. It was concluded that the knowledge of general practitioners and family practice doctors is reflected in their medical attitude and practice. Therefore we consider essential to train physicians in this specific problem.
