Epidemiology, demographics, and genetics of sarcoidosis.

Epidemiological knowledge of sarcoidosis is based mainly on studies performed more than 30 years ago. These early case-control studies produced some interesting risk factor-disease associations, but a clear causative mechanism in sarcoidosis remains unknown. Studies in military and veteran populations showed a clear preponderance of sarcoidosis in African Americans compared with Caucasians. Our recent sarcoidosis incidence study in a racially heterogeneous population found African Americans at three- to fourfold greater risk, which was less than the 10 to 17 times greater risk previously reported. Females are consistently found at greater risk than males, although the relative risk difference generally does not exceed two. The striking racial differences and numerous reports of familial clustering suggest genetic susceptibility. We have found that familial sarcoidosis is almost three times more common in African-American (17%) than Caucasian cases (6%). Future genetic studies can benefit from the extensive catalog of candidate genes that is emerging from the human genome project. The epidemiological evidence to date strongly suggests that studies seeking causes for sarcoidosis need to consider both environmental and genetic risk factors to be successful because the two likely interact with each other to produce disease.

[The Sociedad Española de Cardiología on the Internet: current resources and future prospects. The Internet Committee of the Sociedad Española de Cardiología].

The Internet can help physicians to identify needed clinical information quickly providing continued medical education. Internet also improves medical information of the non-medical population. Researchers have quick access to library catalogs, Medline and other important databases from the most recognized research centers. Furthermore, it can put physicians in ready contact with other specialists for communication and consultation, facilitates administrative procedures of multicenter studies and accelerates editorial processes of biomedical journals. Since its creation, the website of the Spanish Society of Cardiology has evolved rapidly to the present model, providing different kinds of services to its members including faster communication, information from national and international societies and congresses, earlier access to the full content of Revista Española de Cardiología and to a variety of graphic resources and of continuing education. Nowadays, the website of the Spanish Society of Cardiology is consulted by one thousand visitors a week, even at weekends. The degree of activity increases from 3 h P.M. with a peak from 11 h P.M. to 1 h A.M. In the near future, our website will incorporate its own courses of continuing medical education with on-line evaluation and credit granting, will give support to multicenter studies and will initiate the publication and discussion of clinical cases of interest.

[Leadership in nursing in Brazil: approaching its demystification].

This study aims at understanding the view about the nursing leadership question, through critical analysis of the literature produced predominantly in Brazil on this theme, from 1930 to 1995, contributing to its "dismythification". From the assumption that nursing is a social practice, and therefore articulated with other health practices, we conducted the investigation under a historical-social approach. Empirical data were obtained from the discourses about the issue or related themes, contained in 14 periodicals, from which 12 were of nursing. Besides them, we used proceedings from nursing events, as well as research and researcher catalogs and 3 theses. We analysed data according to time delimitation, distributed in four historical periods, intending to maintain the similarity of the peculiar discourses of each period, according to the context in these moments. The study demonstrated that the nursing leadership question has been treated, in the profession, in an idealized, mythical and crystalized way, contributing, thus, to the maintenance of the status quo and nursing professional's alienation.

Acupuncture in the treatment of asthma: a critical review.

BACKGROUND: Increasingly, patients are turning to treatments and drugs that are considered "alternative" or "complementary" as part of their healthcare. In response, the National Institutes of Health (NIH) established an Office of Alternative Medicine (OAM) in 1992 to facilitate identification and evaluation of alternative and complementary therapies. Acupuncture, a therapy that has been used to treat disease in China for approximately 2,500 years has attracted considerable attention. NIH in a consensus statement derived from a Fall of 1997 consensus conference to assess acupuncture has indicated that acupuncture was useful in pain control and maybe a useful adjunct treatment for the management of asthma. Further, the US Food and Drug Administration has removed acupuncture from the "experimental medical devices" category. METHODS: We have extensively searched the literature for reports addressing the use of acupuncture in asthma treatment. We sought these using the National Library of Medicine, the Office of Alternative Medicine's database as well as other databases both English language based as well as other languages that catalog literature pertaining to alternative and complementary therapies. We then reviewed these reports and weighted the validity of the conclusions reached in the reports based on assessment of study design, number of subjects studied, duration of studies, types and number of controls, and statistical analyses used. RESULTS: Data presently in the literature do not provide sufficient support for a useful role for acupuncture in asthma management. CONCLUSIONS: Further properly designed clinical studies examining the use of acupuncture in asthma are extremely important and urgently needed.

Searching the databases: a quick look at Amazon and two other online catalogues.

The Amazon Online Catalogue was compared with the Library of Congress Catalogue and the British Library Catalogue, both also available online, by searching on both neutral (Gay, Lesbian, Homosexual) and pejorative (Perversion, Sex Crime) subject terms, and also by searches using Boolean logic in an attempt to identify Lesbian Fiction items and religion-based anti-gay material. Amazon was much more likely to be the first port of call for non-academic enquiries. Although excluding much material necessary for academic research, it carried more information about the individual books and less historical homophobic baggage in its terminology than the great national catalogues. Its back catalogue of second-hand books outnumbered those in print. Current attitudes may partially be gauged by the relative numbers of titles published under each heading--e.g., there may be an inverse relationship between concern about child sex abuse and homophobia, more noticeable in U.S. because of the activities of the religious right.

Library cooperation: wave of the future or ripple?

Little of the literature in library cooperation applies specifically to library service for the health sciences. Based on experience in and observations of the cooperation of health science libraries, this short lecture reviews aspects of general library cooperation, networks, and consortia. The effects on library operation of several cooperative activities are enumerated and cooperation management is discussed briefly.

The PHILSOM system--one user's experience.

The University of Texas Health Science Center at San Antonio joined the PHILSOM system, a comprehensive serials control network, in 1971. The experiences of the library in using the system are described. The major benefit of the system has been multiple copies of the holdings list which have made the serial records publicly accessible and significantly increased their value. Tallies of these lists' use indicate that more than half of serials-related questions are now answered directly by the users. The effects of PHILSOM on the procedures of the serials department--processing, claiming, bindery, and personnel are described. Costs to the network and the UTHSCSA Library are briefly summarized.
