Evaluation of the pharmacological selectivity profile of alpha 1 adrenoceptor antagonists at prostatic alpha 1 adrenoceptors: binding, functional and in vivo studies.

1. The profile of a range of alpha 1 adrenoceptor antagonists was determined in vitro against cloned human alpha 1A, alpha 1B and alpha 1D adrenoceptors and against noradrenaline-mediated contractions of rat aorta and human prostate. The in vivo profile of compounds was determined in an anaesthetized dog model which allowed the simultaneous assessment of antagonist potency against phenylephrine-mediated increases in blood pressure and prostatic pressure. 2. The quinazoline antagonists, prazosin, doxazosin and alfuzosin displayed high affinity but were non selective for the three cloned human alpha 1 adrenoceptors. Indoramin and SNAP 1069 showed selectivity for alpha 1A and alpha 1B adrenoceptors relative to the alpha 1D subtype. Rec 15/2739, WB 4101, SL 89,0591, (+)- and (-)- tamsulosin showed selectivity for alpha 1A and alpha 1D adrenoceptors relative to the alpha 1B subtype. RS 17053 showed high affinity and selectivity for alpha 1A adrenoceptors (pKi 8.6) relative to alpha 1B (pKi = 7.3) and alpha 1D (pKi = 7.1) subtypes. 3. (+)-Tamsulosin, (-)-tamsulosin, SL 89,0591, Rec 15/2739, SNAP 1069 and RS 17053 appeared to act as competitive antagonists of noradrenaline-mediated contractions of rat aorta yielding pA2 affinity estimates which were similar to binding affinities at cloned human alpha 1D adrenoceptors. The following rank order was obtained: prazosin = (-)-tamsulosin > doxazosin > SL 89,0591 = (+)-tamsulosin > Rec 15/2739 > RS 17053 = SNAP 1069. 4. (-)-Tamsulosin was a very potent, insurmountable antagonist of noradrenaline-mediated contractions of human prostate, yielding an approximate pA2 estimate of 9.8 at 1 nM. The corresponding (+)-enantiomer was 30 fold weaker. SL 89,0591, SNAP 1069 and Rec 15/2739 yielded pA2 estimates which compared well with their alpha 1A binding affinities. The affinity estimate for prazosin on human prostate was lower than the corresponding binding affinity determined at alpha 1A adrenoceptors and RS 17053 was a very weak antagonist on human prostate (pA2 = 6.0) relative to the high affinity (pKi = 8.6) determined at cloned human alpha 1A adrenoceptors. 5. In the anaesthetized dog, in vivo pseudo "pA2' values showed that doxazosin, (+)- and (-)-tamsulosin inhibited phenylephrine-induced increases in prostatic and blood pressure with similar affinity, implying that these agents show little or no selectivity for prostatic responses in this model. SL 89,0591 and SNAP 1069 were moderately selective (3 and 6 fold respectively) for prostatic pressure relative to blood pressure. Rec 15/2739 was a more potent antagonist of phenylephrine-mediated increases in prostatic pressure ("pA2' = 8.74) compared to blood pressure ("pA2' = 7.51). 6. Data in this study suggest that the alpha 1 adrenoceptor mediating noradrenaline-induced contractions of human prostate, whilst having some of the characteristics of an alpha 1A adrenoceptor, cannot be satisfactorily aligned with cloned alpha 1A, alpha 1B or alpha 1D adrenoceptors. In addition, studies in the anaesthetized dog have shown that agents having high affinity and selectivity for prostatic alpha 1 adrenoceptors, particularly over the alpha 1D subtype, appear to inhibit phenylephrine-induced increases in prostatic pressure selectively compared to blood pressure.

The relationship between global myocardial ischemia, left ventricular function, myocardial redox state, and high energy phosphate profile. A phosphorous-31 nuclear magnetic resonance study.

UNLABELLED: The onset of global myocardial ischemia was related to mechanical function (intraventricular pressure), cellular redox state (NADH fluorophotography), and high energy profile (phosphorous-31 nuclear magnetic resonance). Ten rabbit hearts were excised and perfused on a modified Langendorff apparatus (37 degrees C; pO2 480 Torr). Developed pressure and positive and negative dp/dt were determined at control, 1-10, 15, 30, 45, and 60 sec of acute global ischemia. NADH fluorophotographs were taken at control, 1-10, 15, 20, 30, 60 sec, and 5, 10, and 30 min. P-31 NMR spectra in 14 guinea pig hearts under identical conditions were obtained at control, 1, 5, 10, 20, 40, and 60 min of acute global ischemia. LV contractility diminished within 1 sec (P less than 0.01) of ischemia and dropped to less than 35% of control by 1 min. Reduction of mitochondria was detected by epicardial NADH fluorophotography at 2 sec of ischemia. Cellular pH diminished 0.3 pH units by 5 min. Adenosine triphosphate (ATP) concentration remained at control levels while phosphocreatine (PCr) dropped to 63 +/- 8.5% of control by 1 min of ischemia. CONCLUSIONS: After the onset of global ischemia (1) mitochondrial electron transport ceases by 2 sec; (2) acidosis develops immediately; (3) LV contractility diminishes by 1 sec; (4) ATP concentration appears to be buffered by PCr, and is dissociated from myocardial function.

Use of Fourier analysis to determine age-related changes in the facial profile.

This study demonstrated the feasibility and utility of fitting a Fourier function to the facial profile to accurately characterize changes in shape associated with aging, orthodontic or prosthodontic treatment, and orthognathic surgery. In contrast to conventional metric approaches, Fourier analysis provided a detailed measurement of the facial profile and facilitated the quantitative assessment of changes in its shape. Fourier functions, corrected for size, were computed for each of the 77 facial profiles and used to measure longitudinal changes in the midsagittal facial outline. The facial profile segment between the maxillary and mandibular lips showed the greatest variability and the tip of the nose the least variability in persons with intact dentitions.

Analytical platforms for activity-based protein profiling--exploiting the versatility of chemistry for functional proteomics.

The field of proteomics aims to develop and apply technologies for the characterization of protein function on a global scale. Toward this end, synthetic chemistry has played a major role by providing new reagents to profile segments of the proteome based on activity rather than abundance. Small molecule probes for activity-based protein profiling have been created for more than a dozen enzyme classes and used to discover several enzyme activities elevated in disease states. These innovations have inspired complementary advancements in analytical chemistry, where new platforms have been introduced to augment the information content achievable in chemical proteomics experiments. Here, we will review these analytical platforms and discuss how they have exploited the versatility of chemical probes to gain unprecedented insights into the function of proteins in biological samples of high complexity.

Gene expression profiling in human peripheral blood mononuclear cells using high-density filter-based cDNA microarrays.

Microarray technology has provided the ability to analyse the expression profiles for thousands of genes in parallel. The need for highly specialised equipment to use certain types of microarrays has restricted the application of this technology to a small number of dedicated laboratories. High-density filter-based cDNA microarrays provide a low-cost option for performing high-throughput gene expression analysis. We have used a model system in which filter-based cDNA microarrays representing over 4000 known human genes were used to monitor the kinetics of gene expression in human peripheral blood mononuclear cells (PBMCs) stimulated with phytohaemagluttinin (PHA). Using software-based cluster analysis, we identified 104 genes that altered in expression levels in response to PHA stimulation of PBMCs and showed that there was a considerable overlap between genes with similar temporal expression profiles and similar functional roles. Comparison of microarray quantitation with quantitative PCR showed almost identical expression profiles for a number of genes. Coupled with the fact that our findings are in agreement with a large number of independent observations, we conclude that the use of filter-based cDNA microarrays is a valid and accurate method for high-throughput gene expression profiling.

The rating form of IBD patient concerns: a new measure of health status.

Health status assessment for persons with chronic illness includes not only symptoms, but also an appraisal of the psychosocial concomitants of illness. In this national study of persons with inflammatory bowel disease (IBD), we standardized a disease-specific 25-item measure of perceived health status: the Rating Form of IBD Patient Concerns (RFIPC). Factor analysis yielded four indices: a) impact of disease (e.g., being a burden, loss of energy, loss of bowel control); b) sexual intimacy; c) complications of disease (e.g., developing cancer, having surgery, dying early); and d) body stigma (e.g., feeling dirty or smelly). A higher level of IBD concerns was associated with greater disease severity, female gender, and lower educational status. When controlling for these factors, as well as disease type and age, we found that concerns about: a) impact of disease was positively associated with poorer perception of health and well-being, greater psychological distress (SCL-90), and poorer daily function (Sickness Impact Profile) (p less than 0.0001); b) sexual intimacy was related to poorer psychologic function (p less than 0.01); and c) complications of disease was related to several measures of poorer daily function (p less than 0.0001 to 0.01). This standardized measure of the worries and concerns of persons with IBD may be used in clinical care and research to evaluate the effects of interventions on IBD patient outcomes.

Does parallel item content on WOMAC's pain and function subscales limit its ability to detect change in functional status?

BACKGROUND: Although the Western Ontario and McMaster University Osteoarthritis Index (WOMAC) is considered the leading outcome measure for patients with osteoarthritis of the lower extremity, recent work has challenged its factorial validity and the physical function subscale's ability to detect valid change when pain and function display different profiles of change. This study examined the etiology of the WOMAC's physical function subscale's limited ability to detect change in the presence of discordant changes for pain and function. We hypothesized that the duplication of some items on the WOMAC's pain and function subscales contributed to this shortcoming. METHODS: Two eight-item physical function scales were abstracted from the WOMAC's 17-item physical function subscale: one contained activities and themes that were duplicated on the pain subscale (SIMILAR-8); the other version avoided overlapping activities (DISSIMILAR-8). Factorial validity of the shortened measures was assessed on 310 patients awaiting hip or knee arthroplasty. The shortened measures' abilities to detect change were examined on a sample of 104 patients following primary hip or knee arthroplasty. The WOMAC and three performance measures that included activity specific pain assessments--40 m walk test, stair test, and timed-up-and-go test--were administered preoperatively, within 16 days of hip or knee arthroplasty, and at an interval of greater than 20 days following the first post-surgical assessment. Standardized response means were used to quantify change. RESULTS: The SIMILAR-8 did not demonstrate factorial validity; however, the factorial structure of the DISSIMILAR-8 was supported. The time to complete the performance measures more than doubled between the preoperative and first postoperative assessments supporting the theory that lower extremity functional status diminished over this interval. The DISSIMILAR-8 detected this deterioration in functional status; however, no significant change was noted for the SIMILAR-8. The WOMAC pain scale demonstrated a slight reduction in pain and the performance specific pain measures did not reflect a change in pain. All measures showed substantial improvement over the second assessment interval. CONCLUSIONS: These findings support the hypothesis that activity overlap on the pain and function subscales plays a causal role in limiting the WOMAC physical function subscale's ability to detect change.

Long-term effects of maternal diabetes on vascular reactivity and renal function in rat male offspring.

Fetal growth impairment can occur in pregnancy complicated by diabetes. Although several studies have focused the effects of nutritional status on intrauterine development, the long-term impact of maternal diabetes on vascular and renal function in the offspring is poorly investigated. In the present study, blood pressure profiles and renal function parameters were investigated in the offspring of diabetic rats (DO). Female rats were made diabetic throughout gestation with a single dose of streptozotocyn (STZ) 10 d before mating. After weaning, the offspring had free access to food and water. Arterial pressure was evaluated every 15 d. Functional and morphometric kidney studies were performed in newborn, 3, 6 and 12-mo-old male rats in DO and in controls, C. Although maternal diabetes did not affect nephron number in the young adult rat, glomerular hypertrophy developed from 3 mo on. Glomerular Filtration Rate and Renal Plasma Flow were observed to be significantly decreased in DO when compared with C, from 3 mo on. In DO, hypertension was observed from 8 wk on and persisted elevated throughout the experimental period (12 mo). Vascular reactivity, evaluated in mesenteric arterial bed showed a decreased endothelium-dependent vasodilatation in 12-mo-old DO animals, while preserved response to sodium nitroprusside was demonstrated. Our data show that exposure to intrauterine diabetes induced by STZ does not affect nephron number in the young offspring but can cause permanent changes in Nitric Oxide (NO)-related vascular response, which, in turn may accelerate the natural age-related nephron loss.

Pharmacokinetic profile of benoxaprofen in subjects with normal and impaired renal function, prediction of multiple-dose kinetics.

The pharmacokinetic profile of benoxaprofen given as a single oral dose of 600 mg was determined comparatively in 5 healthy volunteers and in 15 patients with various degrees of renal insufficiency (5 of whom were undergoing maintenance hemodialysis). In normal subjects, the half-absorption period (Ka) was 0.63 hours. A mean maximum concentration of 47.3 micrograms/ml could be predicted at 3.6 hours after the dose intake. Serum level decrease was particularly slow. The overall elimination rate constant, Ke, was 0.0234(h-1) and biologic half-life amounted to 28.8 hours. Values for total clearance (Ct=4.8 ml/min/1.73 m2) and renal clearance (Cr=1.6 ml/min/1.73 m2) were low. Of the administered dose of benoxaprofen, 13.9% was recovered in the urine over a 24-hour period. Renal insufficiency did not induce major changes in pharmacokinetic parameters. Under such conditions, it seems advisable to reduce the dose to one-half only in patients with a creatinine clearance of less than 10-20 ml/min/1.73 m2. Predicted serum levels theoretically achieved after repeated administration of a 600-mg dose of benoxaprofen every 6, 12, 24, 36 and 48 hours were calculated. From this evaluation, it appears that therapeutically effective and adequate levels could be obtained after administration of a 600-mg dose every 12 ot 24 hours.

[The analysis of serum lipid profile in anovulatory and ovulatory women in fifth decade of life].

OBJECTIVES: We examined the effect of declining ovarian function on lipid profile. MATERIALS AND METHODS: Serum concentrations of total cholesterol (TC), low density lipoprotein cholesterol (LDL), high density lipoprotein cholesterol (HDL), triglycerides (TG) as well as early follicular phase FSH, periovulatory 17-beta oestradiol and midluteal progesterone levels were estimated. 71 regularly menstruating premenopausal women were divided according to serum progesterone concentration on: anovulatory (progesterone < 4.5 ng/ml) and ovulatory group (progesterone > 4.5 ng/ml). RESULTS: Investigated groups did not differ with respect to mean age, body mass index but hormonal profile showed higher level of FSH in anovulatory (15.1 +/- 21.2 mIU/ml) then in ovulatory group (5.1 +/- 4.2 mIU/ml, p = 0.008). 17-beta oestradiol levels were 91.9 +/- 29.9 pg/ml vs. 123 +/- 48.6 pg/ml, p < 0.05, respectively. Statistically significant difference in LDL concentration between anovulatory and ovulatory group was found (144.9 +/- 29.9 mg/dl vs. 131.4 +/- 24.6 mg/dl, p = 0.04, respectively). Also HDL content of the TC showed statistically significant difference between these two groups (19.3 +/- 4.4% vs. 21.4 +/- 4.3%, p = 0.04, respectively). However, there were no difference in serum concentrations of TC, TG, HDL. CONCLUSION: Based on these findings we concluded that changes in lipid metabolism that lead to increased risk of cardiovascular diseases may appear as early as in the 5th decade of life in apparently healthy women.

An existential/dialectical model for analyzing marital functioning and interaction.

Many experienced marital therapists will acknowledge that it is difficult for them to really understand the dynamics of a couple's interaction and to plan the direction of couple therapy. This article presents a new tool, a Profile of Marital Functioning and Interaction, for developing a broad, many-faceted, but still concise picture of a couple's strengths and weaknesses. The profile is based on an existential/dialectal model for analyzing marital functioning and interaction. It is intended to assist the therapist in selecting a systematic focus in marital therapy.

Comparison of the changes in global gene expression of Escherichia coli induced by four bactericidal agents.

DNA microarrays provide a global view of the physiological state of the cell by parallel analysis of the expression levels of all the genes in an organism. The effects of four bactericidal agents on the expression pattern of Escherichia coli MG1655 were assessed. Compounds were chosen on the basis of their different mechanisms of action and included inhibitors of DNA replication and recombination, translation, transcription and cell wall biosynthesis. The addition of rifampin resulted in increased expression of the target, rpoB, as well as several genes involved in nucleotide salvage and purine biosynthesis. The addition of ampicillin resulted in overall changes in gene expression that showed some similarity to changes induced by rifampin. The addition of the antibiotics kanamycin or norfloxacin resulted in the induction of unique gene expression signatures: a heat shock response to kanamycin and an SOS response to norfloxacin. Several genes of unknown function showed expression profiles similar to the genes associated with the SOS or the heat shock response. Thus, these profiles define families of genes with similar expression phenotypes that can be tested for related function.

Another Look at the Mechanism of the Concerted 1,3-Dipolar Cycloaddition of Fulminic Acid to Acetylene.

The transition structure and energy barrier for the concerted addition of fulminic acid to acetylene, a prototypical 1,3-dipolar cycloaddition, have been determined using various molecular orbital and density functional theory methods (MP2, CCSD(T), G2(MS), G2(CC), CASSCF/CASPT2, and B3LYP) with the aim of obtaining accurate energetics and finding an economical but reliable approach for treating larger substituted systems. Although the activation energy is not particularly sensitive to the geometries employed, it is strongly dependent on the treatment of dynamical electron correlation. The approximate G2(MS) appears to be an efficient and reliable treatment. Both CCSD(T) and CASPT2 results agree with each other, suggesting that the energy barrier for the HCNO + HCCH addition amounts to about 14 kcal/mol. The electronic mechanism of the cycloaddition has also been probed further using DFT descriptors, as well as an analysis of the CAS-LMO-CI wave functions. The hardness profile along the minimum energy path shows a minimum in the saddle region, but the position of its minimum is somewhat shifted toward the product side compared to the maximum in energy profile. The variation of the coefficients of the excited configurations in the CAS wave function along the reaction path suggests that the transition state does correlate with a substantial electron movement from the O to the N of HCNO. The O thus behaves as a new bond acceptor center and the C as a new bond donor, in contrast with a picture previously derived from either the net charges distribution, or the motion of the centroids of Hartree-Fock based localized orbitals accompanying the nuclear approach of both reaction partners, or a spin-coupled valence bond analysis.

Calculation of the absolute thermodynamic properties of association of host-guest systems from the intermolecular potential of mean force.

The authors report calculations of the intermolecular potential of mean force (PMF) in the case of the host-guest interaction. The host-guest system is defined by a water soluble calixarene and a cation. With an organic cation such as the tetramethylammonium cation, the calixarene forms an insertion complex, whereas with the Lanthane cation, the supramolecular assembly is an outer-sphere complex. The authors apply a modified free energy perturbation method and the force constraint technique to establish the PMF profiles as a function of the separation distance between the host and guest. They use the PMF profile for the calculation of the absolute thermodynamic properties of association that they compare to the experimental values previously determined. They finish by giving some structural features of the insertion and outer-sphere complexes at the Gibbs free energy minimum.

Combining gene annotations and gene expression data in model-based clustering: weighted method.

It has been increasingly recognized that incorporating prior knowledge into cluster analysis can result in more reliable and meaningful clusters. In contrast to the standard modelbased clustering with a global mixture model, which does not use any prior information, a stratified mixture model was recently proposed to incorporate gene functions or biological pathways as priors in model-based clustering of gene expression profiles: various gene functional groups form the strata in a stratified mixture model. Albeit useful, the stratified method may be less efficient than the global analysis if the strata are non-informative to clustering. We propose a weighted method that aims to strike a balance between a stratified analysis and a global analysis: it weights between the clustering results of the stratified analysis and that of the global analysis; the weight is determined by data. More generally, the weighted method can take advantage of the hierarchical structure of most existing gene functional annotation systems, such as MIPS and Gene Ontology (GO), and facilitate choosing appropriate gene functional groups as priors. We use simulated data and real data to demonstrate the feasibility and advantages of the proposed method.

Overexpression of alpha 2-adrenergic receptors in fetal rat heart: receptors in search of a function.

alpha 2-Adrenergic receptors are transiently overexpressed by many types of developing cells. In the current study, the developmental profile and cellular function of these receptors were examined in the fetal and neonatal rat heart. alpha 2-Receptors, assessed with [3H]rauwolscine binding, were extremely high in fetal hearts on gestational day 19, 30-fold higher than values seen in adults. Receptor binding decreased by two-thirds by gestational day 21 and dropped by half again by postnatal day 3. To assess potential cellular functions controlled by the alpha 2-receptors, the capability of an alpha 2-agonist (clonidine) to inhibit membrane-associated adenylate cyclase activity was measured in three different settings: basal enzyme activity, the enzymatic response to isoproterenol (dependent upon beta-receptor linkages to the regulatory protein, Gs), and the response to forskolin (independent of receptor-Gs interactions). Despite the high number of alpha 2-receptors in fetal hearts, clonidine failed to alter any of the adenylate cyclase activity measures. In light of the postulated role of alpha 2-receptors in the maintenance of fetal/neonatal atrioventricular conduction, the excess alpha 2-receptors are probably linked to other cellular events, such as movement of calcium into the cell.

A planimetric study on the female urethral closure pressure profile.

Integration of the pressures along the functional part of the female urethral profile performed by planimetry adds information to what can be obtained by measuring functional length alone. In 43 patients with urinary stress incontinence, a statistically significant lower area of the profile was found in patients with decreased urethral resistance (diagnosed by pressure-flow study and micturition cystourethrography), whereas patients with cystourethrocele had larger areas than a control group. Patients with cystocele or bladder insufficiency did not differ from the controls.
