Combination acid-etched and coping-superstructure fixed partial prosthesis.

A conservative method of replacing missing teeth is described. The fixed partial denture incorporates a resin-bonded retainer for the intact tooth and a telescopic coping for the carious or fractured tooth. A permanently cemented coping beneath a provisionally cemented superstructure provides the opportunity to recement the prosthesis if the acid-etched retainer is debonded. The advantages of this technique include conservation of enamel on the intact abutment and protection from leakage caused by dissolution of temporary cement on the abutment with a telescopic coping.

The direct/indirect composite resin veneers: a case report.

Veneer restorations are well suited for conservative and aesthetic improvement of the anterior dentition. Laboratory fabricated porcelain and composite resin veneers present optimal aesthetics and durability. Although there are disadvantages associated with direct bonding, it provides control of color and contour for the operator, particularly in the case of a single anterior central incisor. Direct/indirect composite resin veneers utilize the advantages of both direct and indirect techniques in reconstruction of restorations with improved physical properties. The learning objective of this article is to review the available composite resins, opaquers, and tints, and present a step-by-step protocol for predictable restoration of discolored anterior dentition with direct/indirect heat-treated composite resin veneers in a single appointment. Patient evaluation, preoperative aesthetic considerations, selection of the restorative composite resins, and the clinical procedure are discussed.

Esthetic recontouring. A team approach.

The presence of irregular incisal edges, a worn or abraded dentition, and imbalances in tooth morphology compromise a harmonious esthetic smile. A team approach is indicated after extensive orthodontic or prosthetic treatment to reach a final cosmetic result. Intentional changes in tooth form by recontouring provide an improved outcome. This article presents the components of a harmonious smile, evaluation, guidelines and a stepwise approach for the esthetic recontouring of anterior teeth. Two cases illustrate the effects that are achieved with this methodology.

A root perforation and treatment.

According to Lemon (1992) the internal matrix concept of repairing perforations has distinct advantages over the surgical method. The procedure is more time-efficient when compared with other methods. The technique requires direct access and visualization of the perforation site in order to place the repair material. It is therefore restricted to perforations in the furcation area, and the straight portions of the root canal.

Clinical experience with Empress crowns.

The IPS Empress pressed glass-ceramic system was used in this investigation to restore anterior and posterior single teeth. One hundred forty-four crowns were evaluated over a period of 6 to 68 months (mean period of 37 months). According to the Kaplan-Meier analysis, the estimated success rate after this period was 95.35%. Crowns were also investigated using the modified US Public Health Service criteria. Most of the crowns rated as Alpha for color match, contour, marginal integrity, recurrent caries, and marginal discoloration. The results of this study indicate that this material can be successfully used, especially in the anterior area, when the procedures outlined are carefully followed.

A comparative study on the effect of three methods of instrumentation on the shape of root canal (an in-vitro study).

This study compared the ability of three types of engine driven instrumentation techniques to negotiate and prepare small, slightly curved root canals. Forty five mesial roots of extracted maxillary and mandibular first permanent molars were classified into three groups. Group (I) was prepared by Canal Leader system, group (II) was prepared by Nickel Titanium files and group (III) was prepared by Canal Master technique. Radiographs of the root canal space before and after preparation was carried out by the aid of an experimental model. Enlarged photographic prints obtained from radiographs were scored for shaping effectiveness of the three techniques in respect to canal course, position & width of apical constriction and lack of ledge formation. The results indicated that the Canal Leader system was an efficient technique in maintaining canal curvature, absence of zipping, transportation or ledge formation in almost all of the specimen tested, followed by the Nickel Titanium files and then the Canal Master technique which produced more straightening and destruction of apical constriction.

Diagnosis and treatment planning for implant dentistry.

Dental implants are well-established in dentistry and allow the restorative dentist to offer patients the best that dentistry has to offer. Through examination, radiographs, and study models, the restorative dentist and implant surgeon can develop a treatment plan. Comprehensive diagnosis and treatment planning involve much more than a clinical exam--they require an investigation involving past, present and future dental therapy.

The hybrid crown.

A crown type has been described that offers the advantages of both ceramic veneers and metal crowns. More than 200 single crowns and three three-unit bridges of this type have been inserted with no failures after two years. University-based shear and compressive testing is currently being arranged to affirm perceived clinical strength of this new crown type. Comparison testing also will be performed with comparative values for the hybrid crown, all ceramic (various types), and porcelain-fused-to-metal.
