The antihypertensive and hemodynamic efficacy of Betaloc (Egys-Hungary) was studied in 30 hypertensive patients in the II stage of the disease. The drug was applied in a single daily dose of 100 mg in the course of 6 months. The systolic and diastolic pressure and the heart rate decreased reliably (p less than 0.0001) at the end of the first month. The stroke volume increased (p less than 0.045) at the end of the sixth month but the minute volume and the cardiac index did not change (p greater than 0.32). The peripheral vascular resistance dicreased reliably (p less than 0.02) at the end of the sixth month--the degree depending on the initial type of circulation--and it determined the longstanding antihypertensive efficacy of Betaloc. No untoward reactions were registered.

Thirty-seven asthmatic adults (27 women, 10 men), aged 28 to 70 yrs (mean 50. yrs), in a stable condition, were studied to assess the cardiac tolerance of B2SM sprays. These patients had no cardiac arrhythmia (checked by echocardiography), no hypertension and no hyperthyroidism. Two thirds had severe asthma, and 16 suffered from chronic asthma. After their condition was stabilized, they continued their previous treatment, consisting of theophylline (34 cases), moderate corticotherapy (19 cases), atropine spray (14 cases). Holter monitoring was performed before and 3 days after the institution of treatment by Fenoterol spray at 1,600 micrograms per day in 4 doses at fixed times. Gasometry was tested immediately before and during the protocol. Before B2SM treatment, 26 patients had a normal Holter, 11 had an abnormal first recording (4 important atrial ectopic beats (EBs), 5 ventricular EBs, 1 associating the two), 1 patient had an episode of ventricular tachycardia (VT). With Fenoterol, we observed (with no clinical manifestation) the aggravation of 1 pre-existing atrial EBs and 2 ventricular EBs. Two patients had a short episode of tachycardia but the role of Fenoterol was uncertain: one case was an elderly hypoxaemic patient and in the second a high theophylline level (20 mg/l) was found. The arrhythmogenic property peculiar to B2SM in spray form seems low at the dose we used. However in this study and in certain circumstances, such as hypoxemia, metabolic acidosis, or theophylline overload, the use B2SM may have contributed, together with other factors, to induce disquieting infraclinical cardiac dysrhythmia. We stress that this short-term study involved stable asthmatic patients under surveillance in hospital conditions.(ABSTRACT TRUNCATED AT 250 WORDS)
In infants paradoxical bronchoconstriction instead of dilatation was observed after beta-2-agonist inhalation and was thought to be due to an osmolality and pH change of the inhalation mixture in the nebuliser cup. Therefore, we measured lung function in 11 asthmatic children and adolescents, and in a healthy control group, after inhalation of a hyperosmolar beta-2-NaCl mixture. 0.5 ml salbutamol in 1.5 ml physiological saline (2 ml total) or 1.0 ml salbutamol in 3 ml NaCl, respectively, were nebulised for 10 minutes. Following nebulisation the patients were asked to inhale for 5 minutes from the residue in the cup. Before and after this procedure lung function was measured. All the patients were symptom-free and were tested on two days at the same time after being off medication for at least 24 hours. Baseline osmolality of the mixtures at the start of nebulisation was 235 +/- 5.0 mmol/kg and increased in the cup after 10 minutes to 322 +/- 7.5 mmol/kg for the low initial volume (2 ml) and to 261 +/- 2.0 mmol/kg for the high initial volume (4 ml). After the 5 minutes' inhalation session osmolality rose further to 460 +/- 25.0 mmol/kg (2 ml) and 278 +/- 1.0 mmol/kg (4 ml), respectively. In all asthmatic children significant changes (p less than 0.01, paired t-test) before and after inhalation occurred (decrease of airway resistance, increase of FEV1, increase of maximal expiratory flows at 75%, 50% and 25% of vital capacity, increase of peak expiratory flow. Thoracic gas volume, residual volume and vital capacity remained unchanged). No changes were seen in the control group.(ABSTRACT TRUNCATED AT 250 WORDS)

The ratio systolic pressure--telesystolic volume (SP-TV) was determined in 39 patients with II degree hypertension by M-type echocardiography and simultaneous measurement of the arterial pressure before and at the end of the I and VI month of treatment with a cardioselective beta-blocker without a sympathicomimetic activity. The initial values of the ratio SP-TV did not differ from those of healthy persons. At the end of the first month of treatment the ratio was reliably lower (p less than 0.001) and at the end of the sixth month it increased but remained reliably lower than that of healthy persons (p less than 0.045). At the end of the sixth month the left ventricular muscular mass was reliably reduced. A large up to significant negative correlation was established between the ratio SP-TV and the left ventricular muscular mass before and after the treatment. The left ventricular pump function showed favorable dynamics.
3 ml tylose gel containing 500 micrograms PGE2 was injected into the cervical canal of 23 patients prior to first trimester abortion. 11 patients received 5 mg fenoterol orally before the PGE2-gel application and 12 patients a placebo tablet. The PGFM and oxytocin concentrations in plasma were determined radioimmunologically. The results showed the dominant role of elevated PGFM levels in the clinical prevalence of pain during induced abortion.
