[Perioperative administration of bicarbonated solution to a patient with mitochondrial encephalomyopathy].

A 16-year-old man with mitochondrial encephalomyopathy underwent biopsy and nephrectomy under general anesthesia. Mitochondrial encephalomyopathy is caused by mitochondrial dysfunction, and frequently accompanies elevation of lactic and pyruvic acid levels in the blood. It has been considered that problems of anesthesia for the patient with mitochondrial encephalomyopathy are the probability of hyperlactacidemia, the relevance to malignant hyperthermia, the possibility of myocardial disease and dysfunction of heart conduction system, respiratory depression due to muscle weakness, and so on. Therefore, to prevent hyperlactacidemia, we prepared the extracellular fluid solution including bicarbonic acid but no lactic and acetic acid, and infused the solution to the patient during anesthesia. By use of this solution, his lactic acid level was kept within the normal range during anesthesia and no metabolic acidosis occurred. His hemodynamics was stable and he showed normal response to vecuronium, recovering from anesthesia smoothly and postoperative course was uneventful.

GC/ITMS measurement of carbonyls and multifunctional carbonyls in PM2.5 particles emitted from motor vehicles.

A method was developed and tested to identify and quantitate carbonyls and multifunctional carbonyls in fine particulate matter (PM2.5; <2.5 microm aerodynamic diameter). The method relies on ultrasonic extraction of particulate matter on filters at -8 degrees C; derivatization with O-(2,3,4,5,6-pentafluorobenzyl) hydroxylamine (PFBHA), and PFBHA along with bis (trimethylsilyl) trifluoroacetamide (BSTFA); and detection of the derivatives by gas chromatography/ion trap mass spectrometry. Ultrasonic extraction of model compounds from enriched particles was affected by solvent polarity (water > methylene chloride > toluene-isopropanol (2 + 1, v/v). Water provided the highest recovery for dihydroxy acetone, pyruvic acid, and hydroxy acetone, compared to methylene chloride, and toluene-isopropanol. Lowering the ultrasonication bath temperature from 0 degrees to -8 degrees C improved the recoveries of the less water soluble and more volatile species-methacrolein, methyl vinyl ketone, 2,3-butanedione, and tolualdehyde. The power of the method was demonstrated by identification and quantitation of carbonyls and multifunctional carbonyls in sample extracts of fine particulate matter (PM2.5) collected in the Caldecott tunnel, CA. The identities of crotonaldehyde, 2,3-butanedione, glyoxal, 9H-fluoren-9-one, glycolaldehyde, glyoxylic acid, levulinic acid, and 3-hydroxybenzaldehyde were confirmed by comparing the relative retention time and mass spectra of the analyte in the sample extract with an authentic standard. Quantitation of crotonaldehyde, glyoxal, 2,3-butanedione, glyoxylic acid, and levulinic acid was accomplished. This is the first report of glyoxylic acid, levulinic acid, and 3-hydroxybenzaldheyde in PM2.5 particles sampled in a roadway tunnel. It is also the first report of a C10 carbonyl with the molecular formula of C10H16O2, a hydroxy carbonyl with the molecular formula of C17H21NO2, and a hydroxy or dihydroxy carbonyl with the molecular formula of C16H14O2 or C9H10O3. The high-molecular weight hydroxy carbonyls, which were found only in the heavy-duty (diesel) bore, may be tracers of diesel emissions in air.

[Novel methods in diabetes mellitus diagnosis].

Analysis of the lacrimal fluid was carried out for the diagnosis of diabetes mellitus in 68 patients. Collection of tears is absolutely painless, atraumatic, and involves virtually no risk of viral infections. Analysis of the lacrimal fluid is particularly valuable for repeated analyses over the course of disease. A method for postmortem diagnosis of diabetes mellitus and hyperglycemic coma by measuring glucose in the vitreous body is suggested. The content of glucose in the vitreous higher than 17 mmol/liter is a specific marker indicating death from hyperglycemic coma.

[The effect of weight loss caused by tissue dehydration on changes in carbohydrate metabolism under physical load].

Moderate dehydration of rats led to a certain reduction of glycogen content in the liver and muscles. The analogous changes were observed when the animals were subjected to physical load. The previous dehydration also causes a delay in glycogen l-vel restoration in tissue after work is over. Under usual conditions physical load which wrestlers underwent on a veloergograph caused a certain increase in the concentration of lactic and pyruvic acids in blood. Against a background of previous dehydration physical load of the same intensity caused a more considerable change in these values. Consequently, even moderate dehydration of an organism practised by some sportsmen leads to considerable change in certain values of carbohydrate metabolism. It may be one of the reasons of a negative influence produced by weight loss on working capacity.

Ultraviolet spectrometry of serum triglycerides by a totally enzymic method adapted to a centrifugal analyzer.

We described a micromethod for determining serum triglycerides (triacylglycerols) with the centrifugal analyzer. This technique is based on the procedure of Bublitz and Kennedy [J. Biol. Chem. 211, 951 (1954)]. The enzymic hydrolysis requires 10 min at 30 degrees C. Twenty-six serum triglyceride assays can be done in about 30 min. Concentration and absorbance are linearly related up to 5.0 mmol/liter, but higher concentrations can be assayed by changing conditions slightly. Day-to-day reproducibility for the method was satisfactory (CV, 2.7 to 8.4%). Comparison of this method and two other methods for triglycerides, the automated Hantzsch condensation method and a commercial enzymic method (Calbiochem), gave correlation coefficients of 0.976 and 0.990, respectively. Results are unaffected by the presence of endogenous serum ADP, pyruvic acid, phosphatases, or ATPase.

[Effect of experimental diabetes and thyrotropic hormone on carbohydrate and lipid metabolism in heart and liver of rats].

The purpose of this research was to analyse the change in relationships between the carbohydrate and lipid metabolism parameters in the cardiac muscle and liver in the rats, in which the thyroid gland was induced additionally by use of thyrotropic hormone. The tests were performed on 33 white rats of the Wistar breed that were subdivided into four groups. The first group were reference animals. In the rats of the second group thyroid hyperfunction was induced by use of thyrotropic hormone. In animals of group there diabetes was evoked by use of alloxan. Diabetes and the thyroid hyperfunction the animals in of group four. The levels of glycogen, lactic and pyruvic acids, cholesterol, free fatty acids, phospholipids and triglycerides were determined in the cardiac muscle and liver of all the animals. It was found that the occurrence of both hormonal disorders: experimental diabetes and thyroid hyperfunction noticeably changes metabolism of the cardiac muscle as well as the carbohydrate and lipid metabolism and leads to disorders of carbohydrate metabolism in the liver.

[Dimorphism in "Ustilago cynodontis". II--Glucidic metabolism (author's transl)].

The glucidic metabolism has been studied in four strains of Ustilago cynodontis. Two of them--M1 and M7--are mycelial strains, the two others --L1 and L7--being yeast like are respectively issued from M1 and M7. The results obtained show that the choice between the different glucidic catabolism pathways takes place at the phosphofructokinase level. When the phosphofructokinase is lacking (M1) the catabolism occurs via the pentose phosphate cycle followed by the last glycolytic reactions (between triose-phosphates and pyruvic acid). When the phosphofructokinase is present it always enters into competition with a very active phosphoglucose isomerase which makes it ineffective (L1, M7, L7). In those cases the catabolism still proceeds through the pentose cycle either completely (L7) or incompletely (L1, M7), depending in the presence or the absence of transketolase. In the last case it is the phosphoketolases which carry out the conversion between fructose-5-phosphate, xylulose-6-phosphate and acetyl-phosphate.

[Effects of pH on the endocrine system and metabolism].

Changes in extra- and intracellular pH affect metabolic states and hormone responses. In general, alkalosis stimulates and acidosis inhibits glycolysis although the relationship is not a simple one. 6-phosphofructo-1-kinase, a rate limiting enzyme in glycolysis, seems to be activated directly by a rise in pH. Alkalosis stimulates the production of pyruvic acid and lactic acid. Citric acid cycle is stimulated in alkalosis and hexose monophosphate shunt pathway is facilitated in acidosis. Acid-base disorders affect the insulin secretion and insulin action. In acidosis, insulin action in the target tissues is reduced. Other hormones, including parathormone, renin-angiotensin-aldosterone system, and adrenocorticotropine, respond to the changes in pH.

[The relation between oxidative processes and the glycogen content in the heart and liver of rabbits with chronic ischemic heart disease].

While laboratory experimental model of coronary heart disease (according to Frol'kis et al.) is developed, activity of succinate dehydrogenase, alpha-ketoglutarate dehydrogenase, Na+, Ka(+)- and Mg2+ ATPase decreases, but activity of lactate dehydrogenase and concentrations of lactic and pyruvic acids in the heart tissue increase. At the same time concentration of glycogene increases more than twice. As far as we can see there is an evidence of a decrease of glycogene utilization due to change in levels of regulatory processes. Despite a decrease of ATP synthesis by the inhibition of tricarboxylic acid cycle the ATP:ADP relation reduces to ATP, as emphatic inhibition of ATPase in the heart tissues takes place in development of the model of the coronary heart disease. The relation between ATP and ADP is considered as a regulator of glycogene utilization. In the liver tissue activity of succinate dehydrogenase, alpha-ketoglutarate dehydrogenase, Na+, K(+)- and Mg2+ ATPase falls, while concentrations of lactic acid grow. No accumulation of glycogen is observed. It is obvious that there are controversial metabolic processes. Experimental data are discussed.

[Physicochemical properties of the microbial exopolysaccharide ethapolan synthesized on a mixture of growth substrates].

Some physicochemical properties of the microbial exopolysaccharide (EPS) ethapolan synthesized by Acinetobacter sp. 12S depended on whether the producer was grown on a mixture of ethanol and glucose or on single substrates. Irrespective of the carbon source in the nutrient medium, the contents of carbohydrates, pyruvic acid, uronic acids, and mineral components in the EPS remained unchanged. The EPS were also identical in their monosaccharide composition: the molar ratio of glucose, mannose, galactose, and rhamnose was 3:2:1:1. EPS with a higher proportion of fatty acids was synthesized during growth on the mixture of ethanol and glucose. Average molecular weight and the proportion of high-molecular (over two million) fractions were greater in ethapolan produced on the substrate mixture. In the presence of 0.1 M KCl, after transformation into the H+ form, and in the Cu(2+)-glycine system, solutions of these EPS showed higher viscosity than solutions of EPS synthesized on single substrates. The reasons for the improved rheological properties of the EPS produced on the substrate mixture are discussed.
