[A case of recurrent breast cancer responding to combination therapy with mitoxantrone (MIT), 5'-deoxy-5-fluorouridine (5'-DFUR) and medroxyprogesterone acetate (MPA)].

The patient was a 50-year-old woman who had undergone left standard radical mastectomy who had undergone left standard radical mastectomy on June 1, 1986. She showed multiple liver metastases with elevation of CEA level in July, 1991, and 5'-DFUR plus MPA combination therapy was started. The daily dosages were: 800 mg/body and 1,200 mg/body, respectively. After intra-arterial infusion of pirarubicin and Lipiodol, bilateral oophorectomy was performed and an implantable reservoir for intra-arterial infusion chemotherapy was implanted via the proper hepatic artery. Then she was treated by arterial-infusion of mitoxantrone 10mg/body intermittently every two weeks. The metastatic foci responded to this therapy and her CEA level decreased.

[Plasma concentrations of non-esterified fatty acids and lipids in high-dose medroxyprogesterone therapy of metastatic breast cancer].

The objective of this study was to investigate the influence of high dose MPA on serum cholesterol, triglyceride, phospholipids as well as free fatty acids given to 17 patients with metastatic breast cancer. Before the therapy, the patients with cancer revealed higher level in serum cholesterol, triglyceride, phospholipids and C 12:0, C 18:0 as we found it in the control (13 patients without cancer). There was no significant change in lipid parameters by treatment with MPA whereas cortisol decreases and insulin increases under therapy. The results are discussed under biochemical aspects.

[A case of breast cancer with metastasis of multiple organs showing marked efficacy of medroxyprogesterone acetate (MPA)].

The patient, a 55-year-old female, was diagnosed as having right breast cancer (T1aN2M1, Stage IV) with metastasis of multiple organs (lung, pleura, bone, mediastinal and unilateral axillary lymph nodes) when she underwent tumorectomy. Postoperative adjuvant therapy was performed using oral administration of MPA (800 mg/day). After 3 months of this treatment, metastatic lesions in the lung and pleura had disappeared. After 37 months, complete remission (CR) was evaluated in the multiple lesions of bone metastasis. She achieved a complete remission (CR) by this treatment lymph nodes metastasis in both mediastinum after 24 months and unilateral axillae after 15 months. Complete response time was 38 months in both lung and pleura, 15 months in bone metastasis, 15 months in mediastinal lymph nodes, 24 months in unilateral ones. Normalization of tumor markers was acquired after 7 months of this hormonal therapy. This CR condition continues at present without severe complications.

Use of depot medroxyprogesterone acetate contraception in adolescents.

More than 1 million teen pregnancies occur each year in the United States. DMPA offers several distinct advantages over other contraceptive methods in adolescents. The duration of effectiveness and convenience of DMPA are appealing to teens. Most teens who are current users view amenorrhea as a positive feature of DMPA. Although most young women are able to tolerate the irregular bleeding, some find the cycle disruption concerning. Satisfaction rates among adolescent DMPA users is high.

[Therapy of premenstrual syndrome with medroxyprogesterone acetate in various dosages].

The authors report on their observations with the treatment of premenstrual syndrome with gestogens of different dosage. The efficacy of medroxyprogeszteron-acetate therapy with 20 mg/die and 40 mg/die between the 15. and 26. days of menstruation were studied with the help of the premenstrual syndrome index. The connection between the efficacy and dosage of the efficacy and period of therapy were observed. According to the data the elevated dosage of medroxyprogeszteron-acetate does not increase the efficacy of therapy. After 3 month treatment period 67% of the complaints disappeared. Further therapy does not increase efficacy, but has been important to prevent remissions. According to the data of this study the authors opinion is that a dosage of 20 mg/die medroxyprogeszteron-acetate treatment between the 15. and 26. days of menstrual cycle is sufficient to treat premenstrual syndrome.

[High-dose medroxyprogesterone acetate in the treatment of metastatic carcinoma of renal parenchyma].

In animals it was found that the growth of oestrogen induced renal cell carcinoma can be inhibited by progesterone derivatives. In human clinical studies of metastatic renal cell carcinoma treated with MPA doses of 500 mg/24 h the overall response was about or below 10%. However, it has been noted that there is a dose effect relationship in MPA in breast cancer. As the level of hormonal receptors in renal cell carcinoma tissue is low, it could be expected that dose escalation of MPA might influence the therapeutic response. Thirteen patients with metastatic renal cell carcinoma with progressive disease treated with MPA doses of 500 mg/24 h were treated with escalated doses of MPA 2000 mg/24 h for 10-15 consecutive days, the drug free interval being 3-4 weeks. Twelve patients were evaluated for response. Partial response was registered in 3/12 patients with median survival-time of 15 months, 4/12 patients had stable disease and 5/12 patients had progressive disease. All patients with a partial response have been previously nephrectomised, and had lung metastases. The type of the response was also noted in interferon treated patients. During the high-dose MPA treatment several patients displayed thrombocytopenia, a phenomenon difficult to interpret. One patient, previously nephrectomised, developed fatal renal vein thrombosis of the remaining kidney. Other toxic effects (weight gain, transitory psychotic episodes) did not require treatment interruption. There seems to be a dose response relationship of MPA in a sub-category of patients with metastatic renal cell carcinoma, and additional treatment responses may be observed in patients treated with escalated MPA doses.
