How and what do autistic children see? Emotional, perceptive and social peculiarities reflected in more recent examinations of the visual perception and the process of observation in autistic children.

Autistic symptoms become apparent at the earliest during the 2nd-3rd month of life when the spontaneous registration of the meaning of specific-visual stimuli (eyes, configuration of the mother's face) do not occur and also learning experiences by reason of mimic and gestures repeatedly shown by the interaction partner can neither evoke a social smile nor stimulate anticipational behaviour. Even with increasing age an empathetic perception of feelings in the corresponding mimical gesticular formation is very difficult and they themselves are only insufficiently able to express their own feelings intelligibly to everyone. As mimic and gestures are, however, visually perceived, the autistic perceptive child's competence is of great importance. On the basis of the examinations of visual perception (retinal pathology, tunnel vision) perceptual processing (recognition of feelings, sex and age) and the disintegration of multimodal stimuli it can be presumed that social and emotional deficits are to be seen in connection with a deviant perceptive interpretation of the world and irregular processing on the basis of a neuro-biological handicap (the absence of a genetic determined reference-system for emotionally significant stimuli), which can have various causes (comp. Gillberg 1988) and also impede the adequate expression of feelings in mimic, gestures and voice. Autistic people see, experience and understand the world in a specific way in which and by which they differ from non-handicapped people.(ABSTRACT TRUNCATED AT 250 WORDS)

[Identification of the skulls of paratroopers from Ressl Street in Prague].

The paper presents an overall report on the skull identification of paratroopers shot in Ressl Street, Prague. The victims, members of the underground group, assassinated R. Heydrich in June 1942. The skull identification is based on the paratroopers' photographies and, in particular, on the stomatological findings which was compared with the dentition visible in some photographs, especially those showing them smiling. None of the skully examined belonged to the actual assassins (J. Kubis, J. Gabcík). It is highly probable that they were those of the other members of the underground group.

Occipitofrontalis muscle: functional analysis revealed by electromyography.

The occipital and frontal bellies of the right occipitofrontalis muscle were studied electromyographically in thirty volunteers in various movements, using a special steel electrode (80 microns diameter). The occipital and frontal bellies have independent actions despite the galea aponeurotica. The frontal belly initially raises the homolateral eyebrow, followed by a displacement of the galea aponeurotica, drawing the scalp forward; therefore it is active in the formation of the transversal wrinkling in the forehead. The occipital belly eventually can be active during raising of the eyebrown, nevertheless it does not interfere with the action of the frontal belly. The occipital belly alone is responsible for drawing back the galea aponeurotica. The occipital belly is also active during smiling and yawning, and can be active during the movements of the auricula.

[Cast composite veneers].

No matter the reason, patients will no longer tolerate any malformation or staining of anterior teeth. One can not ignore the fact, that with time, esthetics and a pretty smile have become important issues for most individuals. When indicated and properly placed, composite veneers can probably rectify these situations (at least as a temporary measure due to their moderate cost and the minor amount of tissue which has to be destroyed). Professor Françoise Roth, who was one of the first individuals to study this procedure, allows us to profit from her experience with the assistance of Doctor Michel Gouvernet.

The role of bleaching in esthetics.

Bleaching discolored teeth must be considered a viable treatment modality. Although it does not offer the predictability of many cosmetic options, its simplicity and economics demands attention. Combining bleaching with selected bonding and contouring can return a smile to the not-so affluent, conservative, or apprehensive patient who does not wish to face tooth reduction and full coverage.

[Lateralization of the epileptic process and psychology].

In a group of 70 patients with a unilateral epileptic process in the temporal limbic structures, localized in the left half of the brain in 38 patients (26 men, 12 women) and in the right half of the brain in 32 patients (17 men, 15 women) the lateralization of motor functions, asymmetry of emotional mimicry, conjugated deviation of the eye bulbs, intellect, memory, psychomotor rate and personality (laterality test of Matĕjcek--Zlab, Wechsler's intellect and memory scale, Benton, Stroop, VAROS, SUPOS, DSF, Washington Psychosocial--Seizure Inventory WPSI was investigated. Patient with a left-sided epileptic process had only insignificantly lower contact with psychiatry (31.6% as compared with 40.6%). Significantly lower contacts with psychiatry were recorded in 35 patients with a predominance of emotional mimicry (while smiling) on the left (25.0% as compared with 47.1%, p less than 0.05) and particularly significantly low in 18 patients with a combination of signs left-sided epileptic process + predominance of emotional mimicry on the left + deviation of the eyeballs on the right (mathematical stimulus) + genotypical righthandedness (16.7% as compared with 42.3%, p less than 0.025). Men with a left-sided epileptic process displayed a significantly greater anxiety, in women this trend was only indicated. Four women with a left-sided epileptic process anticipating regulation of behaviour was typical, for women with a right-sided epileptic process lower autoregulating inhibitions. A left-sided epileptic process had significantly more often a favourable course of epilepsy (in 65.8% as compared with 25.0%, p less than 0.005). In psychological tests, however, the differentiating criteria were much more than the course of the disease invalid pension, contact with psychiatry and subjective need of assistance from a psychiatrist or psychologist.

The effects of pet therapy on the social behavior of institutionalized Alzheimer's clients.

Twelve (N = 12) Alzheimer's (AD) residents of a Special Care Unit in a large midwest Veterans' Home were observed for the effects of the presence of a pet dog on eight social behaviors: smiles, laughs, looks, leans, touches, verbalizations, name-calling, and others. Observations took place on three separate occasions (absence of dog, temporary presence of the dog, and permanent placement of the dog) in both group and individual settings. The results showed that the presence of the dog increased the number of total social behaviors of the AD clients, but no differences were found in behaviors between the temporary and permanent placement of the dog.

[Solubility and degree of hemolysis as methods for the in vitro evaluation of dental filling materials].

For the biological examination of dental resin filling materials, a solubility and hemolysis test is recommended as an in-vitro testing method. Solubility and degree of hemolysis were examined in various filling materials based on MMA and BIS-GMA. It was found that, at the beginning, solubility was considerably higher in MMA-containing materials than in the BIS-GMA systems, and that after a while the conditions were reversed. The degree of hemolysis decreased in the following order with the individual materials: 1. Posite, Sevriton, Adatpic, 2. Epoxylite, Smile, Palakav, 3. MMA-BPO/Amin and MMA-TBBO. Comparison between the MMA and BIS-GMA monomers showed that corresponding to the same molar amounts, BIS-GMA materials developed a higher degree of hemolysis.

Development of equipment for the standardization of skull photographs in personal identifications by photographic superimposition.

The technique of photographic superimposition of postmortem specimens (dried skull or head) positioned identically to the orientation in an antemortem smiling photograph is now a recognized method for personal identification of human remains. Previous methods used to produce the postmortem photographs were problematic as orientation of the specimen could not be easily adjusted and positions were not reproducible. The objective of this paper is to describe the design and method of use of purpose-constructed equipment intended to alleviate these problems. The equipment comprises a skull-holding jig based conceptually on the Royal Berkshire Hospital halo frame. This is mounted on a pan-and-tilt device, incorporating calibrated measurement scales, enabling independent movements in each of the Cartesian coordinates. A camera is attached to an adjustable mount running on twin parallel rails allowing the camera-to-specimen distance to be varied. The equipment has proven to be straightforward in use and offers considerable advantages over previously described methods for producing postmortem photographs.

Plasmid fingerprinting. A tool for bacterial strain identification and surveillance of nosocomial and community-acquired infections.

Plasmid fingerprinting provides a rapid and dependable means of identifying bacterial isolates of the same strain. The stability, wide distribution, and diverse nature and size of extrachromosomal elements make it suitable for virtually all bacterial genera. There are many different procedures available for plasmid screening, and the one chosen depends primarily on the types of organisms to be analyzed. Some procedures are better suited to gram-positive organisms; others are better for visualizing the very large plasmids often seen in Pseudomonas and Rhizobium species. The key to the plasmid fingerprinting technique is agarose gel electrophoresis. In this step of the technique, it is important to differentiate open circular from closed circular forms of plasmids and to recognize the "smile effect." Plasmid fingerprinting can be utilized for epidemiologic studies of both nosocomial and community-acquired infections. The use of restriction endonuclease analysis can greatly enhance the ability of the investigator to differentiate strains that harbor only a single plasmid. Plasmid fingerprinting often provides the only differential characteristic for strains involved in epidemics.

Effects of the vestibular system on human development, Part I. Overview of functions and effects of stimulation.

Studies concerned with the effects of vestibular stimulation on human development and function are reviewed and some implications for therapy are suggested in this paper. Studies are included that relate to changes in the vestibulo-ocular reflex with age, effects of vestibular stimulation on smiling, crying, general activity, and visual attentiveness of infants; and studies that show that typical sequelae to vestibular stimulation are reduced following long-term stimulation. The importance of reviews of the literature and transference of research results to therapeutic use are stressed, with the implication that vestibular-based therapy will improve as occupational therapists become more aware of related research.

Use of mucogingival surgical procedures to enhance esthetics.

The procedures outlined herein deal primarily with improving esthetics. The importance of esthetics in periodontics is most apparent in patients with a high lip line. Current mucogingival procedures provide the surgeon with the capability to have a major role in the total treatment of complex restorative cases. Periodontal considerations must be included as an integral part of esthetic dentistry. A complete examination of the patient must include a thorough periodontal evaluation. In addition to the usual detection of periodontal pockets and inflammation indicative of periodontal disease, the examination should include a thorough mucogingival evaluation for detection of the following: (1) inadequate keratinized gingiva; (2) gingival recession; (3) excessive gingival display; (4) insufficient clinical crown length; (5) asymmetric gingival margins; (6) flat marginal contour; (7) improper gingival margin relationships; (8) lack of harmony of gingival margins with the lip line; and (9) alveolar ridge deficiencies. Mucogingival surgical procedures should be performed after control of all etiologic factors and in concert with the restorative therapy so that the surgery can produce the most desirable esthetic result. It must be understood that the location and contour of the gingival margin and its relationship to adjacent margins are as important to the final esthetic result as the location, form, and relationship of the incisal margins of the anterior teeth. Attention to the total dental display as framed by the smile, with appropriate mucogingival analysis and treatment, is necessary to achieve the most satisfying esthetic results.

Temperament and the development of inhibited approach.

The early development of inhibited approach was studied through the observation of infants' reaching toward objects. 48 infants were observed longitudinally at 6.5, 10, and 13.5 months as they reached for toys under high- and low-intensity/novelty conditions. It was predicted that if an approach system related to positive affect were in place by 6 months, infants should show relative interindividual stability across age in their latency to approach low-intensity/novelty toys. This latency would also be inversely related to temperamental positive affect, that is, children who smiled and laughed more would reach more quickly. It was also predicted that, if behavioral inhibition to high-intensity/novelty stimuli were developing over this period, relative instability of latency to grasp high-intensity/novelty toys would be found. This latency would also be positively related to temperamental ratings of fearfulness, that is, children who were more fearful, would grasp more slowly. These hypotheses were generally supported, with the exception of the relation between parent-reported fearfulness and latency to approach and grasp. In addition, sex differences in frequency of hesitations were found.

Relationships among dental functional status, clinical dental measures, and generic health measures.

Current measures of dental health status are primarily clinical in nature and rely on clinical and radiographic assessment of the patient's dental health. Information about a patient's ability to perform usual activities related to good dental health--for example, chewing, speaking, and smiling--is not routinely collected. This study investigated what measures contribute to dental functional status, how they are related to traditional clinical measures, whether dental factors contribute to other generic measures of health, and the extent to which dental factors contribute to overall quality of life. Regression analyses of interview and clinical data from 159 dental patients show that while periodontal status and the number of dental symptoms do explain some of dental functional status, the clinical measures of decayed, missing, and filled teeth do not. Severity of medical condition is correlated with decayed/missing teeth and periodontal health. Quality of life is explained by medical functional status, perceived medical health, and perceived dental health. The dental functional status index may be useful in clarifying the relationship between dental health and overall health and quality of life measures.

The early social environment of premature and fullterm infants.

The behaviors of 10 mothers of prematures and 29 mothers of fullterms were compared from 7-h observations made in the home when the infants were 2, 3, 4 and 5 weeks post-term. The observation day was divided into four mutually exclusive interactional contexts that together made up the total day: feeding time, changing or bathing time, time when the mother and infant were in physical contact but the mother was not caretaking, and time the infant was alone. Measures of ten maternal behaviors were also compared: moving, rocking, patting, caressing, talking, looking, engaging in vis-a-vis with the baby, holding or carrying, smiling or laughing, and stimulating the baby to suck. Mothers of prematures left their infants alone more and changed them less than mothers of fullterms. In addition, mothers of prematures moved their infants less often, talked to their infants less, looked at their infants less, and held their infants less. These results indicate that, over the 7-h day, prematures receive markedly less stimulation than fullterms. Since the neurobehavioral characteristics of premature and fullterm infants are known to differ, it is suggested that these differences in maternal behaviors may be in response to infant cues and appropriate for the infants.

[Electrophysiological study of congenital facial paralysis].

The common cause of neonatal facial asymmetry is facial nerve paralysis or "asymmetric crying facies syndrome". In the not uncommon later the lower lip, symmetrical at rest, becomes tilted to the so-called normal side when the patient is smiling or crying, as the congenital hypogenesis of sublabial muscles fail to pull down the lower lip in the opposite side. The electrophysiological differentiation between the two diseases has been performed by orbicularis oculi and oris reflexes with mechanically glabellar and supralabial tapping stimulation, respectively, in addition to needle and/or surface EMG recording. In the facial nerve paralysis of the case 1, R1 and R2 were absent in the orbicularis oculi and oris reflexes. EMG activity was completely lacking over the M. orbicularis oculi and oris innervated by facial nerve. On the contrary, the orbicularis oculi and oris reflexes were normal in the asymmetric crying facial of the case 2. EMG activity was absent only in the sublabial muscles including M. depressor anguli oris and/or M. depressor labii inferioris. Furthermore, needle EMG disclosed no spontaneous activity at rest, which was suggestive of no denervation in the sublabial muscles. It was, however, not possible to determine exactly which muscle the needle was inserted, the M. deprossor anguli oris or the M. depressor labii inferioris. The case 3 might be a variant of asymmetric crying facies with hypogenesis of M. orbicularis oris and/or oculi as well as the sublabial muscle, since the latency was normal but the amplitude was significantly attenuated in the components of orbicularis oculi and oris reflexes.(ABSTRACT TRUNCATED AT 250 WORDS)

Orbital fractures: eyelid approach.

Orbital floor fractures can be explored through the eyelid with a subciliary or a smile-crease incision. A two-plane incision is made to expose the orbital rim. Orbital contents are removed from the sinus, and an alloplastic implant is used to cover the fracture. The periosteum is closed over the implant. Fine suture material is used to close the skin. These approaches give good exposure of the orbital floor and excellent postoperative cosmesis.

[Clinical cases casuistic of oral rehabilitation].

To rehabilitate means to give back to the patient the former and initial function of his teeth. Various clinical cases are shown, different in age, sex, health and financial status of the patient. They illustrate the method to apply to each case definite constructive principles dictated by biomechanics. The solutions are diversified, but the principles underlying the decision are identical. Before starting work, 3 questions must be asked: What do to? How to do it? At what fee? Thus we can fulfil the patient's wish to eat with pleasure, to talk agreeably and to smile without inhibitions.
