[Mosquito bite allergies terminating as hemophagocytic histiocytosis: report of a case].

A 21-year-old woman had suffered from repeated vasculitis and panniculitis with fever and chills after following mosquito bites since age 7. These manifestations were confirmed with a direct mosquito bite test during her admission to another hospital at age 17. The patient presented to our hospital with progressive dyspnea, productive cough and intermittent fever for one week. In addition to the bilateral infiltrative lesions on chest roentgenography, hepatosplenomegaly with an abnormal liver function test, pancytopenia, and elevated IgE were also detected. Blood and sputum cultures grew no microorganisms. Epstein-Barr virus-IgM, Cytomegalovirus-IgM and Mycoplasma pneumonia antibodies were all negative. Bone marrow aspiration and biopsy revealed histiocytosis with hemophagocytosis. No atypical histiocyte was found. The patient was put on dexamethasone treatment with improvement. Unfortunately, symptoms relapsed two weeks later. A repeated bone marrow aspiration and biopsy revealed a picture similar to the previous one. Despite antibiotic administration and ventilator support, a rapidly deteriorated course terminated in the patient's death by respiratory failure. Mosquito bite allergies were reported to be associated with malignant histiocytosis in Japan. However, atypical histiocytes were not found in our case upon repeated bone marrow aspirations, biopsies and skin biopsies. Mature histiocytes with hemophagocytosis were prominent instead. Reactive histiocytosis was thus favored rather than malignant histiocytosis. Although opportunistic infection cannot be excluded, we propose that a mosquito bite allergy with consequent histiocytic activation by antigen, immune complex or IgE is the possible pathogenetic mechanism for hemophagocytic histiocytosis in this patient.

[The diagnostic and treatment characteristics of spinal fractures in the victims of multiple trauma].

The authors present their experience in the treatment of 107 patients with spinal injuries associated with polytrauma. The topical diagnosis of the spinal lesions in the severely injured patients was rather difficult both at the preclinical stage and during the clinical examination. For making the diagnosis in combined spinal trauma urgent laparoscopy and aimed roentgenography was employed with the patient in the dorsal position. When the latter technique is employed, a special attachment ("cassette holder") is used which allows to make the roentgenograms with mobile X-ray units. The authors have proposed a new construction of the reclining hammock for the treatment of the patients with the thoracic and lumbar spine traumas associated with severe injuries of the head, the breast and the extremities. It is pointed out that in the patients with complicated spinal injuries the surgical treatment of the locomotor system is most effective.

[The use of nuclear magnetic resonance in the diagnosis of hypophyseal adenomas (preliminary report)].

The paper is concerned with the use of a new tomographic method based on the principle of nuclear magnetic resonance (NMR-tomography) in the diagnosis of adenohypophyseal diseases. A NMR-tomograph with a resistive magnet (0.234 T) was employed. Altogether 15 patients with the most common types of hypothalamo-hypophyseal diseases including Itsenko-Cushing disease (6) of various degrees of severity, hyperprolactinemic hypogonadism (8), and one patient with acromegaly were investigated. NMR-tomography was shown to possess a high informative value permitting assessment of the presence of adenohypophyseal pathology without irradiation of a patient. NMR-tomography combined with routine methods of examination of patients with hypothalamohypophyseal pathology (roentgenography, computerized tomography, hormonal investigation) made it possible to improve considerably early diagnosis of pituitary adenomas and to specify the volume and type of tumor growth.

[Modeling hyperparathyroid osteodystrophy in animals].

For the investigation of roentgenologic signs of osteodystrophy, especially of their early signs and symptoms, an animal model is proposed. The roentgenologic, micro-roentgenographic and pathomorphologic skeletal image of the disease can be studied in this model over all stages, which is useful for the understanding of pathologic transformations of bone structure in hyperparathyroid osteodystrophy. Especially micro-roentgenography is able to visualize changes of bone architecture in the early stages of the disease.

[Diagnosis and treatment of post-traumatic diaphragmatic hernia].

In the work, the experience with diagnosis and treatment in 23 patients of diaphragmatic hernias, which developed because of diagnostic and tactical errors in acute and more late period after closed thoracic trauma is summarized. Roentgenography, pneumoretroperitoneum and contrast study of the stomach and intestine were the leading methods of investigation. Timely diagnosis of posttraumatic diaphragmatic hernias and early performance of reconstructive operation prevent the development of complications.

[The postcholecystectomy syndrome in peptic ulcer].

Results are reported of examination and treatment of 26 patients with gastric and duodenal ulcer which was the cause of postcholecystectomy syndrome and was often not diagnosed before the operation. Roentgenography of the gastrointestinal tract and fibrogastroduodenoscopy helps to establish the diagnosis. Treatment in a gastroenterological clinic and health-resort treatment allow to improve long-term results in this category of patients.

[Pneumography of the membranous spaces in the spinal cord].

The author describes one-stage investigation of the epidural and preneural spaces at the lumbar level using gas and a needle for lumbar puncture with a longitudinal side hole at the working end with subsequent roentgenography (pneumospondylography). Altogether 140 patients with lumbar vertebral osteochondrosis with neurological disorders were examined. Pneumospondylography gives 3 levels of contrast and ensures information on the sizes of the epidural and arachnoidal spaces, function of the invertebral holes and paravertebral tissues. The phenomenon of differentiation of the dural sac wall helps to diagnose in more detail the posterior protrusions of the intervertebral disks with concomitant epiduritis, pachymeningitis, arachnoiditis by the length and diameter of the vertebral canal.

[Differential diagnosis of tuberculotic lung tumor and peripheral lung cancer].

This report discusses the two diseases differentiated characteristics in age, diseases time, clinical signs and symptoms, location and size of lesions, exercise with radiographs and fiber bronchoscopy and cytologic etc. by experience of diagnosis and treatment to 94 patients of lung tuberculosis tumor and 196 patient of peripheral lung cancer for ten years. This article summarizes that early diagnosis characteristic of lung cancer is: asymptomatic in clinic, most patients are over 40 age, less lesions as satellite on a sectional roentgenography. The lesions still gradually develop, through treatment effectively tuberculosis, and the article suggests that the lung tuberculosis tumors that they may be cancer but diagnosis is doubtful in clinic should be treatment by operation.

Cardiac status after four years in a trial on nutritional therapy for high blood pressure.

A randomized controlled trial demonstrated the ability of nutritional intervention in place of antihypertensive drugs to maintain blood pressure at normal levels for four years in 39% of less severely hypertensive patients whose blood pressure was previously well controlled by pharmacologic treatment. However, average blood pressures during the trial for patients in the intervention group were higher than those for a comparison group that continued to receive drug therapy throughout the study. Holter monitoring, echocardiography, roentgenography, and electrocardiography done at four years to determine whether blood pressure differences between groups were associated with differences in cardiac status did not indicate any differences in cardiac status favorable to one group compared with the other. Further investigation in larger samples is needed to assess any long-term differences in cardiac status based on such alternate therapies.

[Current diagnosis of dissecting aortic aneurysm].

The prerequisites for proper treatment of a dissecting aneurysm are for the diagnosis to be established quickly and for the location and extent to be identified precisely. For this purpose, various partially complementary methods of examination in addition to the history and clinical findings are at our disposal: plain chest roentgenography, abdominal sonography, transthoracic and transesophageal echocardiography, computed tomography, magnetic resonance imaging and angiography. Thirty-one of our patients have been studied to assess the reliability of the different methods. The advantages, diagnostic criteria and limitations of the methods are discussed.

Pharmacologic management of Paget's disease.

The pathogenesis, clinical features, indications for therapy, and current pharamacologic management of Paget's disease are reviewed. Paget's disease is a bone disorder of unknown etiology primarily affecting the elderly. Overactive bone resorption leads to the accelerated formation of disorganized, weak bone. Pain and fractures are common clinical features. Neurologic, cardiovascular, metabolic, and neoplastic complications are also reported. Because most patients are asymptomatic, the disease is often detected during routine roentgenography or laboratory tests. Primary indications for pharmacologic intervention include bone pain, neural compression, immobilization hypercalcemia or hypercalciuria, cardiac failure, and orthopedic surgery. Recurrent or non-healing fractures and rapidly progressing complications are additional indications. Drugs used in the management of Paget's disease include calcitonin, etidronate disodium, and plicamycin. Although these agents are efficacious, each has disadvantages. Clinical resistance to animal calcitonins may develop, and the cost of therapy may be prohibitive. Etidronate may induce ostemalacia. The use of plicamycin is limited by potentially severe toxicities. Dichloromethylene and aminohydroxypropylidene are promising diphosphonate compounds but are still investigational In those patients who are unresponsive to single-agent regimens, combination therapy may prove effective. Although many patients with Paget's disease do not require pharmacologic therapy, calcitonin and etidronate are the agents of choice when it is indicated.

Penetrating thoracic aortic injuries: rare but potentially salvageable sequelae of urban warfare.

Penetrating injuries of the thoracic aorta are rare and often fatal. Victims of such wounds rarely survive to reach the emergency department. Those patients who do arrive will display one of two symptom complexes--either continuing hemorrhage or seeming hemodynamic stability with a widened mediastinum on chest roentgenography. As the results of our study of six such patients show, at least 50% of those who arrive with any signs of life may be salvaged by aggressive surgical management.

[Ultrasonic diagnosis of tendon and ligament injuries in anterior dislocations of the shoulder joint].

The usefulness of ultrasonography in diagnosis of frequently occurring traumatic injuries of the ligament-tendon apparatus of the shoulder has been evaluated and compared with intra-operative nd contrast roentgenography examinations. An analysis of 102 cases, including 85 fresh anterior dislocation, 17 habitual anterior dislocation, has been made. 306 ultrasonographic examinations have been performed. A frequent coexistence of injuries of the tendon of the long head of the biceps muscle with an injury of the rotators cuff, mainly the supraspinous muscle, has been pointed out. The purposefulness of routine ultrasonographic examinations in injuries of the shoulder, especially acute, and full consideration of indications of early surgical treatment have been stressed.

[A case of primary transitional cell carcinoma of the prostate].

A 58-year-old man was admitted to our hospital with the complaint of pollakisuria and micturitional pain. The urine cytology showed malignant cells suggesting the urothelial cancer, but various examinations could not reveal the malignant lesion. The prostate was also normal by the digital examination, endoscopy, roentgenography, ultrasonography and serum markers, and the transperineal prostate biopsy showed no malignancy. Three years after the first admission the prostate showed slight hardness and the transperineal biopsy suggested adenocarcinoma of the prostate. Hormonal therapy was then started and the prostate showed no remarkable change until about two years later, when rapid progression of the prostatic tumor was recognized. The transperineal biopsy of the prostate revealed the transitional cell carcinoma with negative staining of Alcian-Blue, PAS and PSA (prostate specific antigen). The epithelia of the bladder and posterior urethra were normal. The radical cystoprostatectomy was done and the histological diagnosis was the pure type of primary transitional cell carcinoma of the prostate. The literatures were reviewed and the clinical differentiation between transitional cell carcinoma and adenocarcinoma of the prostate was discussed.

[Retroperitoneal plasmacytoma: a case report].

We have experienced a case of retroperitoneal plasmacytoma. A 47-year-old man was admitted to our hospital with the chief complaints of right lumbago and lower abdominal dull pain. Roentgenography revealed retroperitoneal tumors, and right nephrectomy and excision of left pararenal tumors were performed. The histological examination and urine immunoelectrophoresis showed retroperitoneal plasmacytoma. No signs or symptoms of multiple myeloma were recognized postoperatively. The literature is reviewed briefly concerning the diagnosis and treatment of retroperitoneal plasmacytoma.

A new method for differentiating simple intra-abdominal from strangulated small-intestinal obstruction.

We developed a model that aids in the differentiation of simple from strangulated intra-abdominal small-intestinal obstruction and reached an accuracy rate of 97 percent in predicting strangulated obstruction. Fifty-three criteria were analyzed from the records of 229 patients and by the Bayes statistical mode; five statistically significant criteria were identified. Using these, and another 16 criteria that indicated trends, we were able to predict strangulated obstruction with a 97 percent accuracy. Neither laboratory tests nor abdominal roentgenography aided in the diagnosis.
