Cervical cancer: outcome of treatment and causes of failure.

OBJECTIVE: To summarize the data and look into the various treatments offered to cervical cancer patients at Institute of Nuclear Medicine and Oncology (INMOL) to highlight the most likely causes of treatment failure. METHODS: In this retrospective study, case files of all patients presenting with invasive carcinoma of uterine cervix during 1993-2002 were studied in respect to personal profile, disease related risk factors, pathological characteristics, treatment administered and outcome in the form of tumour response and survival. RESULTS: Early age at marriage, multiple marriages of self or spouse, multiparty, prolonged use of contraceptives and smoking were some of the risk factors for cervical cancer in this group of patients. Out of 618 patients presenting with invasive cervical cancer, 65% presented in advanced stages II and III. Apart from advanced stage at presentation, anaemia, poor nutrition, and ignorance about self-hygiene and lack of follow-up were main causes of treatment failure. Outcome of treatment was improved when chemotherapy was added to radiation. CONCLUSION: Advanced stage at diagnosis and lack of follow-up were main causes of treatment failure. Implementation of screening programs on national level for early detection is therefore recommended.

[Significance of the unexpected hypertensive response to exertion. Correlation with ambulatory blood pressure monitoring and echocardiography].

The aim of this study was to analyse the meaning of unexpected hypertensive response during treadmill stress test in resting normotensive individuals without any evidence of cardiovascular pathology. The study group of 52 persons who fulfil the inclusion criteria was compared with a homogenous control population composed by 200 individuals with a suitable blood pressure response to stress. The adopted criteria for hypertensive response were the presence of blood pressure greater than or equal to 180/100 mmHg after 6 stress, in maximal stress greater than or equal to 210/110 mmHg and after 5' recovering greater than or equal to 150/90 mmHg. 74% of patients with hypertensive response appeared to be hypertensive according to the criteria of ambulatory blood pressure monitoring--day blood pressure greater than or equal to 140/90 mmHg and night blood pressure greater than or equal to 120/80 mmHg. After echocardiographic study to define the index of left ventricular mass, 68% of individuals with hypertensive stress response showed left ventricular hypertrophy with a medium index of 145 + 17 g/m2 (P less than or equal to 0.01) in most cases with a concentrical hypertrophy type. The appearance of tensional response of hypertensive type ascertains the probability of hypertensive disease. The analysis of tensional response to stress is an integrant part of a methodology of approaching patients suspect of high blood pressure.

[Myelolipoma of the adrenal gland].

The Authors report a case of adrenal myelolipoma, with clinical picture of abdominal painful syndrome. Myelolipoma is a mixed tumor of mature adipose tissue, intermingled with myeloid tissue. None of the many theories advanced up to day has succeeded in explaining how these myeloid and adipose inclusions occur in the adrenal gland. In the case reported tumor has a mass of huge size, displacing down the homolateral kidney. Useful diagnostic tools for this pathological condition are discussed. In the personal experience of the Authors urography, echotomography combined with C.T. allow a right preoperative diagnosis in a large amount of cases.

Cardiac valve prostheses at autopsy.

We describe the pathologic findings at autopsy in 96 persons who had undergone prosthetic heart valve implantation. The most common indication for implantation was rheumatic heart disease (58%), followed by infective endocarditis (22%). Cardiac failure led to death in two thirds of the cases, most often from infective endocarditis (18%) and congestive heart failure (13%). Failure of the prosthetic valve caused death in eight cases. There were 13 deaths intraoperatively or in the immediate postoperative period. Patients with mechanical prostheses survived longer than did those with bioprostheses. The most common prosthesis-related complication was thrombosis, found in 23% of mechanical valves and 11% of bioprostheses. Thrombosis led to death in only 4 cases. Thromboembolic phenomena led to nine deaths and were not related to anticoagulant therapy. These findings provide an approach to investigation, description, and diagnosis of prosthetic heart valves at autopsy.

[Infiltration of the eyelids and orbit as the first sign of generalized lymphoma].

The authors discusses the clinical signs in patients with tumorous infiltrations. In 11 among 12 persons these signs were the first pathological changes of a systemic condition--malignant lymphoma. Only in one patient one found simultaneously the enlarged peripheral lymphatic nodes++ and the liver. In 3 cases pathological changes afflicted the orbit, in 4 cases the lids, in 5--the orbit and the lids. The collection of the material by means of aspiration fine-needle biopsy and the computer tomography are efficient methods in establishing the diagnosis of lymphoma and its extensiveness. The possibility of repeating of these methods has a considerable importance in the control of the dynamics of the pathological processes.

[Ocular bobbing. A new hypothesis].

Three of 4 patients with ocular bobbing were typical cases while in the last case the ocular disorder was asymmetric. A neuropathologic study was conducted in 2 cases. In both use, findings included variable degrees of pontine destruction without lesion of the medulla, with extension of the lesion into the mesencephalon in 1 case. The various physiopathogenic hypotheses put forward are reviewed but none appears satisfactory. A new hypothesis is proposed, based on available clinico-pathologic data (35 published and 2 personal cases) and recent experimental studies. Two centers, one mesencephalic, the other in the medulla, could exist that would generate saccadic movements downwards, and in the pons a center would provoke inhibition of saccadic movements. Destruction of the latter could activate spontaneous saccadic movements downwards (the only possible ones remaining) with the condition that the mesencephalic generating center and its pathways are intact.

[Determination of free serum phenol by gas-liquid chromatography in hemosorption].

A modified procedure is developed for estimation of free phenol in blood serum by means of gas-liquid chromatography, appropriate in clinico-biochemical assays. The method enabled to evacuate the blood serum free phenol in healthy persons as well as in various pathologies of abdominal cavity before and after hemosorption. After hemosorption a distinct decrease of the free phenol content was observed in blood, correlating with improved clinico-biochemical patterns of the patients, with a decrease in encephalopathy manifestations as well as with normalization of the neurological status.

[Epidemiology of the most widespread bacterial infections and current trends in use of antibiotics].

The last decade has seen a greater use of broad spectrum antibiotics at the expense of those with limited spectrum. With this in mind, the trends in the commonest bacterial, respiratory, gastroenteric and urinary infections from 1963 to 1974 have been analysed with a view to identify correlations between epidemiology and trends in antibotic consumption. The epidemiological investigation was carried out on the INAM health institution assisted population, i.e. on a sample of more than thirty million subjects throughout the country. Diseases with prevalently Gram-positive aetiology (acute and chronic infections of the respiratory tract) and those with prevalently Gram-negative aetiology (infections of the urinary tract) were examined and the data obtained processed statistically. It is personally considered that study of this pathology over the last decade is not enough to explain the tendency to make use almost exclusively of broad spectrum antibiotics and the serious clinical, epidemiological and social implications deriving from the dissemination of multiresistant bacterial strains.

Acute nonlymphocytic leukemia and acute aplastic anemia. The simultaneous occurrence in marital partners.

A young husband and wife couple presented simultaneously with aplastic anemia and acute myelomonocytic leukemia, respectively. No etiologic agent or exposure was identified. With appropriate therapy, the wife achieved a complete remission and the husband has had a good response. Potential etiologic environmental factors known to cause both disorders are discussed. A single causative agent may induce stem-cell changes in genetically dissimilar persons and produce a spectrum of pathologic features ranging from aplastic anemia to acute leukemia.

[Pseudodementia in frontal tumours (author's transl)].

Our examinations have revealed personality changes in eighteen out of fifty patients with frontal tumour. The changes may be classified to a certain extent in the same symptomatological category as pseudodementia. In view of the site, size and histological structure of the tumours, the existence and severity of the disorders seem to be independent of tumour vascularization. Such personality changes may be the only pathological sign, even in the presence of large tumours. Earlier diagnostic difficulties (the group of patients dates back to the period prior to CAT scanning) may be largely overcome by the use of computerized axial tomography. Thus the duration of a case up to time of surgery can be reduced (hitherto as long as 10 years or more) and tumours may no longer grow to excessive size.
