[Occlusion with interrupted titanium clips for ruptured bullae under video-assisted thoracoscopic surgery].

Spontaneous pneumothorax is a common disease and as many as 20% of these patients should be operated because of a recurrent pneumothorax, persistent air leak, or failure to completely reexpand the lung. From January 1993 until March 1994, on the basis of experiment, we performed video-assisted thoracoscopic surgery (VATS) intervention and talc pleurodesis on 8 patients with spontaneous pneumothorax caused by ruptured bullae, and designed a new procedure of occluding the bullae with interruptured titanium clips. This procedure has some advantages of complete occlusion of lung tissue at the base of the bulla, minimal loss of lung tissue, effectively preventing leakage and recurrent pneumothoraces, and enough capacity of anti-tension while the lung expands. Also Authors have advocated giving a negative pressure suction to opsilateral lumen of the Carlen's tube interbation to visualize adequately the intrathoracic structures, while selective lung ventilation is performed. This method keeps hemodynamic stability and is performed more easily and safely than that of positive pressure insufflation is done. Postoperatively, complete lung expansion was obtained in all patients. There were no perioperative complications related to VATS, and no recurrences to date. These early results with thoracoscopic management of spontaneous pneumothorax caused by ruptured bullae have encouraging.

Suppressed renal response to 2,6-dimethyl clonidine but not clonidine in one kidney-one clip hypertensive rats.

The disparate renal actions of clonidine and 2,6-dimethyl clonidine (2,6-DMC), recently reported by the authors, may be the result of stimulation of alpha-2 adrenoceptors and imidazoline-preferring sites, respectively. Studies found that in one kidney-one clip (1K-1C) hypertension, the density of renal binding sites labeled by [3H] idazoxan (imidazoline-preferring sites) but not [3H] rauwolscine (alpha-2 adrenoceptors) was decreased compared with the 1K control rats. The renal response to 2,6-DMC in 1K-1C hypertensive rats and 1K control rats was compared. In 1K control rats, 2,6-DMC infusion directly into the renal artery (0, 1 and 3 micrograms kg-1 min-1) produced a dose-related increase in the percent fractional excretion of water and sodium and osmolar clearance. This effect was similar to that reported for the imidazoline-preferring site agonist moxonidine. Conversely, in 1K-1C hypertensive rats, these infusion rates of 2,6-DMC failed to alter renal water and solute excretion. By contrast, clonidine (3 micrograms kg-1 min-1) produced a similar increase in the percent fractional excretion of water and sodium, free water clearance and osmolar clearance in 1K-1C hypertensive rats and 1K normotensive rats. In renal membranes from 1K-1C hypertensive rats, [3H] idazoxan binding (imidazoline-preferring site antagonist) was decreased, whereas the binding of [3H] rauwolscine (alpha-2 adrenoceptor antagonist) was unaltered. As proposed in a previous study, the decreased response to 2,6-DMC but not clonidine in 1K-1C hypertensive rats indicates that these two agonists, although structurally similar, are functioning at two unique sites.(ABSTRACT TRUNCATED AT 250 WORDS)

A nose stud retainer clip lost within the nasal cavity discovered on dental radiographs.

It is fairly common for a dental surgeon to come across patients with foreign bodies in the nasal cavity. In this paper we report an unusual case of a nose stud retainer clip being displaced into the nasal cavity by trauma, and its discovery on radiographs taken to assess dental injuries. We also present a review of the literature since 1982, and assess the possible sequelae to nasal foreign bodies.

Effect of dietary calcium on the development of hypertension and hypertensive vascular lesions in DOCA-salt and two-kidney, one clip hypertensive rats.

OBJECTIVE: To examine the effect of modest changes in dietary calcium on systolic blood pressure (SBP) and myocardial and renal vascular lesions in Sprague-Dawley rats. DESIGN: Regular- (0.4%, by weight), high- (0.8%) or low-calcium (0.24%) diets were fed to normotensive control, deoxycorticosterone acetate (DOCA)-salt and two-kidney, one clip (2-K, 1C) hypertensive rats for 8 weeks. METHODS: Tail-cuff SBP and metabolic balance were measured once a week. At the end of the study the kidneys and hearts were collected for histological study. RESULTS: Dietary calcium had no effect on SBP in the DOCA-salt rats, but loading with calcium accelerated the rise in SBP in 2-K,1C rats (P < 0.01, high- versus regular-calcium diet). The high-calcium diet reduced the percentage medial area of intramyocardial arteries in the DOCA-salt and 2-K,1C hypertensive rats. The DOCA-salt rats on the low-calcium diet had a higher renal vascular lesions score than those on the regular- or high-calcium diet (P < 0.05). CONCLUSIONS: A high-calcium diet appears to prevent intramyocardial vascular wall thickening in DOCA-salt and 2-K,1C hypertensive rats, and a low-calcium diet aggravates renal vascular lesions in DOCA-salt hypertensive rats. These effects are not related simply to changes in blood pressure.

Baroreceptor and somatic sensory regulation of kidney function in two-kidney, one-clip Goldblatt hypertensive rats.

The aim of this investigation was to characterize the renal haemodynamic and tubular responses to somatic afferent nerve stimulation following the removal of afferent nerve input from the atria or the carotid sinuses in chloralose-urethane-anaesthetized Sprague-Dawley rats and two-kidney, one-clip Goldblatt hypertensive rats. Bilateral stimulation of the brachial nerve plexi at 15 V, 1.3 Hz for 0.2 ms resulted in an increase in systemic blood pressure in each group of 10-40%, while renal perfusion pressure was maintained at a constant level. There were significantly larger falls in left renal blood flow and combined left and right glomerular filtration rate in all groups following selective denervation of either the cardiopulmonary or the carotid sinus baroreceptors, respectively. Brachial nerve stimulation decreased urine flow rate and absolute and fractional sodium excretion from both kidneys in Sprague-Dawley intact animals by 53, 65 and 59%; in vagotomized animals by 68, 77 and 63%; and in carotid sinus denervated animals by 86, 90 and 48%, respectively. The renal response in the Goldblatt group were similar to the normotensive group, but the main contribution of the total response was from the untouched left kidney. The inhibitory influence of the vagus and carotid sinuses on the renal sympathetic nerve-mediated sodium and water resorption appeared to be enhanced in the Goldblatt hypertensive rats when compared with the normotensive rats. The renal functional responses to somatic afferent nerve stimulation appeared to be well preserved in the renovascular hypertensive rats, although there were important differences in the contributions to the responses from the left and right kidneys. Furthermore, the baroreceptors exerted a greater influence on basal renal function in the hypertensive rats.

The influence of age on the development of two-kidney, one-clip hypertension in the rat.

The role of age in the development of two-kidney, one-clip (2K1C) renal hypertension was evaluated. Blood pressure response to aortic constriction was more pronounced in young rats although the alterations of renal renin activity and body fluid volumes were greater in adult ones. Obtained results suggested that 2K1C renal hypertension is maintained by reciprocal interaction of renin-angiotensin system and body fluid volume alterations only in adult rats. In young rats other factors might be more important.

The influence of age on the development of two-kidney, one-clip hypertension in the rat.

The role of age in the development of two-kidney, one-clip (2K1C) renal hypertension was evaluated. Blood pressure response to aortic constriction was more pronounced in young rats although the alterations of renal renin activity and body fluid volumes were greater in adult ones. Obtained results suggested that 2K1C renal hypertension is maintained by reciprocal interaction of renin-angiotensin system and body fluid volume alterations only in adult rats. Alpha-1 adrenoceptor subtypes involved in mediating adrenergically induced antinatriuresis and antidiuresis in two kidney, one clip Goldblatt and deoxycorticosterone acetate-salt hypertensive rats.

This study characterized the alpha-1 adrenoceptor subtypes involved in adrenergically induced antinatriuresis and antidiuresis in pentobarbital-anesthetized deoxycorticosterone acetate (DOCA)-salt and two kidney, one clip (2K1C) Goldblatt hypertensive rats. In the DOCA-salt rats, phenylephrine infusion (100 microgram kg(-1) hr(-1) close renal arterially) caused increases in blood pressure of 5 to 10%, decreases in renal blood flow of 5 to 12%, whereas there were large reversible decreases in urine flow and absolute and fractional sodium excretions of 55 to 70%. The presence of chloroethylclonidine (10 microgram kg(-1) hr(-1) to block alpha-1B adrenoceptors) had no effect on the magnitude of the phenylephrine-induced excretory responses, but after 5-methylurapidil (10 microgram kg(-1) hr(-1) to block alpha-1A adrenoceptors) they were abolished. Infusion of phenylephrine in the 2K1C hypertensive rats caused small hemodynamic and renal blood flow responses with marked 55 to 70% reductions in urine flow and absolute and sodium excretion. The phenylephrine-induced antinatriuresis and antidiuresis, which was probably due to activation of alpha-1 adrenoceptors present on the renal tubular epithelial cells, and the fact that these tubular responses were blocked by 5-methylurapidil but not by chloroethylclonidine in both DOCA-salt and 2K1C Goldblatt hypertensive models, means that the alpha-1A adrenoceptors was the major functional subtype present.

Mechanisms of hypertension during the acute and intermediate phases of the one-clip, two-kidney model in the dog.

The acute and intermediate onset phases of one-clip, two-kidney hypertension were studied in six conscious dogs. Mean arterial pressure (MAP), cardiac output (CO), total peripheral resistance (TPR), serum renin concentration (SRC), plasma aldosterone (PA) and cumulative sodium and water balance were studied prior to unilateral renal artery constriction, at 2, 10, and 24 hours postconstriction, and daily thereafter for 5 days. At 2 hours postconstriction, MAP, CO, TPR, and SRC were elevated, with unchanged fluid balance. At 10 and 24 hours there was a further rise in CO accompanied by positive fluid-sodium balance, with a slight decrease in MAP, TRP, and SRC. During days 2 and 3, MAP, CO, TPR, and SRC remained elevated and stable in the presence of decreasing fluid-sodium balance to preconstriction levels. During days 4 and 5, MAP, SRC, and fluid-sodium balance remained unchanged, TPR demonstrated a secondary increase, whereas CO decreased to preconstriction values. Sequential changes in PA parallel those of SRC, but were significant only at 2 hours postconstriction. These changes suggest that the increased CO is not totally dependent on fluid-sodium balance, and that CO is not the sole determinant of elevated MAP. The temporal relationship between MAP and SRC throughout the study is consistent with renin-mediated hypertension. The secondary rise of TPR may be due to total body autoregulation and/or increased vascular reactivity to high levels of circulating angiotensin. A unitary renin-angiotensin mechanism, therefore, may be responsible for the induction and maintenance of hypertension in this experimental model.

[Effects of taurine and enalapril on blood pressure, platelet aggregation and the regression of left ventricular hypertrophy in two-kidney-one-clip renovascular hypertensive rats].

In two-kidney-one-clip (2k-1c) renovascular hypertensive rats, the blood pressure, left ventricular weight/body weight (LVW/BW) ratio and blood platelet aggregation were increased significantly. Enalapril (Ena) 6 mg . kg-1 . d-1 ig 9 wk and Taurine 30 mg . kg-1 . d-1 ig 9 wk can not only decrease the high blood pressure, LVW/BW ratio, but also the blood platelet aggregation induced by ADP or thrombin, though still different from that of the normal group. When the 2k-1c renovascular hypertensive rats were treated with both Ena and Tau, the blood pressure and blood platelet aggregation were decreased to the same as that of the normal group, and the LVW/BW ratio was also lowered markedly, though still higher than that of the normal group. These results show that both Ena and Tau can reverse the left ventricular hypertrophy, decrease the blood pressure and suppress the blood platelet aggregation in 2k-1c renovascular hypertensive rats. When treated with both drugs, the effects can be improved. It suggests that the two drugs can enhance the effects when used together, and that they may be two good agents for treatment of hypertension.

Videos and film clips as a medium for enhancing critical thinking.

Teaching as an art involves the use of varied methodology which is tried, true and creative. This article presents the use of video and film clips to develop/enhance critical thinking skills at the college level in two different disciplines, nursing and pre-service teacher education.

Suppression of sympathetic nervous system attenuates the development of two-kidney, one-clip Goldblatt hypertension.

PURPOSE: This study tried to assess the possible contribution of the sympathetic nervous system to the onset of two-kidney, one-clip (2K-1C) Goldblatt hypertension. MATERIALS AND METHODS: The effect of chlorisondamine administration with or without subsequent splanchnicotomy on the development of hypertension was examined in 2K-1C rats with special reference to norepinephrine synthesis. RESULTS: The 2K-1C rats were treated either with chlorisondamine or chlorisondamine plus subsequent splanchnicotomy, so that the development of hypertension was effectively arrested for 4 weeks. An apparently high rate of release of norepinephrine in 2K-1C rats was reduced by treatment with chlorisondamine plus splanchnicotomy. A similar trend was also seen in plasma norepinephrine concentration and norepinephrine clearance, to a lesser extent. There were significant positive relationships between percent change of systolic blood pressure and apparent rate of release of norepinephrine, plasma norepinephrine concentration and norepinephrine clearance in 2K-1C rats and 2K-1C + chlorisondamine + splanchnicotomy rats. There were no significant relationships in these parameters in sham-treated rats. CONCLUSION: Increased sympathetic innervation appears to participate in the development of 2K-1C Goldblatt hypertension.

Influence of captopril on the serotonin-induced vasopressor effect in tail artery isolated from two-kidney, one-clip renal hypertensive rats.

The influence of captopril and ritanserin (5-HT2 receptor antagonist) on the serotonin-induced contraction of isolated tail artery of two-kidney, one clip hypertensive (2K, 1C-RHR) and normotensive rats was studied. Captopril, administrated in a single dose of 100 mg/kg, po or in a dose of 30 mg/kg/day, po for one week, diminished the systolic blood pressure in both groups of rats. Ritanserin (0.01 mg/kg/day, sc for one week) administrated alone or in combination with captopril (30 mg/kg/day) evoked such effect only in 2K, 1C-RHR. The reactivity of rat tail artery isolated from 2K, 1C-RHR to serotonin was significantly higher than that obtained from normotensive ones. Captopril given in a single dose or chronically did not change the serotonin-induced contraction of rat tail artery in normotensive rats. The attenuation of serotonin contractile effect was registered in hypertensive rats pre-treated with captopril. Administration ritanserin with or without captopril also inhibited the response to serotonin in both groups. The stronger effect was observed after administration of ritanserin with captopril in 2K, 1C-RHR than in control group. Our results indicate, that in 2K, 1C-RHR the reactivity of rat tail arteries to serotonin is enhanced and this effect can be reduced by chronic captopril administration.

Renal function in one-kidney, one clip sodium-restricted rats: influence of enalapril and losartan.

BACKGROUND: Some observations have suggested that the harmful renal effects of angiotensin converting enzyme inhibitors might be partly mediated by non-angiotensin mechanisms under conditions in which a high intraglomerular pressure is required to maintain the glomerular filtration rate. MATERIALS AND METHODS: The effects of enalapril and losartan (10 and 30 mg/kg per 24 h for 6 days, respectively) on arterial pressure and renal function were assessed in one-kidney, one clip hypertensive rats subjected to dietary sodium depletion before treatment. RESULTS: Arterial pressure and renal vascular resistance were reduced to a similar extent in rats treated with enalapril and with losartan. However, the glomerular filtration rate (867 +/- 40 mu l/min per g kidney weight in untreated rats) decreased to a greater extent in enalapril than in losartan-treated rats (284 +/- 29 and 444 +/- 32 mu l/min per g kidney weight, respectively). In addition, the filtration fraction was reduced in treated compared with untreated rats; however, it was slightly but significantly lower in enalapril- than in losartan-treated rats. CONCLUSIONS: Despite similar final arterial pressure and renal vasodilation, angiotensin converting enzyme inhibitors may exert an effect on intrarenal forces contributing to the regulation of glomerular filtration rate, independently of blockade of the renin-angiotensin system.

[Splenic injury sutured in situ with a single-clip stapler].

A 36 year-old woman with first degree splenic rupture from blunt abdominal trauma underwent laparotomy due to haemodynamic instability. The spleen was enlarged and difficult to mobilize with the rupture on the upper-lateral face of the spleen. Classic splenorrhaphy could not be performed. A single-clip-stapler made for laparoscopic herniotomy was used successfully to close the lesion.

Marking and identifying colon lesions. Tattoos, clips, and radiology in imaging the colon.

Precise knowledge of a lesion's location in the colon is infrequently required. Occasionally, however, this information can be of critical clinical importance. A variety of endoscopic and radiologic techniques have been described to localize an area or site within the colon. Tissue staining or tattooing is the most reliable endoscopic method of colon lesion localization, and India ink provides a long-lasting and probably permanent tattoo of the site. Endoscopic clips are less reliable and remain cumbersome to use. Electronic imaging is an intriguing approach but remains experimental. Sites and lesions also can be identified by barium radiography or fluoroscopy, but these techniques involve added expenses and are not as reliable as tattooing with India ink.
