Conservative aesthetic enhancement of the anterior dentition using a predictable direct resin protocol.

Aesthetic enhancement of the maxillary dentition can be accomplished using a variety of direct and indirect methods. Composite resin procedures enable the clinician to follow a predictable, conservative, and reliable chairside protocol for improving patients' smiles. Combined with advances in adhesive materials themselves, these procedures can be used successfully in the daily practice of restorative aesthetic dentistry. This article will demonstrate the protocol used to enhance the appearance of a patient who presented with concern regarding short crown length.

Lip repositioning for reduction of excessive gingival display: a clinical report.

Excessive gingival display can be managed by a variety of treatment modalities, depending on the specific diagnosis. This case report demonstrates the successful management of excessive gingival display with a lip-repositioning procedure. This is accomplished by removing a strip of mucosa from the maxillary buccal vestibule, then suturing the lip mucosa to the mucogingival line. This results in a narrower vestibule and restricted muscle pull, thereby reducing gingival display during smiling. This article reviews the basic technique for lip repositioning and discusses the indications and contraindications for this novel procedure in dentistry.

Esthetic management of the upper labial frenum: a novel frenectomy technique.

An abnormal upper labial frenum is capable of retracting the gingival margin, creating a diastema, limiting lip movement, and in cases of a high smile line, affecting esthetics also. When an abnormal frenum is present, frenectomy is advised. This technique has been modified in many ways. But in most of the techniques the zone of attached gingiva and esthetics are not considered. This case report highlights a new technique of frenectomy that results in good esthetics, excellent color match, gain in attached gingiva, and healing by primary intention at the site of the thick, extensive abnormal frenum.

Geometric and mathematical proportions and their relations to maxillary anterior teeth.

AIM: One of the critical aspects of esthetic dentistry is creating geometric or mathematical proportions to relate the successive widths of the anterior teeth. The golden proportion, the recurring esthetic dental (RED) proportion, and the golden percentage are theories introduced in this field. The aim of this study was to investigate the existence of the golden proportion, RED proportion, and the golden percentage between the widths of the maxillary anterior teeth in individuals with natural dentition. METHODS AND MATERIALS: Standardized frontal images of 376 dental student smiles were captured. The images were transferred to a personal computer, the widths of the maxillary anterior teeth were measured, and calculations were made according to each of the above mentioned theories. The data were statistically analyzed using paired student T-test (level of significance P<0.05). RESULTS: The golden proportion was found to be accurate between the width of the right central and lateral incisors in 31.3% of men and 27.1% of women. The values of the RED proportion were not constant, and the farther the one moves distally from the midline the higher the values. Furthermore, the results revealed the golden percentage was rather constant in terms of relative tooth width. The width of the central incisor represents 23%, the lateral incisor 15%, and the canine 12% of the width of the six maxillary anterior teeth as viewed from the front. CONCLUSIONS: Both the golden proportion and the RED proportion are unsuitable methods to relate the successive widths of the maxillary anterior teeth. However, the golden percentage theory seems to be applicable to relate the successive widths of the maxillary anterior teeth if percentages are adjusted taking into consideration the ethnicity of the population.

Use of the 810 nm diode laser: soft tissue management and orthodontic applications of innovative technology.

Innovative technologies such as the diode laser have provided considerable benefit to dental patients and professionals. Facilitating efficient cutting of tissue and subsequent coagulation, the soft tissue laser enhances tissue healing and can reduce postsurgical complications. Due to the conservative nature of treatment accomplished with the laser this technology is very useful in orthodontic procedures. The diode laser is utilized in both esthetic enhancement of the smile, and treatment management of soft tissue issues that impede efficient orthodontic treatment. Its clinical application will be illustrated in a series of orthodontic cases.

[Behavioral patterns of autistic children during infancy].

OBJECTIVE: The present study investigated the behavioral patterns of autistic children during infancy to provide clues for early identification of childhood autism. METHODS: The abnormal behaviors of 30 children with autism and 26 children with other developmental disorders in infancy were investigated. RESULTS: The children with autism presented a series of abnormal behaviors, including no social smile, no eye contact and no respond to own name, and joint attention deficiency, which were distinguished from the children with other developmental disorders. The imitation and attachment behaviors were significantly different between the two groups. Repetitive motor actions and interest peculiarity were only seen in children with autism. CONCLUSIONS: The children with autism may present a series of abnormal behaviors as early as in infancy. The abnormal behaviors facilitate early diagnosis of autism.

[Treatment of ureterocele in the adult].

Review of 18 cases of ureterocele in the adult. The first clinical symptoms are unspecific, with a long-term evolution, and were found while carrying out routine urological examinations. The association to secondary lithiasis in over 60% cases and the presence of complete duplicity in 44% cases is highlighted. The choice treatment is by endoscopy in the simple cases without hydronephrosis or associated reflux. The technique of transversal incision in 'smiling mouth' at the base of the ureterocele produces good results.

Adult orthodontics: periodontic and cosmetic enhancements.

Approximately 30 years ago it became apparent that the periodontal, occlusal, and restorative needs inherent in the therapeutic management of the stomatognathic system required repositioning the teeth or the entire dentition. Precedence in adult orthodontics concentrated on correcting diastemas and malpositioned teeth, leveling infrabony defects, reducing periodontal depths, leveling occlusal planes, and creating harmonious gingival-lip line relationships. The desired result can be obtained by any of these procedures, but the perception and acceptance of an improved appearance or a beautiful smile will determine the ultimate success for the patient.

[Association of congenital upper and lower double lip].

The frequency of the congenital double lip is scarcely mentioned in the literature. There is no previous report on the bilabial congenital double lip. This association consists of a mucosae fold at the free edge of the lip, which is projected at smiling and opening of the mouth. Histologically, the mucous salivary glands are either hypertrophic or normal with little connective tissue and covered by a cornified epithelium. There is no muscle tissue found. We present the report of two cases of bilabial congenital double lip treated surgically by the elliptical resection of the mucosae folds.

The extended subperiosteal face lift: a definitive soft-tissue remodeling for facial rejuvenation.

The subperiosteal face lift described by Psillakis has been criticized for not showing a more dramatic improvement over conventional brow/face lift procedures. His approach also has a significantly high incidence of nerve injury. This study reports our anatomic findings and surgical modifications, which have permitted a significant improvement in the safety of execution and clinical results using the subperiosteal face lift concept. Pertinent points of applied local anatomy and dissection techniques are as follows: First, we use extensive interconnected subperiosteal dissection that includes the entire zygomatic arch. This allows better repositioning of the deep soft tissues of the entire upper face, most of the midface, and indirectly, key structures of the lower face. Second, the upward pull of the muscles of the cheek and mouth will produce an elevation of the corner of the mouth, affecting positively the smiling mechanism, the oral frowning, and the jowls. Third, the dissection deep to both layers of the temporal fascia decreases the risk of injury to the frontalis nerve. Fourth, the temporal fascia is used as a lifter and anchoring element of the entire cheek-perioral soft tissues as opposed to the periorbital fibrofatty tissues. This will decrease the risk of injuring the frontal and zygomatic branches of the facial nerve. These modifications have been used in 28 patients. Our rate of patient satisfaction has been high, and no complications with regard to nerve injury have been observed. This compares favorably with our initial 60 patients, in whom the Psillakis or Tessier approach was used. In these patients, there was an 11 and 20 percent rate of nerve injury, respectively.

[Prosthetic rehabilitation of a case of total amelogenesis imperfecta].

A young adolescent girl suffering from Amelogenesis imperfecta was examined. The treatment plan: teeth kept vital, preparation of provisional resin crowns and their replacement by definitive single crowns. Thus re-establishing aesthetics and function, and furthermore the possibility of giving this young patient the pleasure of finally being able to smile, no longer hiding her teeth.

[Nonverbal communication in psychotherapy].

In psychotherapy, use is made of both verbal and nonverbal variants of communications between the psychotherapist + and the patient. The first ones have been studied in a sufficiently detailed manner whereas nonverbal interactions have been elaborated insufficiently. The process of psychotherapy (proceeding from nonverbal prerequisites) is estimated by the author as a combination of nonverbal actions of the patient and the doctor. At the same time special attention is paid to the feeling of distance, postures, gestures, smiles, empathy, definite movements, body incline, free or constrained postures, and so forth. It is believed that nonverbal communication can successfully be used not only in psychotherapy but also in psychodiagnosis and psychotherapy.

Aesthetics and osseointegration for the completely edentulous.

The completely edentulous patient who has been fitted with osseointegrated implants poses new challenges for the restoratie dentist. Besides having to make up for loss of teeth and alveolar bone, the prostheses have to provide function and aesthetics, and be designed to facilitate oral hygiene procedures. The metal superstructure should be strong, have a passive fit, be resistant to corrosion and be easy to clean. Three designs are proposed: the sanitary bridge, the aesthetic bridge and the removable overdenture. The patient's expectations regarding function and aesthetics should be assessed initially by the restorative dentist. General principles of aesthetics in complete dentures are still to be followed: proper upper anterior tooth selection (shape and shade), and correct tooth positioning in relationship to lip line, smile line and profile. Lower teeth should be level with the lower lip and the retromolar pad. Success is based on the patient's capacity to adapt and the dentist's sound application of basic principles.

Developmental changes in synapses of the retino-recipient layers of the opossum superior colliculus.

Changes in synaptic structure were examined in junctions of the retino-recipient layers of the opossum superior colliculus (SC) at critical developmental stages (30, 40 and 61 day-old pouch young and adult controls). Criteria of classification were the presence and direction of curvature (smile, flat, frown and irregular) and the degree of aggregation of paramembranous components (immature, intermediate and mature). The results favor the aggregation of dense components vis-a-vis curvature in the assessment of maturational changes.

Cesarean section and delayed contact: effect on baby's behaviour.

Fifty two babies were assessed by the Brazelton Neonatal Behaviour Assessment Scale (BNBAS) first at 24-48 hours after birth and next at 28 days of age. Of these 26 belonged to the normal vaginal delivery (NVD) group where the babies were roomed in with their mothers immediately after birth. The other 26 were delivered by Cesarean Section (CS) and had a mean separation of 2.8 +/- 1.0 day from their mothers. Babies in the NVD group performed better for the interactive processes at both the initial and follow-up assessment. They were more active initially and on follow up. Although no differences were obtained at the initial assessment for organisational processes (physiological response to stress and state control) the follow up revealed that the NVD babies smiled more, cried less and were less irritable.

Hand, arm, and facial actions of young infants to a social and nonsocial stimulus.

Do 9-15-week-old infants produce differentially organized hand and arm actions in relation to affective states when presented with social and nonsocial stimuli? This question was examined by observing 8 infants longitudinally. They were observed when facing their active and passive mother and an active and passive doll during 4 visits at biweekly intervals. Videotapes were coded in real time using the following measures: Vocalization, Gaze, and Gaze Avert; for face, Smiling, Distressed, and Neutral; for hands, Pointing, Open, Curled, and Closed; and for arms, Extended and At Side. Co-occurrence and lag sequential analyses showed that hand actions were organized with other infant actions to form unique behavioral linkages in each of the 4 conditions. The implications of these findings for the development of nonverbal communication are discussed.

Polymerase chain reaction amplification products separated on rehydratable polyacrylamide gels and stained with silver.

Separation of polymerase chain reaction (PCR) amplification of specific fragment length polymorphisms was carried out on rehydratable polyacrylamide gels on a horizontal flat slab system. A discontinuous sulfate-borate buffer system was employed on 5-8% T gels crosslinked with 3.5% C. Samples were diluted in leading sulfate ion buffer at 1/10 the ionic strength of the separating gel buffer and placed directly onto the surface of the rehydrated gels in 0.5-10 microliters volumes. The trailing ion and counterion were contained in a gel plug and placed directly onto the anodal and cathodal ends of the gel, and the electrodes placed directly onto the surface of the gel plugs. Filter paper wicks, soaked in diluted leading ion buffer, were placed along each side to lower the ionic strength of the edges, thereby increasing mobility at the edge and thus preventing smile effects. The gel-gel contact of the plug and separating gel prevent the production of a junction potential which occurs between dissimilar materials such as a paper wick and the gel. Ten- to 20-cm separations were carried out from 2-5 h, respectively, and resolution in the 20 cm system was 1.6-4 bp (base pairs) between 100 and 500 bp, 4-7 bp between 500 and 1000 bp, 12-20 bp between 1000 and 2000 bp and about 50 bp between 2000 and 3000 bp. Between 3000 and 4000 bp, resolution fell off to +/- 100 bp. Sensitivity, using a silver stain, indicated that one could readily distinguish less than 10 pg of DNA per mm width on the gels.(ABSTRACT TRUNCATED AT 250 WORDS)
