Uterine and cardiovascular effects of beta2-selective sympathomimetic drugs administered as an intravenous infusion.

The uterine and cardiovascular effects of fenoterol, hexoprenaline, ritodrine and salbutamol, given as intravenous infusions in equivalent and therapeutic dosages in oxytocin-induced labour, were studied and compared. All the drugs were effective in reducing the uterine activity to less than 30% of the original. The effect on the cardiovascular system was to produce a moderate tachycardia and a widening of the pulse pressure by raising the systolic and decreasing the diastolic blood pressures.

[Comparative study of a vagolytic and a beta 2 sympathomimetic drug on the respiratory function of the asthmatic patient].

The authors compared the effects of a synthetic vagolytic drug, the SCH 1000, and a beta2 sympathicomimetic one (Fenoterol) in two groups of control children (n = 11) and two groups of asthmatic children (n = 23). They studied the following parameters, flow volumes curves, steady state lung diffusing capacity and the blood gases. No difference of efficiency was found between the two types of bronchodilators although the graphic modification of flow volumes curves was different after Fenoterol and after SCH 1000. The modifications consisted in a great improvement in instantaneous flow, a decrease in diffusion capacity and an increase of the Pa O2 significant in the group of asthmatic children after Fenoterol.

[Comparison of the cost of treatment of premature labor with atosiban or beta-sympathomimetics from the perspective of the health care payer--a pharmacoeconomic model].

OBJECTIVE: To evaluate the cost of treating premature delivery with atosiban or beta-sympatomimetic drugs (fenoterol and hexoprenalin) from the perspective of health care payer--the medical insurance company. DESIGN: A pharmaco-economic model based on the results of randomized, controlled clinical study. SETTING: Hospital Pharmacy at Vitkovice Hospital of Blessed Mary Antonia, Ostrava. METHODS: The study is based on the application of clinical decision-making analysis, which includes results of a randomized controlled clinical study as well as data on the cost of clinical interventions and cost of drug therapy. The pharmaco-economic model was created from the perspective of the payer of health care--the insurance company. This model presumes the administration of atosiban or beta-sympatomimetic drugs (fenoterol and hexoprenalin) for the period of 18 and 48 h and the therapy of possible untoward effects for the next 72 h after the administration of the drugs. The analysis of sensitivity of pharmacokinetic model also employs so called low and high estimate of supplementary cost for the treatment of untoward effects. RESULTS: After the administration of the drugs for the period of 18 h the total cost of the payer of medical care was in the range of 21,914.5-21,974.4 CKr in atosiban, 19,878.7-22,661.4 CKr in fenoterol and 19,942.9-21,974.4 CKr in hexoprenalin. In the administration of the drugs for 48 h, the overall cost of the payer of medical care was in the range of 43,082.5-43,142.4 CKr in atosiban, 19,960.3-23,150.7 CKr in fenoterol and 20,131.3-23,574.0 in hexoprenalin. CONCLUSIONS: This study compared overall cost associated with hospitalization of a premature delivery from the perspective of the medical care payer, i.e. the health insurance company. The authors applied a pharmaco-economic model evaluating hospitalization for the period of 48 h and subsequent therapy of possible untoward effects for the period of up to 72 h. In case of a shorter administration of atosiban (up to 18 h) the overall cost of hospitalization for premature delivery for the period of 48 h from the point of view of medical insurance company is basically comparable with the administration of beta-sympatomimetic drugs. If atosiban is administered for more than 18 h, the overall cost of hospitalization is higher than with beta-sympatomimetic drugs, and the cost increases in relation to the duration of atosiban administration.

[The separation of some racemic beta-blockers and alpha-sympathomimetics with HPLC following derivatization].

Five beta-adrenergic blocking agents and two alpha-sympathicomimetics were separated after derivatization with (R)-(-)-1-(Naphth-1-yl)-ethylisocyanate (NEIC). For two of these analytes the separation was successful also after reaction with (R)-(+)-1-phenylethylisocyanate (PEIC) by using HPLC with RP-18 phases and a water-methanolic eluent by UV-detection on 254 nm. After reaction with (1S)-(+)-campher-10-sulfonylchloride (CSC) an acceptable separation of norphenylephedrine also was possible. Analytes which can be derivated with NEIC and PEIC give no reaction with CSC (and vice versa).

[A patient with pulmonary edema following use of beta-sympathomimetics (tocolytic agents)].

During her pregnancy (29 weeks) a 29-year-old woman complained of pain in the right upper quadrant of her abdomen. Gallbladder stones were demonstrated by ultrasonography. Because of recurrent pain attacks cholecystectomy was performed. Continuous intravenous tocolytic therapy, fenoterol bromide (Parusisten), was given during and following surgery. Two days after this first laparotomy she underwent a second laparotomy because of unexplained abdominal pain and blood loss. A subhepatic haematoma was found without a specific origin for the blood loss. Shortly after this second operation she developed clinical signs and abnormalities on the chest X-ray compatible with severe pulmonary oedema. She needed artificial ventilation for 15 days. There appears to be a pathophysiological relationship between the development of pulmonary oedema and tocolytic therapy. Probably, pulmonary oedema may develop during tocolytic therapy as a result of several factors such as increased hydrostatic pressure, decreased oncotic pressure and perhaps capillary leakage.

[Reproducibility of the bronchospasmolytic effect of anticholinergic drugs and beta-sympathomimetics].

The bronchodilator effect of an anticholinergic and beta-adrenergic drug in the same patient was tested at 3 different times to study the reproducibility. There is a good average reproducibility: the curves concerning the bronchodilation are nearly identical or parallel. On the other hand there is a greater difference in the individual curves at different times: the patients with the most severe airway obstruction show the greatest difference in the bronchodilator effect at different times.

[The bronchospasmolytic test with anticholinergic drugs and sympathomimetics: differences between body plethysmography and spirometry measuring procedures].

The different bronchodilator effects after inhalation of anticholinergic and beta-adrenergic drugs due to the different methods of measurement (i.a. body plethysmography vs. spirometry) were studied in 43 patients with COLD and in 20 healthy subjects. The greatest changes occur when the body plethysmographic value Rt was chosen. The spirometric values FEV1 and PEF show the greatest bronchodilator effect. The patients with COLD have the best bronchodilation after inhalation of the beta-adrenergic drug, whereas the healthy subjects show the greatest effect after anticholinergic drugs.

[Inhalation therapy with selective beta 2-sympathomimetic drugs. Effect of pirbuterol administered via the Autohaler and salbutamol administered via the Diskhaler in obstructive respiratory tract disease].

The therapeutic success of salbutamol inhalation with the powder inhalator (Diskhaler) and of pirbuterol with an inhalation-triggered metered aerosol (Autohaler) was determined by comparing the degrees of bronchodilation in 13 patients suffering from chronic obstructive airways disease. Rt and IGV were determined by whole-body plethysmography and FEV1, peak flow and MEF50 by spirometry, and pulse rates were also measured. On average, Rt values decreased by 40 per cent; the other lung function parameters responded correspondingly. Duration of action extended to 4 hours after dosage. Pulse rates and lung functions measured after administration of the products did not differ, either. In such studies as this, whole-body plethysmographic results are superior to those obtained by spirometry.

[Oral therapy with beta-2-sympathomimetic drugs: comparative study of the bronchospasmolytic effect of tulobuterol and fenoterol in patients with reversible airway obstruction].

In a multicentre, single-blind study, the bronchospasmolytic effect of a single oral dose of tulobuterol 2.0 mg and of fenoterol 2.5 mg was compared in 26 patients suffering from reversible airways obstruction. The bronchospasmolysis achieved with the oral application of tulobuterol and fenoterol is qualitatively satisfactory and reliable, the effect of tulobuterol persisting for a longer period than that of fenoterol.

[Retrograde ejaculation as a cause of aspermia following retroperitoneal lymph node excision and the effective use of alpha sympathomimetic drugs].

Retroperitoneal lymphadenectomy may produce in the majority of patients an irreversible loss of normal ejaculation due to lesion of presacral sympathetic nerve fibers. Libido and orgasm did remain unchanged. In 14.3% of the patients a retrograde ejaculations could evaluated. By application of alpha-sympathicomimetic drugs this could reversed to an antegrade ejaculation. However, fertility was altered strongly because of motility loss.
