Subcellular topological effect of particle monolayers on cell shapes and functions.

We studied topological effects of subcellular roughness displayed by a closely packed particle monolayer on adhesion and growth of endothelial cells. Poly(styrene-co-acrylamide) (SA) particles were prepared by soap-free emulsion copolymerization. Particle monolayers were prepared by Langmuir-Blodgett deposition using particles, which were 527 (SA053) and 1270 nm (SA127) in diameter. After 24-h incubation, cells tightly adhered on a tissue culture polystyrene dish and randomly spread. On the other hand, cells attached on particle monolayers were stretched into a narrow stalk-like shape. Lamellipodia spread from the leading edge of cells attached on SA053 monolayer to the top of the particles and gradually gathered to form clusters. This shows that cell-cell adhesion became stronger than cell-substrate interaction. Cells attached to SA127 monolayer extended to the reverse side of a particle monolayer and engulfed particles. They remained immobile without migration 24h after incubation. This shows that the inhibition of extensions on SA127 monolayer could inhibit cell migration and cell proliferation. Cell growth on the particle monolayers was suppressed compared with a flat TCPS dish. The number of cells on SA053 gradually increased, whereas that on SA127 decreased with time. When the cell seeding density was increased to 200,000 cells cm(-2), some adherent cells gradually became into contact with adjacent cells. F-actin condensations were formed at the frame of adherent cells and the thin filaments grew from the edges to connect each other with time. For the cell culture on SA053 monolayer, elongated cells showed a little alignment. Cells showed not arrangement of actin stress fibers but F-actin condensation at the contact regions with neighboring cells. Interestingly, the formed cell monolayer could be readily peeled from the particle monolayer. These results indicate that endothelial cells could recognize the surface roughness displayed by particle monolayers and the response was dependent on the pitch of particle monolayers.

Building and using a medical ontology for knowledge management and cooperative work in a health care network.

In the context of a health care network, we describe our method for reconstitution of a medical ontology via the translation of a medical database (DB) towards RDF(S) language. Then we show how we extended this ontology among others through natural language processing of a textual corpus. Then, we present the construction of a tool called "Virtual Staff", enabling a cooperative diagnosis by some of the health care network actors, by relying on this medical ontology and on the creation of SOAP and QOC graphs.

On the incidence of diarrhoea among young Indian children.

Diarrhoea, claiming over three million young lives in the world every year, is the second biggest killer of children in developing countries. Using data for over 13,000 children in rural India, under the age of 3 years, this paper examines the relative effects of the different factors--inter alia the quality of the water supply, mother's literacy, housing conditions, and the level of development of the villages in which the children lived--contributing to diarrhoea. The paper highlights the importance of two factors: that children born to undernourished mothers may be more susceptible to infection than children whose mothers are well nourished, and that good hygienic practices within the home, such as washing hands with soap before feeding a child, can reduce the incidence of diarrhoea. The paper also quantifies the relative strength of the factors that determine whether mothers do so. The results emphasize the importance of mothers being literate, of household affluence and of institutional support (through the availability of trained midwives and mother and child centres in villages) in promoting domestic hygiene.

Assessment of marine pollution in Izmir Bay: nutrient, heavy metal and total hydrocarbon concentrations.

Izmir Bay (western Turkey) is one of the great natural bays of the Mediterranean. Izmir is an important industrial and commercial centre and a cultural focal point. The main industries in the region include food processing, oil, soap and paint production, chemical industries, paper and pulp factories, textile industries and metal processing. The mean concentrations showed ranges of 0.01-0.19 and 0.01-10 microM for phosphate, 0.10-1.8 and 0.12-27 microM for nitrate+nitrite, and 0.30-5.8 and 0.43-39 microM for silicate in the outer and middle-inner bays, respectively. The TNO(x)/PO(4) ratio is significantly lower than the Redfield's ratio and nitrogen is the limiting element in the middle-inner bays. Diatoms and dinoflagellates were observed all year around in the bay and are normally nitrogen limited. Metal concentrations ranged between Hg: 0.05-1.3, Cd: 0.005-0.82, Pb: 14-113 and Cr: 29-316 microg g(-1) in the sediments. The results showed significant enrichments during sampling periods from Inner Bay. Outer and middle bays show low levels of heavy metal enrichments except estuary of Gediz River. The concentrations of Hg, Cd and Pb in the outer bay were generally similar to the background levels from the Mediterranean. The levels gradually decreased over the sampling period. Total hydrocarbons concentrations range from 427 to 7800 ng g(-1) of sediments. The highest total hydrocarbon levels were found in the inner bay due to the anthropogenic activities, mainly combustion processes of traffic and industrial activities. The concentrations of heavy metals found in fish varied for Hg: 4.5-520, Cd: 0.10-10 and Pb: 0.10-491 microg kg(-1) in Izmir Bay. There was no significant seasonal variation in metal concentrations. An increase in Hg concentration with increasing length was noted for Mullus barbatus. A person can consume more than 2, 133 and 20 meals per week of fish in human diet would represent the tolerable weekly intake of mercury, cadmium and lead, respectively, in Izmir Bay. Heavy metal levels were lower than the results in fish tissues reported from polluted areas of the Mediterranean Sea.

Residual behaviour of profenofos on some field-grown vegetables and its removal using various washing solutions and household processing.

Profenofos (Selecron 72% EC), was sprayed on field-grown pepper and eggplant at the recommended rate of 1.28 kg a,i/ha. Fruit samples were collected at 1 h to 14 days after application and analysed to determine the content and dissipation rate of profenofos. The effect of different washing solutions and some household processing on the removal of such residues from treated vegetables were also investigated. Profenofos residues were quantified by using gas chromatography. The results showed that the consumable safety time were found to be 10 days on sweet pepper and 14 days on hot pepper and eggplant fruits. The initial disappearance of profenofos appeared to follow first order kinetics with different rates of reaction of 0.38, 0.40 and 0.35 day(-1) for hot pepper, sweet pepper and eggplant, respectively. The corresponding half-lives (t1/2) were 1.84, 1.74 and 1.96 days. Also, the results indicated that tap water, potassium permenganate and acetic acid solution gave high percent removal of profenofos residues from hot and sweet pepper fruits, while no detectable residues was found in eggplant fruit after washing with soap and acetic acid solutions. In general, all tested washing solutions gave higher percent removal of profenofos residues from eggplant fruit than the two other pepper fruits. Blanching and frying of pepper and eggplant fruits resulted in great reduction to almost completely removed (approximately 100%) of the deposited profenofos. In addition, pickling process removed 92.58 and 95.61% from hot pepper fruit after one week and after two weeks, respectively.

Comparative assessment of the acute skin irritation potential of detergent formulations using a novel human 4-h patch test method.

Predictive skin irritation test methods, which do not require use of animals, are needed for the pre-market assessment of detergent formulations. The utility of a novel and ethical human acute skin irritation patch test method, originally developed for chemical skin irritation assessment, was evaluated. In this IRB-approved method, subjects were patched under occlusion for increasing periods of time up to 4h in duration. The total incidence of positive skin reactions for test products was compared to a positive control (20% aqueous sodium dodecyl sulfate [SDS]). Acutely irritating formulas were defined as those showing a significantly increased or equal incidence of positive responders compared with that of SDS. The time of exposure required for 50% of subjects to show a positive skin reaction (TR50 value) was calculated for each product and enabled test product comparisons within and between studies. Using this approach, 24 detergent formulations of various types were tested in seven individual studies. The skin irritation profiles were generally consistent within product types, which could be categorized as follows (by decreasing irritancy): mold/mildew removers (average TR50 = 0.37 h) > disinfectants/sanitizers (0.64 h) > fabric softener concentrate (1.09 h) = aluminum wash (1.20 h) > 20% SDS (1.81 h) > liquid laundry detergents (3.48 h) > liquid dish detergents (4.16 h) = liquid fabric softeners (4.56 h) = liquid hand soaps (4.58 h) = shampoos (5.40 h) = hard surface cleaners (6.34 h) > powder automatic dish detergents (>16 h) = powder laundry detergents (>16 h). In addition to formulation effects, some seasonal effects were noted; particularly greater winter-time reactivity to 20% SDS and the hard surface cleaner and liquid laundry formulations. These results demonstrate the utility of this patch test method for the comparative skin irritation assessment of these different product types.

Toxicology and pharmacology of sodium ricinoleate.

Ricinoleic acid constitutes approximately 90% of the fatty acid content of castor oil. Castor oil is known for its purgative effects and can be used to induce labor. Both castor oil and ricinoleic acid are approved for use in food. The mechanistic basis for purgative actions likely includes the membrane-disruptive effects of detergent-like molecules, such as sodium ricinoleate (a 'soap'). These effects have been shown to be dose-related and to exhibit a threshold below which no laxative response was evident, in both animals and in humans. Castor oil was not toxic in subchronic feeding studies in rodents at doses ranging up to 10-20% of the diet. Sodium ricinoleate, as a surfactant, demonstrates predictable skin and mucus membrane irritant effects, and may induce a Type IV dermal sensitization response in those previously sensitized to it. However, food-grade castor oil and sodium ricinoleate are prepared in such a manner as to be free of the castor bean constituents that have been proven to be the source of reported Type I immediate hypersensitivity responses. Feeding studies with castor oil in rodents provide a basis for a no observable adverse effect level (NOAEL) estimate of 7,500 mg/kg/day and 5,000 mg/kg/day in mice and rats, respectively (). Applying an uncertainty factor of 100 to the lesser of these NOAELs, one can thus estimate an acceptable daily intake (ADI) in man to be 50 mg/kg, or 3,000 mg of castor oil per day in an average 60 kg person. As ricinoleic acid constitutes approximately 90% of castor oil, applying this calculation to the 3,000 mg/day estimated ADI in humans for castor oil (given the rapid hydrolysis of castor oil glyceride in the gastrointestinal tract), the acceptable daily intake of ricinoleic acid may be as high as 2,400 mg/person.

Comparative studies on the ocular and dermal irritation potential of surfactants.

Comparative studies on the irritation potential of 18 surfactants were performed using the same stock solution of surfactant for each study. The ocular irritation potential of surfactants was studied using the red blood cell test (RBC), the hen's egg test-chorioallantoic membrane (HET-CAM) and the Skinethic ocular tissue model. The skin irritation potential was assessed based on data obtained from human studies using a 24h epicutaneous patch test (ECT) and a soap chamber test (SCT). The same pH and active substance (AS) content for all surfactants tested was used depending on the test conducted. In general, clusters of substances with varying irritation potential were identified similarly by most tests. These results show that when using standardized test conditions in which pH and % AS are the same for each surfactant tested, there is a good correlation between the in vitro ocular irritation assays themselves as well as between the dermal and ocular irritation assays. In particular the RBC test seems to be not only highly predictive for ocular irritation (H(50)/DI) but also for dermal irritation and changes in barrier function (DI) induced by surfactants.

In vivo spermatotoxic effect of chromium as reflected in the epididymal epithelial principal cells, basal cells, and intraepithelial macrophages of a nonhuman primate (Macaca radiata Geoffroy).

OBJECTIVE: To understand, through a simulation experiment in a nonhuman primate model, the potential in vivo spermatotoxic toxic effect of hexavalent chromium (CrVI) in men who are occupationally or environmentally exposed to it. DESIGN: Controlled laboratory study. SETTING: Research laboratory in a department of endocrinology in a university in India. ANIMAL(S): Male bonnet monkey, Macaca radiata Geoffroy. INTERVENTION(S): Monkeys were exposed ad libitum to 100, 200, and 400 ppm CrVI, dissolved in drinking water, for a chronic period of 180 days. MAIN OUTCOME MEASURE(S): Examination of epididymis with a transmission electron microscope and assessment of the effect of CrVI in terms of accumulation of sperm-derived lipofuscin (LF) material in the principal cells, basal cells, and intraepithelial macrophages of the epithelium. RESULT(S): The abundance of basal cells and intraepithelial macrophages and the content of LF material in these cell types increased. The principal cells phagocytosed from the lumen the dead sperm resulting from CrVI exposure and processed them partially into LF material, which was acquired by the basal cells and intraepithelial macrophages and processed further. The LF material-laden basal cells and intraepithelial macrophages appeared to leave the epithelium, accompanied by recruitment of fresh basal cells and intraepithelial macrophages. CONCLUSION(S): Occupational or environmental exposure to CrVI, as would occur in the tannery, soap, and other industries in developing and underdeveloped countries, can be toxic in vivo to spermatozoa.

Evaluation of the One-Step ELISA kit for the detection of buprenorphine in urine, blood, and hair specimens.

A solid-phase enzyme immunoassay involving microtiter plates was recently proposed by International Diagnostic Systems corporation (IDS) to screen for buprenorphine in human serum. The performance of the kit led us to investigate its applicability in other biological matrices such as urine or blood, and also hair specimens. Low concentrations of buprenorphine were detected with the ELISA test and confirmed by HPLC/MS (buprenorphine concentrations measured by HPLC/MS: 0.3 ng/mL in urine, 0.2 ng/mL in blood, and 40 pg/mg in hair). The intra-assay precision values were 8.7% at 1 ng/mL of urine (n = 8), 11.5% at 2 ng/mL in serum (n = 8), and 11.5% at 250 pg/mg of hair (n = 8), respectively. The immunoassay had no cross-reactivity with dihydrocodeine, ethylmorphine, 6-monoacetylmorphine, pholcodine, propoxyphene, dextromoramide, dextrometorphan at 1 and 10 mg/L, or codeine, morphine, methadone, and its metabolite EDDP. A 1% cross-reactivity was measured for a norbuprenorphine concentration of 50 ng/mL. Finally, the immunoassay was validated by comparing authentic specimens results with those of a validated HPLC/MS method. From the 136 urine samples tested, 93 were positive (68.4%) after the ELISA screening test (cutoff: 0.5 ng/mL) and confirmed by HPLC/MS (buprenorphine concentrations: 0.3-2036 ng/mL). From the 108 blood or serum samples screened, 27 were positive (25%) after the ELISA test with a cutoff value of 0.5 ng/mL (buprenorphine concentrations: 0.2-13.3 ng/mL). Eighteen hair specimens were positive (72%) after the screening (cutoff: 10 pg/mg) and confirmed by LC/MS (buprenorphine concentrations: 40-360 pg/mg). The ELISA method produced false positive results in less than 21% of the cases, but no false negative results were observed with the immunological test. Four potential adulterants (hypochloride 50 mL/L, sodium nitrite 50 g/L, liquid soap 50 mL/L, and sodium chloride 50 g/L) that were added to 10 positive urine specimens (buprenorphine concentrations in the range 5.3-15.6 ng/mL), did not cause a false negative response by the immunoassay.

Flexible information storage in MUDR(II) EHR.

An important research task of the EuroMISE Centre is the applied research in the field of electronic health record (EHR) design including electronic medical guidelines and intelligent systems for data mining and decision support. The research in this field was inspired by several European projects. We have proposed a mathematical meta-description of a flexible information storage model based on the experience gathered in cooperation in those projects. In this model, we use two basic structures called a knowledge base and data files. We describe those two structures using the graph theory concepts. Furthermore, we use logical formulas to express conditions that should be valid. Additionally, we present a description of a global system architecture of a 3-tier EHR application with interfaces based on the latest technologies; predominately on Web Services, SOAP, XML, HTTP, CORBA, etc. According to our experience and test results gained from the MUDR EHR usage, we describe an open universal solution, which can be applied as the EHR kernel of hospital information systems. To realize this approach in a daily practice for health professionals we have started a co-operative project with clinical information systems developers. Within that project we are developing a new system for continual shared health care.

The effectiveness of a nurse-initiated intervention to reduce catheter-associated bloodstream infections in an urban acute hospital: an intervention study with before and after comparison.

BACKGROUND: Catheter care is considered to be important for prevention of catheter-associated bloodstream infections (CABSIs) although epidemiological evidence is sparse. OBJECTIVES: To identify problems associated with catheter care and evaluate the effectiveness of nurse-initiated interventions to reduce CABSIs. DESIGN: An intervention study with before and after comparison. SETTINGS: CABSI surveillance was conducted in a 560-bed acute hospital located in a major urban area in Japan. PARTICIPANTS: Patients were enrolled in this study from April 2000 to December 2002 based on the following criteria: (1) adult inpatients; and (2) those in whom central venous lines or Swan-Ganz catheters were inserted for 2 days or longer. METHODS: In the first year, risk factors for CABSI and problems associated with catheter care were identified by inspection of the infection control nurse (ICN) or four trained link nurses, and the laboratory results. In the subsequent 2 years, the following interventions based on the surveillance results were implemented: (1) enhanced skin preparation by scrubbing with regular bathing soap and tap water; (2) a new method for stabilisation of the catheter inserted into the internal jugular vein, where additional dressing was placed over the sterilised dressing; (3) educating the staff on maximal sterile precautions by teaching staff members at their section meetings and displaying posters; (4) use of a check list and observation of catheter insertion by link nurses to monitor compliance; and (5) selection of a disinfectant that requires shorter contact time and has longer residual effect. RESULTS: After these interventions were implemented, the overall bloodstream infection (BSI) rate declined from 4.0/1000 device-days to 1.1/1000 device-days (p<0.005). CONCLUSIONS: We identified four problems-those related to skin preparation, dressing, sterile precautions and disinfectant. We implemented a series of interventions to reduce CABSIs; the overall CABSI rate decreased significantly.

The choice of a compressor for the aerosolisation of tobramycin (TOBI) with the PARI LC PLUS reusable nebuliser.

The performance of five different compressors (CR60), Porta-Neb, Pulmo-Aide, TurboBoy and Freeway Freedom) was studied in combination with the widely recommended PARI LC PLUS nebuliser for the aerosolisation of a marketed tobramycin solution (TOBI). The droplet size distribution of the generated aerosol was measured with laser diffraction technique at stationary inspiratory flow rates through the nebuliser cup of 20, 30 and 40l N/min. The different compressors showed a distinct difference in droplet size distribution of the aerosol and nebulisation time till dry running. The finest droplets with a volume (equals mass) median diameter (mmd) of 1.84 microm (which was the same at all flow rates), as well as the narrowest size distribution were obtained with a CR60. The Freeway Freedom generated the largest droplets: mmd ranged between 2.63 and 3.72 microm depending on the inspiratory flow rate. The aerosol produced with this compressor also had the widest size distribution. The differences between the compressors could be explained with differences in the jet flow. A higher jet flow resulted in finer droplets, less dependence on the inspiratory flow rate and a shorter time till dry running. Thus, to obtain the required fineness of the aerosol for peripheral airway deposition of the tobramycin, independent of the inspiratory flow rate, the use of the CR60 compressor is preferred over the use of Porta-Neb, Pulmo-Aide, TurboBoy and Freeway Freedom (in order of decreasing preference). Finally, it was found that careful cleaning with warm water and liquid soap of the nebuliser cup is essential to obtain adequate performance of the LC PLUS.

New perspectives on epidermal barrier dysfunction in atopic dermatitis: gene-environment interactions.

Atopic dermatitis (AD) is a multifactorial, chronic inflammatory skin disorder in which genetic mutations and cutaneous hyperreactivity to environmental stimuli play a causative role. Genetic mutations alone might not be enough to cause clinical manifestations of AD, and this review will propose a new perspective on the importance of epidermal barrier dysfunction in genetically predisposed individuals, predisposing them to the harmful effects of environmental agents. The skin barrier is known to be damaged in patients with AD, both in acute eczematous lesions and also in clinically unaffected skin. Skin barrier function can be impaired first by a genetic predisposition to produce increased levels of stratum corneum chymotryptic enzyme. This protease enzyme causes premature breakdown of corneodesmosomes, leading to impairment of the epidermal barrier. The addition of environmental interactions, such as washing with soap and detergents, or long-term application of topical corticosteroids can further increase production of stratum corneum chymotryptic enzyme and impair epidermal barrier function. The epidermal barrier can also be damaged by exogenous proteases from house dust mites and Staphylococcus aureus. One or more of these factors in combination might lead to a defective barrier, thereby increasing the risk of allergen penetration and succeeding inflammatory reaction, thus contributing to exacerbations of this disease.

A camera's view of consumer food-handling behaviors.

OBJECTIVE: To compare consumer food-handling behaviors with the Fight BAC! consumer food-safety recommendations. DESIGN: Subjects were videotaped in their home while preparing a meal. Videotapes were coded according to Fight BAC! recommendations. A food-safety survey was administered and temperature data was collected. SUBJECTS/SETTING: A market research company randomly recruited subjects by telephone. Ninety-nine consumers participated (92 women, seven men). STATISTICAL ANALYSIS PERFORMED: Descriptive statistics were used. RESULTS: Overall, subjects did not follow the Fight BAC! recommendations for safe food handling. Handwashing was inadequate. The average hand wash length was significantly lower than the 20-second recommendation. Only one-third of subjects' hand wash attempts were with soap. Surface cleaning was inadequate with only one-third of surfaces thoroughly cleaned. Moreover, one-third of subjects did not attempt to clean surfaces during food preparation. Nearly all subjects cross-contaminated raw meat, poultry, seafood, eggs, and/or unwashed vegetables with ready-to-eat foods multiple times during food preparation. Unwashed hands were the most common cross-contamination agent. Many subjects undercooked the meat and poultry entrees. Very few subjects used a food thermometer. APPLICATIONS/CONCLUSIONS: Consumers make many food-handling errors during food preparation, increasing their risk of foodborne illness. Dietetics professionals need to familiarize themselves with the Fight BAC! consumer food-safety recommendations; understand where consumers are making food-handling errors; increase food safety awareness; and educate consumers, especially those in high-risk populations, about safe food handling at home.

Degumming of silk fabric with several proteases.

A crêpe silk fabric was treated with different alkaline (3374-L, GC 897-H), neutral (3273-C), and acid (EC 3.4 23.18) proteases with the aim to study their effectiveness as degumming agents. Proteases were used under optimum conditions of pH and temperature, while enzyme dosage (0.05-2 U/g fabric) and treatment time (5-240 min) were changed in order to study the kinetics of sericin removal. Degumming loss with soap and alkali was 27 wt.%. The maximum amount of sericin removed in 1 h was 17.6, 24, and 19 wt.% for 3374-L (2 U/g fabric), GC 897-H (1U/g fabric), and 3273-C (0.1 U/g fabric), respectively. Under the experimental conditions adopted, EC 3.4 23.18 was almost ineffective as a degumming agent. Degumming loss increased as a function of the treatment time, reaching a value of 25 wt.% with 1 U/g fabric of 3374-L. The morphological analysis showed that sericin was completely removed from the warp yarns of the crêpe fabric, while the highly twisted weft yarns still exhibited the presence of sericin deposits within the most internal parts of the close fibre texture. The chromatographic pattern of soluble sericin peptides changed as a function of the kind of enzyme used, enzyme dosage, and treatment time. A mixture of peptides from 5 to 20 kDa in weight, with a weight-average molecular weight of about 12 kDa was obtained.

Molecular-scale observation of the surface of polystyrene particles by AFM.

The surface morphology of polystyrene particles prepared by soap-free emulsion polymerization was observed in situ using the tapping mode of an atomic force microscope (AFM). Here, the use of a cationic initiator enabled us to synthesize positively charged polystyrene, which then adsorbed electrostatically onto the negatively charged mica plate with molecular-scale smoothness. The following was found from AFM measurements in water. The surface of polystyrene particles changed from smooth to rough as the polymerization proceeded in the experimental condition, where the bulk generated tiny particles continuously, because of the heterocoagulation in the growth process between the newborn and tiny particles and the existing particles. However, it is easy to prepare the particles with smooth surfaces on the condition of rich monomers because the monomers in the bulk dissolve the polymers which form the surface morphology to be deformable and reduce the interfacial area.

Effect of chelating functional polymer on the size of CdS nanocluster formation.

Chelating poly(acrylates-co-2-methylacrylic acid 3-(bis-carboxymethylamino)-2-hydroxy-propyl ester) microspheres of diameter 250-310 nm were prepared by the soap-free emulsion polymerization method for varying amounts of GMA-IDA. Then CdS/copolymer composite was generated by chemical deposition on the surface of the copolymer microspheres. By XRD analysis it is found that the chelated CdS nanoparticles are a pure cubic zinc blende structure. The CdS/copolymer composite is examined by UV-vis absorbance, photoluminescence, and TEM observation. Average CdS nanoparticle size calculated from Henglein's empirical curve is in the range of 3.0-8.0 nm and varies according to the GMA-IDA molar ratio during polymerization, pH value during chelation, and postchelation annealing temperature. Higher ratio of chelating group, pH value, and annealing temperature produce larger CdS nanoparticles. As GMA-IDA ratio increases, photoluminescence exhibits a red shift from 510 to 520 nm, photoluminescence increases, and bandwidth decreases. Photoluminescence of the CdS nanoparticle becomes negligible when diameter exceeds 5 nm.
