[The therapeutic procedure in thromboembolism of the pulmonary artery].

Clinical symptomatology, ECG, roentgenography, echocardiography, blood biochemistry, angiopulmonography with manometry of the pulmonary artery and right heart compartments were used to diagnose in 14 patients (age: 25-65 years) massive and submassive pulmonary artery thromboembolism. The angiopulmonographic index exceeded 18 points. Treatment included embolectomy, medical therapy (heparine, indirect anticoagulants), thrombolytic agents (celiase) and symptomatic therapy. The results of medical treatment were good in 80% of patients. Good results in embolectomy were observed in 77.7%. The authors formulate absolute and relative indications to urgent embolectomy in conditions of artificial circulation.

[Pleural involvement in patients with chronic bronchitis].

The extension and pathogenetic role of pleural changes in the development of unspecific chronic pulmonary diseases in 146 workers of different professional groups suffering of chronic bronchitis, were investigated. Ultrasonography and roentgenography were used. The functional state of the lungs was evaluated by means of spirographic and radiological methods. It is concluded that pleural changes are an endogenous risk factor in the development of chronic unspecific pulmonary diseases.

[Accumulation of 111In-antimyosin antibody at the focal site of inflammation].

An p6-year-old man with anterior acute myocardial infarction was administered antimyosin antibody labeled with 74 MBq (2 mCi) 111In, and abnormal hot activity was observed in the left lung by immunoscintigraphy. 67Ga-citrate scintigraphy also showed abnormal uptake in this lesion. At this time, this patient complained of fever and cough with sputum, and roentgenography showed a coin lesion-like shadow at the same location of the lung. By treatment with antibiotics, his symptom was improved and coin lesion-like shadow disappeared. So we thought the RI uptake area in the left lung was inflammatory lesion. This case suggests that 111In-antimyosin antibody can be trapped to the focal site of inflammation.

Early detection and control of cancer in clinical practice.

As part of the Community Cancer Care Evaluation, a random-sample survey of practicing physicians in 12 geographic areas was conducted in 1985 to provide information about physician practice patterns with reference to cancer detection, control, and treatment. All respondents were asked whether they routinely performed comprehensive physical examinations, breast palpations, mammography, rectal examinations, chest roentgenography, and stool guaiac examinations on normal healthy patients older than 50 years. Responses were examined in terms of American Cancer Society and National Cancer Institute (Bethesda, Md) recommendations. Conformity with recommendations was dependent on the geographic area, the specific procedure, and the specialty of the physician. Across all procedures, frequency of performance varied with years since graduation from medical school, with more recent graduates more likely to conform to recommended standards.

[Intrathoracic goiter. Apropos of 36 cases].

This article is concerned with 36 cases of plunging goiters responding to Eschapasse and Merlier definition. The frequency of plunging goiters is 5.2%, women are predominating with 96% and the average of age is 45 years. In 34%, these goiters gave rise to fow symptoms, while there was mediastinal complication in the others cases. Paraclinic investigations are limited to a face and profile pulmonary roentgenographies, and scintigraphy of the thyroid. In the treatment, cervicotomy proved to the completely satisfactory to removal of all the goiters and there were no need to sternotomy or thoracotomy. The pathology revealed two malignant tumors. Post operative evolution was uneventful except one death in the 10th post operative day by pulmonary emboli.

The vascular repair of an experimental osteotomy held in an external fixator.

The vascular repair of a lower tibial transverse osteotomy in the New Zealand white rabbit held in an external fixator was studied in three groups. In the first group, composed of 18 animals, the osteotomy gap was maintained throughout repair. Three animals were killed every two weeks up to 12 weeks postoperatively. In the second group, composed of three animals killed at six weeks, the fragments were brought into contact. In a third and similar group, the osteotomy was compressed. Plain roentgenograms were taken weekly, and intraarterial perfusion with Micropaque was performed when the animals were killed. Roentgenography using fine-grain film, microradiography, and histology were carried out on a midsagittal tibial slice. The results showed that following transverse osteotomy in which the gap was maintained, vascular union of the proximal and distal cortices and associated external callus masses had usually occurred by ten weeks postoperatively. However, an occasional hypovascular zone at the osteotomy site at 12 weeks was associated with fibrous delayed union. In contrast, when apposition or compression of the fragments was used, repair was accelerated, and cortical bone union was established at six weeks. Vascular union of the fragments occurred predominantly through the medullary vessels. The results emphasize the overriding clinical importance of abolition of a fracture gap to achieve rapid revascularization of a fracture and its union by bone.

A case of rapidly growing gastric leiomyosarcoma: observations over 18 months.

A gastric leiomyosarcoma in a 68-yr-old female was followed for 18 months by serial roentgenography. On initial x-ray, the lesion was diagnosed as a benign submucosal tumor. However, it rapidly increased in size, with doubling time subsequently calculated as 5 months. After 18 months' periodic observation, a gastrectomy was performed for a suspected malignancy. Histologic examination of the resected tumor revealed a leiomyosarcoma originating in the muscular layer of the stomach. It appears that the poor prognosis of leiomyosarcoma is related to its rapid growth; in addition, it is seldom detected at an early stage. Follow-up of the patient with a gastric submucosal tumor should be done first, 6 months after the initial examination, and thereafter, be done continuously according to the patient's condition. It is useful in differentiating a benign from a malignant myogenic tumor.

[Value of electroroentgenography in the diagnosis of skeletal degenerative and dystrophic changes and lesions].

The authors have carried out an electroroentgenographic study of degenerative and dystrophic changes of various portions of the skeleton in 635 patients, 30 of whom had had surgical interventions on the hip joints. The findings concerning the frequency of the occurrence and the peculiarities of the representation of some manifestations of the degenerative and dystrophic changes of the skeleton using electroroentgenography and film roentgenography have been compared. It has been established that electroroentgenography is characterized by a high degree of informativity in the diagnosis of this kind of pathology and the evaluation of the results of its treatment, with its efficacy being, on the whole, not lower than that of film reontgenography.

[Cytological examination of bronchoalveolar lavage fluid and its clinical significance in patients with diffuse interstitial lung diseases].

Cytological examination of bronchoalveolar lavage fluid (BALF) was carried out in 48 patients with diffuse interstitial lung diseases based on chest roentgenography. 7 cases manifested with respiratory symptoms but without any abnormality on both chest roentgenogram and fibroptic bronchoscopy served as controls. The total cell counts of BALF in the study group were all higher than those of the control group (3.1 x 10(5)/ml) and the differential count of the cells showed that neutrophilic granulocytes was found in patients with fibrosing alveolitis, Neutrophilic granulocytes accounted for 21.3 +/- 2.4% of all the cells and the percentage was significantly higher than that of the control group (1.8 +/- 0.5% P less than 0.01). On the contrary, lymphocytosis was found in patients with allergic alveolitis and sarcoidosis; lymphocytes accounted for 30.8 +/- 5.3% and 29.0 +/- 1.1% of all the cells in these two diseases respectively and the percentage was also higher than that of the control group (3.0 +/- 0.6% P less than 0.01). However, the differential cell count of BALF in alveolar carcinoma showed no significant difference with that of the control group (P greater than 0.05). The factors influencing the quality control of both BAL and cytological examination were evaluated and the clinical significance of these results was discussed.

Effective use of ultrasound in the management of congenital dislocation and/or dysplasia of the hip.

The use of ultrasound and roentgenography was evaluated in 50 patients with congenital hip dislocation and/or dysplasia treated with the Pavlik harness. The patients were divided into two groups based upon the predominant imaging technique used to monitor the course of treatment. Ultrasound was found to be an effective monitoring technique. A roentgenogram at conclusion of treatment is recommended to assess the acetabular development most accurately.

[An autopsy case of adenocarcinoma of the lung with signet ring cells, manifesting with aphasia and hemiparesis due to nonbacterial thrombotic endocarditis].

A 42-year-old male was admitted to Tokyo University Hospital because of confusion, aphasia and right hemiparesis. Cranial computed tomography and cerebral angiography demonstrated cerebral infarction due to occlusion of the left middle cerebral artery, while chest roentgenography disclosed a nodular shadow in the right upper lobe and swelling of right hilar and paratracheal lymph nodes. These findings suggested carcinoma of pulmonary origin and tumor-associated cerebral thrombosis, but a possibility of gastric cancer was raised by the finding of cervical lymph node biopsy which revealed signet ring cells in metastatic adenocarcinoma. He developed disseminated intravascular coagulation syndrome and died on the 83rd hospital day. Autopsy revealed adenocarcinoma of the lung with signet ring cells and non-bacterial thrombotic endocarditis which appeared to be responsible for the cerebral infarction. The relationship between adenocarcinoma of the lung with signet ring cells and non-bacterial thrombotic endocarditis was discussed.

[Abdominal pregnancy. Its conservative management. A case report with a live conceptus].

This is a case of abdominal pregnancy in an 18 year old woman. It has been diagnosed as a 30.4 weeks gestation abdominal pregnancy by clinical antecedents and findings supported by VSG and roentgenography. Conservative management for a period of two weeks with continual fetal monitoring and a series of maternal hemoglobin levels as well as induction of lung fetal maturation, were carried out. Exploratory laparatomy was done at 32.4 weeks of gestation; a living masculine product was found, weighing 1,100 g and Apgar of 8-9. The placenta was situated on the surface of the anterior leaf of the broad ligament, proximal round ligament portion, and proximal tubal portion on the right side. Removal of the placenta was made and right salpingectomy. Evolution was satisfactory of both mother and newborn. Mother was healthy and was discharged from hospital 3 days after. Newborn at 40 days from birth was healthy and weighing 2,000 g. Although most authors recommend surgical treatment for these cases, this case was treated conservatively as we consider the following to be good prognostic factors: age of the patient, general condition, position of placenta, state of fetal membranes, antecedents of partial detachment of placenta during pregnancy.

[Clinical evaluation of 20K gold alloy inlay restorations on young permanent teeth from the viewpoint of tooth-emergence age].

UNLABELLED: We observed 173 cases of 20K gold alloy inlay restorations on young permanent premolars and molars and investigated the emergence age of all these teeth. The observation period of inlay restorations was from 1 year to 11 years and 6 months. Clinical evaluations were made by exploring with an explorer, roentgenography and color photography. RESULTS: 1. Changes in inlay restorations occurred in 1 case (0.6%) of discoloration and 4 cases (2.3%) of inlay dislodgement. 2. Changes in teeth occurred in 8 cases (4.6%) of secondary caries, 15 cases (8.7%) of new caries and 4 cases (2.3%) of pulpitis. 3. Clinical failures occurred in 32 cases, and 24 of those cases were restored within 2 years after tooth emergence. 4. Almost clinical failures occurred on the maxillary and mandibular first molars. 5. In 27 cases (15.6%), repeat restoration was necessary. In 10 of these cases, the original 20K gold alloy inlay restorations continued in use.

[Facial circumscribed ossifying myositis. Anatomo-clinical and tomodensitometric study].

1 case of circumscribed myositis ossificans of the temporal muscle is reported in a 21 years-old man. The initial complaint was a sudden non traumatic swelling. Roentgenography showed a characteristic feature: peripheral radio-opacity around a non-opacified area. Under microscope, we could find 3 zones: a central fibroblastic area, an intermediate zone with immature osteoid bone, a peripheral zone with mature lamellar bone. In addition, we used histoenzymological and electron microscopical methods. The difficulties of diagnosis, the pathogenesis and the prognosis of this disease were also discussed.

[Intercepted orthopedic treatment of skeletal maxillary protrusion deformity].

Maxillary protrusion deformity may be divided into three types: functional, skeletal and their combination. The diagnosis of which should be differentiated carefully. The treatment of skeletal maxillary protrusion is very difficult. It must be diagnosed in the growing stage of children and treated with intercepted orthopedic appliance in time. In order to bring the mandible into harmony with maxilla, the growth of maxilla should be inhibited while the mandible is stimulated. In this article an efficient orthopedic appliance using orthopedic force to inhibit the growth of maxilla was introduced. The factors which influenced the orthopedic effect were introduced by cases report. The use of the roentgenography cephalometric in diagnosing the maxillary protrusion deformity was discussed.

Tumoral calcinosis: seasonal biochemical studies and chemical studies of eyelid lesion.

We recently described (Arch Ophthalmol 1988; 106:725-6) the presence of unique calcific lesions in the eyelids of a young woman with a history of hyperphosphatemic tumoral calcinosis. Here we document that no immediate family members showed similar lesions and that none was hyperphosphatemic. Dental roentgenography revealed characteristic abnormalities in the patient that confirmed the clinical diagnosis of tumoral calcinosis. Seasonal biochemical studies demonstrated persistently increased concentrations of phosphorus and 1,25-dihydroxyvitamin D in her serum. A calcific eyelid excrescence removed from the patient, studied by x-ray diffraction, was found to consist of crystals of hydroxyapatite. Microprobe analysis indicated the major elements in the deposit to be Ca, P, S, and Cl, just as in the periarticular deposits found in tumoral calcinosis. The Ca concentration in the patient's tear fluid, measured by atomic absorption spectrometry, was within the range found in tears of healthy volunteers. Phosphorus was undetectable (less than 30 mumol/L) in tears of the patient and the volunteers. These findings suggest that the eyelid lesions represent a new manifestation of the pathological process that produces the characteristic periarticular calcific masses of tumoral calcinosis.

[A gamma scintigraphic assessment of blood supply of the hip joint in Perthes disease].

In order to specify the peculiarities of the blood supply of the hip joint in Perthes' disease the authors carried out gamma scintigraphy by 99m Tc-pyrophosphate in 34 children. Patients with roentgenologically confirmed diagnosis of Perthes' disease at various stages as well as those in whom Perthes' disease was suspected, but convincing roentgenologic changes were absent, were examined. The data of scintigraphy and roentgenography were compared. The following phases of disturbances in blood supply were distinguished: ischemia, instable blood supply and stable revascularization. It is supposed that recurrences of ischemia may take place at the stage of resorption. Radionuclide investigation allows to make early diagnosis even before the appearance of roentgenologic changes with differentiation of doubtful cases.

[The diagnostic importance of a quantitative evaluation of gamma scintigraphy in Perthes' disease in children].

The investigation was performed in 28 children with Perthes disease, stages I-III. The comparison of visual and quantitative assessment of gamma-scintigrams has shown that the use of the coefficient of differential accumulation as a criterion of asymmetry of RP accumulation in the hip joints makes it possible to increase 4-fold the effectiveness of diagnosis at stage I of Perthes disease, the most difficult one for x-ray diagnosis. At more advanced stages neither visual nor quantitative assessment of gamma-scintigrams is superior to roentgenography. Therefore the use of gamma-scintigraphy seems appropriate in Perthes disease at stage I.
