[The prognosis of ovarian cancer depends on the CA125 value at the end of induction therapy].

The prognosis of ovarian cancer depends on the CA125 value at the end of induction therapy. The relationship between the prognosis and the CA125 value at the end of induction therapy was analysed in 16 ovarian cancers. In the group with a CA125 value less than 35 units/ml after the induction therapy (initial surgery + 3 courses of chemotherapy) all patients have survived for 18-45 months since the initial treatment to the present, whereas in the group with a CA125 value above 35 units/ml all patients died within 16 months after the initial treatment. Comparison of the survival rates in these two groups showed a significant difference. These results suggest that the most important factor that affects prognosis in patients with ovarian cancer is the normalization of the CA125 value after the induction therapy.

[Computer-assisted analysis of the prognosis in patients with upper gastrointestinal hemorrhage].

The prognosis of 189 patients with upper gastrointestinal bleeding was studied with the assistance of a computer. The safety of the prognosis given by the doctor was partially improved with the computer. However, when both judgements were simultaneously considered, the best results were obtained. A scoring-system constructed with simple clinical data proved useful to obtain prognostic judgements with high sensitivity and specificity. It is concluded that the reliability of the prognosis given in patients with upper gastrointestinal bleeding can be improved with objective methods. The improvement is maximal if objective information is evaluated together with clinical information.

Value of clinico-pathological factors in prognosis of breast cancer patients.

Two hundred and twenty nine cases of breast cancer diagnosed and treated in CMC, Ludhiana were studied for their clinical details, histological features and followed up. Various factors have an effect on the prognosis. The relationship between various clinicopathological features and prognosis were evaluated. The good prognostic factors in this study were middle age, small size of the tumour, low histological grade with marked lymphocytic infiltrate around the tumour, and sinus histiocytosis and no tumour metastasis in axillary lymph nodes. Pregnancy, lactation, younger age, large size, high grade tumour, skin invasion, and increased number of metastatic axillary lymph nodes were associated with significantly poor prognosis.

The prognosis of depression in the elderly.

Controversy prompted a systematic and critical review of original research articles to determine the prognosis of depression in the elderly. A search of three computer databases for articles published from January 1980 to May 1989 and of the bibliographies of articles located revealed 10 reports, involving 990 subjects, that met the following inclusion criteria: original research, published in English or French since 1950, sample of at least 25 patients, inclusion of only patients over 60 years, mean follow-up period of 1 year or more and description of the patients' mental state during follow-up. The methods and study designs were then assessed with the use of the criteria for prognostic studies established by McMaster University Health Sciences Centre, Hamilton, Ont. All of the studies had serious, multiple flaws. Nevertheless, when the results were combined it appeared that at least 60% of the patients remained well or had relapses with recovery (mean follow-up 31.9 months); up to 25% remained continuously ill. Physical illness, cognitive impairment and severe depressive symptoms were frequently related to poor prognosis; most of the social factors studied were not related to prognosis. Future studies must pay attention to methods and design, particularly the composition of study populations, the assessment of outcomes and the control of extraneous prognostic factors.

[Interferon-producing capacity in patients with prostate cancer: prognosis value].

The interferon (IFN)-alpha and -gamma producing capacities were measured in 47 patients with prostatic cancer by the whole blood method. The mean IFN-alpha producing capacity in prostatic cancer patients was 3,657 IU/ml, which indicated a significantly (P less than 0.005) lower value than that in healthy male control aged 50 years or more (mean value, 4,988 IU/ml), while the IFN-gamma producing capacity was almost normal. The IFN-alpha and -gamma producing capacities in patients with stage D disease was decreased as compared with that in patients with stage A, B or C. The higher the grade of the diseases, the lower the IFN producing capacity tended to be. In patients with prostatic cancer, the prognosis was worse in low IFN producing capacity group than in normal or high IFN producing capacity group. These results suggest that the measurement of IFN producing capacity is useful for the estimation of the prognosis of patients with prostatic cancer, and that the IFN producing capacity may be a factor which decides the prognosis.

An international survey on the prognosis of occupational contact dermatitis of the hands.

Prognosis is a crucial topic in contact dermatitis. We recently reviewed the extensive literature on the prognosis of contact dermatitis. The majority of patients in these studies had persistent dermatitis. Job changes usually did not lead to a significant improvement for most patients with occupational contact dermatitis. We performed a worldwide survey of colleagues with expertise in treating patients suffering from occupational contact dermatitis of the hands. Of 65 experts, 51 returned questionnaires for analysis. 67% of European and 29% of respondents from Sweden, Denmark, Finland, United States of America, Canada, and Mexico stated that more than 75% of workers with severe hand dermatitis required a job change. 78% of the respondents found that chromate and 57% found that nickel allergy were associated with the worst possible prognosis. 98% felt that barrier creams were no more effective than bland emollients in the prevention of hand dermatitis.

Angiographic arterio-venous shunt and venous thrombosis in the prognosis of hepatoma.

Angiograms and clinical data for 249 patients with hepatoma were evaluated, with particular attention paid to the arteriovenous (A-V) shunt and venous thrombosis (including occlusion). There were 43 cases (17.3%) with A-V shunt and 140 cases (56.2%) with venous thrombosis. A statistically significant difference in survival was noted among the unresectable patients with and without these vascular changes. Furthermore, in the resectable patients, presence of venous thrombosis also showed a poor prognosis. A-V shunt did not influence the survival in the unresectable patients with venous thrombosis, and 93.0% of A-V shunt was associated with venous thrombosis. This seemed to imply that these two vascular changes might be the same entity. In the presence of either A-V shunt or venous thrombosis, the survival rate in the surgical group was still better than that of medical groups treated by either transarterial embolization (TAE) or conservative methods. Most of the A-V shunt (81.4%) and venous thrombosis (56.4%) occurred in the diffuse type of hepatoma, and the prognosis for unresectable patients was the worst in this type, with a mean survival time of 3 months. Since A-V shunt and venous thrombosis are factors in poor prognosis and occur frequently, careful preoperative evaluation of these vascular changes is mandatory. However, in patients with localized tumor, coexisting with such vascular involvement, surgical resection is still recommended.

[Treatment and prognosis of stomach cancers in the population of the Cote d'Or's District (1976-1985)].

The present study on treatment and prognosis of gastric cancers is based on the data of the digestive tract cancer registry set up for the French department of Côte-d'Or. During the 1976-1985 period, 784 cases of gastric cancer were diagnosed. Resection for cure was performed in 41.3% of gastric cancers and that proportion remained the same during the course of the study period. Operative mortality was 18.5% after resection for cure and 26.7% after palliative surgery. The overall survival rate was 31.7% at 1 year, 17.0% at 3 years, and 13.0% at 5 years. Ten different criteria (personal data, histologic and therapeutic parameters) were studied in relation to survival. With the univariate analysis, the survival rates were related to age, duration of the symptoms, history of previous gastric ulcer, tumour location, stage of diagnosis and treatment. Only 4 aspects were of good prognosis according to the multivariate analysis (Cox model): extension limited to the gastric wall, resection for cure, duration of the symptoms and location to the antrum or the angulus. These results underline the fact that the prognosis of gastric cancer in a well-defined population remains poor. Because of its severity, gastric cancer still represents a major cancer problem.

[Factors affecting the prognosis of acute lymphoblastic leukemia in childhood].

The authors tried to test the value of some clinical and laboratory characteristics for the prognosis of acute lymphoblastic leukaemia (ALL) in a group of 69 children treated according to three different protocols. The results were evaluated by methods of one-dimensional and multidimensional analysis. The absolute number of blasts in the peripheral blood stream and initial leucocytosis during establishment of the diagnosis proved to be the most important risk factors influencing the prognosis of the patients. Other adverse signs for the prognosis of ALL in the group were a mediastinal tumour, L2 type of leukaemic blasts according to the morphological FAB classification and age above 10 years when the diagnosis was established. The patient's sex, immunophenotype of the leukaemic blasts, chromosomal abnormality of the karyotype in the leukaemic cells, marked hepatosplenomegaly, thrombocytopenia, haemoglobin values and PAS reaction in the blasts, did not affect the therapeutic results in the author's group of patients.

[Wegener's granulomatosis. New aspects of diagnosis, treatment and prognosis].

In its classical form, Wegener's granulomatosis (WG) is characterized by acute granulomatous vasculitis of the upper and lower respiratory tract together with glomerulonephritis, and when untreated, mortality is high. The use of cyclophosphamide and prednisolon has improved the prognosis dramatically, while azathioprine and trimetoprim-sulfamethoxazole are of limited therapeutic value in these patients. Only 1/3 of subacute and chronic cases show renal impairment, and the prognosis is better in limited Wegener's granulomatosis without renal disease than in the classical form. This case-report and review takes up various new aspects of diagnosis, treatment and prognosis.

[Treatment and prognosis of primary malignant melanoma of the oral cavity--an analysis of 12 cases and 96 reported cases].

The treatment methods and prognosis were discussed in 12 patients of oral melanoma who were treated at Dept. of Oral and Maxillofacial Surgery, Kobe Univ. School of Medicine from 1975 to 1989, with a review of 96 cases published in the Japanese literature between 1984 and 1988, and the following findings were obtained. 1. Eleven our 12 cases were acral lentiginous melanoma (ALM), and one case was nodular melanoma (NM). 2. The early two cases were died of distant metastasis. Of the recent ten cases which were underwent surgical excision combined with adjuvant immunochemotherapy with DTIC, ACNU, VCR and OK-432, nine cases had good prognosis, and one case was died of local recurrence. 3. The prognosis of the reported cases was poor because of local recurrence and distant metastasis.

[The degree of coincidence of the long-term extrapolated expert prognosis with the real morbidity of tick-borne encephalitis in the USSR].

Tick-borne encephalitis morbidity over the period of 1952-1981 was assessed and the prognosis was made for 29 administrative territories for the 1980s and early 1990s. The changes in the morbidity in 1982-1988 have shown the coincidence with the prognosis in 70% of cases, in 20% of cases stability was noted instead of the predicted growth or drop in the morbidity, in 10% of cases real changes were contrary to the predicted ones. The peak of the disease onset was predicted correctly on 16 out of 22 territories, in 3 cases peak of the disease was observed 2 years earlier, in 1 case it was noted 4 and in 1 case 8 years earlier. Measures for improvement of the quality of the prognosis are suggested.

[A critical study of the prognosis of renal adenocarcinoma and an attempted classification. Apropos of a series of 322 cases].

The prognosis of adult renal adenocarcinoma was evaluated by analyzing 322 cases treated by extensive nephrectomy (1979-1988). Five-year actuarial survival curves showed that prognosis was closely correlated with anatomic spread: no clear prognostic significance could be ascribed to venous invasion but involvement of retroperitoneal lymph nodes proved to be nearly as negative a prognostic factor as metastatic disease. The grade of malignancy also had a major impact on prognosis. Because the TNM classification, although accurate, is difficult to use and relies solely on descriptive data, a simple five-stage classification is proposed but remains to be validated. The classification system advocated by Robson groups stages with very different outcomes and consequently seems unhelpful.

[Utility of percutaneous biliary drainage in patients with hepatic hilar carcinoma in reference to prognosis and associated complication].

We investigated prognosis in 48 patients with hepatic hilar carcinoma (35 treated with percutaneous biliary drainage (PBD) and 13 without PBD) and complication associated with PBD. Survival rate in 20 patients who had no cholangitis within 1 month after PBD was significantly superior to that in 15 patients with cholangitis. The former was 51.9% at 6 months and 13.0% at 1 year. We thought that this result would be helpful in estimating prognosis in patients with hepatic hilar carcinoma treated with combined therapy. Cholangitis within 1 month after PBD was caused from inadequate drainage for separated intrahepatic bile ducts, ERC before PBD and dislodgement of drainage catheter. Survival rate in patients with PBD not accompanied by cholangitis in 1 month after was superior to that in those without PBD. So we thought that PBD could improve prognosis in patients with hepatic hilar carcinoma if they had no cholangitis within 1 month after PBD.

[A study of lung cancer with presence of pleural effusion at the time of thoracotomy--cytologic evaluation of a relatively small amount of pleural effusion and prognosis after removal].

The cytologic evaluation of a relatively small amount of pleural effusion in lung cancer at the time of thoracotomy has not been previously considered, and prognosis after removal has not been clarified. In order to clarify these points, 99 cases of removed lung cancer with presence of pleural effusion at the time of thoracotomy were examined following cytology. Our study was conducted with regard to the amount and nature of pleural effusion, pleural effusion cytology, tumor development and its relation to prognosis. On the basis of this study a formula was developed relating the occurrence rate of cytologically positive pleural effusion with the development of tumor employing multivariate analysis, specifically the multiple regression analysis. 21% of these cases showed cytologically positive pleural effusion, indicating an absence of its correlation to the amount of pleural effusion; the conducting of cytology regardless of the amount of pleural effusion was found to be significant in determining the precise stage. The occurrence rate of cytologically positive squamous cell carcinoma was significantly few compared with those in other cell types. Regarding the pleural effusion of a relatively small amount, the prognosis after removal showed a relatively high three-year survival rate of 32% for those cases with negative pleural metastasis despite the positive showing of pleural effusion cytology, indicating the viability of surgery. From the occurrence rate of cytologically positive pleural effusion (Y) and the degree of pleural metastasis (X1)/the degree of pleural invasion (X2)/the degree of lymph nodes metastasis (X3), the following formula was obtained employing multiple regression analysis: Y = 0.344X1 + 0.050X2 + 0.034X3 + 0.075 (proportion 0.840).(ABSTRACT TRUNCATED AT 250 WORDS)

Relation of histopathological features to prognosis of gallbladder cancer.

Sixty-four cases of gallbladder cancer surgically removed were histologically divided into three groups, that is, papillary adenocarcinoma, tubular adenocarcinoma, and undifferentiated carcinoma, and were compared clinicopathologically. The results are as follows. Papillary adenocarcinoma showed little gender preponderance and had a low association with gallstones. Underlying chronic cholecystitis or epithelial metaplasia was infrequent. Most cases were in the early stage, with favorable prognosis. Tubular adenocarcinoma, seen predominantly in females with gallstones, showed a downward invasive growth with unfavorable prognosis. Chronic cholecystitis and epithelial metaplasia were frequent. Undifferentiated carcinomas were common in females with gallstones, and their prognosis was the worst of the three. It was concluded that papillary adenocarcinoma differed from both tubular adenocarcinoma and undifferentiated carcinoma in biological behavior. The former appears to arise mainly from the native gallbladder epithelium, whereas the latter two types frequently arise from metaplastic epithelium.

[Relation between blood coagulability and prognosis in patients with ovarian cancer].

In 13 patients with ovarian cancer who had been treated with remission induction chemotherapy, we measured the levels of platelet PT, APTT, Hepa T, ATIII, alpha 2PI and FDP, and we also measured such molecular markers as fibrinopeptide A(FPA), fibrinopeptide B beta 1-42(FPB beta 1-42), and fibrinopeptide B beta 15-42 (FPB beta 15-42) before and after chemotherapy. Then the relation between the post chemotherapeutic trends of these substances and the prognosis for patient with ovarian cancer were investigated. 1) After chemotherapy, the levels of fibrinogen were increased, ATIII was decreased significantly in cases in the NC.PD group compared with the PR.CR group (p less than 0.05). The alpha 2PI concentrations were also slightly low, and the levels of FDP were within the normal range in cases in the PR.CR group, but in many cases in the NC.PD group were increased. 2) After chemotherapy, the concentrations of molecular markers (FPA, FPB beta 1-42 and FPB beta 15-42) were increased significantly in cases in the NC.PD group compared with the PR.CR group (p less than 0.05). 3) In cases in the recurrent group, the levels of ATIII and alpha 2PI were decreased significantly compared with the after therapy group, FDP, FPB beta 1-42 and FPB beta 15-42 were increased. Accordingly, the recovery of hemostatic balance with effective chemotherapy is related to the prognosis for patients with ovarian cancer, and the levels of fibrinogen, PT, alpha 2PI, ATIII, FDP, FPA, FPB beta 1-42 and FPB beta 15-42 may be able to be used in the prognosis for patients after chemotherapy.

Poor prognosis of children with pre-B acute lymphoblastic leukemia is associated with the t(1;19)(q23;p13): a Pediatric Oncology Group study.

The prognostic significance of chromosomal translocations, particularly t(1;19) (q23;p13), was evaluated in children with pre-B and early pre-B acute lymphoblastic leukemia (ALL). Patients were treated on a risk-based protocol of the Pediatric Oncology Group (POG) between February 1986 and May 1989. An abnormal clone was detected in 46% (130 of 285) of pre-B cases and 56% (380 of 679) of early pre-B cases. Translocation of any type was associated with a worse treatment outcome than other karyotypic abnormalities: 15 of 66 versus 3 of 64 failed therapy in the pre-B group (P = .001), and 37 of 141 versus 23 of 239 failed in the early pre-B group (P less than .001). The t(1;19) (q23;p13) occurred significantly more often in cases of pre-B ALL with a clonal abnormality than in early pre-B ALL cases (29 of 130 v 5 of 380, P less than .001). Among the 285 pre-B cases in which bone marrow was studied cytogenetically, those with t(1;19) had a significantly worse treatment outcome than all others (11 of 29 v 27 of 256 have failed therapy, P less than .001). This difference is significant (P less than .001) after adjustment for leukocyte count, age, and other relevant features. Cases with the t(1;19) also had a worse prognosis than pre-B patients with other translocations (4 of 37 have failed, P less than .01) or with any other karyotypic abnormality (7 of 101 have failed, P less than .001). We conclude that chromosomal translocations confer a worse prognosis for non-T, non-B-cell childhood ALL, and that the t(1;19) is largely responsible for the poor prognosis of the pre-B subgroup.
