[The nucleus of ossification in kidney calculus].

The presence of bone tissue in renal stones has been described for a long time. Two cases of heterotopic ossification are reported in this study. Stone analysis was performed by stereoscopic microscopy, infrared spectrophotometry and fine slice petrographic section and polarized light microscopy. This last technique revealed the existence of osteocytic filling defects inside the stones, an unquestionable sign of mineralized bone tissue.

Heterotopic ossification following total knee arthroplasty requiring surgical excision.

We present a case of progressive heterotopic ossification (HO) after cementless total knee arthroplasty causing painful stiffness that was treated with surgical excision. The patient had few risk factors associated with HO, including minimal anterior notching and dissection of the distal femoral cortex. The patient did undergo manipulation; however, this occurred after the diagnosis of HO was made. This report documents a rare case of HO following total knee arthroplasty that required surgical excision.

Neonatal mucolipidosis II (I-cell disease) with dysharmonic epiphyseal ossification and butterfly vertebral body.

I-cell (mucolipidosis 2) is a rare, autosomal recessive neurodegenerative lysosomal storage disease. Neonatal skeletal radiographs are distinctive; a transient osteopathy that has features resembling hyperparathyroidism and rickets is present during the first year of life. The case of a 3-week old infant with I-cell disease is presented. Additional radiographic findings in this case include a butterfly vertebral body and dysharmonic epiphyseal ossification.

[Heterotopic ossification as a complication of fracture-dislocations of the forearm in children].

Under observation there were 59 patients ranging in age from 4 to 14 years with posttraumatic ossifications in the elbow joint areas. All the patients were given conservative treatment, 23 patients needed operative interventions on the elbow joint. Long-term results were studied at the period from 6 months to 8 years. Good and satisfactory functional results were noted in 96.8% of the patients after the conservative treatment. After the operative treatment good and satisfactory functional and anatomical results were obtained in 90% of the patients.

[Dendriform diffuse pulmonary ossification. Report of 2 cases].

Two cases of diffuse dendriform pulmonary ossification are reported which was diagnosed postmorten. Both patients had a previous history of chronic obstructive pulmonary disease with pneumonic complications. The radiological study was interpreted as pulmonary fibrosis in the first case, an entity which ranks first in the differential diagnosis. In some cases the obtention of highly contrasted X-ray films can be useful for recognizing this are entity.

[Preoperative radiotherapy for prevention of heterotopic ossifications after hip endoprosthesis].

Preliminary results of a prospective study which investigates the efficacy of preoperative radiotherapy (RT) for prevention of heterotopic ossification (HO) after total hip arthroplasty are summarized. A total number of 20 hip joints (18 patients) were irradiated with a single dose of 6.0 Gy < 4 hours before the surgical procedure. After an average observation period of 6.9 months (range: 6-11 months) the results were evaluated using radiological and clinical criteria. At follow-up, one case (5%) had evidence of high-grade HO (> Brooker grade II). The functional outcome quantified with the Harris score was improved by an average of 37.9 points. The authors conclude that preoperative RT is an effective alternative for postoperative irradiation.

[Ossification of the stylohyoid chain in computerized tomography--Eagle syndrome].

The computed tomographic morphology of a typical Eagle syndrome is presented on the basis of a case history. In a 40-year old male patient [correction of female] presenting with bilateral tinnitus, globus hystericus, and increasing hoarseness computed tomography revealed bilateral ossification of the stylohyoid ligament. The incidence of stylalgia is very low in comparison to the occurrence of a elongated styloid process or an ossified stylohyoid ligament. However, in cases of unexplained complaints in the head and neck region it should be considered in the differential diagnosis as it has therapeutic consequences.

Perineurioma with ossification: a case report with immunohistochemical and ultrastructural studies.

We present the case of a peripheral nerve sheath tumor arising from the sciatic nerve in the thigh of a 42-year-old woman. Histologically, the tumor was composed of fascicles of spindle cells with extremely attenuated cytoplasmic processes. Regions of the tumor were heavily mineralized and fibrotic. The tumor was epithelial membrane antigen-positive and S100-negative. Staining for Leu-7 was positive in a patchy distribution. Type IV collagen was present between cells, and CD34 was negative in the tumor cells. Ultrastructural features included elongate cellular processes surrounded by discontinuous basal lamina and collagen. The features of the tumor therefore meet criteria for perineurioma. This rare tumor is found most often in the extremities in middle-aged woman. To our knowledge, the presence of bone formation in a perineurioma has not been described previously. The differential diagnosis includes other peripheral nerve sheath tumors, low-grade fibrous tumors, and synovial (monophasic) tumors. Evidence provided by electron microscopy and immunohistochemistry supports the diagnosis and classification of this unusual nerve sheath tumor as perineurioma with ossification. These findings expand our knowledge and criteria for classifying peripheral nerve sheath tumors.

Circumferential surgery for the management of cervical ossification of the posterior longitudinal ligament.

Can simultaneous anterior and posterior (circumferential) surgery in patients with cervical ossification of the posterior longitudinal ligament (OPLL)/stenosis, achieving both decompression and stabilization, be accomplished with acceptable risk? Between 1989 and 1996, 22 circumferential procedures were performed, including an average 2.5-level anterior corpectomy with 5-level posterior wiring and fusion. These patients were severely myelopathic (average Nurick grade 3.5) and were followed for a mean interval of 22 months (range 4-52 months). Circumferential procedures required an average of 9.8 h and 3.5 U of blood transfused. Postoperatively, patients improved approximately +3.0 Nurick grades. Simultaneous circumferential surgery for OPLL/stenosis may be successfully performed in <10 h with limited blood loss.

Retinoid and bone metabolic marker in ossification of the posterior longitudinal ligament.

The pathogenesis of ossification of the posterior longitudinal ligament (OPLL) remains to be elucidated, though etiologic factors for OPLL have been identified. High levels of serum retinol and retinol binding protein (RBP) have been observed in patients with diffuse idiopathic skeletal hyperostosis (DISH). OPLL is often associated with DISH. In this study, the levels of serum retinol and RBP were determined in 70 patients with OPLL in the cervical spine, and compared with those in normal subjects. Bone metabolic markers of serum intact osteocalcin, urinary pyridinoline and deoxypyridinoline were examined as well. Among female patients, level of serum RBP was significantly higher in those in their 60's, and those with mixed type OPLL. Level of serum RBP was significantly higher in both sexes, and retinol was exhibited higher in female patients, if they were associated with DISH. Patients with OPLL exhibited no abnormal bone metabolic marker levels. These findings suggest that vitamin A may play a role in the development of OPLL.

Heterotopic ossification after cemented or uncemented bateman bipolar hemiarthroplasty.

BACKGROUND: This is a retrospective study to evaluate the incidence of heterotopic ossification (HO) in cemented and uncemented femoral neck fractures treated with Bateman bipolar hemiarthroplasty. METHODS: From October 1985 to July 1990, there were 451 cases of displaced intracapsular fractures of the femoral neck treated with Bateman bipolar hemiarthroplasty (261 uncemented vs 190 cemented) at the Veterans General Hospital-Taipei. RESULTS: After an average follow-up of 46 months, there was more HO in the cemented group than in the uncemented group (p = 0.014, chi-squared test). The average surgical time was approximately 20 minutes longer and the average blood loss was approximately 160 ml more in the cemented Bateman bipolar hemiarthroplasty group. CONCLUSIONS: HO formation is more common in cemented Bateman bipolar hemiarthroplasty than in the uncemented procedure. The incidence of HO is not related to age or sex.

[Ossification of the coracoclavicular ligaments (author's transl)].

Joint-like connections between clavicula and coracoid process are rarely diagnosed. In routine radiological examinations of the thorax, the authors found a posttraumatic ossification of the coracoclavicular ligament in four patients. This phenomenon can occasionally be the cause of shoulder pain. The anatomy and pathogenesis are described, with x-ray film examples, on the basis of four cases.

Cervical myelopathy due to ossification of the posterior longitudinal ligament: a clinicopathologic study.

A clinicopathologic study was done of a Japanese patient with symptomatic ossification of the posterior longitudinal ligament in the cervical spine. Post-mortem specimen showed characteristic distribution of degenerations at C5-6 segments similar to chronic cervical spondylosis or disk protrusion. The degenerative changes were confined to the ventral two thirds of the posterior and lateral columns. In addition to demyelination, loss of axons, and neuronal cell death, thickening of venular wall and fine capillary neovascularization strongly suggested chronic circulatory insufficiency. Spinal immobilization produced by the ossified ligament may have contributed to the long clinical course, lasting for 30 years. Review of literature showed that the myelopathy can be distinguished clinically and radiographically from that produced by cervical spondylosis.
