Effect of cyclophosphamide on rat urinary bladder epithelium treated wtih N-butyl-N-(4-hydroxybutyl)nitrosamine.

The effect of cyclophosphamide (CP) on the urinary bladder epithelium of rats treated with N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) was examined histologically to detect preneoplastic papillary or nodular hyperplasia. The urinary bladder epithelium of rats injected intraperitoneally with 40 or 80 mg/kg body weight of CP after treatment with 0.025% BBN for 2 weeks showed extensive epithelial erosion and then simple, or papillary or nodular hyperplasia, but this soon almost disappeared. In contrast, rats given BBN alone showed papillary or nodular hyperplasia later. These findings indicate that epithelial damage by CP inhibited the early neoplastic process of BBN.

Mycosis fungoides: diagnostic criteria based on quantitative electron microscopy.

Quantitative electron microscopy can provide objective measurements of nuclear atypia in lymphoid cells in skin biopsies during early stages of infiltration of the skin as part of inflammatory or neoplastic processes. This potentially provides a method for establishing reproducible criteria for determining whether an individual skin biopsy represents mycosis fungoides or is unlikely to represent mycosis fungoides. To investigate such criteria, quantitative electron microscopic studies of skin biopsies from 109 patients were performed. Seventy-seven patients had benign disorders, 16 patients had early mycosis fungoides, and 16 patients had controversial lesions. Measurements were made of lymphocyte nuclear profile perimeter and areas in electron micrographs. A nuclear contour index was calculated. On the basis of the mean degree of nuclear convolution alone, mycosis fungoides was not adequately distinguished from benign disorders. Better discrimination was achieved when the proportion of cells with a anuclear contour index of 9.0 or more was considered as an additional actor. When statistical methods were used to select discriminating factors, the best electron microscopic discrimination between benign and mycosis fungoides groups occurred when the proportion of cells with a nuclear contour index of 7 or more and the proportion of cells with a nuclear profile area greater than 30 sq. micrometer. were used together. Nomograms are presented which demonstrate the use of these criteria for a given case.

[Metastatic melanoma of the brain].

Clinical and clinico-pathoanatomical comparisons (60 observations) have shown that in patients with primary cutaneous melanoma and its metastases treated by modern methods the tumour metastasis to the brain occurs in late periods of the disease at the stage of the neoplastic process generalization. Histological examinations of the melanoma metastases of various localization revealed a much lesser differentiation of the tumour cells and a greater vascularization of the tumour. The pronounced vascularization is characteristic for metastases of any localization, both visceral and cerebral, at the stage of the process generalization.

[Myxoma of the heart].

Twenty-one observations of cardiac myxomas (17 samples of operation material, 4 autopsies) were studied. Among them, 18 cases were located in the left atrium, 2 in the right atrium, 1 in the right ventricle. Macro- and microscopic characteristics of cardiac myxomas are presented. No signs of an organized thrombus were found. These studies confirm the current concept of cardiac myxoma as a neoplastic process.

Effects of continuous and discontinued exposure to 2-AAF on urinary bladder hyperplasia and neoplasia.

The effect of discontinued feeding of 2-acetylaminofluorine (2-AAF) was compared with the effect of continuous 2-AAF feeding on the urinary bladder of BALB/c female mice. Dietary concentrations of 2-AAF examined in this regard were 60, 75, 100, and 150 ppm. These diets were fed for 9, 12, or 15 months and then replaced with control feed, and the animals were killed at 18 or 24 months. Observations on these animals were contrasted with observations made at the same intervals on animals continuously fed these concentrations of 2-AAF throughout the study. Discontinuing 2-AAF exposure decreased prevalence of bladder tumors below that found in continuously exposed mice and resulted in a decrease in severity and prevalence of hyperplasia. Mild hyperplasia was retained in a portion of each population and the prevalence of this hyperplasia was dependent on both the period of exposure and the dose of 2-AAF. Mild persistent hyperplasia was correlated closely with neoplasia and may be a necessary stage in neoplastic development. Moderate or severe hyperplasia apparently regress when 2-AAF feeding is discontinued and do not appear to be essential steps in the neoplastic process.

Pathologic fracture of the clavicle caused by arteriovenous malformation.

An anusual pathologic fracture of clavicle was caused in a 48-year-old woman by an arteriovenous malformation. The lesion radiologically mimicked a neoplastic process. Arteriography disclosed the vascular cause. Treatment was achieved by interrupting the feeding vessels and resecting the involved bony segment.

Cell-mediated cytotoxicity in preinvasive and invasive squamous cell carcinoma of the cervix.

Cell-mediated immunity was tested by coincubation of target cell line (2043) of human squamous cell carcinoma and peripheral blood leukocytes (PBL) of 22 patients with squamous cell neoplasia of the uterine cervix, 9 patients with other tumors, and 9 normal females. The percentage of cell reduction in mild dysplasia was 90.1 +/- 6.8% (.001 less than P less than .005), in moderate dysplasia was 91.1 +/- 6.4% (.001 less than P less than .005), in severe dysplasia was 91.6 +/- 15.6% (P less than .001), in carcinoma in situ was 85.0 +/ 2.6% (P less than .001), and in invasive squamous cell carcinoma of the cervix was 85.0 +/- 6.9% (P less than .001). Peripheral blood leukocytes of patients with squamous neoplasia did not show any significant cytocoxicity against cell line SKOV-3, developed from an ovarian adenocarcinoma, nor did the PBL of patients with "other tumors" show any significant cytotoxicity against cell line 2043. This study shows that even in early stages of preinvasive squamous cell carcinoma of the cervix, the PBL are sensitized against the neoplastic process, confirming that different stages of cervical intraepithelial neoplasia and invasive squamous cell carcinoma are different intensities of the same biologic process.

[A comparative assessment between transrectal echography, venous urography, CAT and morphology in patients with prostatic carcinoma].

By incidence rate prostate gland carcinoma ranks second among all neoplastic diseases. As all tumorous neoformations, it is conspicuous for its progressive character. Clinical, laboratory and roentgen methods, thus far routinely used in practice, are hardly in a position to establish correct and accurate diagnosis alone. The diagnostic potentials of transrectal ultrasound examination, intravenous urography, CAT and morphological patterns are comparatively studied in patients presenting prostate gland carcinoma. The obtained results point to a greater diagnostic relevance of transrectal ultrasound examination, by comparison with CAT and intravenous urography. It is a matter of a noninvasive and highly effective procedure for primary diagnosis of the neoplastic process. Emphasis is laid on the necessity to confirm the echographic diagnosis in all instances by performing morphological study in conjunction with the remaining diagnostic methods.

[The C-reactive protein level of patients after the extracorporeal lithotripsy of urinary calculi].

Within 24 to 48 hours of sustaining a variety of severe clinical conditions, such as general bacterial infection, tissue necrosis, acute rheumatism, myocardial infarction, active pulmonary tuberculosis and neoplastic processes, the serum level of C-reactive protein may show a considerable increase. It may also serve as a criterion for the evolution of the disease, and for the effect of the therapy in course. The task undertaken is to use the serum level of C-reactive protein as a criterion for evaluating the extent of renal parenchyma damage following extracorporeal lithotripsy of urinary calculi (ELUC). Proceeding from the results of the study, it is assumed that ELUC is a nontraumatic operative intervention for the renal parenchyma. In case of extreme increase in C-reactive protein level, a higher incidence rate of post-ELUC complications may be anticipated.

Detection of clonal histiocytes in Langerhans cell histiocytosis: biology and clinical significance.

Although the first clinical description of Langerhans cell histiocytosis (LCH) was published over a century ago, the aetiology and pathogenesis of this enigmatic disorder are still remained unknown. Viral, immunological, neoplastic and other pathogenetic mechanisms have been considered, but none has been proven. The prevailing opinion is that LCH is a reactive disorder rather than a neoplastic process, but this presumption has never been definitively tested. A key feature of a neoplasm is its clonal derivation from a single cell. To determine if LCH is a polyclonal reactive or a clonal disorder, we and others have recently used molecular biological techniques to assess clonality in LCH. Using X chromosome-linked DNA probes that can detect clonal or polyclonal X chromosome inactivation patterns in female tissues, clonal CD1a+ histiocytes have now been detected in the lesional tissues in each of 16 females affected with LCH. Most of these patients were studied prior to the initiation of therapy. Lymphoid clonality was excluded in all cases in which it was studied, confirming that the clonal cells in LCH are the CD1a+ dendritic cells presumed to be pivotal in this disorder. Two distinct lesions (a pre-treatment bone biopsy and a lymph node biopsied 3 years later) have been studied in only one case to date; the same clonal pattern of X chromosome inactivation was observed, consistent with persistence of the same clone during this patient's disease course.(ABSTRACT TRUNCATED AT 250 WORDS)

[Cancer of the head of the pancreas and common bile duct (an anatomical, cholangiographic and clinical study)].

A study was carried out on the principal alterations of the common bile duct in pancreatic neoplasms by gross and microscopic morphological examinations and intraoperative cholangiographies. Correlation of the data furnished evidence of various coinvolvement mechanisms of the lower common bile duct by the neoplastic process, mechanisms that lead to a closer understanding of the clinical symptomatology and interpretation of intraoperative cholangiographies.

Cytokeratin patterns of liver carcinomas induced by diethylnitrosamine in monkeys.

BACKGROUND: Although the general conception is that hepatocyte- and bile duct-specific cytokeratin (CK) patterns are maintained throughout the neoplastic process, there is an increasing number of reports showing deviation from the rule. CK patterns have been found to be similar across species barriers, so it could be expected that studying the CK patterns of experimentally induced liver tumors may contribute to the elucidation of these controversies. EXPERIMENTAL DESIGN: A CK immunohistochemical study was carried out on histologic sections from hepatocellular carcinomas (HCCs) and preneoplastic lesions from 118 monkeys chronically dosed with diethylnitrosamine (DEN), using mAbs to CK 8, CK 18, CK 7, and CK 19. RESULTS: Normal monkey hepatocytes differed from human hepatocytes by displaying CK 19 in addition to the CK 8/CK 18 pairs, whereas the CK pattern of the bile duct epithelial cells was identical in monkey and human liver. In association with DEN-induced hepatocarcinogenesis, heterogeneity was observed in the CK expression, both in the HCCs and nontumorous parts of the livers. The majority of the HCC cases displayed one of the three CKs normally present in monkey hepatocytes, whereas positive expression of all three CKs (CK 8, CK 18, CK 19) and negative CK 7 was preserved in only 19.5% of the HCC cases. A so-called mixed staining pattern (negative and positive CK staining within the same tumor) was observed in approximately one-fourth of the cases. There was no correlation between the preservation of the hepatocyte-specific CK pattern and the degree of differentiation, tumor grade, or DNA ploidy of the HCCs. In approximately 10% of the primary tumors, CK 7 was expressed in the entire parenchymal cell compartment of the HCC nodules, whereas it was present in a mixed staining pattern in more than half of the cases. In lung metastases, CK 7 was less common, only expressed in approximately one-fourth of the cases. Alterations in the CK patterns were observed in the nonneoplastic hepatocytes of the tumor-bearing monkeys. These included mixed staining patterns in which the CKs appeared as positive and negative regenerating nodules side-by-side. As was observed in the HCCs, CK 7 was more commonly expressed in the nonneoplastic parenchyma in the form of mixed staining pattern than the other three CKs. Moreover, CK 7-negative HCCs occurred more frequently in CK 7-negative livers than in positive livers. Proliferation of CK 7- and CK 19-positive bile ductules and bile ductular-like (oval) cells was frequently associated with the DEN-induced liver injury and hepatocarcinogenesis. CONCLUSIONS: This is the first report on CK expression in monkey liver. The findings show that the hepatocyte specific pattern is not always preserved during DEN-induced hepatocarcinogenesis and may therefore not be useful in differentiating between HCCs and cholangiocarcinomas.

Human cervical cancer. Retinoids, interferon and human papillomavirus.

Our studies highlight the importance of dietary vitamin A (retinol) and other retinoids in maintaining normal cervical cell function and in inhibiting the growth of cervical tumors. Based on our results we conclude that 1) HPV 16-immortalization enhances cervical cell sensitivity to retinoids, 2) cytokeratin expression may be useful as a marker for evaluating the success of retinoid therapy in vivo, 3) retinoids do not necessarily act to inhibit proliferation of HPV-immortalized cervical cells via effects on HPV E6 and E7 RNA levels and 4) retinoids may act to inhibit cervical proliferation by "suppressing" the activity of the EGF and IGF signalling pathways. Based on these and other results, it is worth considering the possibility that vitamin A or related retinoids could be administered therapeutically, early in the neoplastic process (either systemically or locally), to inhibit the progress of the disease. These results also suggest that combined interferon/retinoid therapy may provide an enhanced beneficial effect to reduce cervical tumor size due to the fact that each agent is inhibiting cervical cell proliferation via distinct, but reinforcing, pathways (i.e., IFN gamma reduces E6/E7 expression, RA inhibits the function of the EGF and IGF1 signalling pathways).

Transorbital approaches to the cranial base.

Orbital neoplasms and their treatment may have significant negative impact on patient's survival, function, and appearance. It is essential to establish early and accurate diagnosis. The anatomic location of the orbit allows neoplastic processes to threaten structures in the intracranial cavity relatively easily. This adds significantly to the complexity of managing orbital tumors. Improved surgical approaches allow better visualization of the neoplasm and greater protection of essential anatomic structures. This also permits immediate functional and aesthetic reconstruction. A multidisciplinary approach involving the head and neck surgeon, neurosurgeon, reconstructive surgeon, ophthalmologist, radiologist, radiation oncologist, and medical oncologist enhances the quality of management of orbital lesions. Further development of new techniques as well as improved application of adjuvant modalities will, it is hoped, continue to improve the prognosis of patients with orbital neoplasms.

[Use of polymerase chain reaction (PCR) in oncohematology. Detection of minimal residual disease].

The early detection of clonogenic cells able to survive cytotoxic therapy and to initiate a neoplastic process, defined as Minimal Residual Disease (MRD), represents a major objective for the development of new and more sensitive diagnostic methods. The cloning of different chromosomal translocations along with the application of the Polymerase Chain Reaction (PCR) technique has allowed the characterization of various hematological disorders at the molecular level. This approach has led to the development of PCR based assays able to detect a minimum number of malignant cells with very high specificity. In the present paper we discuss the use of PCR and its value as a predictor for relapse in lymphomas, chronic myelogenous leukemia and acute promyelocytic leukemia, characterized by the t(14;18) t(9;22) and t(15;17) translocations, respectively.

[Inflammatory pseudotumor of the breast. Report of a case and review of the literature].

Inflammatory pseudotumor (IPT) truly represents an enigmatic entity. A tumor-like lesion known with many synonyms according to the various patronymic authors who named it on the basis of the different observed morphological appearances. Among these names plasma cell granuloma, plasma cell/histiocytoma complex, xanthomatous pseudotumor, xanthoma, fibrous xanthoma, histiocytoma, xanthogranuloma, inflammatory myofibroblastic tumor, inflammatory myofibrohistiocytic proliferation are just the most renown. The enigma consists in the fact that it can easily be misunderstood as a sarcoma with a marked inflammatory component just as reciprocally an inflammatory sarcoma can be misinterpreted as an IPT. As it was not enough some authors believe that some so-called IPT actually represent or may evolve into a true neoplastic process. For this reason a big debate has been raised concerning the biologic behaviour of this entity and a sense of scepticism often surrounds pathological diagnoses termed under this rubric. The predominant pattern is that of a plasma cell infiltration with a definitely histiocytic component and mostly bland spindle shaped cells which occasionally can look somewhat atypical, and can form fascicles in some areas. Anyway putting apart diagnostic misinterpretations, IPT pathogenetically is intended as the result of an inflammatory process mediated locally by an inappropriate production of monokines. It has been described in many different anatomic visceral and somatic sites, from the classical ones such as soft tissues and retroperitoneum to the most unusual such as nervous system and its covering to the most recently observed such as skin or lymph nodes or salivary glands. Breast is definitely an exoteric site of location of such kind of lesion. Only one case has previously been described. This case regards a lady aged 38 years which was locally treated by surgical excision: this case aside the standard microscopic examination was also studied immunohistochemically including proliferation markers (Ki-67/MIB-1) and on flow cytometry which are helpful means to confidently diagnose such entity. The differential diagnoses and diagnostic difficulties concerning the correct interpretation of this lesion are dealt with.

Guided fine needle absorption biopsy in the diagnosis of cysts in maritime workers.

Cysts in the organs of the abdominal cavity revealed in ultrasonographic examinations in patients with none or with unclear clinical symptoms cause diagnostic and fitness for work assessment problems. The aim of the study was to evaluate the usefulness of ultrasonography of cysts, combined with biochemical, bacteriological, cytological examinations and analysis of markers CEA and AFP of the fluid collected with fine needle aspiration biopsy (FAB). The study material examined were maritime workers who were submitted to these examinations for diagnostic purposes and/or previous to issue of certificate of health for work at sea. Examinations results provided support for usefulness of the method discussed in diagnosis and differentiation of inflammatory and neoplastic processes with simple cysts. This was crucial for final diagnosis, for decision about treatment and recognizing fitness for work at sea. It was concluded that the method FAB is safe both in hospital as well as in ambulatory conditions.

The role of surgery in primary osteomyelitis of the chest wall.

Seven patients with primary osteomyelitis of the chest wall are described. All patients presented with pain at the site of infection, and four patients had a tumor-like mass that could easily be confused with a neoplastic process. All were heroin addicted and pseudomonas was the most common organism cultured. Conservative management with antibiotics for a minimum of 4 weeks is recommended with operation assuming a secondary role, namely, to assist in the initial diagnosis or in the form of limited resection of residual sinuses. Primary wide resection is discouraged.
